Repetitiveness of the oral glucose tolerance test in children and adolescents

BACKGROUND Data regarding the most suitable diagnostic method for the diagnosis of glucose impairment in asymptomatic children and adolescents are ***,limited data are available on the reproducibility of the oral glucose tolerance test(OGTT)in children and adolescents who are obese(OB).AIM To investigate the usefulness of the OGTT as a screening method for glucose dysregulation in children and *** Eighty-one children and adolescents,41 females,either overweight(OW),OB or normal weight(NW)but with a strong positive family history of type 2 diabetes mellitus(T2DM),were enrolled in the present observational study from the Outpatient Clinic of Paediatric Endocrinology of the University Hospital of Patras in *** or two 3-h OGTTs were performed and glucose,insulin and Cpeptide concentrations were measured at several time points(t=0 min,t=15 min,t=30 min,t=60 min,t=90 min,t=120 min,t=180 min).RESULTS Good repetitiveness was observed in the OGTT response with regard to T2DM,while low repetitiveness was noted in the OGTT response with regard to impaired glucose tolerance(IGT)and no repetitiveness with regard to impaired fasting glucose(IFG).In addition,no concordance was observed between IFG and *** the 1st and 2nd OGTTs,no significant difference was found in the glucose concentrations between NW,OW and OB patients,whereas insulin and C-peptide concentrations were higher in OW and OB compared to NW patients at several time points during the ***,OW and OB patients showed a worsening insulin and C-peptide response during the 2nd OGTT as compared to the 1st *** In mild or moderate disorders of glucose metabolism,such as IFG and IGT,a diagnosis may not be reached using only one OGTT,and a second test or additional investigations may be *** glucose metabolism is profoundly impaired,as in T2DM,one OGTT is probably more reliable and adequate for establishing the *** weight and/or a positiv

Background:This study aimed to determine the effect of different carbohydrate(CHO)doses on exercise capacity in patients with McArdle disease—the paradigm of“exercise intolerance”,characterized by complete muscle glycogen unavailability—and to determine whether higher exogenous glucose levels affect metabolic responses at the McArdle muscle cell(in vitro)***:Patients with McArdle disease(n=8)and healthy controls(n=9)underwent a 12-min submaximal cycling constant-load bout followed by a maximal ramp test 15 min after ingesting a non-caloric *** a randomized,double-blinded,cross-over design,patients repeated the tests after consuming either 75 g or 150 g of CHO(glucose:fructose=2:1).Cardiorespiratory,biochemical,perceptual,and electromyographic(EMG)variables were ***,glucose uptake and lactate appearance were studied in vitro in wild-type and McArdle mouse myotubes cultured with increasing glucose concentrations(0.35,1.00,4.50,and 10.00 g/L).Results:Compared with controls,patients showed the“classical”second-wind phenomenon(after prior disproportionate tachycardia,myalgia,and excess electromyographic activity during submaximal exercise,all p<0.05)and an impaired endurance exercise capacity(-51%ventilatory threshold and55%peak power output,both p<0.001).Regardless of the CHO dose(p<0.05 for both doses compared with the placebo),CHO intake increased blood glucose and lactate levels,decreased fat oxidation rates,and attenuated the second wind in the ***,only the higher dose increased ventilatory threshold(+27%,p=0.010)and peak power output(+18%,p=0.007).In vitro analyses revealed no differences in lactate levels across glucose concentrations in wild-type myotubes,whereas a doseresponse effect was observed in McArdle ***:CHO intake exerts beneficial effects on exercise capacity in McArdle disease,a condition associated with total muscle glycogen *** of these benefits are dose dependent.

Common psychiatric disorders(CPDs)and depression contribute significantly to the global epidemic of type 2 diabetes(T2D).We postulated a possible pathophysiological mechanism that through Bridge-Symptoms present in depression and CPDs,promotes the establishment of emotional eating,activation of the reward system,onset of overweight and obesity and,ultimately the increased risk of developing *** plausibility of the proposed pathophysiological mechanism is supported by the mechanism of action of drugs such as naltrexonebupropion currently approved for the treatment of both obesity/overweight with T2D and as separate active pharmaceutical ingredients in drug addiction,but also from initial evidence that is emerging regarding glucagon-like peptide 1 receptor agonists that appear to be effective in the treatment of drug *** hope that our hypothesis may be useful in interpreting the higher prevalence of CPDs and depression in patients with T2D compared with the general population and may help refine the integrated psychiatric-diabetic therapy approach to improve the treatment and or remission of T2D.

Endoluminal vacuum-assisted therapy(EVT)has been introduced recently to treat colorectal anastomotic leaks in clinically stable non-peritonitic *** application has been mainly reserved to low colorectal and colo-anal *** main advantage of this new procedure is to ensure continuous drainage of the abscess cavity,to promote and to accelerate the formation of granulation tissue resulting in a reduction of the abscess *** reported results are promising allowing a higher preservation of the anastomosis when compared to conventional treatments that include trans-anastomotic tube placement,percutaneous drainage,endoscopic clipping of the anastomotic defect or stent ***,despite this procedure is gaining acceptance among the surgical community,indications,inclusion criteria and definitions of success are not yet standardized and extremely heterogeneous,making it difficult to reach definitive conclusions and to ascertain which are the real benefits of this new ***,long-term and functional results are poorly *** present review is focused on critically analyzing the theoretical benefits and risks of the procedure,short-and long-term functional results and future direction in the application of EVT.

Multidisciplinary treatment of rectal cancer in 2014: Where are we going?

In the present review we discuss the recent developments and future directions in the multimodal treatment of locally advanced rectal cancer, with respect to staging and re-staging modalities, to the current role of neoadjuvant chemo-radiation and to the conservative and more limited surgical approaches based on tumour response after neoadjuvant combined therapy. When initial tumor staging is considered a high accuracy has been reported for T pre-treatment staging, while preoperative lymph node mapping is still suboptimal. With respect to tumour re-staging, all the current available modalities still present a limited accuracy, in particular in defining a complete response. The role of short vs long-course radiotherapy regimens as well as the optimal time of surgery are still unclear and under investigation by means of ongoing randomized trials. Observational management or local excision following tumour complete response are promising alternatives to total mesorectal excision, but need further evaluation, and their use outside of a clinical trial is not *** preoperative selection of patients who will benefit from neoadjuvant radiotherapy or not, as well as the proper identification of a clinical complete tumour response after combined treatment modalities,will influence the future directions in the treatment of locally advanced rectal cancer.

Effect of grassland mosaic structure and dynamism on the reproductive habitat suitability for Alectoris graeca in central Apennines

Over the last several decades,the Mediterranean region has been subjected to mountain abandonment and farming cessation,leading to changes in vegetation and coenological features of grasslands,contextually to the strong decline of the rock partridge(Alectoris graeca).Our hypothesis was that the ongoing dynamic processes leading to the compositional changes of grasslands in central Apennines affect the habitat suitability for the singing male(territorial male defending its reproductive site).In 2015,we defined the presence/absence of spring territorial singing males in seven sites(1,250-2,400 m a.s.l.),by the census in 74 playback stations,distributed along 15 *** characterized the topography and the vegetation mosaic of such sites according to the collected topographic and vegetation cover data in 59 randomly selected plots(100 m×100 m)along the *** understand the relations among the environmental variables and the effect of the composition and dynamism of the vegetation mosaics on the presence/absence of the singing male,we used principal components analysis and generalized linear mixed-effect *** results emphasized the marked overlap between the general environmental conditions,which proved to define the habitat suitability for ***,and the characteristics of the site chosen by the singing male for starting the reproductive ***,we found that the site suitability for the singing male decreases when the vegetation recovery processes are ongoing,because of the spread of coarse tall grassesdominated communities and/or grassland types with dense *** grass-dominated communities and thick-turf grasslands exert their negative effect decreasing the habitat suitability for the singing male starting from low cover ***,it is conceivable that singing male’s suitable habitat will be dramatically restricted to the steepest south-facing slopes,where topographic and soil conditions do not allow the spread of grassl

Modeling egg development of the pest Clavipalpus ursinus （Coleoptera： Melolonthidae） using a temperature-dependent approach

Predicting the population dynamics of insects in natural conditions is es- sential for their management or preservation, and temperature-dependent development models contribute to achieving this. In this research the effects of temperature and soil moisture content on egg development and hatching of Clavipalpus ursinus （Blanchard） were evaluated. The eggs were exposed to seven temperature treatments with averages of 7.2, 13.0, 15.5, 19.7, 20.6, 22.0 and 25.3~C, in combination with three soil moisture contents of 40%, 60% and 80%. A linear and two non-linear （Lactin and Briere） models were evaluated in order to determine the thermal requirements of this developmental stage. Temperature affected significantly the time of development and egg hatching, while no significant effect was observed for moisture content. Thermal requirements were set as： 7.2℃ for lower developmental threshold, 20.6℃ for optimum developmental threshold, 25.3℃ for maximum temperature and 344.83 degree-days for the thermal constant. The linear model described satisfactorily egg development at intermediate temperatures; nev- ertheless, a slightly better fit of the observed data was obtained with the Lactin model. Egg development took place inside a narrow range of temperatures. Consequently, an increment of soil temperature could generate a negative impact on the population size of this species or changes in its biological parameters.
