Simultaneous, Asymmetrical, Comminuted and Bilateral Ballistic Fractures by Multiple Gunshot Wounds and a Review of Its Literature: A Case Report in Chad

Although open leg fractures are very common in orthopedics and traumatology, bilateral open leg fractures are extremely rare and are usually associated with an increased risk of complications. We report a case of a simultaneous, asymmetrical, comminuted and bilateral open fracture of the tibia and fibula by a firearm that occurred during an escape attempt in a 29-year-old prisoner. The pre-operative radiological assessment found an open fracture of both legs of the ballistic type;multiple traumatic gunshot wounds with a narrow entry hole and a wide exit hole. Early surgical intervention of intravenous antibiotics, tetanus prophylaxis and open fracture irrigation and debridement was performed within the 6 hour rule. The treatment consisted of a posterior splint followed by trimming andosteosynthesis using a FESSA external fixator from the military health service. A second operation was not needed. Evolution was favorable with ambulation starting from the 45th day. Simultaneous and bilateral tibia-fibula fractures by a firearm are exceptional, therefore, the treatment was surgical with the pre-operative and post-operative protocols well managed.

The Cambrian-lower Ordovician volcanic units of the South Armorican and Occitan domains are ana- lysed in a tectonostratigraphic survey of the French Variscan Belt. The South Armorican lavas consist of continental tholeiites in middle Camhrian-Furongian sequences related to continental break-up. A significant volcanic activity occurred in the Tremadocian, dominated by crustal melted rhyolitic lavas and initial rifting tholeiites. The Occitan lavas are distributed into five volcanic phases：（I） basal Cambrian rhyolites, （2） upper lower Cambrian Mg-rich tholeiites close to N-MORBs but crustal contaminated, （3） upper lower-middle Cambrian continental tholeiites, （4） Tremadocian rhyolites, and （5） upper lower Ordovician initial rift tholeiites. A rifting event linked to asthenosphere upwelling took place in the late early Cambrian but did not evolve. It renewed in the Tremadocian with abundant crustal melting due to underplating of mixed asthenospheric and lithospheric magmas. This main tectono-magmatic conti- nental rift is termed the ＂Tremadocian Tectonic Belt＂ underlined by a chain of rhyolitic volcanoes from Occitan and South Armorican domains to Central Iberia. It evolved with the setting of syn-rift coarse siliciclastic deposits overlain by post-rift deep water shales in a suite of sedimentary basins that fore- casted the South Armorican-Medio-European Ocean as a part of the Palaeotethys Ocean.

A highly distorted chiral nanographene structure composed of triple corannulene-fused[5]helicenes is prepared with the help of the Heck reaction and oxidative photocyclization with an overall isolated yield of 28%.The complex three-dimensional(3D)structure of the bowl-helix hybrid nanostructure is studied by a combination of noncontact atomic force microscopy(AFM)and scanning tunneling microscopy(STM)on the Cu(111)surface,density functional theory calculations,AFM/STM simulations,and high-performance liquid chromatography-electronic circular dichroism *** examination reveals a molecular structure in which the three bowl-shaped corannulene bladesd position themselves in a C3-symmetric fashion around a highly twisted triphenylene *** molecule appears to be shaped like a propeller in which the concave side of the bowls face away from the connected[5]helicene *** chirality of the nanostructure is confirmed by the direct visualization of both MMM and PPP enantiomers at the single-molecule level by scanning probe *** results underline that submolecular resolution imaging by AFM/STM is a powerful real-space tool for the stereochemical characterization of 3D curved chiral nanographene structures.

高载流子迁移率和器件性能的一致性对于有机场效应晶体管在阵列和集成电路上的应用至关重要.然而,同时具备高性能和小批间差的方法当前仍然是个挑战.本论文在原子力显微镜和偏振荧光显微镜表征的基础上,报道了在PTCDI-C1_(3)分子模板上生长具有大晶畴的dif-TES-ADT类液晶状薄膜的方法.我们进一步将所获得的薄膜用作有机场效应晶体管的载流子传输通道,其饱和载流子迁移率高达1.62±0.26 cm^(2)V^(-1)s^(-1).更重要的是,在重复三个批次之间,每次50个器件的迁移率批间差仅为16%.这一发现为设计材料和器件结构以同时实现高载流子迁移率和器件均匀性提供了一种新思路.

AIM: To evaluate the impact of computed b = 1400 s/mm2(C-b1400) vs measured b = 1400 s/mm2(M-b1400) diffusion-weighted images(DWI) on lesion detection rate, image quality and quality of lesion demarcation using a modern 3T-MR system based on a small-field-of-view sequence(sFOV). METHODS: Thirty patients(PSA: 9.5 ± 8.7 ng/mL; 68 ± 12 years) referred for magnetic resonance imaging(MRI) of the prostate were enrolled in this study. All measurements were performed on a 3T MR *** DWI, a single-shot EPI diffusion sequence(b = 0, 100, 400, 800 s/mm2) was utilized. C-b1400 was cal-culated voxelwise from the ADC and diffusion images. Additionally, M-b1400 was acquired for evaluation and comparison. Lesion detection rate and maximum lesion diameters were obtained and compared. Image quality and quality of lesion demarcation were rated accord-ing to a 5-point Likert-type scale. Ratios of lesion-to-bladder as well as prostate-to-bladder signal intensity(SI) were calculated to estimate the signal-to-noise-ratio(SNR). RESULTS: Twenty-four lesions were detected on M-b1400 images and compared to C-b1400 images. C-b1400 detected three additional cancer suspicious lesions. Overall image quality was rated significantly better and SI ratios were significantly higher on C-b1400(2.3 ± 0.8 vs 3.1 ± 1.0, P < 0.001; 5.6 ± 1.8 vs 2.8 ± 0.9, P < 0.001). Comparison of lesion size showed no significant differences between C- and M-b1400(P = 0.22). CONCLUSION: Combination of a high b-value extrap-olation and sFOV may contribute to increase diagnostic accuracy of DWI without an increase of acquisition time, which may be useful to guide targeted prostate biopsies and to improve quality of multiparametric MRI(mMRI) especially under economical aspects in a pri-vate practice setting.

Room for improvement: Patterns of referral misdiagnosis to a vascular anomalies center

Objective: Vascular anomalies are a diagnostic and therapeutic challenge. Errors in diagnosis lead to treatment delay, inappropriate interventions and prolonged suffering. The aim of this study was to analyze patterns of misdiagnosis among patients referred to a vascular anomalies center (VAC). This will better define the problem and may be used to refine and improve referral guidelines for patients with vascular anomalies. Patients and Methods: After IRB approval, we performed a single-center retrospective review of all patients referred to a vascular anomaly between January 1, 2008 and December 15, 2011. Evaluation of both referral and final diagnosis was made. Data regarding accuracy of diagnosis were determined and compared for both vascular tumors and malformations. Results: Mean age was 7.9 ± 7.7 (13 days - 66 years). 42% had a correct diagnosis at the time of referral. Vascular tumors were correctly diagnosed more often than vascular malformations (58% vs 38%). The most common misdiagnosis for infantile hemangioma (IH) was venous malformation (VM). The most common misdiagnosis for VM was IH. Nonspecific and historical terms such as “mass”, “lymphangioma”, and “cavernous hemangioma” frequently appear as the referral diagnosis. Conclusion: Referral misdiagnosis is common. IH and VM are frequently confused and if there is any uncertainty in the diagnosis, these patients should also be referred to a VAC, in addition to the more complicated anomalies. Outdated nomenclature remains prevalent and continued efforts should be made to adhere to International Society for the Study of Vascular Anomalies (ISSVA) classification. Improvements in diagnostic accuracy are likely to greatly improve patients’ care.
