Oral Health and Self-Perception in the Elderly

Background: Investigating self-perception of oral health among the elderly may contribute towards changing public health and assistance policies in the area. Methods: The present study assessed the oral health status (tooth decay, need for treatment, periodontal status, use of and need for dental prosthesis, soft tissue alterations and self-perception of oral health) of 102 elderly people from two community groups in Fortaleza, Ceará State, Brazil. Results: Mean age was 69.9 years and 82.4% were female. Mean DMFT was 30.17 with a predominant missing component;63.7% were edentulous. Regarding prosthesis, 29.4% and 67.6% of elderly patients did not use upper and lower dentures respectively, while 66.7% and 78.4% required some type of upper and lower prosthesis respectively. Among valid sextants, 44.3% presented dental calculus and 34.3% presented loss of attachment from 6 to 8 mm. Soft tissue alterations were found in 58.8%. despite of precarious clinical conditions and dental treatment needs, good oral health perception was found (49%), as well as satisfaction with the appearance of teeth and gums (60.8%). Conclusion: Subjects displayed unsatisfactory oral health conditions, accumulated over time and resulting from the lack of policies favoring this age group. There is a need for oral rehabilitation that enables the recovery of function and esthetics among the elderly, restoring their self-esteem and improving social interaction.

de-escalating therapy in gastric aggressive lymphoma

The treatment of primary gastric diffuse large B-cell lymphoma(DLBCL) has changed radically over the last 10–15 years, with the abandonment of routine gastrectomy in favor of more conservative therapies. Low-level evidence suggests that consolidation radiotherapy could be avoided in patients with limited-stage DLBCL of the stomach who achieve complete remission after rituximab-CHOP combination. Small, recent prospective trials suggest that selected patients with limited-stage Helicobacter pylori(H. pylori)-positive DLBCL of the stomach and favorable prognostic factors can be managed with antibiotics alone, with excellent disease control and cure rates, keeping chemo-radiotherapy for unresponsive patients. This recommendation should equally regard patients with mucosa-associated lymphoid tissue-related or de novo DLBCL. Future studies should be focused on the establishment of reliable variables able to distinguish the best candidates for exclusive treatment with H. pylori eradication from those who need for conventional chemo-immunotherapy.

A Comparison of the Estimators of the Scale Parameter of the Errors Distribution in the L₁ Regression

The L1 regression is a robust alternative to the least squares regression whenever there are outliers in the values of the response variable, or the errors follow a long-tailed distribution. To calculate the standard errors of the L1 estimators, construct confidence intervals and test hypotheses about the parameters of the model, or to calculate a robust coefficient of determination, it is necessary to have an estimate of a scale parameterτ. This parameter is such that τ2/n is the variance of the median of a sample of size n from the errors distribution. [1] proposed the use of , a consistent, and so, an asymptotically unbiased estimator of τ. However, this estimator is not stable in small samples, in the sense that it can increase with the introduction of new independent variables in the model. When the errors follow the Laplace distribution, the maximum likelihood estimator of τ, say , is the mean absolute error, that is, the mean of the absolute residuals. This estimator always decreases when new independent variables are added to the model. Our objective is to develop asymptotic properties of under several errors distributions analytically. We also performed a simulation study to compare the distributions of both estimators in small samples with the objective to establish conditions in which is a good alternative to for such situations.

Objective:To investigate the isolation of enterobacteria associated with Macrobrachium amazonicum(***) farming and evaluate the in vitro antimicrobial susceptibility of Vibrio ***:Strains were isolated from female *** prawns and environmental and hatchery *** assays were used to identify bacterial genera and those belonging to the genus Vibrio were submitted to further analyses for species identification,through Vitek 2 automated system and *** test was performed according to Clinical Laboratory Standards ***:The following genera of enlerobacteria were recovered:Enterobacter(n=11),Citrobacter(n=10),Proteus(n=2),Serratia(n=2),Kluyvera(n=2),Providencia(n=2),Cedecea(n=1),Escherichia(n=1).Edwardsiella(n=1) and BulliauxeUa(n=1).As for Vibrio,three species were identified:Vibrio cholerae non-O1/non-O139(n=4).Vibrio vulnificus(***)(n=1) and Vibrio mimicus(n=1).Vibrio *** minimum inhibitory concentrations values within the susceptibility range established by Clinical Laboratory Standards Institute for almost all antibiotics,except for ***,which presented intermediate profile to ***:Enterobacteria do not seem to be the most important pathogens associated with *** farming,whereas the recovery of Vibrio *** larviculture,with emphasis on Vibrio cholerae and ***,deserves special attention due to their role as potentially zoonotic aquaculture-associated ***,the intermediate susceptibility of *** to ampicillin reflects the importance of monitoring drug use in prawn farming.

CeO_(2)-Nb_(2)O_(5)photocatalysts for degradation of organic pollutants in water

The photocatalytic properties of CeO2-Nb2O5 photocatalysts in heterogeneous photocatalysis(under ultraviolet and visible radiation)and in Fenton-like process were *** blue dye(MB)and phenol(Ph)were used as models of pollutant molecules for these reactions,and the photocatalysts were characterized by X-ray diffraction(XRD),diffuse reflectance spectroscopy(DRS),scanning electron microscopy(SEM),energy-dispersive X-ray spectroscopy(EDX)and thermally stimulated luminescence(TL).The results indicated that the addition of CeO2(0.3 wt%,1.0 wt%and 2.0 wt%)to Nb2O5 sensitized the resultant materials,increasing light absorption in the visible ***,there is a suitable formulation of CeO2-Nb2O5 photocatalysts to improve each photocatalytic *** heterogeneous photocatalysis,the addition of small CeO2 quantities to Nb2O5 was enough to improve the photocatalytic activity of CeO2-Nb2O5 photocatalysts(The best composition reported was CeO20.3 wt%.).The effectiveness of the catalyst was explained by the decrease in the number of trapping and luminescence centers in the conduction band of the material after the addition of CeO2 to Nb2O5,but a large amount of CeO2 decreased the number of trapping,luminescent and active centers to a large ***,in a Fenton-like process,the addition of CeO2 to Nb2O5 was favorable in all the proportions studied.(The best composition was 2.0 wt%CeO2.)In this case,the effectiveness was explained by the influence of the adsorption process(adsorption-triggered process),and the interactions between H2 O2 and Ce3+of the CeO2 in each photocatalyst thus formed surface peroxide species O22-,which induced the removal of the organic molecules under visible light.

Serum Adiponectin levels in Different Phenotypes of Polycystic Ovary Syndrome

Objectives: to evaluate and compare serum adiponectin levels in different phenotypes of polycystic ovary syndrome (PCOS) and to investigate their correlation with endocrine and metabolic parameters. Material and methods: we studied 5 groups of patients: A (n = 20): H (hyperandrogenism) + O (oligoanovulation) + P (polycystic ovary) [classic phenotype];B (n = 17): H + O [classic phenotype but normal ovaries];C (n = 15): H + P [Ovulatory phenotype];D (n = 17): O + P [Normoandrogenic phenotype];and E (n = 16) control group. Body mass index, waist circumference, waist/hip ratio, blood pressure and hirsutism were evaluated. Serum concentrations of adiponectin, insulin, Creactive protein, SHBG, androgens and lipids were measured. Oral glucose tolerance test was performed. Results: there were no differences between the groups in terms of age and BMI. Total cholesterol, LDL-C and triglyceride levels were higher in phenotype A than in C (P P = 0.03). HOMA-IR, insulin and glucose/insulin ratio were significantly higher in phenotypes A and D vs C and E (P P P dentify phenotypes at increased metabolic risk.

Low phase angle is associated with the development of hepatic encephalopathy in patients with cirrhosis

AIM Evaluate the association between phase angle and the development of hepatic encephalopathy in the longterm follow-up of cirrhotic *** This was a prospective cohort study. Clinical, nutritional and biochemical evaluations were performed. MannWhitney's U and χ2 tests were used as appropriate. Kaplan-Meier curves and Cox proportional Hazards analysis were used to evaluate the prediction and incidence of hepatic *** Two hundred and twenty were included; the most frequent etiology of cirrhosis was hepatitis C infection, 52% of the patients developed hepatic encephalopathy(18.6% covert and 33.3% overt); the main precipitating factors were infections and variceal bleeding. KaplanMeier curves showed a higher proportion of HE in the group with low phase angle(39%) compared to the normal phase angle group(13%)(P = 0.012). Furthermore, creatinine and phase angle remained independently associated to hepatic encephalopathy in the Cox regression multivariate analysis [hazard ratio = 1.80(1.07-3.03)]. CONCLUSION In our cohort of patients low phase angle was associated with an increased incidence of hepatic encephalopathy. Phase angle is a useful nutritional marker that evaluates cachexia and could be used as a part of the integral assessment in patients with cirrhosis.
