Cardiac resynchronization therapy(CRT)is an evidence-based effective therapy of symptomatic heart failure with reduced ejection fraction refractory to optimal medical ther-apy and associated with intraventricular conduction disturbance,that results in a significant electrical ventricular dyssynchrony.However,the non-response rate to CRT is still 20%−40%.[1]The potential underlying causes of CRT nonresponse are:(1)suboptimal localization of the left ventricular(LV)electrode,far away from the latest activated LV region.

Cardiac resynchronization therapy(CRT)is an evidence-based effective therapy of symptomatic heart failure with reduced ejection fraction refractory to optimal medical treatment associated with intraventricular conduction disturbance,that results in electrical dyssynchrony and further deterioration of systolic ventricular function.However,the non-response rate to CRT is still 20%−40%,which can be decreased by better patient selection.The main determinant of CRT outcome is the presence or absence of significant ventricular dyssynchrony and the ability of the applied CRT technique to eliminate it.The current guidelines recommend the determination of QRs morphology and QRs duration and the measurement of left ventricular ejection fraction for patient selection for CRT.However,QRs morphology and QRs duration are not perfect indicators of electrical dyssynchrony,which is the cause of the not negligible non-response rate to CRT and the missed CRT implantation in a significant number of patients who have the appropriate substrate for CRT.Using imaging modalities,many ventricular dyssynchrony criteria were devised for the detection of mechanical dyssynchrony,but their utility in patient selection for CRT is not yet proven,therefore their use is not recommended for this purpose.Moreover,CRT can eliminate only mechanical dyssynchrony due to underlying electrical dyssynchrony,for this reason ECG has a greater role in the detection of ventricular dyssynchrony than imaging modalities.To improve assessment of electrical dyssynchrony,we devised two novel ECG dyssynchrony criteria,which can estimate interventricular and left ventricular intraventricular dyssynchrony in order to improve patient selection for CRT.Here we discuss the results achieved by the application of these new ECG dyssynchrony criteria,which proved to be useful in predicting the CRT response in patients with nonspecific intraventricular conduction disturbance pattern(the second greatest group of CRT candidates),and the significance of other new ECG dyssynchrony

Temperature is a key factor that shapes the distribution of organisms.Having knowledge about how species respond to temperature is relevant to devise strategies for addressing the impacts of climate change.Aquatic insects are particularly vulnerable to climate change,yet there is still much to learn about their ecology and distribution.In the Yungas ecoregion of Northwestern Argentina,cold-and warm-adapted species of the orders Ephemeroptera,Plecoptera,and Trichoptera(EPT)are segregated by elevation.We modeled the ecological niche of south American EPT species in this region using available data and projected their potential distribution in geographic space.species were grouped based on their ecogeographic similarity,and we analyzed their replacement pattern along elevation gradients,focusing on the ecotone where opposing thermal preferences converge.Along this interface,we identified critical points where the combined incidence of cold and warm assemblages maximizes,indicating a significant transition zone.We found that the Montane Cloud Forest holds the interface,with a particularly greater suitability at its lower boundary.The main axis of the interface runs in a N-s direction and falls between 14°C-16°C mean annual isotherms.The probability of a particular location within a basin being classified as part of the interface increases as Kira’s warmth index approaches a score around 150.Understanding the interface is critical for defining the thermal limits of species distribution and designing biomonitoring programs.Changes in the location of thermal constants related to mountainous ecotones may cause vertical displacement of aquatic insects and vegetation communities.We have recognized significant temperature thresholds that serve as indicators of suitability for the interface.As global warming is anticipated to shift these indicators,we suggest using them to monitor the imprints of climate change on mountain ecosystems.

A 58-year-old man with hypertension and half a year ago documented left bundle branch block(LBBB)in his past history presented with progressive dyspnea.A pre-admission echocardiography showed severely depressed systolic left ventricular function[ejection fraction(EF)=27%],diffuse hypokinesis and dilated heart chambers consistent with dilated cardiomyopathy(DCM).

Metabolomics for the diagnosis of bladder cancer: A systematic review

Objective: Metabolomics has been extensively utilized in bladder cancer (BCa) research, employing mass spectrometry and nuclear magnetic resonance spectroscopy to compare various variables (tissues, serum, blood, and urine). This study aimed to identify potential biomarkers for early BCa diagnosis.Methods: A search strategy was designed to identify clinical trials, descriptive and analytical observational studies from databases such as Medline, Embase, Cochrane Central Register of Controlled Trials, and Latin American and Caribbean Literature in Health sciences. Inclusion criteria comprised studies involving BCa tissue, serum, blood, or urine profiling using widely adopted metabolomics techniques like mass spectrometry and nuclear magnetic resonance. Primary outcomes included description of metabolites and metabolomics profiling in BCa patients and the association of metabolites and metabolomics profiling with BCa diagnosis compared to control patients. The risk of bias was assessed using the Quality Assessment of studies of Diagnostic Accuracy.Results: The search strategy yielded 2832 studies, of which 30 case-control studies were included. Urine was predominantly used as the primary sample for metabolite identification. Risk of bias was often unclear inpatient selection, blinding of the index test, and reference standard assessment, but no applicability concerns were observed. Metabolites and metabolomics profiles associated with BCa diagnosis were identified in glucose, amino acids, nucleotides, lipids, and aldehydes metabolism.Conclusion: The identified metabolites in urine included citric acid, valine, tryptophan, taurine, aspartic acid, uridine, ribose, phosphocholine, and carnitine. Tissue samples exhibited elevated levels of lactic acid, amino acids, and lipids. Consistent findings across tissue, urine, and serum samples revealed downregulation of citric acid and upregulation of lactic acid, valine, tryptophan, taurine, glutamine, aspartic acid, uridine, ribose, and phosphocholine.

The Puna grasslands support grazing systems that produce meat and wool in multi-species herds,especially from llama(Lama glama)and sheep.However,it is yet unknown whether grazing pressure can modify grassland structure and Plant Functional Types diversity and cover in Puna grasslands.We analyzed the relationship between grazing pressure and Plant Functional Types by comparing transects located near stockyards(high grazing pressure)and far from them(low pressure)and by evaluating the relationship between the Plant Functional Types cover to a Grazing Pressure Index(GPI).This index incorporates the heterogeneity of traditional pastoral management.At two ecological sites(Ciénego and Tolar),we sampled paired transects(near-far).Plots near and far from corrals were similar in all the variables measured except for plots in Tolar,which tend to have higher total cover,possibly due to fecal fertilization near stockyards.Furthermore,we recorded an increase in total cover and grass cover at higher GPI in Ciénego,while in Tolar we found lower values of total cover and herbaceous dicotyledonous cover at intermediate intensities of GPI.The only negative relationship found was the cover of clonal and non-clonal shrubs,possibly explained by the greater use of these shrubs as firewood near stockyards.In summary,our study does not show important changes in Plant Functional Types as a function of grazing pressure in the studied grasslands probably because grazing pressure is diluted or diminished when livestock rotates in different areas and grazing durations vary.Finally,the long grazing history with which the Puna has evolved could also contribute to the co-structuration between native vegetation and livestock farming.

sex-biased mortality can occur in birds during development,for example due to sexual differences in energy requirement and/or environmental sensitivity,or the effects of sex hormones or sex differences in the expression of mutations linked to sex chromosomes.The extent of sex-bias in mortality may also be related to environmental conditions that influence offspring development and survival.Urban areas often provide poorer conditions for nestling development resulting in higher offspring mortality compared to natural areas,which may accelerate sex differences in offspring mortality in cities.To test this hypothesis,we examined the sex ratio of dead offspring in Great Tits(Parus major),using 427 samples of unhatched eggs and dead nestlings collected in two urban and two forest sites between 2013 and 2019.The ratio of males in the whole sample of dead offspring(56.9%)was significantly higher than expected by an 1:1 ratio,and the strongest sex biases were detected in urban areas(57.6%males)and in young nestlings(s old,59.0%males).However,the sex ratios of dead offspring did not differ significantly among study sites and between offspring developmental stages.29.3%of unhatched eggs contained a visible embryo,and the proportion of embryo-containing unhatched eggs did not differ significantly between urban and forest study sites.These results suggest male-biased offspring mortality in Great Tits,and highlight the need of large datasets to detect subtle differences between habitats and developmental stages.

The present study aimed to evaluate multi-element concentrations(Al,As,Ca,Cd,Co,Cr,Cu,Fe,Mg,Mn,Mo,Ni,Pb,sr)in suspended particulate material(sPM)collected from Ensenada de La Paz(ELP)lagoon,Baja California sur,Mexico in two different periods(september and May)to comprehend their origin,geochemical behavior,mass transfer and associated ecotoxicological risks.The 24 hr variation coefficient of volumetric sPM levels were found to be 51.7%in september and 40.5%in May,signifying the effects of oceanic waters.The calculated enrichment factor(EF)values for all the studied elements were of higher magnitude because of the high surface area and oxide nature of sPM,and in this study,Mo had the highest EF of 46.77 probably due to its origin from continental weathering.From the ecotoxicological perspective,the integrated toxic risk index revealed low toxic risk to the benthic community.However,the mean-ERM-Quotient calculated using the particulate concentrations of As,Cd,Cr,Cu,Ni,Pb indicated 9%probability of toxicity to biota.The comprehensive geochemical and ecotoxicological assessment of particulate metal concentrations in the ELP lagoon signify low to moderate contamination.

BACKGROUND Current cardiac resynchronization therapy(CRT),devised to eliminate dyssynchrony in left bundle branch block(LBBB),works by pacing the latest activated left ventricular site(LALVs).We hypothesized that patients with nonspecific intraventricular conduction disturbance(NICD)pattern respond less favorably to CRT,because their LALVs is far away from that in LBBB.METHODs By measuring the amplitude and polarity of secondary sT-segment alterations in two optional frontal and hori-zontal surface electrocardiogram(ECG)leads and using a software,we determined the resultant 3D spatial secondary sT vector,which is directed 180o away from the LALVs,in 110 patients with LBBB pattern and 77 patients with NICD pattern and heart failure.To validate the ECG method,we also estimated the LALVs by echocardiography using 3D parametric imaging and 2D speckle tracking in 22 LBBB patients and 20 NICD patients.Patients with NICD pattern were subdivided according to their non-overlapping frontal plane resultant secondary sT vector ranges to the NICD-1 subgroup(n=44)and the NICD-2 subgroup(n=33).REsULTs Based on the software determined coordinates of the resultant 3D spatial secondary sT vector directed 180o away from the LALVs,the LALVss were located leftward,posterosuperior in the LBBB group,slightly left,superior in the NICD-1 sub-group,and slightly left,posteroinferior in the NICD-2 subgroup.The LALVs determined by ECG and echocardiography matched in all patients,except two.CONCLUsIONs In the NICD-2 subgroup,a remote LALVs was found from that in LBBB pattern,which might explain the high non-response rate of the NICD pattern to the current CRT technique.

BACKGROUND Pancreatic neuroendocrine tumors(PNETs)are relatively rare but rank as the second most common pancreatic neoplasm.They can be functional,causing early metabolic disturbances due to hormone secretion,or non-functional and diagnosed later based on tumor size-related symptoms.Recent diagnoses of PNETs under 2 cm in size have sparked debates about their management;some practitioners advocate for surgical removal and others suggest observation due to the tumors’lower potential for malignancy.However,it is unclear whether managing these small tumors expectantly is truly safe.AIM To evaluate poor prognostic factors in PNETs based on tumor size(>2 cm orsurgically treated patients.METHODs This cohort study included 64 patients with PNETs who underwent surgical resection between 2006 and 2019 at a high-complexity reference hospital in Medellín,Colombia.To assess patient survival,quarterly follow-ups were conducted during the first year after surgery,followed by semi-annual con-sultations at the hospital's hepatobiliary surgery department.Qualitative variables were described using absolute and relative frequencies,and quantitative variables were expressed using measures of central tendency and their corresponding measures of dispersion.REsULTs The presence of lymph node involvement,neural involvement,and lymphovascular invasion were all associated with an increased risk of mortality,with hazard ratios of 5.68(95%CI:1.26–25.61,P=0.024),6.44(95%CI:1.43–28.93,P=0.015),and 24.87(95%CI:2.98–207.19,P=0.003),respectively.Neural involvement and lymphovascular invasion were present in tumors smaller than 2 cm in diameter and those larger than 2 cm in diameter.The recurrence rates between the two tumor groups were furthermore similar:18.2%for tumors smaller than 2 cm and 21.4%for tumors larger than 2 cm.Patient survival was additionally comparable between the two tumor groups.CONCLUsION Tumor size does not dictate prognosis;lymph node and lymphovascular involvement affect mortality,which high-lights that histopath
