Introduction: Insomnia and depressive symptoms are common among patients with chronic heart failure (HF). Aim: The aim was to describe the prevalence of insomnia and depressive symptoms, as well as to examine the association between insomnia and depressive symptoms in patients with HF. Method: A crosssectional descriptive study including 212 patients with HF. All patients responded to questionnaires regarding sleeping difficulties (Uppsala sleep Inventory-Chronic Heart Failure), daytime sleepiness (Epworth sleepiness scale) and depressive symptoms (The Mental Health scale). Results: Patients with depressive symptoms (34%) had, compared to those without, significantly more major complaints of insomnia as indicated by the sub-types;non-restorative sleep (66% vs. 32%, p sleep (35% vs. 16%, p = 0.003), difficulty in initiating sleep (29% vs. 14%, p = 0.009) and early morning awakenings (25% vs. 10%, p = 0.004). The odds ratio (OR) to suffer from depressive symptoms;were for;non-restorative sleep 5.2 (CI 95%, 2.2 - 12.3), difficulties maintaining sleep 2.5 (CI 95%, 1.2 - 4.9), difficulties in initiating sleep 2.2 (CI 95%, 1.1 - 4.4) and early morning awakenings 2.4 (CI 95%, 1.1 - 5.4). When categorising insomnia into three severity groups, 1) non insomnia, 2) mild insomnia, and 3) severe insomnia, the OR for depressive symptoms for the mild insomnia and severe insomnia group were 2.2 (CI 95%, 1.1 - 4.2) and 7.4 (CI 95%, 2.4 - 22.8) respectively, compared to the non insomnia group. Conclusion: Insomnia is independently associated to depressive symptoms. Assessment of depressive symptoms and insomnia in patients with HF is important since treatment could be targeted to depressive symptoms only and/or to the sleep disturbance.

AIM To investigate the total blood loss(TBL) and the safety with respect to the re-amputation rate after transtibial amputation(TTA) conducted with and without a tourniquet. METHODs The study was a single-centre retrospective cohort study of patients with a primary TTA admitted between January 2013 and April 2015. All patients with a primary TTA were assessed for inclusion if the amputation was performed because of arteriosclerosis or diabetic complications. All patients underwent a standardized TTA procedure that was performed approximately 10 cm below the knee joint and performed with sagittalflaps. The pneumatic tourniquet, when used, was inflated around the femur to a pressure of 100 mmH g above the systolic blood pressure. The number of blood transfusions within the first four postoperative days was recorded. The intraoperative blood loss(OBL), which is defined as the volume of blood lost during surgery, was determined from the suction volume and by the weight difference of the surgical dressings. The trigger for a blood transfusion was set at a decrease in the Hgb level sfusions were performed with pooled red blood cells containing 245 m L per portion, which equals 55 g/L of haemoglobin. The TBL during the first four postoperative days was calculated based on the haemoglobin level and the estimated blood volume. The re-amputation rate was evaluated within 30 d. REsULTs seventy-four out of 86 consecutive patients who underwent TTA within the two-year study period were included in the analysis. Of these, 38 were operated on using a tourniquet and 36 were operated on without using a tourniquet. There were no significant preoperative differences between the groups. The patients in both groups had a postoperative decrease in their Hgb level compared with preoperative baseline values. AIM:To evaluate long-term complications and survival in patients with Budd-Chiari syndrome (BCs) referred to a Danish transjugular intrahepatic portosystemic shunt (TIPs) ***:Twenty-one consecutive patients from 1997-2008 were retrospectively included [15 women and 6 men,median age 40 years (range 17-66 years)].Eighteen Danish patients came from the 1.8 million catchment population of Aarhus University Hospital and three patients were referred from scandinavian *** consisted of tests for underlying haematological,endocrinological,or hypercoagulative disorders parallel to initiation of specific treatment of ***:BCs was mainly caused by thrombophilic (33%) or myeloproliferative (19%) *** percents had symptoms for less than one week with ascites as the most prevalent *** (67%) were treated with TIPs and 7 (33%) were manageable with treatment of the underlying condition and *** median follow-up time for the TIPs-treated patients was 50 mo (range 15-117 mo),and none required subsequent liver *** control was achieved in all TIPs patients with a marked reduction in the dose of diuretics.A total of 14 TIPs revisions were needed,mostly of uncovered *** died during follow-up:One non-TIPs patient worsened after 6 mo and died in relation to transplantation,and one TIPs patient died 4 years after the TIPs-procedure,unrelated to ***:In our BCs cohort TIPs-treated patients have near-complete survival,reduced need for diuretics and compared to historical data a reduced need for liver transplantation.

Undiagnosed obstructive sleep apnea in hypertensive outpatients in primary care—Associations with sleep complaints, depressive symptoms and global perceived health

＜正＞ Functional magnetic resonance imaging （fMRI） is increasingly recognized as being a non-invasive way to perform pharmacological investigations in experimental animal models and patients with neurotransmitter deficits such as in Parkinson’s disease （PD）. Previously, we have demonstrated in normal rhesus monkeys that pharmacological MRI （phMRI, i. e. functional MRI using drugs to evoke CNs responses） can be used to map age-associated changes in the nigrostriatal system following dopaminergic stimulation （NeuroImage 14: 1159 ?1167, 2001）. In the present study, phMRI was used to map dopamine （DA） receptor-mediated neuronal activity, specifically the D1 and D2 receptors in the basal ganglia of hemiparkinsonian rhesus monkeys. Twelve female rhesus monkeys rendered hemiparkinsonian via unilateral infusion of MPTP into the right carotid artery were used in this study. Three sec and age-matched normal controls were used for comparison. Apomor-phine, a mixed D1/D2 receptor agonist, evoked strong activations seen in the MPTP-lesoned putamen and substantia nigra. In parallel, a significant increase in movement speed was seen in these animals, as quantified using an automated video -tracking system. In normal monkeys, apomorphine evoked a small deactivation in the putamen, while no changes were seen in the sN. Accordingly, motor speed remained unchanged compared to baseline levels, in response to apomorphine challenge. Results suggest that phMRI is capable to detect MPTP-induced changes in the basal ganglia with dopaminergic stimulation and also prove that phMRI may be a potential powerful tool for Parkinson’s disease research in human.

Objective: 1) To describe the prevalence of undiagnosed obstructive sleep apnea (OsA) and depressive symptoms in hypertensive men and women below 65 years of age, and 2) to describe the association of OsA to subjective sleep complaints, depressive symptoms and global perceived health. Design: Cross-sectional design focusing on nursing care outcomes of obstructive sleep apnea. setting: Four primary care health centres in sweden. PATIENTs: 411 consecutive patients (52% women), mean age 57.9 years (sD 5.9 years), with diagnosed hypertension (BP > 140/90). Main Outcome Measures: Prevalence of OsA and depressive symptoms, and association of OsA to sleep complaints, depressive symptoms and global perceived health. REsULTs: Mild, moderate and severe OsA was seen among 29%, 16% and 14% of patients, respectively. Depressive symptoms were seen in 16% of the total group, with a higher prevalence among men, compared to women, 21% vs. 12%. No differences were found regarding blood pressure, estimated sleep need, sleep sufficiency index, insomnia symptoms, daytime sleepiness or depressive symptoms with respect to different degrees of OsA. Apnea-hypopnea index was significantly associated to perceived health after adjustment for gender and comorbidities, but when depressive symptoms and non-restorative sleep were added to the model, 33% of the variance in global perceived health was explained. Conclusion: OsA is highly prevalent among patients with hypertension in primary care and does together with sleep complaints and depressive symptoms have a negative impact on global perceived health. Hypertensive patients without subjective sleep complaints or depressive symptoms may still have OsA.
