Abdominoplasty, Liposuction and Brazilian Butt Lift (BBL) in Kinshasa in the Democratic Republic of Congo. My Experience in This Aesthetic Plastic Surgery with Black African Women in a Low-Income Country

Background: In low-income African countries, the demand and use of plastic surgery operations including abdominoplasty, liposuction and Brazilian butt lift (BBL) are increasing. The goal of this work is to present my experience and the challenges of this aesthetic plastic surgery among black African women. Material and Methods: A retrospective study was conducted about the abdominoplasties, liposuctions and Brazilian butt lift (BBL) which I operated in public and private hospitals at Kinshasa (Democratic Republic of Congo) in black African women. It covers a period of 13 years, going from December 1, 2010 to December 1, 2023. For this study, I had at least 6 months period (to Juin 1, 2024) to assess the occurrence of early and late postoperative complications. Results: I performed 84 abdominoplasties, 144 liposuctions and 23 Brazilian butt lifts (BBL). The average age at the time of abdominoplasty was 44 years with extremes ranging from 26 to 55 years and a concentration of cases (60.7%) in the age group of 40 to 49 years. The age group of 20 to 29 years old represented the lowest rate of requests for abdominoplasty (4.7%). Patients with a BMI of 30 to 2 were the majority (61.9%), followed by those between 25 to 2 (29.7%). 67.8% of patients were obese (BMI ≥ 30 kg/m2). The average age at the time of liposuction was 41 years with extremes ranging from 21 years to 69 years;and more than half of cases (68%) in the age group between 30 and 49 years. As for Brazilian butt lift (BBL), the average age was 33 years with extremes ranging from 24 to 42 years and a concentration of patients (91.3%) between 20 and 39 years. The immediate postoperative complications of abdominoplasties observed were: seroma in 7% of cases, hematoma and partial infection of the surgical site in 5% of cases. Pathological scars (hypertrophic, keloid) after abdominoplasties were observed in 9% of cases. The most common complication of liposuction was contour deformity. I observed 16 patients (11.1%)

Health economics for intra-capsular hip fractures undertaking fixation

First Reconstructive Plastic Surgery of the Perineum in a Hominin Pan paniscus (Bonobo) and Plea by a Plastic Surgeon for Surgery for the Benefit of All Hominins (Members of the Human Lineage)

We present, for the first time, the repair by plastic surgery procedures of a large loss of perineal substance in a bonobo (hominin Pan paniscus). A 20-year-old male bonobo living in captivity in the Lola Ya Bonobo Sanctuary in Kinshasa in the Democratic Republic of the Congo was attacked by another bonobo. It was a large perineal wound through which urine flowed, located between the penis and the testicles, taking part of the anterior and posterior urethra, the bladder, as well as the elements of the spermatic duct. To repair the large loss of perineal substance, a pedicled flap of scrotal skin was removed, and then turned over, cutaneous surface on the trench of the loss of bladder substance and the urethral lumen. The postoperative course was satisfactory with healing by the first intention of the surgical wound, despite the wild postoperative behavior of the patient (removal of the vesicourethral catheter and protective plaster). We observed in the short term an urethro-cutaneous fistula, left in natural healing. The reconstructive surgery procedures applied in humans can also be applied with satisfactory results in bonobos, a species in the process of extension and whose members are our closest cousins. The bonobo is genetically close to humans. Based on this first successful experience, we advocate for reconstructive plastic surgery for all hominins (members of the human lineage), if indicated.

Surgical Treatment of Olecranon Fractures Using a Medartis Screw-Locked H-Shaped Plate (Medartis Trilock Olecrane Plate) at the Moulins Yzeure Hospital Center (France)

Introduction: Olecranon fractures represent approximately 10% of upper limb fractures, orthopedic treatment is often doomed to failure and surgical treatment consists of fixing the fracture sites. Patients and Methods: This study retrospectively analyzed 130 patients, 90 men and 40 women, aged on average 48.7 ± 11.9 years (30 to 65) treated between 2018 and 2020 in the Orthopedics and trauma department at the Moulins-Yzeure hospital center, for olecranon fracture using the Medartis H-locked plate. The study aimed to evaluate the results of this surgical method, particularly with regard to postoperative complications. Results: Among the 130 patients, there were 90 men and 40 women, with an average age of 48.7 ± 11.9 years (30 to 65). 50% of all patients were aged over 51 years. The plate used was standard, the same for the 130 patients with the same operating technique. The left side was more affected at 53.8%, the dominant side was less affected at 46%;the majority of patients had been temporarily immobilized with a posterior cast splint before surgery. The average time between trauma and operation was 9 days with a minimum time of 1 day and a maximum of 30 days. At a maximum follow-up of 2 years postoperatively, all patients reported satisfactory results with a return to their previous activities within 60 days after surgery. Conclusion: Medartis H-locked plate osteosynthesis is a reliable solution for the surgical treatment of olecranon fractures.

Evaluation of Intraoperative Iatrogenic Lesions and Postoperative Complications in 1140 Patients Treated for Carpal Tunnel Syndrome in the Orthopedic Surgery and Traumatology Department of the Moulins-Yzeure Hospital Center in France

Introduction: Carpal tunnel syndrome is a more common form of upper limb canal syndrome, resulting from compression of the median nerve in the carpal tunnel, but is particularly troublesome. Medical treatment is often unsuccessful, and surgical treatment usually involves transection of the annular ligament. The aim of this study was to assess iatrogenic intraoperative and postoperative complications, as well as patient outcomes following the use of conventional and endoscopic surgery in the surgical management of carpal tunnel syndrome. Hypothesis: Are nerve, vascular and tendon injuries of iatrogenic origin always present in the surgical management of carpal tunnel syndrome, even though this surgery is performed on an outpatient basis? Patients and methods: This retrospective series is composed of 1140 patients, 230 men and 910 women, mean age 58.6 ± 16.4 years, operated on between 2010 and 2020 for carpal tunnel syndrome by conventional surgery and under endoscopy. Medical records, operative reports and consultation letters were consulted. All patients were reviewed regularly at one month post-op until recovery. Results: No nerve, vascular or tendon damage was noted, and at a maximum follow-up of 2 years, 20 patients had recurred, i.e. a 2.51% failure rate. Scar disunion was observed in 0.9%, wound infection in 0.9% and scar fibrosis in 0.9%. 92.98% of patients underwent outpatient surgery, irrespective of the type of anesthesia or surgical technique used. Patients who stayed in hospital for a short time were suffering from carpal tunnel syndrome associated with compression of the ulnar nerve in Guyon’s canal, for which both the median and ulnar nerves were freed during the same operation, under general anaesthetic. All patients were able to return to their previous activity within 30 days of surgery. Conclusion: Intraoperative iatrogenic complications, notably nerve, vascular and tendon lesions, were not identified despite the large sample size. On the other hand

Setting: The epidemiology of tuberculosis (TB) among children in the Democratic Republic of Congo (DRC) is not well known. Objective: This study aimed to describe the trends in TB epidemiology among children in the DRC and to compare these trends in children and adults. Design: Data from the National TB program, the WHO Global TB Report, and a demographic survey of health in the DRC were retrospectively analyzed. The study period was from 1995 to 2014. The notification rate, absolute incidence and incidence rate of TB per 100,000 population were reported. Results: In 2014, 12,785 (12.6% of adult cases) TB cases were reported in children and 101,303 in adults. Among children, 3438 (26.89%) had PTB+;2828 (22.11%) had PTB–;and 6519 (50.98%) had extrapulmonary TB (EPTB). Children under 5 years had a lower reported prevalence of TB (184 cases). The incidence rate per 100,000 population was 10 in children and 181 in adults. The TB incidence decreased between 2010 (11.47) and 2014 (10.46). The proportion of children in overall cases of PTB+ was 4% to 5% in all districts. Conclusion: Caring for childhood TB remains a challenge in the DRC. Improved diagnostic procedures and effective training of providers who care for childhood TB are needed.

Virtual Reality Toolkit for the Assembly of Nanotube-based Nano-electro-mechanical Systems

Nano assembly and manipulation technologies are the basis for nano-electro-mechanical systems（NEMS）. Atomic force microscope（AFM） is widely used to manipulate nanotubes to assemble NEMS. Manipulating nanotubes with AFM is a challenging and difficult task. One of the major reasons is the deficiency of visual information during the manipulation process. To address these difficulties, this research aims to put forward novel virtual tools and assembly strategies to improve the efficiency, accuracy and ease of the assembly process of NEMS. This paper begins by the discussion of the principles and implementation of a virtual nano-assembly simulator, which serves as a benchmark to test the proposed NEMS assembly techniques and virtual tools. Then, a general framework of nanotube-based NEMS assembly is proposed. Several nano-assembly strategies and virtual tools, such as automated path planning for NEMS assembly, a four-step scheme of nanotube manipulation, virtual fixtures for assembly finalization and safe manipulation, are introduced. These virtual tools and methods are experimented for justification. An assembly task of moderate complexity was performed in our virtual nano-assembly simulator with and without the help of the proposed toolkit. Experimental results suggest that the proposed methods tend to greatly enhance the efficiency and accuracy of nanotube-based NEMS assembly. In general, the proposed virtual reality toolkit not only ensures the safety, but also enhances the accuracy and efficiency of the assembly of nanotube-based NEMS.

In this paper, we report an experimental demonstration of enabling technology exploiting resonant properties of plasmonic nanoparticles, for the realization of wavelength-sensitive ultra-minituarized(4 μm× 4 μm) optical metadevices. To this end, the example of a 1.3/1.6 μm wavelength demultiplexer is considered. Its technological implementation is based on the integration of gold cut-wire-based metalines on the top of a silicon-on-insulator waveguide. The plasmonic metalines modify locally the effective index of the Si waveguide and thus allow for the implementation of wavelength-dependent optical pathways. The 1.3/1.6 μm wavelength separation with extinction ratio between two demultiplexers' channels reaching up to 20 dB is experimentally demonstrated. The considered approach, which can be readily adapted to different types of material planar lightwave circuit platforms and nanoresonators, is suited for the implementation of a generic family of wavelength-sensitive guided-wave optical metadevices.

Unveiling the lamellar structure of the human cornea over its full thickness using polarization-resolved SHG microscopy

A key property of the human cornea is to maintain its curvature and consequently its refraction capability despite daily changes in intraocular *** is closely related to the multiscale structure of the corneal stroma,which consists of 1-3µm-thick stacked lamellae made of thin collagen ***,the distribution,size,and orientation of these lamellae along the depth of the cornea are poorly characterized up to *** this study,we use second harmonic generation(SHG)microscopy to visualize the collagen distribution over the full depth of 10 intact and unstained human corneas(500-600µm thick).We take advantage of the small coherence length in epi-detection to axially resolve the lamellae while maintaining the corneal physiological ***,as raw epi-detected SHG images are spatially homogenous because of the sub-wavelength size of stromal collagen fibrils,we use a polarimetric approach to measure the collagen orientation in every *** a careful validation of this approach,we show that the collagen lamellae(i)are mostly oriented along the inferior-superior axis in the anterior stroma and along the nasal-temporal axis in the posterior stroma,with a gradual shift in between and(ii)exhibit more disorder in the anterior *** results represent the first quantitative characterization of the lamellar structure of the human cornea continuously along its entire thickness with micrometric *** also shows the unique potential of P-SHG microscopy for imaging of collagen distribution in thick dense tissues.
