Pulmonary fibrosis and emphysema: Is the emphysema type associated with the pattern of fibrosis?

AIM: To investigate whether the predominant emphysema type is associated with the high resolution computed tomography(HRCT) pattern of fibrosis in combined pulmonary fibrosis and emphysema(CPFE).METHODS: Fifty-three smokers with upper lobe emphysema and lower lobe pulmonary fibrosis on- HRCT-were retrospectively evaluated. Patients were stratified into 3 groups according to the predominant type of emphysema: Centrilobular(CLE), paraseptal(PSE), CLE = PSE. Patients were also stratified into 3 other groups according to the predominant type of fibrosis on HRCT: Typical usual interstitial pneumonia(UIP), probable UIP and nonspecific interstitial pneumonia(NSIP). HRCTs were scored at 5 predetermined levels for the coarseness of fibrosis(Coarseness), extent of emphysema(emphysema), extent of interstitial lung disease(Tot Ext ILD), extent of reticular pattern not otherwise specified(Ret NOS), extent of ground glass opacity with traction bronchiectasis(extG GOBx), extent of pure ground glass opacity and extent of honeycombing. HRCT mean scores, pulmonary function tests, diffusion capacity(DLCO) and systolic pulmonary arterial pressure were compared among the groups.RESULTS: The predominant type of emphysema was strongly correlated with the predominant type of fibrosis. The centrilobular emphysema group exhibited a significantly higher extent of emphysema(P < 0.001) and a lower extent of interstitial lung disease(P < 0.002), reticular pattern not otherwise specified(P < 0.023), extent of ground glass opacity with traction bronchiectasis(P < 0.002), extent of honeycombing(P < 0.001) and coarseness of fibrosis(P < 0.001) than the paraseptal group. The NSIP group exhibited a significantly higher extent of emphysema(P < 0.05), total lung capacity(P < 0.01) and diffusion capacity(DLCO)(P < 0.05) than the typical UIP group. Differential colour-ring loss among Dalmatian Pelican(Pelecanus crispus) colonies and its consequence on survival estimates

The analysis of bird ringing data often comes with some potential sources of error and bias,as ring wear and/or loss could affect mark-recapture analyses and produce erroneous estimates of survival.Furthermore,ring wear and loss rates may differ between and within species based on the habitat they use or the species’ life-history traits and behaviour as well as the type of the ring.In this study we use resighting data from a long-term double marking experiment to directly estimate the rate of colour-ring loss among different Dalmatian Pelican colonies over time,evaluate any possible factors that could contribute to differential ring loss and assess how it may bias the results of mark-resighting analyses.Based on 14,849 resightings from 1275 individuals and using multi-state continuous-time hidden Markov models(HMMs) we showed that probability of ring loss was markedly different among colonies,ranging from 0.10 to 0.42 within the first year of marking,whereas the cumulative probability of losing a ring after ten years ranged 0.64 to 0.99.These rates are among the highest estimated when compared to previous studies in waterbirds.Our approach assessing the intra-specific variance in ring loss provided several factors potentially involved,such as the use of glue and the fledgling age accuracy and we could further hypothesise the effect of environmental factors.Finally,our results showed that ring loss can be a significant challenge for the assessment of the species’ population dynamics using mark-recapture methods as survival was consistently underestimated when not accounting for ring loss and varied significantly among different colonies.

Grafts in Peyronie’s surgery without the use ofprostheses:a systematic review and meta-analysis

Peyronie’s disease(PD)is characterized by abnormal penile curvature,and various surgical methods have been developed usingdifferent graft materials.However,there is currently no universal agreement on which type of graft is the best.The objective of thisreview was to evaluate the available literature and identify the most effective graft material for penile curvature correction in PD.A literature search was conducted using electronic databases,including PubMed,Scopus,and the Cochrane Library.The patients,intervention,comparison,and outcome(PICO)approach was used to define the eligibility of studies.Two authors independentlyselected studies,evaluated them,and extracted data.Random-effect models using the DerSimonian–Laird method were used.Moststudies were single-arm studies and had a high risk of bias.Buccal mucosa grafts(BMG)were found to result in the highest penilestraightening rates and were associated with the least de novo erectile dysfunction.TachoSil grafts demonstrated a high successrate in straightening despite a higher mean preoperative curvature,while Tutoplast grafts had a higher incidence of postoperativeerectile dysfunction.BMG had the highest percentage of postoperative penile straightening.Overall,the TachoSil graft showed thebest performance when preoperative curvature is taken into account.Based on the available literature,BMG appear to be the mosteffective for penile curvature correction in PD,but this is offset by the requirement for low preoperative curvature.The TachoSilgraft shows the best overall performance when preoperative curvature is considered.Comparative randomized clinical trials arestill needed to determine graft superiority.

Percutaneous antegrade management of large proximal ureteral stones using non-papillary puncture

Objective:To evaluate the feasibility and the safety of medial non-papillary percutaneous nephrolithotomy(npPCNL)for the management of large proximal ureteral stones.Methods:We evaluated prospectively collected data of 37 patients with large proximal ureteral stones more than 1.5 cm in diameter treated by prone npPCNL.Depending on stone size,in-toto stone removal or lithotripsy using the Lithoclast®Trilogy(EMS Medical,Nyon,Switzerland)was performed.Perioperative parameters including operative time(from start of puncture to the skin suturing),stone extraction time(from the first insertion of the nephroscope to the extraction of all stone fragments),and the stone-free rate were evaluated.Results:Twenty-one males and 16 females underwent npPCNL for the management of large upper ureteral calculi.The median age and stone size of treated patients were 58(interquartile range[IQR]:51-69)years and 19.3(IQR:18.0-22.0)mm,respectively.The median operative time and stone extraction time were 25(IQR:21-29)min and 8(IQR:7-10)min,respectively.One case(2.7%)of postoperative bleeding and two cases(5.4%)of prolonged fever were managed conservatively.The stone-free rate at a 1-month follow-up was 94.6%.Conclusion:The npPCNL provides a straight route to the ureteropelvic junction and proximal ureter.Approaching from a dilated portion of the ureter under low irrigation pressure with larger diameter instruments results in effective and safe stone extraction within a few minutes.

We need to talk about red blood cells.Eukaryotic cells are defined as cells containing distinct nuclei and other membranous organelles.However,red blood cells(RBCs)lack a nucleus and organelles—perhaps to limit the generation of reactive oxygen species(Zhang et al.,2011)—and are filled with hemoglobin(Hb).This oxygen carrier is limited to the cytosol by the cell membrane and cytoskeleton,which maintain cellular integrity and deformability as RBCs pass through narrow capillaries(Gratzer,1994)to perform their primary function of gas exchange.

Background Early-life respiratory infections and asthma are major health burdens during childhood.Markers predicting an increased risk for early-life respiratory diseases are sparse.Here,we identified the predictive value of ultrasound-monitored fetal lung growth for the risk of early-life respiratory infections and asthma.Methods Fetal lung size was serially assessed at standardized time points by transabdominal ultrasound in pregnant women participating in a pregnancy cohort.Correlations between fetal lung growth and respiratory infections in infancy or early-onset asthma at five years were examined.Machine-learning models relying on extreme gradient boosting regressor or classifier algorithms were developed to predict respiratory infection or asthma risk based on fetal lung growth.For model development and validation,study participants were randomly divided into a training and a testing group,respectively,by the employed algorithm.Results Enhanced fetal lung growth throughout pregnancy predicted a lower early-life respiratory infection risk.Male sex was associated with a higher risk for respiratory infections in infancy.Fetal lung growth could also predict the risk of asthma at five years of age.We designed three machine-learning models to predict the risk and number of infections in infancy as well as the risk of early-onset asthma.The models’R2 values were 0.92,0.90 and 0.93,respectively,underscoring a high accuracy and agreement between the actual and predicted values.Influential variables included known risk factors and novel predictors,such as ultrasound-monitored fetal lung growth.Conclusion Sonographic monitoring of fetal lung growth allows to predict the risk for early-life respiratory infections and asthma.

Fragipan Horizon Changes Using Annual Ryegrass and Other Admendments

A greenhouse experiment was conducted involving intact fragipan soil cores of 50 cm thickness after removing the topsoil horizons. The cores were maintained in moist condition throughout the experiment and received several treatments with various amendments for different periods ranging from 9 to 17 months. The amendments included annual ryegrass or Festulolium residues, powder limestone and various humate compounds alone or in combination with the grass residues. The results suggested a significant effect of ryegrass and Festulolium in reducing penetration resistance into the top 10 cm of the fragipan within 9 - 17 months, particularly when used in combination with certain humate materials such as Leonardite. Apparently, this is the result of the release of certain soluble organic compounds from the plant residues or the humate amendments that increase the solubility of Si and Al associated with the fragipan brittleness, thus decreasing the density of the compacted fragipan material.

Alcohol use disorder(AUD)represents a major public health issue which affects millions of people globally and consist a chronic relapsing condition associated with substantial morbidity and mortality.The gut microbiome plays a crucial role in maintaining overall health and has emerged as a significant contributor to the pathophysiology of various psychiatric disorders.Recent evidence suggests that the gut microbiome is intimately linked to the development and progression of AUD,with alcohol consumption directly impacting its composition and function.This review article aims to explore the intricate relationship between the gut microbiome and AUD,focusing on the implications for mental health outcomes and potential therapeutic strategies.We discuss the bidirectional communication between the gut microbiome and the brain,highlighting the role of microbiotaderived metabolites in neuroinflammation,neurotransmission,and mood regulation.Furthermore,we examine the influence of AUD-related factors,such as alcohol-induced gut dysbiosis and increased intestinal permeability,on mental health outcomes.Finally,we explore emerging therapeutic avenues targeting the gut microbiome in the management of AUD,including prebiotics,probiotics,and fecal microbiota transplantation.Understanding the complex interplay between the gut microbiome and AUD holds promise for developing novel interventions that could improve mental health outcomes in individuals with AUD.

Medical transportation and the contribution of air medical evacuation in isolated mountainous areas of Greece-Case study,Municipality of Agrafa

In every emergency medical situation,access to emergency medical services is an inalienable right of any citizen.The average acceptable time to provide first aid assistance is 60 minutes.In this paper,the possibility of using helicopters for air travel in mountainous areas was examined for the first time for all of Greece and especially for a mountainous and isolated area such as the Municipality of Agrafa.Initially,the time distance-virtual distance,from the hospitals was calculated at the national level of Greece,with an acceptable first contact time of 60 minutes(golden hour),i.e.,less than two hours from the nearest hospital to the incident and back to the hospital and the relevant conclusions are drawn.Then,the case study of Municipality of Agrafa,one of the most mountainous areas in Greece,was examined.The time distance of the settlements from the nearest primary health point,namely the Primary Health Centre(PHC)of West Fragkista,was calculated and the transportation by ground means was compared with the corresponding time by flying means,to investigate the contribution of air transportation or not,of the timely arrival of the patients in the appropriate health structure.Finally,possible helicopter landing locations were identified,using a geographic information system to facilitate the process of air medical transportation in the Municipality of Agrafa.

The coronary sinus(CS)is a tubular venous structure in the left atrioventricular groove that drains around 55% of the deoxygenated coronary arterial blood supply into the right atrium.[1,2]The CS,predominantly a small structure,can be significantly dilated leading to a condition known as giant CS.Giant CS is a clinically important but rare echocardiographic finding,which can be caused by a variety of factors.
