Pulmonary fibrosis and emphysema: Is the emphysema type associated with the pattern of fibrosis?

AIM: To investigate whether the predominant emphysema type is associated with the high resolution computed tomography(HRCT) pattern of fibrosis in combined pulmonary fibrosis and emphysema(CPFE).METHODS: Fifty-three smokers with upper lobe emphysema and lower lobe pulmonary fibrosis on- HRCT-were retrospectively evaluated. Patients were stratified into 3 groups according to the predominant type of emphysema: Centrilobular(CLE), paraseptal(PSE), CLE = PSE. Patients were also stratified into 3 other groups according to the predominant type of fibrosis on HRCT: Typical usual interstitial pneumonia(UIP), probable UIP and nonspecific interstitial pneumonia(NSIP). HRCTs were scored at 5 predetermined levels for the coarseness of fibrosis(Coarseness), extent of emphysema(emphysema), extent of interstitial lung disease(Tot Ext ILD), extent of reticular pattern not otherwise specified(Ret NOS), extent of ground glass opacity with traction bronchiectasis(extG GOBx), extent of pure ground glass opacity and extent of honeycombing. HRCT mean scores, pulmonary function tests, diffusion capacity(DLCO) and systolic pulmonary arterial pressure were compared among the ***: The predominant type of emphysema was strongly correlated with the predominant type of fibrosis. The centrilobular emphysema group exhibited a significantly higher extent of emphysema(P < 0.001) and a lower extent of interstitial lung disease(P < 0.002), reticular pattern not otherwise specified(P < 0.023), extent of ground glass opacity with traction bronchiectasis(P < 0.002), extent of honeycombing(P < 0.001) and coarseness of fibrosis(P < 0.001) than the paraseptal group. The NSIP group exhibited a significantly higher extent of emphysema(P < 0.05), total lung capacity(P < 0.01) and diffusion capacity(DLCO)(P < 0.05) than the typical UIP group. Impact of earthquake-induced bridge damage and time evolving traffic demand on the road network resilience

Damage from recent earthquakes has shown that substandard bridges are particularly vulnerable to strong ground motions being the weakest components of a road *** and foundation damages in bridges lead to a significant loss related to both repair process and a prolonged traffic disruption, which in turn results in large indirect loss in the affected area. Along these lines, the estimation of the overall loss related to earthquake-induced damage in highway bridges and overpasses must be based on a wider network analysis rather than on a single structural assessment. Key concept in such a comprehensive loss estimation procedure is the network resilience, expressing the extent of both direct and indirect loss, as well as the system's ability to quickly recover its preearthquake state. In this paper, a recently developed framework for assessing the loss and resilience associated with seismic impact on the structural and geotechnical components of a road network, as well as the relevant software developed are extended to further consider the implications of post-earthquake traffic demand variation. Moreover, a sensitivity analysis is conducted for a case study network to investigate the impact of traffic demand variation after a major earthquake event and the subsequent trip cancelations on the time-variant, cumulative cost at a network level. The results clearly highlight that not only the seismic resilience of a highway network should be assessed in a holistic manner coupling seismic hazard, structural and traffic analysis, but the latter shall include realistic scenarios with respect to the potential variation of origin-destination demand after the earthquake and during the recovery period.

Comparison of blood rheological models in patient specific cardiovascular system simulations

Newtonian, Quemada and Casson blood viscosity models are implemented in order to simulate the rheological behavior of blood under pulsating flow conditions in a patient specific iliac bifurcation. The influence of the applied blood constitutive equations is monitored via the wall shear stress （WSS） distribution, magnitude and oscillations, non-Newtonian importance factors, and viscosity values according to the shear rate. The distribution of WSS on the vascular wall follows a pattern which is independent of the theological model chosen. On the other hand, the WSS magnitude and oscillations are directly related to the blood constitutive equations applied and the shear rate. It is concluded that the Newtonian approximation is satisfactory only in high shear and flow rates. Moreover, the Newtonian model seems to overestimate the possibility for the formation of atherosclerotic lesions or aneurysms at sites of the vascular wall where the WSS are oscillating.

Malware-Propagative Mobile Ad Hoc Networks:Asymptotic Behavior Analysis

In this paper, the spreading of malicious software over ad hoc networks, where legitimate nodes are prone to propagate the infections they receive from either an attacker or their already infected neighbors, is analyzed. Considering the Susceptible-Infected-Susceptible （SIS） node infection paradigm we propose a probabilistic model, on the basis of the theory of closed queuing networks, that aims at describing the aggregated behavior of the system when attacked by malicious nodes. Because of its nature, the model is also able to deal more effectively with the stochastic behavior of attackers and the inherent probabilistic nature of the wireless environment. The proposed model is able to describe accurately the asymptotic behavior of malware-propagative large scale ad hoc networking environments. Using the Norton equivalent of the closed queuing network, we obtain analytical results for its steady state behavior, which in turn is used for identifying the critical parameters affecting the operation of the network. Finally, through modeling and simulation, some additional numerical results are obtained with respect to the behavior of the system when multiple attackers are present, and regarding the time-dependent evolution and impact of an attack.

In every emergency medical situation,access to emergency medical services is an inalienable right of any *** average acceptable time to provide first aid assistance is 60 *** this paper,the possibility of using helicopters for air travel in mountainous areas was examined for the first time for all of Greece and especially for a mountainous and isolated area such as the Municipality of ***,the time distance-virtual distance,from the hospitals was calculated at the national level of Greece,with an acceptable first contact time of 60 minutes(golden hour),i.e.,less than two hours from the nearest hospital to the incident and back to the hospital and the relevant conclusions are ***,the case study of Municipality of Agrafa,one of the most mountainous areas in Greece,was *** time distance of the settlements from the nearest primary health point,namely the Primary Health Centre(PHC)of West Fragkista,was calculated and the transportation by ground means was compared with the corresponding time by flying means,to investigate the contribution of air transportation or not,of the timely arrival of the patients in the appropriate health ***,possible helicopter landing locations were identified,using a geographic information system to facilitate the process of air medical transportation in the Municipality of Agrafa.

Medical Diagnosis Based on Multi-Attribute Group Decision-Making Using Extension Fuzzy Sets, Aggregation Operators and Basic Uncertainty Information Granule

Accurate medical diagnosis, which involves identifying diseases based on patient symptoms, is often hindered by uncertainties in data interpretation and retrieval. Advanced fuzzy set theories have emerged as effective tools to address these challenges. In this paper, new mathematical approaches for handling uncertainty in medical diagnosis are introduced using q-rung orthopair fuzzy sets (q-ROFS) and interval-valued q-rung orthopair fuzzy sets (IVq-ROFS). Three aggregation operators are proposed in our methodologies: the q-ROF weighted averaging (q-ROFWA), the q-ROF weighted geometric (q-ROFWG), and the q-ROF weighted neutrality averaging (qROFWNA), which enhance decision-making under uncertainty. These operators are paired with ranking methods such as the similarity measure, score function, and inverse score function to improve the accuracy of disease identification. Additionally, the impact of varying q-rung values is explored through a sensitivity analysis, extending the analysis beyond the typical maximum value of 3. The Basic Uncertain Information (BUI) method is employed to simulate expert opinions, and aggregation operators are used to combine these opinions in a group decisionmaking context. Our results provide a comprehensive comparison of methodologies, highlighting their strengths and limitations in diagnosing diseases based on uncertain patient data.

Background:Femtosecond-laser assisted clear cornea cataract surgery may hold promise in safer and more effective *** decided to perform a comparative study to standard manual incision phacoemulsification ***:This is a single-center,single-intervention,and prospective comparative data evaluation of 133 consecutive cases subjected to cataract ***-A(Phaco),manual capsulorhexis&ultrasound phacoemulsification(n=66);Group-B femtosecond-laser assisted capsulorhexis and lens fragmentation(n=67),employing the LenSx laser(Alcon Surgical,***,TX).All cases were evaluated for refraction,visual acuity,keratometry,tomography,pachymetry,endothelial cell counts,intraocular pressure,and type of intraocular lens(IOL)*** groups were matched for age,gender,pre-operative vision metrics,and cataract grade,and were followed up to 1 ***:In group-A post-operative uncorrected distance visual acuity(UDVA)was 20/20 or better in 61.5%and 20/25 or better in 78.5%of the *** femtosecond laser group-B had improved outcomes(p=0.075 and p=0.042,respectively):post-operative UDVA was 20/20 or better in 62.7%of the eyes and 20/25 or better in 85.1%.Linear regression scatterplots of achieved versus attempted spherical equivalent had excellent regression coefficients(r^(2)=0.983 in group-A and 0.979 in group-B).There were 75.2%cases in group-A and 80.6%in group-B(p=0.8732)within±0.50 D of targeted refractive *** trend of under-correction was noted in *** residual manifest cylinder in the toric subgroup-A was-0.50 D(95%Limit-of-Agreement(LoA)=-0.78 D),and in toric subgroup-B-0.45 D(LoA=-0.45 D).Conclusions:Mean spherical equivalent refraction and visual acuity are comparable with laser cataract surgery compared with manual capsulorhexis&ultrasound *** astigmatism correction may be among the benefits of femtosecond laser–assisted cataract *** corneal edema may be a first day transient

A preliminary investigation on the topology of Chinese climate networks

Complex networks have been studied across many fields of science in recent years. In this paper, we give a brief introduction of networks, then follow the original works by Tsonis et al （2004, 2006） starting with data of the surface temperature from 160 Chinese weather observations to investigate the topology of Chinese climate networks. Results show that the Chinese climate network exhibits a characteristic of regular, almost fully connected networks, which means that most nodes in this case have the same number of links, and so-called super nodes with a very large number of links do not exist there. In other words, though former results show that nodes in the extratropical region provide a property of scale-free networks, they still have other different local fine structures inside. We also detect the community of the Chinese climate network by using a Bayesian technique; the effective number of communities of the Chinese climate network is about four in this network. More importantly, this technique approaches results in divisions which have connections with physics and dynamics; the division into communities may highlight the aspects of the dynamics of climate variability.
