目的:研究和分析仰卧位(S-DAA)和侧卧位(L-DAA)髋关节置换(THA)的早期结果来评价临床效果差异性。方法:搜集2021年1月至2022年9月在南昌大学第二附属医院骨科进行髋关节置换手术患者的资料作为研究对象,共60例患者,将这些患者随机分入观察组(S-DAA组)(n=30)和对照组(L-DAA组(n=30)进行髋关节置换,搜集临床结果,并进行统计学分析。结果:(1)患者性别、年龄、随访情况、固定方式、髋关节活动程度及切口长度等均无显著差别,无统计学意义(P>0.05);(2)两种不同体位的DAA在手术时间、术中出血量、肢体长度差异方面存在显著差异(P0.05)、两组患者术后髋关节功能评分(Harris评分)无明显差异(P>0.05);(4)所有患者均无手术并发症。结论:S-DAA和L-DAA均取得了良好的临床效果,但L-DAA在手术过程中减少了对专用特殊器械和折叠手术台的依赖,操作更便利,手术时间更短,更符合后路关节外科医生的操作习惯;但S-DAA在控制双下肢长度差异方面更具优势。

Cholangiocarcinoma, Primary Sclerosing Cholangitis, or IgG4-Sclerosing Cholangitis: Similar Presentations with Different Managements

In our case, we present a case of an 80-year-old male who was referred to the gastroenterologist for evaluation of a suspicious mass. CT imaging at the time had shown intrahepatic and extrahepatic biliary dilations, and the patient was thought to have a pancreatic or a common bile duct mass. A mass biopsy showed no malignancy, and further evaluation was warranted. The patient was found to have elevated IgG4 levels and was diagnosed with IgG4-sclerosing cholangitis (IgG4-SC). IgG4 has been found to create a wide array of pathologies, including autoimmune pancreatitis, dacryoadenitis, and sialadenitis. These pathologies have been grouped under an IgG4-Related Disease (IgG4-RD) category. In some cases, this IgG4-RD can present as a subclass of primary sclerosing cholangitis due to immune depositions and swelling of the CBD. Due to the strictures caused by the sclerosing cholangitis, intrahepatic and extrahepatic dilations might be found on endoscopic ultrasound (EUS). It is imperative to differentiate this from a malignant mass as the early recognition and treatment of IgG4-SC can lead to complete resolution. In this case report, we present a case of a patient who was found to have IgG4-SC and responded well to steroid treatment.

Impacts of Sletr1-1 and Sletr1-2 mutations on the hybrid seed quality of tomatoes

Shelf life is an important breeding trait in tomato, especially for the tomato production in subtropical and tropical regions. Previously we have isolated and characterized ethylene receptor mutants, Sletr1-1 and Sletr1-2 from mutant population based on Micro-Tom cultivar. Sletr1-1 showed insensitivity to ethylene while Sletr1-2 showed reduced sensitivity to ethylene. We also have demonstrated that the traits are useful for extending fruit shelf life of the hybrid tomato cultivars. For commercializing the hybrid cultivars, the seed quality is another important trait. In this study, we evaluated the effects of the Sletr1-1 and Sletr1-2 mutations on the seed quality characteristics of F_1 hybrid lines generated by crossing Sletr1-1 and Sletr1-2 with three commercial tomato cultivars, Intan, Mutiara and Ratna. Sletr1-1 mutation conferred insensitivity to ethylene in the F_1 hybrid seedlings, resulting in negative effects including reduced germination rate, vigor index and emergence speed index. Interestingly Sletr1-2 mutation had almost no effect on the seed quality characteristics of the F_1 hybrid lines, suggesting that Sletr1-2 was suitable for producing high quality of hybrid seeds.

Improving the phenotypic expression of rice genotypes:Rethinking “intensification” for production systems and selection practices for rice breeding

Intensification in rice crop production is generally understood as requiring increased use of material inputs: water, inorganic fertilizers, and agrochemicals. However, this is not the only kind of intensification available. More productive crop phenotypes, with traits such as more resistance to biotic and abiotic stresses and shorter crop cycles, are possible through modifications in the management of rice plants, soil, water, and nutrients, reducing rather than increasing material inputs. Greater factor productivity can be achieved through the application of new knowledge and more skill, and(initially) more labor, as seen from the System of Rice Intensification(SRI), whose practices are used in various combinations by as many as 10 million farmers on about 4 million hectares in over 50 countries. The highest yields achieved with these management methods have come from hybrids and improved rice varieties, confirming the importance of making genetic improvements. However,unimproved varieties are also responsive to these changes, which induce better growth and functioning of rice root systems and more abundance, diversity, and activity of beneficial soil organisms. Some of these organisms as symbiotic endophytes can affect and enhance the expression of rice plants' genetic potential as well as their phenotypic resilience to multiple stresses, including those of climate change. SRI experience and data suggest that decades of plant breeding have been selecting for the best crop genetic endowments under suboptimal growing conditions, with crowding of plants that impedes their photosynthesis and growth, flooding of rice paddies that causes roots to degenerate and forgoes benefits derived from aerobic soil organisms, and overuse of agrochemicals that adversely affect these organisms as well as soil and human health. The liver is an organ that withstands a lot of insults due to various things such as infection, toxins and even our own immune system. There are injuries to the liver that are relatively common in medicine such as viral hepatitis caused by different strains of Hepatitis A-E, autoimmune hepatitis, and injury by drugs such as acetaminophen. However, syphilis causing hepatitis is not seen often and there are certain features that distinguish syphilitic hepatitis that should be reported more to distinguish its characteristic features.

Bedside score predicting retained common bile duct stone in acute biliary pancreatitis

BACKGROUND Retained common bile duct(CBD) stone after an acute episode of biliary pancreatitis is of paramount importance since stone extraction is *** To generate a simple non-invasive score to predict the presence of CBD stone in patients with biliary *** We performed a retrospective study including patients with a diagnosis of biliary pancreatitis. One hundred and fifty-four patients were included. Thirty-three patients(21.5%) were diagnosed with CBD stone by endoscopic ultrasound(US).RESULTS In univariate analysis, age(OR: 1.048, P = 0.0004), aspartate transaminase(OR:1.002, P = 0.0015), alkaline phosphatase(OR: 1.005, P = 0.0005), gamma-glutamyl transferase(OR: 1.003, P = 0.0002) and CBD width by US(OR: 1.187, P = 0.0445)were associated with CBD stone. In multivariate analysis, three parameters were identified to predict CBD stone;age(OR: 1.062, P = 0.0005), gamma-glutamyl transferase level(OR: 1.003, P = 0.0003) and dilated CBD(OR: 3.685, P = 0.027),with area under the curve of 0.8433. We developed a diagnostic score that included the three significant parameters on multivariate analysis, with assignment of weights for each variable according to the co-efficient estimate. A score that ranges from 51.28 to 73.7 has a very high specificity(90%-100%) for CBD stones, while a low score that ranges from 9.16 to 41.04 has a high sensitivity(82%-100%). By performing internal validation, the negative predictive value of the low score group was 93%.CONCLUSION We recommend incorporating this score as an aid for stratifying patients with acute biliary pancreatitis into low or high probability for the presence of CBD stone.

Right Post-Traumatic Diaphragmatic Hernia with Liver Dislocation

Diaphragmatic hernia is a rare consequence of thoraco-abdominal trauma. It may be associated with high morbidity and mortality, particularly if surgical intervention is delayed. We report a case of a right diaphragmatic hernia in a 75-year-old woman. The patient was referred to our hospital with mild dyspnea. Chest radiograph showed an overtly elevated right hemi-diaphragm. Thoracic and abdominal computed tomographic scan was requested and showed a defect of the right diaphragmatic muscle wall with intrathoracic ascension of the liver. During the postoperative course, the patient was still on mechanical ventilation, hemodynamically unstable. She developped urinary peritonitis and an extensive bowel ischemia worsening. We report this case to show that the prognosis is related to associated injuries and possible complications. The possibility of a diaphragmatic rupture should be kept in mind and surgery is mandatory in order to avoid complications.

New insights into diet breadth of polyphagous and oligophagous aphids on two Arabidopsis ecotypes

We investigated whether plant ecotype might affect aphid performance and behavioi. The probing behaviors of the polyphagous aphid Myzus persicae and the oligophagous aphid Brevicoryne brassicae on two ecotypes of Arabidopsis thaliana, WS and Col-0 were recorded using the direct current electrical penetration graph method (DC-EPG). Myzus persicae displayed a significant preference for the WS ecotype but was not greatly disturbed on Col-0, while B. brassicae discriminated between the two A. thaliana ecotypes, feeding less on WS than on Col-0. A Principal Component Analysis of aphid probing behavior data recorded on Col-0 and WS ecotypes showed that the one of M. persicae was positively correlated with the phloem ingestion phases while the one of B. brassicae was more related to nonfeeding phase. The survival of the aphid species was followed during early larval stages on the two ecotypes and a significantly higher mortality was observed of B. brassicae neonates compared to M. persicae. Both reared on WS. Moreover, transcriptomic analysis of non infested plant leaves from both ecotypes was monitored and underlined constitutive differences between Col-0 and WS gene expression that might explain the different aphid behaviors. Among a unigene set comprising 39 042 sequences for A. thaliana, 6% were differently expressed affecting, for example, the secondary metabolites and cell wall pathways: two third upregulated in WS and one third upregulated in Col-0. Thus, the "ecotype” variable should be taken into account when setting up a plant-insect experimental research.
