Determinants and Risks Factors of Renal Failure in Patients with Heart Failure in Cardiology Department of CHU Sylvanus Olympio of Lomé(Togo)

Introduction: The morbidity and mortality of patients with heart failure are known to increase rapidly in the presence of renal insufficiency, which is usually the cause but may be a consequence. To organize better prevention of renal failure, we undertook this study to identify the determinants of renal failure in the population of patients with heart failure. Methodology: This was a retrospective descriptive and analytical study of heart failure (HF) cases hospitalized from January 1st to December 31st, 2016, over a period for twelve (12) months at CHU Sylvanus Olympio. Patients who performed cardiac Doppler ultrasound were included in our study. Renal failure was defined as eGFR (estimated glomerular filtration rate) less than 60 ml/min/1.73m2. Multivariate logistic regression was performed to investigate associated factors. The dependent variable was DFG status: coded 1 if the GFR is less than 60 ml/min and 0 if not. Results: A total of 216 patients were included. The majority were female (54.17%). The median age of patients was 53 years [IQI = 32 - 61 years] with extremes of 15 and 96 years. 16.49% of patients had a GFR of less than 60 ml/min. In multivariate analysis the average standard of living (OR = 2.40, p = 0.0456), diabetes (OR = 2.67, p = 0.0300), hypertension (OR = 5.66, p = 0.0399), alcoholism (OR = 4.00, p = 0.0063) were the main factors in the development of an RF/HF. Conclusion: The average standard of living, diabetes, hypertension, and chronic alcoholism are the determinants of renal failure in HF.

Laryngeal Tuberculosis and Laryngeal Cancer: Two Similar Diagnoses in an Elderly Person in Tuberculosis-Endemic Area

Laryngeal tuberculosis is a rare form of extrapulmonary tuberculosis, often complicating pulmonary tuberculosis that may be unrecognized. Its clinical presentation is nonspecific, often pointing to cancer. We report the case of a 77-year-old woman, with no reported pathological history. She also has no alcohol or tobacco intoxication, who presented with chronic dysphonia evolving for 2 months, associated with an altered general condition. The examination of the larynx by direct laryngoscopy and anatomical pathology study of the biopsies led to the diagnosis of laryngeal tuberculosis. A search for secondary sites revealed a concomitant pulmonary infection. The evolution was favorable under standard anti-tuberculosis treatment, with complete voice recovery and improved performance status. Laryngeal tuberculosis should be suspected in patients living in endemic areas and suffering from chronic dysphonia, even if they are not alcoholics or smokers.

Prevalence and Factors Associated with Hepatitis B Virus Surface Antigen and Human Immunodeficiency Virus Antibodies in Chronic Hemodialysis Patients Followed-Up in Lomé, Togo

Introduction: In haemodialysis patients, the risk of acquiring blood-borne viral infections is an important cause of the dialysis’s process. Indeed, viral infections are quite common in chronic haemodialysis patients including those due to viral hepatitis B and HIV. Objective: Determine the prevalence of HIV infection and hepatitis B infection among chronic hemodialysis patients of Togo. Methods: A cross-sectional study was conducted in chronic hemodialysis patients of Togo from January 1st to December 31st, 2016. Sociodemographic and clinical data were collected. Antigen HBs was screened by a rapid immunochromatographic test (SD BIOLINE? HBsAg), and HIV antibodies were detected using a combination of 2 rapid diagnostic test, SD BIOLINE? HIV-1/2 (first step) and TRIDOT test (second step). Results: During the study period, 95 subjects were included. Patient’s age was ranking from 13 to 80 years with a mean of 46.6 years. The sex-ratio (M/F) was 1.8 and a duration average of dialysis was 51.7 months. The aetiologies of the chronic endstage kidney failure were related respectively to a vascular renal disease for 42.1% of the patients, glomerular nephropaty for 34.74%, interstitial nephropaty for 9.47% and hereditary for 3.16%. For 10.53% of the patients, the initial renal disease remained indefinite. Prevalence of antigen HBs and HIV antibodies were respectively 10.5% and 7.4%. The frequency of co-infection HBV and HIV was 1.1%. Conclusion: The rates of HBV infection and HIV infection remain high in chronic hemodialysis in Togo, somewhat which is linked to an endemic aspect of these two viruses in the country. The type of initial renal disease (glomerular) and the vascular way (the central catheters) established the risk factors for the HIV infection while for the infection of HVB no factor was incriminated.

An Atypical Ovarian and Peritoneal Pelvic Tuberculosis Complicated by Toxidermia

Background: Ovarian tuberculosis is a rare form of tuberculosis. Its clinical presentation mimics an ovarian tumor, leading to misdiagnosis. Proper treatment with anti-tuberculosis drugs can lead to a cure for the disease but can sometimes cause adverse effects that compromise therapeutic management. Observation: We report a 71-year-old female Senegalese patient who presented with a chronic abdominopelvic mass with an elevated Cancer Antigen 125 (CA125) level, raising the suspicion of an ovarian tumor. She underwent an exploratory laparotomy and the anatomopathological study confirmed the diagnosis of ovarian and peritoneal tuberculosis. After starting treatment with anti-tuberculosis drugs, she developed toxidermia in the form of generalized urticaria. Discontinuation of the four-drug therapy and separate reintroduction of anti-tuberculosis drugs led to the incrimination of rifampicin. Progress was then favorable on Isoniazid (H) pyrazinamide (Z) and Ethambutol (E). Conclusion: Ovarian tuberculosis should be suspected in the presence of an abdominopelvic mass in a woman living in an endemic area. The occurrence of adverse reactions to anti-tuberculosis treatment is not uncommon, hence the importance of regular monitoring.

Impact of the Intermittency of Photovoltaic Power Plants on the Frequency Management: Case of the Senegalese Electricity Grid

This paper investigates the impact of the intermittency of the photovoltaic (PV) power plants on the electric grid frequency management and the consequences that can occur. To meet these purposes, three years data of the Senegalese grid frequency are used, combined with three PV power plants data since their starting-up. Using these data, we analyzed two days of heavily disturbed operation of the Senegalese grid in order to emphasize the real impact of the presence of PV power plants on the electrical grid. Our study has highlighted that how the intermittency of photovoltaic power plants affects frequency management. It obviously appears that the PV production curves follow the irradiation variations. Instantaneous variations of PV production make it difficult to manage the grid frequency leading to more frequent load shedding as we have shown with the data after the commissioning of the PV power plants. We also estimate the power then energy losses during the load shedding induced by the PV integration on the power grid by comparing a period corresponding to one month before (February 2017) and one month after (February 2018) the integration of variable PV production.

Air quality has been a major health issue in urban areas in recent decades. Human activities release a large number of pollutants into the atmosphere which has a direct impact on plant health and leads to ecosystem degradation. The objective of this study is to contribute to a better evaluation of the impact of the air quality of the city of Togo on biological resources. The determination of pollutants was done on samples of plant species with a strong link with the source of pollution. The determination of Sulfur dioxide (SO2) was done by the ripper method. The determination of carbon and estimation of CO2 and CO by the colorimetric method. The determination of nitrogen was done by the Kjeldhal method. The increase in the urban population and the high concentration of many anthropogenic activities in certain regions of the world lead to atmospheric disturbances. The excess concentration of several chemical elements leads to air pollution. In order to identify the types of air pollutants, a study was carried out in the city of Lomé, the capital of Togo. The objective of this study is to achieve a better knowledge of the types of air pollutants in the city of Lomé. The identification of the various atmospheric pollutants linked to the sources of pollution was made using micro-sensors. The standardized method was used to access target gas concentration levels. The average concentrations of NO2 gaseous pollutants at landfills are on average 0.333 μg/m3;0.403 mg/m3 at the industrial level, at the transport level they are on average 0.434 mg/m3 for the morning and 0.457 mg/m3 for the evening. Concentrations of carbon dioxide (CO2) are higher in industrial areas than in others sources of pollution
