Treatment with Intensity-Modulated Radiation Therapy (IMRT) and Chemotherapy in Advanced Inoperable Non-Small Cell Lung Cancer (NSCLC): Toxicity, Survival and Patterns of Failure in Relation to Treatment with High and Low Radiation Dose

Purpose: To investigate the toxicity, survival and patterns of failure in patients with advanced lung cancer treated with intensity modulated radiation therapy (IMRT) and chemotherapy. Methods and Materials: Retrospective chart review of 68 total patients: 46 academic and 22 community center. Endpoints: Grade ≥ 3 pneumonitis, Grade ≥ 2 esophagitis, local, regional and distant failure, progression-free survival (PFS) and overall survival (OS). Results: For the academic center patients, median follow-up was 19.2 months. Esophagitis: 0% Grade 3, 35% Grade 2, no significant difference between dose bins: <70 Gy vs. 70 Gy, 25% vs. 45% (p = 0.22), <66 Gy vs. 66 - 70 Gy, 28% vs. 39% (p = 0.53). Lung dose metrics and PTV size were not associated with Grade ≥ 3 pneumonitis. Esophageal V35, V50, and mean dose but not PTV size was associated with Grade 2 esophagitis. 1 year local, regional and distant failure = 6.5%, 6.5%, and 30.4%. No endpoint differences were seen between dose bins, though patients with smaller PTVs treated with 70 Gy did demonstrate improved OS (ns) when compared to those treated with <70 Gy. Community Center: Median follow-up 6.2 months with 15% Grade 2 esophagitis, no Grade 3 esophagitis. Two patients (9%) experienced Grade ≥ 3 pneumonitis. Conclusions: IMRT chemoradiation was well tolerated in a population with advanced NSCLC both in the academic and community settings. Severe pneumonitis rates were low and comparable to other series using IMRT and chemotherapy. Esophagitis was mild and associated with V35, V50 and mean dose. No significant benefit was seen for higher doses regarding survival, local, regional or distant control despite that higher dose bins had smaller tumors. Though not statistically significant, we did find a trend toward worse OS for <70 Gy when the PTV was less than the median PTV.

BACk.ROUND Internal hernia(IH)is a rare culprit of small bowel obstruction(SBO)with an incidence ofk.ow,India between September 2010 and August 2023 were *** Out of 586 patients,7(1.2%)were diagnosed with *** these,4 had congenital IH and 3 had acquired *** male-to-female ratio was 4:*** median age at presentation was 32 ***-enhanced computed tomography(CECT)was the most reliable investigation for preoperative identification,demonstrating mesenteric whirling and clumped-up bowel *** paraduodenal hernia and transmesenteric hernia occurred with an equal frequency(approximately 43%each).Intraoperatively,one patient was found to have bowel ischemia and one had associated malrotation of *** follow-up,no recurrences were *** IH,being a rare cause,must be considered as a differential diagnosis for SBO,especially in young patients in their 30s or with unexplained abdominal pain or discomfort post-surgery.A rapid imaging evaluation,preferably with CECT,is necessary to aid in an early diagnosis and prompt intervention,thereby reducing financial burden related to unnecessary investigations and preventing the morbidity and mortality associated with closed-loop obstruction and strangulation of the bowel.

In the present work. analytical solutions for laminated composite doubly curved panels on rectangular plan form undergoing small deformations and subjected to uniformly distributed transverse load have been obtained. The problem is formulated using first order shear deformation theory. The spatial descretization of the linear differential equations is carried out using fast converging finite double Chebyshev series. The effect of panel thick.ess, curvature, boundary conditions, lamination scheme as well as material property on the static response of panel has been investigated in detail.

Biotic and abiotic stresses impose a serious limitation on crop productivity worldwide. Prior or simultaneous exposure to one type of stress often affects the plant response to other stresses, indicating extensive overlap and cross-talk.between stress-response signaling pathways. Systems biology approaches that integrate large genomic and prot-eomic data sets have facilitated identification of candidate genes that govern this stress-regulatory crosstalk. Recently, we constructed a yeast two-hybrid map around three rice proteins that control the response to biotic and abiotic stresses, namely the immune receptor XA21, which confers resistance to the Gram-negative bacterium, Xanthomonas oryzae pv. oryzae; NH1, the rice ortholog of NPR1, a k.y regulator of systemic acquired resistance; and the ethylene-responsive transcription factor, SUBIA, which confers tolerance to submergence stress. These studies coupled with transcriptional profiling and co-expression analyses identified a suite of proteins that are positioned at the interface of biotic and abiotic stress responses, including mitogen-activated protein k.nase 5 （OsMPk.）, wall-associated k.nase 25 （WAk.5）, sucrose non-fermenting-l-related protein k.nase-1 （SnRk.）, SUBIA binding protein 23 （SAB23）, and several WRk. family tran- scription factors. Emerging evidence suggests that these genes orchestrate crosstalk.between biotic and abiotic stresses through a variety of mechanisms, including regulation of cellular energy homeostasis and modification of synergistic and/or antagonistic interactions between the stress hormones salicylic acid, ethylene, jasmonic acid, and abscisic acid.

Complex variable approach in studying modified polarization saturation model in two-dimensional semipermeable piezoelectric media

A modified polarization saturation model is proposed and addressed math- ematically using a complex variable approach in two-dimensional （2D） semipermeable piezoelectric media. In this model, an existing polarization saturation （PS） model in 2D piezoelectric media is modified by considering a linearly varying saturated normal electric displacement load in place of a constant normal electric displacement load, applied on a saturated electric zone. A centre crack.d infinite 2D piezoelectric domain subject to an arbitrary poling direction and in-plane electromechanical loadings is considered for the analytical and numerical studies. Here, the problem is mathematically modeled as a non-homogeneous Riemann-Hilbert problem in terms of unk.own complex potential functions representing electric displacement and stress components. Having solved the Hilbert problem, the solutions to the saturated zone length, the crack.opening displace- ment （COD）, the crack.opening potential （COP）, and the local stress intensity factors （SIFs） are obtained in explicit forms. A numerical study is also presented for the proposed modified model, showing the effects of the saturation condition on the applied electrical loading, the saturation zone length, and the COP. The results of fracture parameters obtained from the proposed model are compared with the existing PS model subject to electrical loading, crack.face conditions, and polarization angles.

The solution to evolution equations has developed an independent theory within nonlinear analysis dealing with the existence and approximation of such solution （ fixed point） of pseudocontractive operators and its variants. The object is to introduce a perturbed iteration method for proving the convergence of sequence of Lipschitzian pseudocontractive mapping using approximate fixed point technique. This iteration can be ued for nonlinear operators which are more general than Lipschitzian pseudocontractive operator and Bruck.iteration fails for proving their convergence. Our results generalize the results of Chidume and Zegeye.

Extent of mismatch between the period of circadian clock. and light/dark.cycles determines time-to-emergence in fruit flies

Circadian clock. time developmental stages of fruit flies Drosophila melanogaster, while light/dark.（LD） cycles delimit emergence of adults, conceding only during the ＂allowed gate.＂ Previous studies have revealed that time-to-emergence can be altered by mutations in the core clock.gene period （per）, or by altering the length of LD cycles. Since this evidence came from studies on genetically manipulated flies, or on flies maintained under LD cycles with limited range of periods, inferences that can be drawn are limited. Moreover, the extent of shortening or lengthening of time-to-emergence re- mains yet unk.own. In order to pursue this further, we assayed time-to-emergence of D. rnelanogaster under 12 different LD cycles as well as in constant light （LL） and constant dark.conditions （DD）. Time-to-emergence in flies occurred earlier under LL than in LD cycles and DD. Among the LD cycles, time-to-emergence occurred earlier under T4-T8, followed by T36-T48, and then T12-T32, suggesting that egg-to-emergence duration in flies becomes shorter when the length of LD cycles deviates from 24 h, bearing a strong positive and a marginally negative correlation with day length, for values shorter and longer than 24 h, respectively. These results suggest that the extent of mismatch between the period of circadian clock. and environmental cycles determines the time-to-emergence in Drosophila.
