Is it time to replace propranolol with carvedilol for portal hypertension?

Beta-adrenergic receptor antagonists(β-blockers) have been well established for use in portal hypertension for more than three decades. Different Non-selective β-blockers like propranolol, nadolol, timolol, atenolol, metoprolol and carvedilol have been in clinical practice in patients with cirrhosis. Carvedilol has proven 2-4 times more potent than propranolol as a beta-receptor blocker in trials conducted testing its efficacy for heart failure. Whether the same effect extends to its potency in the reduction of portal venous pressures is a topic of on-going debate. The aim of this review is to compare the hemodynamic and clinical effects of carvedilol with propranolol, and attempt assess whether carvedilol can be used instead of propranolol in patients with cirrhosis. Carvedilol is a promising agent among the beta blockers of recent time that has shown significant effects in portal hypertension hemodynamics. It has also demonstrated an effective profile in its clinical application specifically for the prevention of variceal bleeding. Carvedilol has more potent desired physiological effects when compared to Propranolol. However, it is uncertain at the present juncture whether the improvement in hemodynamics also translates into a decreased rate of disease progression and complications when compared to propranolol. Currently Carvedilol shows promise as a therapy for portal hypertension but more clinical trials need to be carried out before we can consider it as a superior option and a replacement for propranolol.

In recent years,natural orifice specimen extraction surgery(NOSES)in the treatment of colorectal cancer has attracted widespread *** potential benefits of NOSES including reduction in postoperative pain and wound complications,less use of postoperative analgesic,faster recovery of bowel function,shorter length of hospital stay,better cosmetic and psychological effect have been described in colorectal *** significant decrease in surgical trauma of NOSES have been observed,the potential pitfalls of this technique have been ***,several issues including bacteriological concerns,oncological outcomes and patient selection are raised with this new ***,it is urgent and necessary to reach a consensus as an industry guideline to standardize the implementation of NOSES in colorectal *** three rounds of discussion by all members of the International Alliance of NOSES,the consensus is finally completed,which is also of great significance to the long-term progress of NOSES worldwide.

Purpose The Large Hadron Collider(LHC)at European Organization for Nuclear Research is planned to be upgraded to the high luminosity *** the luminosity makes muon triggering reliable and ofﬂine reconstruction very *** enhance the redundancy of the Compact Muon Solenoid(CMS)Muon system and resolve the ambiguity of track reconstruction in the forward region,an improved Resistive Plate Chamber(iRPC)with excellent time resolution will be installed in the Phase-2 CMS *** iRPC will be equipped with Front-End Electronics(FEE),which can perform high-precision time measurements of signals from both ends of the *** Back-End Electronics(BEE)need to be researched and developed to provide sophisticated functionalities such as interacting with FEE with shared links for fast,slow control(SC)and data,in addition to trigger primitives(TPs)generation and data acquisition(DAQ).Method The BEE prototype uses a homemade hardware board compatible with the MTCA standard,the back-end board(BEB).BEE interacts with FEE via a bidirectional 4.8 Gbps optical paired-link that integrates clock,data,and control *** clock and fast/slow control commands are distributed from BEB to the FEE via the *** uplink is used for BEB to receive the time information of the iRPC’sﬁred strips and the responses to the fast/slow control *** have a pipelined detector data for clusterﬁnding operation,recover(DeMux)the time relationship of which is changed due to the transmission protocol for the continuous incoming MUXed data from *** at each bunch crossing(BX),clusteringﬁred strips that satisfy time and spatial constraints to generate *** incoming raw MUXed detector data and TPs in a time window and latency based on the trigger signal are read out to the DAQ *** Ethernet(GbE)of SiTCP and commercial 10-GbE are used as link standards for SC and DAQ,respectively,for the BEB to interact with the *** The joint test results of
