Human intravenous immune globulin(IVIg),a purified IgG fraction composed of~60%IgG1 and obtained from the pooled plasma of thousands of donors,is clinically used for a wide range of *** biological actions of IVIg are incompletely understood and have been attributed both to the polyclonal antibodies therein and also to their IgG(IgG)Fc ***,we demonstrated that multiple therapeutic human IgG1 antibodies suppress angiogenesis in a target-independent manner via FcγRI,a high-affinity receptor for *** we show that IVIg possesses similar anti-angiogenic activity and inhibited blood vessel growth in five different mouse models of prevalent human diseases,namely,neovascular age-related macular degeneration,corneal neovascularization,colorectal cancer,fibrosarcoma and peripheral arterial ischemic *** was mediated by the Fc region of IVIg,required FcγRI and had similar potency in transgenic mice expressing human Fcγ***,IVIg therapy administered to humans for the treatment of inflammatory or autoimmune diseases reduced kidney and muscle blood vessel *** data place IVIg,an agent approved by the US Food and Drug Administration,as a novel angioinhibitory drug in doses that are currently administered in the clinical *** addition,they raise the possibility of an unintended effect of IVIg on blood vessels.

背景在哈佛南岸计划工作框架下,旨在探索一条系统的创伤后应激障碍(PTSD)临床药物循证治疗路径。方法更新1999年版PTSD药物治疗指南;重点关注了治疗PTSD的药物疗效证据以及合并症对治疗决策的影响。结果 SSRIs、SNRIs对PTSD的疗效不如预期,建议初始治疗选用哌唑嗪和曲唑酮来治疗PTSD的睡眠障碍,可用抗抑郁剂治疗PTSD的核心症状,可加用抗精神病药治疗伴随的精神病症状,可序贯应用两到三种抗抑郁剂应对难治性个案,若疗效不满意或仍有症状残留,可扩大治疗,建议选用非典型抗精神病药、可乐定、托吡酯或拉莫三嗪。结论这一探索性治疗策略可能利于快速缓解PTSD症状,减少副作用和增加依从性。

Diffusion-weighted magnetic resonance imaging to predict response of hepatocellular carcinoma to chemoembolization

AIM: To investigate whether intra-procedural diffusion- weighted magnetic resonance imaging can predict response of hepatocellular carcinoma (HCC) during trans- catheter arterial chemoembolization (TACE). METHODS: Sixteen patients (15 male), aged 59 ±11 years (range: 42-81 years) underwent a total of 21 separate treatments for unresectable HCC in a hybrid magnetic resonance/interventional radiology suite. ana- tomical imaging and diffusion-weighted imaging (b = 0, 500 s/mm2) were performed on a 1.5-T unit. Tumor enhancement and apparent diffusion coefficient (ADC, mm2/s) values were assessed immediately before and at 1 and 3 mo after TACE. We calculated the percent change (PC) in ADC values at all time points. We compared follow-up ADC values to baseline values using a paired t test (α = 0.05). RESUlTS: The intra-procedural sensitivity, specificity, and positive and negative predictive values (%) for detecting a complete or partial 1-mo tumor response using ADC PC thresholds of ±5%, ±10%, and ±15% were 77, 67, 91, and 40; 54, 67, 88, and 25; and 46, 100, 100, and 30, respectively. There was no clear predictive value for the 3-mo follow-up. Compared to baseline, the immediate post-procedure and 1-mo mean ADC values both increased; the latter obtaining statistical significance (1.48 ± 0.29 mm2/s vs 1.65 ± 0.35 × 10-3 mm2/s, P lUSION: Intra-procedural ADC changes of > 15% predicted 1-mo anatomical HCC response with the greatest accuracy, and can provide valuable feedback at the time of TACE.

Background:Hypertrophic scars and keloids,common complications following median sternotomy for cardiac surgery,signiﬁcantly impact patient quality of life due to their aesthetic and symptomatic *** advancements in laser therapy have made it a prominent option for managing these complex scars,yet a com-prehensive understanding of its efﬁcacy is *** aim of this scoping review is to explore the effects of laser therapy in managing hypertrophic scars and keloids after median ***:This scoping review ana-lyzed studies up to February 2024 from databases including PubMed,EMBASE,CINAHl,Scopus,Web of Science,and the Cochrane *** included any study that assessed laser therapy’s effects on hypertrophic scars and keloids following median *** were selected based on predeﬁned inclusion criteria with-out publication year,design,or origin ***:Six studies met the inclusion criteria,involving a total ofﬁve RCTs and one *** studies primarily tested 585 and 595-nm pulsed dye laser(PDl)treatments,focusing on scar appearance,patient symptoms,and treatment *** studies reported signiﬁcant improvements in scar height reduction and patient symptom relief after treatment,with mixed results for scar erythema and *** events were generally mild and ***:laser therapy offers a beneﬁcial approach for improving the appearance and symptoms of hypertrophic scars and keloids post-median ***,further research is necessary to optimize treatment parameters and explore the long-term psychosocial impacts of this *** review highlights the need for more comprehensive studies to establish standardized treatment protocols and evaluate their effectiveness.

Vitamin D and its association with cardiovascular disease (CVD) are currently a topic of investigation. The objective of this study was to explore the association between vitamin D status (serum 25-hydroxyvitamin D (25(OH)D)) and a cardiometabolic risk score (CMRS) derived from markers of cardiac autonomic nervous system activity, vascular dynamics, and body composition, using an innovative non-invasive technology. We found that individuals who were vitamin D sufficient ((25(OH)D) ≥ 30 ng/ml, n = 51), compared to those who were vitamin D insufficient (l, n = 44), had significantly higher heart rate variability (as measured by time and frequency domain variables) and lower photoplethysmography analysis markers and CMRS. These outcomes show that vitamin D insufficient subjects had reduced cardio protective parasympathetic nervous system activity, increased endothelial dysfunction, and hence were at greater cardiometabolic risk, implying vitamin D may play a meaningful role in CVD.

The zwitterionic Streptococcus pneumoniae serotype 1 polysaccharide（Sp1） is an important anchor point for our immune system to act against streptococcal *** can recognize Sp1 saccharides,and it has been postulated that Sp1 can elicit a T-cell-dependent immune reaction as it can be presented by MHC-II *** unravel the molecular mode of action of this unique polysaccharide we here describe the chemical synthesis of a set of Sp1 fragments,ranging from 3 to 12 monosaccharides in *** outline a unique synthetic approach to overcome the major synthetic challenges associated with the complex Sp1 structure and provide a stereoselective route of synthesis for the oligosaccharide backbone as well as a strategy to introduce the carboxylic acid *** dynamics（MD） simulations together with NMR spectroscopy studies reveal that the oligosaccharides take up helical structures with the nona-and dodecasaccharide completing a full helical *** 3 D structure of the oligosaccharides coincides with the topology required for good interaction with anti-Sp1 antibodies,which has been mapped in detail using *** study has revealed the Sp1 nona-and dodecasaccharides as promising synthetic antigens,displaying all（3 D） structural elements required to mimic the natural polysaccharide and required to unravel the molecular mode of action of these unique zwitterionic polysaccharides.

Introduction: 68Ga-PSMA-11 is considered the gold standard in detection of micro and oligometastases in advanced prostate cancer, being used for therapeutic planning, as well as, potentially, for evaluating response to treatment. The development of ready-to-use lyophilized kit of PSMA-11 adds quality and safety to the routine use of this radiopharmaceutical and represents a pharmacotechnical challenge as it must preserve the integrity and specificity of the ligand. Methods: PSMA-11 kit formulation was proposed, considering radiolabeling parameters and the preservation of the peptide during the lyophilization process, using mannitol as an excipient. Critical temperature characterization studies were carried out using DSC equipment and the freeze-drying process was developed. The direct radiolabeling conditions were evaluated and standardized using 68Ge/68Ga generator eluate from two different manufacturers (ITG and Eckert & Ziegler). The radiochemical purity was evaluated by TlC and HPlC. Biological evaluation was carried out with lyophilized PSMA-11 to demonstrate the integrity of the peptide and preservation of biological activity after the lyophilization process. Results: Based on critical temperature characterization studies, the freeze-drying cycle was designed to reach a freezing temperature of around −40˚C and primary drying at 2˚C. Using 20 mg of mannitol, an intact and elegant lyophilized cake was obtained. PSMA-11 lyophilized kit was directly labeled with 68Ga eluate from 68Ge/68Ga GMP generators (ITG and Eckert & Ziegler) resulting in % RP > 95% at pH 4.0 to 4.5. The results obtained from in vitro and in vivo biological competition studies confirmed the preservation of PSMA-11 affinity for the receptor after lyophilization. Conclusion: A lyophilized formulation (Kit) of PSMA-11 was successfully obtained, which preserved the integrity and biological activity of the peptide and guaranteed radiolabeling efficiency.

Tuberculosis is one of the 10 leading causes of death in the world. Its symptoms include fever, malaise, weakness, weight loss, chest pain, cough, expectoration, shortness of breath and sepsis, which is its most frequent complication. Due to these symptoms, many patients with tuberculosis require admission to the Intensive Care Unit, where they usually are placed on mechanical ventilation. Tuberculosis is more prevalent in the population in situations of social and economic vulnerability. The main factors that interfere with adherence to treatment and the prognosis of these patients are the patient’s support network and their socioeconomic status. We present the following report of a long-time smoker patient, with chronic kidney disease and previous treatment for tuberculosis, who was brought to the emergency room due to lowering of the sensorium, and was immediately placed on mechanical ventilation. The tests suggested sepsis with a respiratory focus, so a search for Alcohol-Acid-Resistant Bacillus was carried out, with a positive result. The patient was transferred to the Intensive Care Unit due to tachycardia and acute respiratory failure. The patient was a smoker with a high tobacco load, chronic kidney disease, in addition to precarious economic, educational and self-care conditions. This report shows that patients with tuberculosis and its complications can be stabilized through known pharmacological treatment. However, the most effective measure to interrupt the transmission of the disease remains the early diagnosis and the provision of adequate treatment, with guidelines aimed at the cessation of harmful habits, such as smoking.
