The prevalence of coronary artery disease(CAD)increases in patients with endstage liver disease,with part of them receiving the percutaneous coronary intervention(PCI)as a treatment *** antiplatelet therapy(DAPT),a standard of care after PCI,could result in catastrophic consequences in this *** PCI and the start of DAPT,it is recommended to assess patient bleeding *** on novel findings,liver cirrhosis does not necessarily lead to a significant increase in bleeding ***,conventional methods,such as the international normalized ratio,might not be appropriate in assessing individual bleeding *** highest bleeding risk among cirrhotic patients has a subgroup with severe thrombocytopenia(l)and elevated portal ***,every effort should be made to maintain thrombocyte count above>50×10^(9)/l and prevent variceal *** is no solid evidence for DAPT in patients with ***,randomized trials investigating short(one month)DAPT duration after PCI with new drug-eluting stents(DES)in a high bleeding risk patient population can be implemented in patients with *** on retrospective studies(with older stents and protocols),PCI and DAPT appear to be safe but with a higher risk of bleeding complications with longer DAPT ***,novel methods in assessing CAD severity should be performed to avoid unnecessary PCI and potential risks associated with *** indicated,PCI should be performed over radial artery using contemporary *** medical therapy,such as proton pump inhibitors and beta-blockers,should be prescribed for lower bleeding risk *** approaches,such as thromboelastography and“preventive”upper endoscopies in PCI circumstances,warn clinical confirmation.

New and compiled detrital zircon U-Pb ages from the southern Neoproterozoic-Cambrian Ribeira Belt,SE Brazil,demonstrate laurentian affinity of the Embu Terrane which is statistically distinct from the adjoining Apiaíand São Roque terranes with cratonic affinity(e.g.,São Francisco Craton).Zircon provenance results indicate that the type-area of the Embu Terrane is dominated by detrital zircon age modes at ca.1200 Ma,1400 Ma,and 1800 Ma,with maximum depositional age of ca.1000 *** contrast,the Apiaíand São Roque terranes are dominated by Paleoproterozoic detrital zircon ages(ca.2200-2000 Ma age dominant component),with maximum depositional ages of ca.1400 Ma and 1750 Ma,*** scaling(MDS)analysis of non-parametric similarity measurements on zircon age populations indicates for the first time that the Embu Terrane encompass two statistically distinct detrital zircon age spectra,which is also reflected in the metamorphic zircon age *** statistical characterization of the Embu Terrane through populational metrics allow a quantitative comparison with surrounding tectonic domains and rock samples classified such as *** results clearly highlight the distinction between the statistically differentiated Embu Terrane from the Apiaíand São Roque terranes,supporting an allochthonous *** addition,we demonstrate that rocks samples previously classified as Embu-type are significantly dissimilar to the definition of Embu Terrane,failing to support alternative tectonic models(e.g.,intracontinental evolution).Detrital zircon age spectra reveal that the Apiaíand São Roque terranes have similar zircon provenance to domains sourced from the São Francisco Craton,whereas detrital zircon populations from the Embu Terrane have greater affinity with SW laurentia basins(and their inferred sediment sources),consistent with previous ***,we interpret the Embu Terrane as a Rodinia descendant developed along the active margin o

AIM: To study the effect of dichloromethylene diphos-phonate (DMDP), a selective Kupffer cell toxicant in reference to liver damage and postnecrotic liver regeneration in rats induced by sublethal dose thioacetamide (TA). METHODS: Rats, intravenously (iv ) pre-treated with a single dose of DMDP (10 mg/kg), were intraperitoneally (ip ) injected with TA 6.6 mmol/kg (per 500 mg/kg body weight). Hepatocytes were isolated from rats at 0, 24, 48 and 72 h following TA intoxication and blood and liver samples were obtained. To evaluate the mecha-nisms involved in the postnecrotic regenerative state, DNA distribution and ploidy time course were assayed in isolated hepatocytes. Circulating cytokine tumor necrosis factor-alpha (TNF-α) was assayed in serum and determined by reverse transcriptase-polymerase chain reaction in liver extract. RESUlTS: The effect of DMDP induced noticeable changes in postnecrotic regeneration, causing an increased percentage of hepatocytes in the cell cycle S phase. The increase at 24 h in S1 population in rats pretreated with DMDP + TA was significantly (P vs 8.57%). Hepatocytes increased their proliferation as a result of these changes. Also, TNF-α expression and serum level were increased in rats pre-treated with DMDP. Thus, DMDP pre-treatment reduced TA-induced liver injury and accelerated postnecrotic liver regeneration. CONClUSION: These results demonstrate that Kupffer cells are involved in TA-induced liver, as well as in post-necrotic proliferative liver states.

AIM:To assess the appropriate use and the diagnostic yield of upper gastrointestinal endoscopy and colonoscopy in this subgroup of ***:In total,789 consecutive outpatients referred for gastrointestinal(GI) endoscopy [381 for esophagogastroduodenoscopy(EGD) and 408 for colonoscopy] were prospectively enrolled in the *** American Society for Gastrointestinal Endoscopy(ASGE) guidelines were used to assess the relationship between appropriateness and the presence of relevant endoscopic f ***:The overall inappropriate rate was 13.3%.The indications for EGD and colonoscopy were,respectively,appropriate in 82.7% and 82.6% of the exams,uncertain in 5.8% and 2.4% and inappropriate in 11.5% and 15%.The diagnostic yield was signif icant higher forEGDs and colonoscopies judged appropriate and uncertain when compared with those considered inappropriate(EGD:36.6% vs 36.4% vs 11.4%,P = 0.004;Colonoscopy:24.3% vs 20.0% vs 3.3%,P = 0.001).Of the 25 malignant lesions detected,all but one was detected in exams judged appropriate or ***:This study shows a good adherence to ASGE guidelines by the referring physicians and a significant increase of the diagnostic yield in appropriate examinations,namely in detecting neoplastic *** underscores the importance that the appropriateness of the indication assumes in assuring high-quality GI endoscopic procedures.

In order to further improve the impact of the continuously evolving neurointerventional techniques, the interventional neuroradiologist needs to have at his disposal more powerful techniques to image the central nervous system. With the recent development of diagnostics techniques that are computed tomography and magnetic resonance based we are now able to assess not just morphology, but also physiology, physiopathology and function. This review discusses the place of diagnostic techniques in the evaluation that the interventional neuroradiologist hast to make when confronted with patients. We provide an overview of current techniques for the brain and spine.

AIM： To study the histological and pancreatitis-associated protein mRNA accumulation changes of pancreas from acute phase of caerulin-induced pancreatitis to recuperation in rats. METHODS： Acute pancreatitis was induced by caerulein in male Wistar rats and followed up for 90 d by histological and mRNA analyses of pancreas. Pancreases were dissected at 0, 9, 24 h and 3, 5, 15, 30, 60, 90 d post-induction. Edema （E）, polymorphonuclear neutrophU （pIvlN） infiltration, cytoplasmic vacuolization （v）, zymogen granule depletion （ZD） and acinar disorganization （AD） were microscopically evaluated. Accumulation of pancreatitis- associated protein （PAP） and l13A mRNAs were quantified by real-time PCR. RESUlTS： The main histological changes appeared at 9 h post-induction for PMN infiltration and cytoplasmic v, while at 24 h and 3 d for E and ZD, respectively. All the parameters were recovered after 5 d, except for ZD which delayed more than 30 d. The main AD was observed after 15 d and values returned to normal after 30 d. Similarly to histological changes, accumulation of the PAP mRNA was increased at 9 h with the highest accumulation at 24 h and differences disappeared after 5 d. CONClUSION： From the acute phase to recuperation of pancreatitis, regeneration and re-differentiation of pan- creas occur and PAP expression is exclusively an acute response of pancreatitis.

AIM: To investigate the association between epidermal growth factor (EGF) +61A/G polymorphism and susceptibility to gastric cancer, through a cross-sectional study. METHODS: Polymerase chain reaction resctriction fragment lenght polymorphism analyses were used to genotype EGF +61 in 207 patients with gastric lesions (162 patients with gastric adenocarcinomas, 45 with atrophy or intestinal metaplasia) and 984 controls. All subjects were Caucasian. RESUlTS: Genotype distribution was 23.5% for GG and 76.5% for GA/AA in the control group, 18.4% for GG and 68.6% for GA/AA in the entire group with gastric lesions and 17.9% for GG and 82.1% for GA/AA in the group with gastric adenocarcinoma. No statistically significant associations were found between EGF +61 variants and risk for developing gastric cancer [odds ratios (OR) = 1.41, 95% confidence intervals (CI): 0.90-2.21, P = 0.116]. However, the stratification of individuals by gender revealed that males carrying A alleles (EGF +61A/G or AA) had an increased risk for developing gastric cancer as compared to GG homozygous males (OR = 1.55, 95% CI: 1.05-2.28, P = 0.021). CONClUSION: In summary, we found that males who were A carriers for EGF +61 had an increased risk for developing gastric cancer. This result may be explained by the suggestion that women secrete less gastric acid than men.

Natural killer(NK)cell-mediated antibody-dependent cellular cytotoxicity(ADCC)through CD16 plays a critical role in antihuman immunodeficiency virus(HIv)responses.1–3 CD300a is a surface receptor highly expressed on NK cells that has the capacity to inhibit NK cell-mediated cytotoxicity in healthy donors.4 The CD300a molecule has been related to several viral infections and is able to diminish the NK cell killing of pseudorabies-infected cells through interactions with its ligands phosphatidylserine and phosphatidylethanolamine.5 In addition,CD300a expression on B and CD4+T lymphocytes is altered during HIv-1 infection,and combined antiretroviral therapy(cART)does not restore nonpathological expression levels.5,6 However,the expression and function of CD300a on NK cells during HIv-1 infection is still *** have determined the surface expression of CD300a on different NK cell subsets and the capacity of this receptor to inhibit CD16-induced NK cell effector functions in healthy and HIv-1 infected individuals.

To determine the number of mutations in the NS5A region of the hepatitis C virus (HCv) and its relationship to the response to antiviral therapy in patients with chronic hepatitis C genotype 1 who are non-responders to two or more treatments. METHODSSequences within HCv NS5A [PKR binding domain (PKRBD) and the interferon-sensitivity-determining region (ISDR)] were analysed via direct sequencing in a selected cohort of 72 patients, with a total of 201 treatments [interferon-alpha (IFN-α), n = 49; IFN-α + ribavirin (RBv), n = 75; pegylated (peg) IFN-α + RBv, n = 47; first-generation direct-acting antivirals (DAAs), n = 13; and second-generation DAAs, n = 17]. Of these, 48/201 achieved a sustained virological response (SvR) and 153/201 achieved no virological response (NvR). RESUlTSFor both regions, treatments resulting in SvR were associated with more baseline mutations than were treatments resulting in NvR (SvR vs NvR; PKRBD: 5.82 ± 3 vs 4.86 ± 2 mutations, P = 0.045; ISDR: 2.65 ± 2 vs 1.51 ± 1.7 mutations, P = 0.005). A decrease or no change in the number of mutations over time between treatments in the PKRBD or ISDR, as shown by sequencing, was associated with patients who usually failed to respond to treatment (PKRBD, P = 0.02; ISDR, P = 0.001). Moreover, patients showing a post-treatment baseline viral load > 600000 IU/ml and increased ISDR mutations with respect to the previous treatment were 9.21 times more likely to achieve SvR (P = 0.001). CONClUSIONThe obtained results show that among patients who have shown no response to two or more antiviral treatments, the likelihood of achieving SvR increases with the genetic variability in the ISDR region (≥ 2 mutations or number of substitutions from the HCv-J and HCv-1 prototype), especially when the viral load is greater than 600000 IU/ml.
