Indoor Air Mycological Survey and Occupational Exposure in Libraries in Mato Grosso-Central Region—Brazil

Background: Indoor air quality in environments where there is great circulation of people, posing risks to the health of its occupants, including allergic problems, infections and contaminations, can be aided by climatic factors, chemicals and biological agents housed in these environments, influencing the location and providing favorable conditions for the degradation of bibliographic collections. The present study investigated the presence of fungi in indoor environments in seven public and private libraries in the central region of Brazil, Mato Grosso, and verified the impact on occupational health. Results: A total of 26,194 fungal specimens were isolated from 342 dust samples collected using three techniques: Andersen’s sampler (12.3%), exposure plate dish (25.1%) and sterile swab (62.6%). A total of 184 fungal species were identified: 156 (84.8%) mycelial fungi and 28 (15.2%) yeast fungi, belonging 54 fungal genera, 43 (79.6%) mycelial fungi and 11 (20.4%) yeast fungi. The genus Aspergillus (40.6%) was one of the main fungi present in indoor air. Aspergillus niger (12.3%) was identified as the most prevalent species in literary environments, followed by Cryptococcus spp. (7.1%) and Cladosporium cladosporioides (7.0%). In relation to seasonal distribution, there was a greater fungal isolation in the dry season (54%);followed by the rainy season (46%). Conclusion: These results suggest the substrates researched in the evaluated environments presented in the form of documents, books and papers associated with dust and air humidity become suitable for microbiological proliferation. These findings highlight the importance of minimizing the risk of exposure to fungal agents, identified in pathogenic and toxigenic microenvironments in library collections.

Schistosomiasis mansoni is a neglected disease and key public health problem,mainly due to its high prevalence,the scarcity of public policies,and the severity of some clinical *** fibrosis(PPF)is the commonest complication of chronic schistosomiasis mansoni and its diagnosis requires different *** though wedge biopsy of the liver is considered the gold standard,it is not justified in non-surgical patients,and percutaneous liver biopsy may be informative but does not have sufficient *** PPF tests mostly include biological(serum biomarkers or combined scores)or physical assessments(imaging assessment of fibrosis pattern or tissue stiffness).Moreover,imaging techniques,such as ultrasound,computed tomography,magnetic resonance imaging,and elastography are applied not only to support the diagnosis of schistosomiasis,but also to assess and detect signs of portal hypertension and organ damage due to chronic schistosomiasis.A combination between a comprehensive history and physical examination with biomarkers for liver fibrosis and imaging methods seems to offer the best approach for evaluating these *** addition,understanding their strengths and limitations will allow a more accurate interpretation in the clinical context and can lead to greater accuracy in estimating the degree of fibrosis in patients with Schistosomiasis mansoni(***)*** review will discuss the different noninvasive methods that are currently available for the evaluation of PPF in *** infection,and their application,advantages,and limitations in clinical practice.

BACKGROUND Toll-like receptor-2(TLR2) is responsible for recognizing Helicobacter pylori(***) and activating the immune response. Polymorphisms in TLR2 may modulate gastric *** To evaluate whether the TLR2 19216 T/C(rs3804099) and TLR2-196 to-174 ins/del(rs111200466) polymorphisms contribute to gastric carcinogenesis in the Brazilian population, and to determine the influence of both polymorphisms and H. pylori infection on TLR2 mRNA *** DNA was extracted from 854 peripheral blood leukocyte or gastric tissue samples[202 gastric cancer(GC), 269 chronic gastritis(CG), and 383 control/healthy(C)]and genotyped by allele-specific PCR or restriction fragment length polymorphism(RFLP)-PCR. Quantitative polymerase chain reaction by Taq Man■ assay was used to quantify TLR2 mRNA levels in fresh gastric tissues(48 GC, 36 CG, and 14 C).RESULTS Regarding the TLR2-196 to -174 polymorphism, the ins/del and del/del genotypes were associated with a higher risk of GC by comparison with the C in all of the analyzed inheritance models(codominant, dominant, recessive, overdominant and log-additive;P < 0.0001). Similarly, an increased risk was observed when comparing the GC and CG groups [codominant(P < 0.0001), dominant(P <0.0001), recessive(P = 0.0260), overdominant(P < 0.0001) and log-additive(P <0.0001)]. In contrast, TLR2 19216 T/C was associated with a protective effect in the GC group compared to the C group [dominant(P = 0.0420) and log-additive(P =0.0300)]. Regarding the association of polymorphisms with H. pylori infection,individuals infected with H. pylori and harboring the TLR2-196 to-174 ins/del polymorphism had an increased risk of gastric carcinogenesis [codominant(P =0.0120), dominant(P = 0.0051), overdominant(P = 0.0240) and log-additive(P =0.0030)], while TLR2 19216 T/C was associated with a protective effect[codominant(P = 0.0039), dominant(P < 0.0001), overdominant(P = 0.0097) and log-additive(P = 0.0021)]. Anesthetical Management of a Patient with Hereditary Muscle Sensory Neuropathy Type 2: Case of a 17-Year Old with Sacral Dermoid and a Short Overview of the Anesthesiological Considerations

We hereby present a short overview of the anaesthesiological considerations regarding the patient with Charcot-Marie-Tooth disease also known as hereditary muscle and sensory neuropathy, which affects peripheral nerves and muscles. Due to pathophysiology of the disease certain anaesthesiological complications associated with HMSN can be related. A case report describing protocol of the total venous anesthesia in the 17-year old patient operated on sacral dermoid with fistulae is presented. The patient recovered without any further complications. In the conclusion we would like to bring the importance of awareness to prepare the HMSN patient for a surgical procedure as well from anesthesiological as from surgical point of view to avoid possible unwanted event such as malignant hyperthermia, hyperkalemia, seizures, prolonged effect of muscle relaxants and worsening of the disease. As an important alternative to general anesthesia regional anesthesia should be considered.
