Pediatric functional constipation treatment with Bifi dobacterium-containing yogurt:A crossover,double-blind,controlled trial

AIM: To evaluate the treatment of pediatric functional chronic intestinal constipation (FcIc) with a probiotic goat yogurt. MeTHODS: A crossover double-blind formula-controlled trial was carried out on 59 students (age range: 5-15 years) of a public school in Belo Horizonte, MG, Brazil, presenting a FcIc diagnostic, according to Roma Ⅲ criteria. The students were randomized in two groups to receive a goat yogurt supplemented with 109 colony forming unit/mL Bifidobacterium longum (***) (probiotic) daily or only the yogurt for a period of 5 wk (formula). Afterwards, the groups were intercrossed for another 5 wk. Defecation frequency, stool consistency and abdominal and defecation pain were ***: Both treatment groups demonstrated improvement in defecation frequency compared to baseline. However, the group treated with probiotic showed most signif icant improvement in the f irst phase of the study. An inversion was observed after crossing over, resulting in a reduction in stool frequency when this group was treated by formula. Probiotic and formula improved stool consistency in the f irst phase of treatment, but the improvement obtained with probiotic was significantly higher (P = 0.03). In the second phase of treatment, the group initially treated with probiotic showed worseningstool consistency when using formula. However, the difference was not signif icant. A signif icant improvement in abdominal pain and defecation pain was observed with both probiotic and formula in the first phase of treatment, but again the improvement was more signif icant for the group treated with B. longum during phase I (P en all data of the crossover study were analyzed, significant differences were observed between probiotic yogurt and yogurt only for defecation frequency (P = 0.012), defecation pain (P = 0.046) and abdominal pain (P = 0.015).

In vitro antileishmanial effects of Physalis angulata root extract on Leishmania infantum

Objective： In the present study, we evaluated the effects of the aqueous extract of Physalis angulata root （AePa） on Leishmania infanturn proliferation, morphology, and the driving mechanism in leishmanicidal activity and modulatory action on macrophages. Methods： I., infantum promastigotes were treated with 50 and 100 μg/mL AePa for 72 h and then antipro- mastigote assay was performed by counts in a Newbauer chamber, morphological changes were analyzed by transmission electron microscopy and the mechanism of the leishmanicidal activity was detected. In addition, macrophages were infected with L. infantum and were used to evaluate anti-amastigote activity of AePa and effects of AePa on cytokine secretion after 72-hour treatment. Results： Treatment with AePa reduced the numbers ofL infantum promastigotes （50% inhibitory concen- tration （Icso） = 65.9μg/mL; selectivity index （SI）= 22.1） and amastigotes （Icso = 37.9 μg/mL; SI = 38.5） compared with the untreated control. Amphotericin B reduced 100% of the promastigote numbers after 72 h of treatment （Ic50 = 0.2μg/mL）. AePa induced several morphological changes and increased the production of reactive oxygen species and apoptotic death in promastigotes after treating for 72 h. AePa （100 μg/mL） promoted tumor necrosis factor-α secretion in macrophages infected with L. infantum after 72 h of treatment, but did not induce an increase in this cytokine in noninfected macrophages. In addition, AePa showed no cytotoxic effect on J774-A1 cells （50% cytotoxic concentration 〉1000μg/mL）. conclusion： AePa presented antileishmanial activity against the promastigotes and amastigotes of I.. infantum without macrophage cytotoxicity; these results show that natural products such as P. angulata have leishmanicidal potential and in the future may be an alternative treatment for leishmaniasis.

AIM:To measure circulating angiotensins at different stages ofhuman cirrhosis and to further evaluate a possible relationship between renin angiotensin system (RAS) components and hemodynamic ***: Patients were allocated into 4 groups: mild-to-moderate liver disease (MLD), advanced liver disease (ALD), patients undergoing liver transplantation, and healthy controls. Blood was collected to determine plasma renin activity (PRA), angiotensin (Ang) , Ang Ⅱ, and Ang-(1-7) levels using radioimmunoassays. During liver transplantation, hemodynamic parameters were determined and blood was simultaneously obtained from the portal vein and radial artery in order to measure RAS ***: PRA and angiotensins were elevated in ALD when compared to MLD and controls (P contrast, Ang was signicantly reduced in MLD. Ang-(1-7)/Ang Ⅱ ratios were increased in MLD when compared to controls and ALD. During transplantation, Ang Ⅱ levels were lower and Ang-(1-7)/Ang Ⅱ ratios were higher in the splanchnic circulation than in the peripheral circulation (0.52 ± 0.08 vs 0.38 ± 0.04, P ereas the peripheral circulating Ang /Ang ratio was elevated in comparison to splanchnic lev-els (0.18 ± 0.02 vs 0.13 ± 0.02, P ely correlated with cardiac output (r= 0.66) and negatively correlated with systemic vascular resistance (r =-0.70).cONcLUSION: Our ndings suggest that the relation-ship between Ang-(1-7) and Ang Ⅱmay play a role in the hemodynamic changes ofhuman cirrhosis.

目的分析社会隔离量表(Social Isolation Scale,SIS)在葡萄牙社区老年人中应用的信度和效度。方法选取250名社区老年人(≥65岁)进行横向描述性调查研究。调查工具包括社会人口学问卷、SIS、老年抑郁量表-4 (Geriatric Depression Scale-4,GDS-4)、6项卢本社会网络量表(6-item Lubben Social Network Scale,LSNS-6)、孤独自我评估问题和生活满意度量表(Satisfaction with Life Scale,SWLS)。分析结构效度(验证性因素分析)和收敛效度,评估ISI内部信度(综合信度)、外部信度(重测、组内相关系数)和评分者间信度(cohen's kappa系数)。结果验证性因素分析显示双因素模型具有良好的拟合指数。SIS得分与LSNS-6(r=0.47)、SWLS(r=0.26)、孤独自我评估(r=0.35)和GDS-4(r=-0.16)得分相关。SIS组合信度良好(0.708)。组内相关系数0.84～0.98。cohen's kappa系数为0.936～1。结论 SIS可作为评估葡萄牙社区老年人社会隔离的有效和可靠工具。医疗保健专业人员,特别是在社区工作的护士,可使用SIS评估老年人的社会隔离状况,以设计、实施和评估干预措施。

evaluation of Quality of Life and Oral changes of Patients in Head and Neck Radiotherapy: Observational Study

Radiotherapy (RT) in the head and neck, despite coming from technological evolution, is challenged by the acute and late side effects of local irradiation, including permanent loss of saliva, osteoradionecrosis, dental caries induced by radiation and necrosis of the oral cavity, mucositis, xerostomia and secondary infections like candidiasis. Since these manifestations are limiting and the integrity of the vital organs contemplates the patient’s general health status, the oral health-related quality of life (OHRQoL) can be significantly affected by the treatment of head and neck cancer (HNC), since cancer can cause important changes in vital functions related to communication, food and social contact, causing disturbances in the patient’s life. An observational and longitudinal study was carried out with 16 cancer patients submitted to RT in the head and neck, between the years 2019-2020 with the objectives of collecting clinical and epidemiological data on the main oral changes caused by RT in the head and neck and correlating them with the impact on OHRQoL. To analyze the OHRQoL, the University of Washington’s Quality of Life Assessment questionnaire (UW-QOL) was applied once a week for 4 weeks and to observe oral manifestations, a dental clinic file standardized by the researchers was applied. The main oral manifestations clinically observed were hyposalivation, trismus and oral mucositis. The UW-QOL presented pain, chewing and taste as the main complaints reported by patients, however, appearance, salivation and chewing showed statistically significant differences over the weeks. Thermodynamic aspects of inclusion engineering in steels

Tailoring non-metallic inclusions in accordance to the desired effect on steel properties has gained widespread acceptance in the last decades and has become known as “inclusion engineering”. effective inclusion engineering involves three steps：（a） a good knowledge of how inclusions influence properties, （b） understanding what is the effect of each type of inclusions on these properties and thus which is the most desirable inclusion in a given product and （c） adjusting the processing parameters to obtain these inclusions. A significant portion of the process adjustment is done during steel refining, where the steel can be tailored so that the desired chemical composition of the non-metallic inclusions that will precipi- tate can be altered. Understanding the relations between steel chemistry, processing variables and inclusion chemical composition requires significant understanding of the thermodynamics of the systems involved. These complex equilibrium calculations are best done using computational thermodynamics. In this work some of the basic techniques used to control inclusion composition are reviewed and the thermodynamic information required to perform this task is presented. Several examples of the application of computational thermodynamics to inclusion engineering of different steels grades are presented and compared with experimental results, whenever possible. The potential and limitations of the method are highlighted, in special those related to thermodynamic data and databases.

care Measures and Health Outcomes in a Pediatric Intensive care Unit in Brazil

Introduction: In low- and middle-income countries, including Brazil, conditions that favor mortality in the PIcU remain significant. compared to developed countries, there is a shortage of skilled human resources, lower availability of technological resources, greater difficulty of access and a higher incidence of infections, including both those acquired prior to admission and those resulting from treatment and hospitalization (i.e., healthcare-associated infections (HAIs)). HAIs in the PIcU include ventilator-associated pneumonia and catheter-related bloodstream infections. Actions for the prevention of HAIs can minimize the occurrence of negative outcomes. Materials and Methods: This is an epidemiological study comparing admissions at the PIcU of a high-complexity hospital in South Brazil over two three-year periods: 2012-2014 (before the measures were adopted) and 2015-2017 (after the measures). The care measures were adopted mainly at the beginning of 2015 and consisted of expansion of physical therapy care, adoption of care protocols, acquisition of new materials and equipment (transparent dressings for central catheters, high-tech mechanical ventilators and multiparametric monitors) and multidisciplinary team training. The frequency of the outcomes mortality, length of PIcU stay, diagnosis of catheter-related bloodstream infection, need for and duration of ventilatory support and diagnosis of ventilator-associated pneumonia were compared between the two trienniums using logistic regression with adjustment for age in months and need of vasoactive drugs. Results: A total of 1140 admissions were analyzed (470 in the first triennium and 670 in the second), representing an increase in the admission rate of 42.6% after the adoption of the measures. After adjustments, significant reductions in the frequency of mortality (adjusted OR [adjOR] = 0.54;cI 95%: 0.34 - 0.84), length of PIcU stay > 7 days (adjOR = 0.75;cI 95%: 0.57 - 0.97) and duration of ventilatory suppor
