Utility of the balloon-overtube-assisted modified over-thewire stenting technique to treat post-sleeve gastrectomy complications

AIM To describe a modified technique of deployment of stents using the overtube developed for balloonassisted enteroscopy in post-sleeve gastrectomy(SG) complications. METHODS Between January 2010 and December 2015, all patients submitted to an endoscopic stenting procedure to treat a post-SG stenosis or leakage were retrospectively collected. Procedures from patients in which the stent was deployed using the balloon-overtube-assisted modified over-the-wire(OTW) stenting technique were described. The technical success, corresponding to proper placement of the stent in the stomach resulting in exclusion of the SG leak or the stenosis, was evaluated. Complications related to stenting were also *** Five procedures were included to treat 2 staple line leaks and 3 stenoses. Two types of stents were used, including a fully covered self-expandable metal stent designed for the SG anatomy(Hanarostent, ECBB-30-240-090; M.I. Tech, Co., Ltd, Seoul, SouthKorea) in 4 procedures and a biodegradable stent(BD stent 019-10A-25/20/25-080, SX-ELLA, Hradec Kralove, Czech Republic) in the remaining procedure. In all cases, an overtube was advanced with the endoscope through the SG to the duodenum. After placement of the guidewire and removal of the endoscope, the stent was easily advanced through the overtube. The overtube was pulled back and the stent was successfully deployed under fluoroscopic guidance. AIM To evaluate viral hepatitis knowledge among individuals from different resource areas and health conditions to identify possible *** A cross-sectional, descriptive study was carried out among 447 individuals from five distinct populations in Brazil: Southeast Viral Hepatitis Ambulatory(n = 100), South(n = 89) and Northeast(n = 114) Health Center, Southeast(n = 77) and Northeast(n = 67) low resource areas. All individuals answered a questionnaire assessing sociodemographic characteristics and viral hepatitis awareness. The perception was scored based on the average number of correct answers of all participants and categorized as "low"(0-28 correct answers) or "desirable"(29-46 correct answers). Associations between sociodemographic characteristics and perception were also *** A low level of knowledge was observed in individuals from Northeast Health Center, Northeast and Southeast low resource areas while desirable knowledge was observed in individuals from Viral Hepatitis Ambulatory and South Health Center. According to sociodemographic characteristics, desirable scores were more common among those with secondary education(47.1%), those who declared themselves as white(46.3%), and those who lived in houses with three individuals(25.5%). Multivariate analysis showed an association between viral hepatitis perception and type of population. CONCLUSION The results demonstrated high level of knowledge among study participants from health clinics from the Southeast region of Brazil and the importance of education programs in increasing the level of knowledge in low resource areas.

Yield of capsule endoscopy in obscure gastrointestinal bleeding:A comparative study between premenopausal and menopausal women

AIM To evaluate differences in capsule endoscopy(CE) performed in the setting of obscure gastrointestinal bleeding(OGIB) among premenopausal women(PMW) and menopausal women(MW).METHODS Retrospective, single-center study, including female patients submitted to CE in the setting of OGIB between May 2011 and December 2016. Patients were divided into 2 groups according to age, considering fertile age as ≤ 55 years and postmenopausal age as > 55 years. The diagnostic yield(DY), the rebleeding rate and the time to rebleed were evaluated and compared between groups. Rebleeding was defined as a drop of Hb > 2 g/dL or need for transfusional support or presence of melena/*** A hundred and eighty three female patients underwent CE for OGIB, of whom 30.6%(n = 56) were PMW and 69.4%(n = 127) were MW. The DY was 30.4% in PMW and 63.8% in MW. The most common findings were angiodysplasias in both groups(PMW: 21.4%, MW: 44.9%)(P = 0.003). In PMW, only 1.8% required therapeutic endoscopy. In 17.3% of MW, CE findingsled to additional endoscopic treatment. Rebleeding at 1, 3 and 5 years in PMW was 3.6%, 10.2%, 10.2% and 22.0%, 32.3% and 34.2% in MW. Postmenopausal status was significantly associated with higher DY(P < 0.001), TY(P = 0.003), rebleeding(P = 0.031) and lower time to rebleed(P = 0.001).CONCLUSION PMW with suspected OGIB are less likely to have significant findings in CE. In MW DY, need for endoscopic treatment and rebleeding were significantly higher while time to rebleed was lower.

<i>FAS</i>-670 Gene Promoter Region Polymorphism in Cervical Intraepithelial Neoplasia Grade 3 (CIN 3) and Invasive Cervical Cancer

The human papillomavirus (HPV) is the etiologic agent of genital cancer, but alone it is incapable of inducing oncogenesis. Rather, progression to invasive lesions is associated with host immunity and interference in the process of cellular apoptosis. Among the several genes involved in cell death, theFAS gene appears to be an important factor. The aim of this study was to evaluate whether there is an association between polymorphisms of the FAS-670 gene promoter region and preinvasive and invasive lesions of the cervix. The FAS gene was evaluated for the presence of polymorphisms using polymerase chain reaction and restriction fragment length polymorphism techniques in 225 blood samples for the control group, as well as cervical tissue from patients with cervical intraepithelial neoplasia grade 3 (CIN 3;75 cases) or invasive carcinoma (214 cases). The AG genotype of the FAS-670 gene promoter region was significantly more frequent in women with CIN 3, with an estimated risk of three times (OR = 3.0). No difference, however, was observed in the control group and women with cervical cancer. In women with cancer, the genotypes were similar in the different histological types and degree of tumor differentiation. Assessing allelic distribution (A or G), we observed no difference in frequency of genotypes in studied groups. These data suggest that polymorphism of the promoter region of the FAS-670 gene is associated with increased risk of CIN 3, but not for invasive cancer of the cervix.

Neisseria meningitidis is a gram-negative diplococcus which causes invasive disease. The most frequent clinical manifestations caused by infection with this pathogen, are meningitis and fulminant septic shock. More benign forms with a subacute or chronic pattern are rare but still can evolve to meningitis and have a fatal outcome. This entity is difficult to recognize, as febrile erythema with otherwise good general condition may simulate other diseases, namely viral infections. The authors reported the cases of two healthy young adults who were misdiagnosed with a viral infection. What they really had was a meningococcemia without meningitis presenting with mild and unspecific symptoms, such as, intermittent fever and a discrete macular rash. We pretend to highlight this rare, unspecific and heterogeneous symptoms which can provoke negligence of a potentially life threatening condition.

Viability of Lactobacillus acidophilus La-5 in the Presence of Williopsis saturnus vat. suaveolens

Maintaining the viability of probiotic microorganisms from production to consumption has long been a technological challenge for the food industry. The objectives of this study were to evaluate the in vitro interaction between Lactobacillus acidophilus La-5 and Williopsis saturnus var. suaveolens and the effect of this yeast on acidification kinetics, viability ofLactobacillus acidophilus and post-acidification in fermented milk during refrigerated storage at 5℃. The in vitro study showed a positive interaction between the acid cell free-supernatant （CFS） ofprobiotic bacteria La-5 and the yeast. The addition of W. saturnus var. suaveolens increased the fermentation time due to consumption of the organic acids produced by L. acidophilus. During the refrigerated storage of the samples, the presence of the yeast increased the viability of L. acidophilus and reduced post-acidification. However, the mechanism of such interaction of bacteria and yeast is not fully understood.

analysis of the Risks of Development Injury Related to Surgical Positioning in a University Hospital of the State of Goiás/Brazil: An Observational-Epidemiological Study

Introduction: The perioperative nurse plays an important role throughout the surgical period, especially in the context of predicting the risks to which patients are exposed when entering a surgical center. These can be skin, muscle, tendon, and even burn injuries from the scalpel or chemicals. Objective: The aim of this study was to verify the degree of risk for the development of injuries resulting from surgical positioning in patients of a university hospital. Methods: This is cross-sectional, exploratory research with a quantitative approach conducted with 70 surgical patients in a Municipal University Hospital with data collection through a socioeconomic and health questionnaire and the Risk Assessment Scale for Development of Surgical Positioning Injuries (SISP). Statistical analyses included simple descriptive evaluation, correlations of variables using the Chi-square test and Pearson’s coefficient with a significance level of 5%. Results: The results found pointed to a population mainly composed of women (60%), married, with an average age of 49 years, with hypertension and high-risk scores to develop injuries due to surgical positioning. There was also a positive and significant correlation between having hypertension and having a higher risk of developing lesions, with R2 = 96.4% and p Conclusion: The results reinforce the need to implement protocols and the acquisition of materials aimed at reducing the risk of injuries related to surgical positioning in order to improve the quality of care offered and thus minimize the harm to users.

Recovering Wood Waste to Produce Briquettes Enriched with Commercial Kraft Lignin

Aiming to use lignocellulosic biomass as energy source, one of the process that may aggregate values is the densification process, which allows the production of bioenergy using solid fuels, mainly for reducing transportation costs. In this research, solid fuel from co-briquetting of wood residues from sawmill using commercial kraft lignin as binder was investigated. The effects of compression pressure (900, 1200 and 1500 PSI) and briquette formulation (varying wood and kraft lignin proportion) on the quality and characteristics of briquettes were evaluated. The main findings were that briquetting of wood residues with kraft lignin resulted in an improvement of bulk density, strength rupture modulus, low heating value (LHV) and high heating value (HHV). The briquettes using 4% and 6% of kraft lignin, and submitted to 1200 to 1500 PSI, presented higher bulk density and strength resistance, respectively. On the other hand, the heating values showed the highest results with the addition of 2% lignin at 900 PSI, being the legal range for additives in briquettes for many countries such as in European Union.

Rheological Study of Polymers Used as Viscosity Index Improvers for Automotive Lubricant Oils

In this work, a rheological study of automotive lubricant oils, neutral light paraffinic (NLP) and neutral medium paraffinic (NMP), additivated with two commercial viscosity index improvers-VIIs, based on hydrogenated poly (isoprene-co-styrene), at different concentrations, was carried out. The study included the determination of the kinematic viscosity of the mixtures, the calculation of the viscosity indexes and their behavior when subjected to various rheometric experiments, including stress sweep, steady state flow, creep and recovery, and temperature ramp oscillatory tests. The obtained results showed the efficacy of the additives as viscosity index improvers, enabling a significant increase in the viscosity index of the pure oils at relatively low concentrations. The determination of the storage (G’) and of the loss (G”) moduli for the mixtures oil/VII additives showed a large predominance of the viscous effect over the elastic one. Except when under low shear stress, the mixtures showed a pseudoplastic behavior, with the flow curves being adjusted to fit the Ostwald-de-Waele model (Power Law), with negative flow behavior indexes.

Impact of the timing of capsule endoscopy in overt obscure gastrointestinal bleeding on yield and rebleeding rate-is sooner than 14 d advisable?

AIM To evaluate the impact of the timing of capsule endoscopy(CE) in overt-obscure gastrointestinal bleeding(OGIB). METHODS Retrospective, single-center study, including patients submitted to CE in the setting of overt-OGIB between January 2005 and August 2017. Patients were divided into 3 groups according to the timing of CE(≤ 48 h; 48 h-14 d; ≥ 14 d). The diagnostic and therapeutic yield(DY and TY), the rebleeding rate and the time to rebleed were calculated and compared between groups. The outcomes of patients in whom CE was performed before(≤ 48 h) and after 48 h(> 48 h), and before( 48 h groups(P = 0.03).CONCLUSION Performing CE within 48 h from overt-OGIB is associated to a higher TY and a lower rebleeding rate and longer time to rebleed.
