Epidemiology and Clinical Features of Mycobacterium avium Complex Pulmonary Disease in a Brazilian Amazon Cohort

The incidence of Mycobacterium avium complex pulmonary disease (MACPD) has been globally increasing. The present study aims to provide a comprehensive analysis of clinical characteristics and treatment response among patients in the Amazon region of Brazil and conduct a retrospective cohort study at a prominent referral hospital in the State of Par, Brazil, from 2012 to 2020. The M. avium group represented 58.9% of cases, followed by M. intracellulare (35.7%), M. colombiense (3.6%), and M. chimaera (1.8%). The majority of patients (73.2%) were female and with an average age of 63 years. Primary clinical manifestations included persistent cough and sputum production. The Charlson comorbidity index (CCI) averaged 2.47, with 36.3% of patients having a CCI score equal to higher than 3. Notably, over 70% of patients had received treatment for pulmonary tuberculosis before initiating treatment for MAC infection. The fibrocavitary radiologic form was the most prevalent (55.4%), frequently exhibiting a bilateral distribution (53.6%). Antimicrobial susceptibility revealed a significant prevalence of MAC resistance to drugs in the therapeutic regimen. Despite this, most of the patients experienced clinical improvement (50%). In conclusion, this study highlights a higher prevalence of MAC infections among middle-aged women, with a history of pulmonary tuberculosis treatment and presenting the fibrocavitary radiological form was predominant.

Marine algae have been used as food since ancient times and today are consumed as a regular part of the diet. In this study, we hypothesized that H. grandifolius phlorotannins-enriched extract exerts cytotoxic selective effects against tumor cell lines promoting cell death trough apoptosis mechanisms. The aim of the present study is to characterize chemically and investigate the selective cytotoxic effect of the H. grandifolius extract on epithelial tumour cell lines (A375, A549, Hep-2, HeLa) compared to non-malignant cell line (Hek-293), and evaluate possible molecular mechanisms involved in the programmed cell death pathway. High-resolution directly-infusion mass spectrometry (HR-DIMS) analysis with electrospray ionization (ESI) was performed in positive and negative mode. Cytotoxicity was evaluated through colorimetric assay and morphological alterations were observed in giemsa stained cells after extract treatment. Apoptosis was further evaluated by annexin V staining. Spectra showed peaks m/z between 370 and 430 and molecular formula was defined upon isotopic ratio. Extract showed selectivity to the non-tumor line with enhanced cytotoxicity in tumor cells according to the concentration and exposure time. After 72 h treatment, the HeLa strain was more susceptible to the extract, followed by lines Hep2, A375 and A549. Morphological changes by giemsa were observed after increased doses of extracts and staining for annexin V showed majority of tumor cells at early stages of the apoptotic process. Here, selective anti-tumor activity of extract taken from the alga H. grandifolius was able to suppress proliferation and promote apoptosis-mediated cell death with induction of initial stages of apoptosis in different cell lines.

Mayer-Rokitansky-Kuster-Hauser Syndrome (MRKH) is a rare disease characterized by total or partial vagina agenesis, karyotype 46, XX with normal secondary sexual characters. Still, it is the second leading cause of primary amenorrhea. The absence of obvious signs and symptoms often causes the syndrome to be diagnosed only after puberty. The case presented here highlights exactly this difficulty of early diagnosis, which meets the objective of the study, and is precisely to provide reliable material that facilitates the diagnosis and management of patients with MRKH syndrome.

The aim of this study was to evaluate the effects of silver nitrate in the in vitro development and conservation of passion fruit plants. Experiments were carried out at the Laboratory of Culture and Tissues, in the National Cassava and Fruits Research Center (EMBRAPA), using microcuttings of Passiflora gibertii N. E. Br., previously cultivated in vitro. The microcuttings were placed in MS media supplemented with silver nitrate at concentrations of 0, 1, 2, 4 and 8 mg·L-1, and, during 30, 60 and 90 days, the following variables were identified: Shooting length (cm), number of roots, number and coloration of leaves. A completely randomized design with 30 replications in a split-plot scheme was employed. Polynomial regression equations were adjusted in analysis of variance to compare averages of treatments. The obtained results support the assumption of high sensitivity of passion fruit tissues to ethylene, which reflects in the lost of vigor and in the induction of culture senescence by its accumulation. Based on obtained results, and in order to mitigate this problem, it is possible to indicate an addition of 2 mg·L-1?of silver nitrate in culture media, for the micropropagation of passion fruit plant, allowing the controlling of the atmosphere inside the culture test tubes, during the establishment of explants and other steps of the process.

研究目的:比较三明治修复技术(玻璃离子水门汀[GIC]或树脂改性玻璃离子水门汀[RMGIC]垫底,复合树脂[CR]充填)和仅用复合树脂充填修复非龋性牙颈部缺损(NCCLs)充填体的保留率。材料与方法:检索以下数据库:the Cochrane ***,Scopus和Web of Science。同时检索了灰色文献和IADR正在进行和未发表的摘要(1990-2017)。应用Cochrane风险评估工具评估纳入文献质量。应用随机效应模型对随访1.2.3年的相关结果进行meta分析。应用GRADE法进行证据质量评价。结果:最初共检索到3645篇文献。经过对题目、摘要和全文进行筛选,共纳入6篇文献。其中有3篇文献是对同一随机临床试验的随访研究,所以最终对4项研究进行分析。4项研究均未报道偏倚风险程度。在所有结果中,仅在随访3年时应用三明治修复技术的充填体脱落率低于仅应用复合树脂充填(风险比[RR]:7.5.95%Cl:2.1-27.2:p=0.002)。结论:本系统综述纳入研究数量有限,随访3年时应用三明治修复技术的充填体保留率高于仅应用复合树脂充填。缺损修复方式不影响次要结果。除充填体保留率的质量等级为中等,其他次要结果的质量等级都为低。

目的：评估2种粘接系统[Adper Single Bond plus（3MESPE）和Clearfil SEBOND（Kuraray）]与预先是否经过Nd：YAG激光或Nd：YAG激光加氟处理的正常或龋坏牙本质粘接后的微拉伸力。材料和方法：将60颗离体第三磨牙的牙本质表面暴露后分为12组。1～6组经过pH循环产生人工龋，7～12组保持正常牙本质。牙本质表面采用3种处理方法：Nd：YAG激光照射（60mJ，15Hz．09w）1min：Nd：YAG激光照射联合氟凝胶处理：不处理（对照组）。实验组根据产品说明书要求应用粘接材料并分层堆塑制成复合树脂块（FiItek Z250．3M ESPE），牙齿沿X和Y轴方向做连续片切．应用万能试验仪进行微拉伸力试验。结果：通过ANOVA和Tukey统计分析（P〈0.05）发现正常牙本质应用Clearfil SE Bond组的平均粘接力最大（4065MPa）．SingleBond组次之（342MPa）。龋坏牙本质组无论是否事先经过激光照射应用两种粘接系统后的粘接力明显下降。但激光照射后应用Clearfil SE Bond组微拉伸粘接力最高。另外．激光照射联合氟化物处理后应用两种粘接系统的微拉伸力有所下降。结论：与全酸蚀粘接系统和激光照射相比．临床上在备洞时用激光照射龋坏牙本质．然后应用自酸蚀粘接系统可以获得较高的粘接力。

The new coronavirus (SARS-CoV-2) broke out in Wuhan in China in December 2019, causing severe pneumonia and deaths, soon in March 2020, it reached pandemic level, affecting several countries including Brazil. The disease was named COVID-19, with characteristics of most infected having mild and moderate symptoms and a part severe symptom. The disease has already reached 158 ethnic groups, which have high vulnerability and limited access to health services. The objective is to investigate the clinical and spatial characteristics of Severe Acute Respiratory Syndrome of COVID-19 in the indigenous peoples of Brazil. It is an epidemiological, cross-sectional, analytical ecological study, based on data from the OpenDataSUS platform from 01/01/2020 to 31/08/2020. Profile variables, signs and symptoms and risk factors/comorbidities. The data were analyzed by Bioestat 5.3. There were 1,207 cases and 470 deaths. Profile: male gender (59.48%) means age 53 years. Signs and symptoms: fever (74.23%), cough (77.71%), sore throat (35.62%), dyspnea (69.34%), respiratory discomfort (62.80%), O2 saturation 2 saturation < 95% (69.36%). Risk factors and comorbidities (45.89%) were associated with deaths (54.04%). About comorbidities, chronic cardiovascular diseases represented (18.97%) and Diabetes Mellitus (18.97%), and associated with deaths: Chronic Cardiovascular Disease (24.46%). Being admitted to the ICU has a risk of death in (OR-3.96- < 0.0001-CI-2913/5383) followed by not being vaccinated against influenza (OR-1.85- < 0.0001-CI-1358/2528). The public and health policies of Brazil should be directed to control the dissemination of COVID-19 in this population, that COVID-19 evolves in the same intensity, however, the indigenous have vulnerabilities that can increase the impact of the pandemic in this population.

This study aimed to describe structure, functionality and development of pediatric heart transplant families, from the understanding of the experienced chronic condition. A descriptive, exploratory study using a qualitative approach was developed in a hospital situated in a city of Northeast Brazil, in 2012. The study was conducted with three families, using the Calgary Family Assessment Model (CFAM). In addition to the genogram, an analysis of the children’s/adolescents’ family members was conducted at the time of interview. Two families were binuclear families and one was mononuclear. From the evaluation, it was possible to determine the care and support network of children and adolescents with heart transplants, as well as some difficulties faced by the investigated families. Nurses are important in this family context since the visualization of the family genogram can generate a change in the nursing care to each family.
