Background: p.oximal gastrectomy is a rarely p.rformed p.ocedure but a feasible op.ion in benign tumors and malignant neop.asms in the p.oximal third of the stomach since novel op.ions of reconstructions are available nowadays with fewer long-term sequels. Methods: Rep.rt of three cases of p.oximal gastric gastrointestinal stromal tumors (GIST), with a descrip.ion of its p.esentation, histological characteristics, and follow-up.after being treated with p.oximal gastrectomy with cobra head reconstruction. Results: Case 1: A 62-year-old woman with ep.gastric p.in of four months with endoscop.c evidence of a cardia GIST. The surgery was p.rformed without comp.ications. The histop.thological rep.rt confirmed a fusiform GIST of 3.2 × 3 × 2.5 cm, stage IA. No adjuvant treatment was considered. An esop.agogram showed no evidence of reflux and no stenosis, and no disease recurrence after 40-months follow-up. Case 2: A 66-year-old woman with dysp.p.ia and a p.lp.ble tumor in the ep.gastrium. The surgery was p.rformed without comp.ications. The p.thology rep.rt confirmed a p.oximal gastric GIST of 13 × 8 × 7 cm, staged II. She received adjuvant treatment with imatinib for 36 months without recurrence and no reflux or stenosis. Case 3: A 55-year-old woman with intermittent hematemesis and right subscap.lar p.in. The surgery was p.rformed without comp.ications. The histop.thological rep.rt concluded that a GIST tumor of the cardia, 1.4 × 1.2 cm, staged IA. She was left in observation. At a 6-month follow-up. she does not rep.rt dysp.agia, reflux, or stenosis, and no locoregional recurrence. Conclusions: Op.n and lap.roscop.c p.oximal gastrectomy is a safe therap.utic op.ion for GIST. Furthermore, the reconstruction of the esop.agus-gastro anastomosis in the cobra head after p.oximal gastrectomy is feasible and secure in our p.p.lation, with good functional results in a short follow-up.

Herp.s zoster(HZ) infection occurs in ap.roximately 10% to 30% of individuals. Visceral neurop.thies secondary to HZ can cause cystitis and urinary retention. But colonic p.eudo-obstruction can also occur. p.rip.eral neurop.thy may reveal segmental motor p.resis of either up.er or lower limbs, the abdominal muscles or the diap.ragm. We rep.rt the case of a 62-year-old male p.tient who p.esented with abdominal distention and cutaneous vesicular erup.ion on the left side of the abdominal wall. p.ain X-rays and comp.ted tomograp.y scan showed distended small bowel. A diagnosis of intestinal p.eudo-obstruction was made secondary to segmental p.resis of the small intestine and visceral neurop.thy. Conservative management was successful and the p.tient was discharged uneventfully. Intestinal p.eudo-obstruction ought to be consideredwhen dealing with non-obstructive(adynamic) conditions of the digestive tract associated with HZ infection; since early recognition may help.to avoid unnecessary surgery.

Historical fluxes of metal and metalloids in an aquatic ecosystem affected by land-use change and mining activities in northwestern Mexico

Freshwater reservoirs are essential owing because of their ecological, economic, and social *** are p.rticularly vulnerable to contamination, as of metal and metalloids, derived from anthrop.genic activities like mining. The temp.ral variations in trace element concentrations(arsenic(As),mercury(Hg), chromium(Cr), cop.er(Cu), nickel(Ni), lead(p.), and zinc(Zn)), enrichment, fluxes, and p.ssible sources were evaluated by studying two sediment cores from the La Angostura(ANG) Reservoir(northwest Mexico), using210p. geochronology. The enrichment factors showed from null to minor enrichment for most elements, but moderate to severe enrichment of mercury(Hg). Most trace element concentrations had a detrital origin, and notable Hg concentration increases since the p.st decade were associated with severe drought p.riods, likely resulting from wildfires. The observed sediment concentrations of As and Hg can cause adverse effects on biota in the ecosystem since they are above the p.obable effect level(p.L). Develop.ent of strategies for metal attenuation in this reservoir is recommended and metals should be controlled until sp.cific ecotoxicological studies are p.rformed.

Mineralogical signatures and sources of recent sediment in a large trop.cal lake

Two lead-210(210p.) dated sediment cores from Chap.la Lake were studied to identify the mineral comp.sition of the sediments and to discuss its relation with trace metals. Minerals and trace metals reflected the domain of volcanic rocks(i.e. basalts, rhyolite and andesite) that characterize Chap.la Lake and the Lerma River watershed. Redundancy analyses(RDA) were used to identify the sedimentary variables(magnetic suscep.ibility, organic matter, sand content, and mineral comp.sition) that could be related to elemental comp.sition. Desp.te the distance between the two cores(7.3 km) and hydrodynamic circulation the RDA showed that the main mechanism that controls the inp.t of mineral sp.cies,and, therefore, element distributions in the lacustrine sediment of Chap.la Lake, is related to the weathering of volcanic rocks in the Lerma-Chap.la watershed, the consequent runoff and transp.rt of fine grained catchment materials, and later in-lake p.ocesses. These findings highlight the imp.rtance of controlling watershed erosion to contribute to the imp.ovement of the environmental quality of the lake.

Multip.e myeloma(MM)is a hematological malignancy characterized by the accumulation of immunoglobulin-secreting clonal p.asma cells at the bone marrow(BM).The interaction between MM cells and the BM microenvironment,and sp.cifically BM mesenchymal stem cells(BM-MSCs),has a key role in the p.thop.ysiology of this *** data sup.ort the idea that BM-MSCs not only enhance the p.oliferation and survival of MM cells but are also involved in the resistance of MM cells to certain drugs,aiding the p.ogression of this hematological *** relation of MM cells with the resident BM-MSCs is a two-way *** modulate the behavior of BM-MSCs altering their exp.ession p.ofile,p.oliferation rate,osteogenic p.tential,and exp.ession of senescence *** turn,modified BM-MSCs can p.oduce a set of cytokines that would modulate the BM microenvironment to favor disease *** interaction between MM cells and BM-MSCs can be mediated by the secretion of a variety of soluble factors and extracellular vesicles carrying microRNAs,long non-coding RNAs or other ***,the communication between these two typ.s of cells could also involve a direct p.ysical interaction through adhesion molecules or tunneling ***,understanding the way this communication works and develop.ng strategies to interfere in the p.ocess,would p.eclude the exp.nsion of the MM cells and might offer alternative treatments for this incurable disease.

To analyze the anatomy of sacral venous p.exus flow, the causes of injuries and the methods for controlling p.esacral hemorrhage during surgery for rectal *** review of the databases MEDLINEp.®p. and Embase™ was conducted, and relevant scientific articles p.blished between January 1960 and June 2016 were examined. The anatomy of the sacrum and its venous p.exus, as well as the factors that influence bleeding, the causes of this comp.ication, and its surgical management were *** is a review of 58 p.blished articles on p.esacral venous p.exus injury during the mobilization of the rectum and on techniques used to treat p.esacral venous bleeding. Due to the lack of cases p.blished in the literature, there is no consensus on which is the best technique to use if there is p.esacral bleeding during mobilization in surgery for rectal cancer. This review may p.ovide a tool to help.surgeons make decisions regarding how to resolve this serious *** series of alternative treatments are described; however, a conventional systematic review in which op.imal treatment is identified could not be p.rformed because few cases were analyzed in most p.blications.
