Monitoring of Ultrafine Particles in the Surrounding Urban Area of In-Land Passenger Ferries

Maritime transportation, widely used both in international transport of goods and touristic purposes, has been identified as a significant source of ultrafine particles (UFP). In-land passenger ferry is a source of UFP far less addressed;however, in locations with relatively high frequency of this transportation mode, it is expected that they contribute to an increase of their concentration. Moreover, the negative effects of UFP on human health and environment are known and therefore, monitoring UFP produced by ferries is vital to assess the degree of exposure of who work or live close to ferries’ terminals or downwind to their cruising path. This work aims to study the influence of in-land ferries activities on UFP in the urban/suburban areas near ferries’ terminals and downwind across the cruising path. The UFP monitoring campaign was carried out from September to December 2018 for 19 non-consecutive periods. The sampling sites were chosen in order to maximize measurements under downwind conditions and allow the association between ferry operation and UFP concentration response. Based on data collected, correlation analysis was performed between ferry’s traffic and particle number counting (PNC) of UFP, and also with meteorological variables. Results show an increase in PNC ranging from 25 to 197% during the third minute around a ferry movement and are moderate to positive significant correlations between PNC values and the number of ferry operations (r = 0.79 to r = 0.94), showing that UFP emitted by in-land ferries contributes to PNC increase. Moreover, negative correlations (r = -0.85 to r = -0.93) between PNC and wind intensity were also found.

Measuring External Debt in a Context of Macroeconomic Imbalances

The exploitation of new methodologies and indicators is considered valuable in measuring a key macroeconomic indicator like the external debt. The recent Portuguese economic developments illustrate the need and usefulness of having a multidimensional approach of this indicator, with several balance of payment items, such as the current and the capital account balances, the foreign direct investment or the reserves assets being carefully analysed when reading the external conditions faced by the economy. Only such an approach can provide with a comprehensive measure of external debt consistent across the range of debt instruments, institutional sectors and valuation methods used. This paper develops an assessment of external debt measures and concludes about their potential advantages and disadvantages. Comparisons are made by focusing on alternatives like gross external debt against net external debt, external debt vis-h-vis international investment position, and external debt at nominal value against external debt at market value.

Portuguese Society of Intensive Care Score for Predicting SARS-CoV-2 Infection Applied to Inpatients with Pneumonia: A Reliable Tool?

Objectives: Early identification of patients with the novel coronavirus induced-disease 2019 (COVID-19) and pneumonia is currently challenging. Few data are available on validated scores predictive of Severe Acute Respiratory Syndrome Coronavirus-2 (SARS-CoV-2) infection. The Portuguese Society of Intensive Care (PSIC) proposed a risk score whose main goals were to predict a higher probability of COVID-19 and optimize hospital resources, adjusting patients’ intervention. This study aimed to validate the PSIC risk score applied to inpatients with pneumonia. Methods: A retrospective analysis of 207 patients with pneumonia admitted to a suspected/confirmed SARS-CoV-2 infection specialized ward (20/03 to 20/05/2020) was performed. Score variables were analyzed to determine the significance of the independent predictive variables on the probability of a positive SARS-CoV-2 rRT-PCR test. The binary logistic regression modeling approach was selected. The best cut-off value was obtained with the Receiver Operating Characteristic (ROC) curve together with the evaluation of the discriminatory power through the Area Under the Curve (AUC). Background: Atherosclerosis and thrombosis are the major manifestations underlying cardiovascular diseases (CVD), which are the leading cause of mortality and morbidity worldwide. Both result from an interaction between genetic and environmental risk factors. The goal of our study was to evaluate several polymorphisms identified as predisposing factors to atherosclerosis and thrombosis. Material and Methods: A series of 155 healthy unrelated individuals of Azorean origin were analyzed using the CVD StripAssay (ViennaLab Diagnostics, Austria) for the most established polymorphisms involved in blood coagulation (F2, F5, F13A1, FGB), fibrinolitic system (SERPINE1), platelet adhesion (ITGB3), homocysteine metabolism (MTHFR), reninangio-tensin system (ACE) and lipid metabolism (APOE). Results: No significant differences were observed in allelic frequencies when comparing our data to mainland Portugal. Group stratification according to the number of “increased” risk alleles, demonstrated that 116/155 (75%) individuals belong to the moderate risk group (5 - 10 risk alleles). Conclusions: Although acknowledging the fact that the allelic states at the analysed loci lack predictive value, the fact that a high frequency of individuals presents at least 5 risk alleles (124/155;80%) is important for the establishment of the appropriate preventive measures in the Azorean population.

Autoimmune hepatitis and anti-tumor necrosis factor alpha therapy:A single center report of 8 cases

This article describes cases of anti-tumor necrosis factor(TNF)-α-induced autoimmune hepatitis and evaluates the outcome of these patients in relation to their immunosuppressive strategy. A retrospective analysis of medical records was performed in our center, in order to detect cases of autoimmune hepatitis(AIH) associated with anti-TNF biologic agents. We describe and analyze eight cases of AIH following anti-TNF therapy, 7 with infliximab and 1 with adalimumab. A distinction should be made between induction of autoimmunity and clinically evident autoimmune disease. Liver biopsy is useful in detecting the role of the TNF-α antagonist in the development of AIH. The lack of relapse after discontinuing immunosuppressive therapy favors, as in this case series, an immune-mediated drug reaction as most patients with AIH have a relapse after treatment is suspended. Although AIH related to anti-TNF therapy is rare, a baseline immunological panel along with liver function tests should be performed in all patients with autoimmune disease before starting biologics.

New Highlights About Worries, Coping, and Well-Being During Childhood and Adolescence

The aim of this study was to understand what children"s and adolescents" worries are and the strategies used to face them. Two studies were carried out. Study one had a sample of 1,207 Portuguese children and adolescents, 48.4% boys, attending 5th to 9th grades, mean age 12.5, participated in a first approach to children and adolescents" theories about worries and coping with worries. Study two had a sample of 527 Portuguese children and adolescents, 46.8% boys, 5th to 9th grades, mean age 12.4, aimed to replicate study one and to go in depth on worries and coping and gender differences. Confronted with questions about the frequency of worries, the impact of worries, the kind of worries and how to cope with them, it was clear that the majority of children and adolescents get worried frequently, part of them let worries interfere significantly in their lives, and the best coping strategy is distraction, less frequently they seek social support, try to calm down or engage in health compromising behaviours such as drinking or eating or being violent. Other strategies were reported but none of them was frequent as these ones. Family, peers (getting supported) and leisure time (getting distracted) are important issues in young people"s well-being but they play differential roles. School seems part of "the problem" but never "part of the solution". The role of personal competences such as resilience, self-regulation, and problem-solving appears less frequently in these age groups. Mental health is a forgotten area in health promotion in schools, but indeed being worried frequently without being able to solve worrying issues does not contribute at all for children and adolescents" well being and furthermore is able to interfere with the so feared school success. The implications for health promotion interventions in families and in schools are discussed.
