Gastric cancer in a Caucasian population: Role of pepsinogen C genetic variants

AIM： To study the role of an insertion/deletion polymorphism in the pepsinogen C （PGC） gene, an effective marker for terminal differentiation of the stomach mucosa, in the susceptibility to the development of gastric lesions. METHODS： The study was performed with 99 samples of known gastric lesions and 127 samples without evidence of neoplastic disease. PCR was employed and the 6 polymorphic alleles were amplified： Allele 1 （510 bp）, Allele 2 （480 bp）, Allele 3/4 （450/460 bp）, Allele 5 （400 bp） and Allele 6 （310 bp）. RESUlTS： Our results revealed that Allele 6 carriers seemed to have protection against the development of any gastric lesion （OR = 0.34; P 〈 0.001）, non-dysplastic lesions associated with gastric adenocarcinoma such as atrophy or intestinal metaplasia （OR = 0.28; P 〈 0.001） or invasive GC （OR = 0.39; P = 0.004）. CONClUSION： Our study reveals that the Allele 6 carrier status has a protective role in the development of gastric lesions, probably due to its association with higher expression of PGC. Moreover, the frequency of Allele 6 carriers in the control group is far higher than that obtained in Asian populations, which might represent a genetic gap between Caucasian and Asian populations.

AIM: To investigate the association between epidermal growth factor (EGF) +61A/G polymorphism and susceptibility to gastric cancer, through a cross-sectional study. METHODS: Polymerase chain reaction resctriction fragment lenght polymorphism analyses were used to genotype EGF +61 in 207 patients with gastric lesions (162 patients with gastric adenocarcinomas, 45 with atrophy or intestinal metaplasia) and 984 controls. All subjects were Caucasian. RESUlTS: Genotype distribution was 23.5% for GG and 76.5% for GA/AA in the control group, 18.4% for GG and 68.6% for GA/AA in the entire group with gastric lesions and 17.9% for GG and 82.1% for GA/AA in the group with gastric adenocarcinoma. No statistically significant associations were found between EGF +61 variants and risk for developing gastric cancer [odds ratios (OR) = 1.41, 95% confidence intervals (CI): 0.90-2.21, P = 0.116]. However, the stratification of individuals by gender revealed that males carrying A alleles (EGF +61A/G or AA) had an increased risk for developing gastric cancer as compared to GG homozygous males (OR = 1.55, 95% CI: 1.05-2.28, P = 0.021). CONClUSION: In summary, we found that males who were A carriers for EGF +61 had an increased risk for developing gastric cancer. This result may be explained by the suggestion that women secrete less gastric acid than men.

AIM:To compare the natural history and course of acute diverticulitis in a younger age group with an older population and to evaluate whether younger patients should be managed differently. METHODS:This study was a retrospective review of 157 patients treated with acute diverticulitis between January 1,2004 and December 31,*** was stratified according to the Hinchey classification. Patients were divided into 2 populations:group A≤ 50 years(n=31) ;group B>50 years(n=126) .Mean patient follow-up was 15 mo. RESUlTS:The median age was 60 years.A significantly higher proportion of patients in group B presented with complicated diverticulitis(36.5%vs 12.9%,P=0.01) .Recurrence was more frequent in group A(25.8%vs 11.1%,P=0.03) and the mean time-torecurrence was shorter(12 mo vs 28 mo,P=0.26) . The most severe recurrent episodes of acute diverticulitis were classified as Hinchey stageⅠand none of the patients required emergency *** multivariate analysis,only age(P=0.024) was identified as an independent prognostic factor for recurrence. CONClUSION:Based on the results of this study,we recommend that diverticulitis management should be based on the severity of the disease and not on the age of the patient.

The extraction of the physical parameters of long period gratings from the spectral response is not an easy process. We present a demodulation technique to synthesize the physical parameters of a long period grating recorded in an optical fibre. The demodulation is achieved through the implementation of a genetic algorithm. The extracted parameters are in agreement with the typical values known for long period gratings.

lNovel lithium doped tris 8 hydroxylquinoline aluminium(Alq3:li) layer is deposited between emission layer and electron injection aluminium electrode as an electron injection assistant layer in different organic light emitting diodes(OlED) to lower the electron injection barrier. In these devices, Alq3 is used as emission layer, and a bilayer film of N,N bis (1 naphhyl) N,N diphenyl 1,1 biphenyle 4,4' diamine(NPB) and 4,4,4' tris(3 methyl phenylphenylamino) triphenylamine( m MTDATA) used as hole transport layer(HTl). The electroluminescent performance of devices with different thicknesses of Alq3∶li shows that the insertion of the lithium doped Alq3 layer can reduce the turn on voltage by at least 2 volts, and the stability of devices with this lithium doped Alq3 layer is improved too. It can also change the efficiency of devices. Compared with an ultra thin lithium fluoride(liF) layer, Alq3∶li sheet gives similar effects but higher efficiency and can be much thicker and hence it is easier to control the deposition.

Successful double fertilization and subsequent seed development in flowering plants requires the delivery of two sperm cells, transported by a pollen tube, into the embryo sac of an ovule. The embryo sac cells tightly control synergid cell death, and as a result the polyspermy block. Arabinogalactan proteins are highly glycosylated proteins thought to be involved in several steps of the reproductive process. We show that JAGGER, Arabinogalactan Protein 4, is an important molecule necessary to prevent the growth of multiple pollen tubes into one embryo sac in Arabidopsis thaliana. In jagger, an AGP4 knockout mutant, the pistils show impaired pollen tube blockage as a consequence of the survival of the persistent synergid. JAGGER seems to be involved in the signaling pathway that leads to a blockage of pollen tube attraction. Our results shed light on the mechanism responsible for preventing polyspermy in Arabidopsis and for safe- guarding successful fertilization of all ovules in one pistil, ensuring seed set and the next generation.

Pregnancy is a period of major transformations in a woman’s life;increased stress,and mood and sleep disorders are *** review evaluates mindfulness interventions during pregnancy and their ability to help manage stress,anxiety,depression,emotional regulation,level of mindfulness and sleep quality.A search of English language scientific literature relevant to mindfulness interventions for pregnant women was conducted using PubMed,Scopus and Web of Science,without restriction on publication *** criteria were randomized clinical trials with pregnant women,using mindfulness as an intervention for at least three weeks,in one of our main areas of interest,and using only validated scales to measure *** hundred and thirty studies were identified in our searches of research databases,and thirteen were included in our *** found a large diversity of mindfulness programs,heterogeneity among the instruments used to evaluate outcomes,and inconsistency in the gestational periods used in the *** interventions were beneficial for stress,anxiety and *** was also effective when applied in pregnant women with a history of depression or experiencing *** emotional regulation and the level of mindfulness,there were signs of improvement,but more studies are *** of the studies evaluated sleep *** review provides information about current mindfulness programs,an overview of the effects of mindfulness interventions,a description of the measurements used so far,and recommendations for developing highquality mindfulness protocols for pregnant women.

AIM:To assess the efficacy and safety of endoscopic papillary large balloon dilation after biliary sphincterotomy for difficult bile duct stones ***:Retrospective review of consecutive patients submitted to the technique during 18 *** main outcomes considered were:efficacy of the procedure(complete stone clearance;number of sessions;need of lithotripsy) and ***:A total of 30 patients with a mean age of 68 ± 10 years,23 female(77%) and 7 male(23%) were *** 10 patients,a single stone was found in the common bile duct(33%) and in 20 patients multiple stones(67%) were *** median diameter of the stones was 17 mm(12-30 mm).Dilations were performed with progressive diameter Through-TheScope balloons(up to 12,15) or 18 *** retrieval of stones was achieved in a single session in 25 patients(84%) and in two sessions in 4 patients(13%).Failure occurred in 1 case(6%).Mechanical lithotripsywas performed in 6 cases(20%).No severe complications *** patient(3%) had mild-grade post-endoscopic retrograde cholangiopancreatography(ERCP) ***:Endoscopic balloon dilatation with a large balloon after endoscopic sphincterotomy is a safe and effective technique that could be considered an alternative choice in therapeutic ERCP.

A much larger amount of bonded hydrogen was found in thick nanocrystalline diamond （NCD） films produced by only adding 0.24% N2 into 4% CH4/H2 plasma, as compared to the high quality transparent microcrystalline diamond （MCD） films, grown using the same growth parameters except for nitrogen. These experimental results clearly evidence that defect formation and impurity incorporation （for example, N and H） impeding diamond grain growth is the main formation mechanism of NCD upon nitrogen doping and strongly support the model proposed in the literature that nitrogen competes with CHx （x = 1, 2, 3） growth species for adsorption sites.
