Safety and efficacy of dual antiplatelet therapy after percutaneous coronary interventions in patients with end-stage liver disease

The prevalence of coronary artery disease(CAD)increases in patients with endstage liver disease,with part of them receiving the percutaneous coronary intervention(PCI)as a treatment *** antiplatelet therapy(DAPT),a standard of care after PCI,could result in catastrophic consequences in this *** PCI and the start of DAPT,it is recommended to assess patient bleeding *** on novel findings,liver cirrhosis does not necessarily lead to a significant increase in bleeding ***,conventional methods,such as the international normalized ratio,might not be appropriate in assessing individual bleeding *** highest bleeding risk among cirrhotic patients has a subgroup with severe thrombocytopenia(50×10^(9)/L and prevent variceal *** is no solid evidence for DAPT in patients with ***,randomized trials investigating short(one month)DAPT duration after PCI with new drug-eluting stents(DES)in a high bleeding risk patient population can be implemented in patients with *** on retrospective studies(with older stents and protocols),PCI and DAPT appear to be safe but with a higher risk of bleeding complications with longer DAPT ***,novel methods in assessing CAD severity should be performed to avoid unnecessary PCI and potential risks associated with *** indicated,PCI should be performed over radial artery using contemporary *** medical therapy,such as proton pump inhibitors and beta-blockers,should be prescribed for lower bleeding risk *** approaches,such as thromboelastography and“preventive”upper endoscopies in PCI circumstances,warn clinical confirmation.

Although,liver transplantation serves as the only curative treatment for patients with end-stage liver diseases,it is burdened with complications,which affect survival *** addition to clinical risk factors,contribution of recipient and donor genetic prognostic markers has been extensively studied in order to reduce the burden and improve the *** of single nucleotide polymorphisms(SNPs)is one of the most important tools in development of personalized transplant *** provide a better insight in recent developments,we review the studies published in the last three years that investigated an association of recipient or donor SNPs with most common issues in liver transplantation:Acute cellular rejection,development of new-onset diabetes mellitus and non-alcoholic fatty liver disease,hepatocellular carcinoma recurrence,and tacrolimus concentration *** studies confirmed previously established SNP prognostic factors,such as PNPLA3 rs738409 for nonalcoholic fatty liver disease development,or the role of CYP3A5 rs776746 in tacrolimus concentration *** also identified several novel SNPs,with a reasonably strong association,which have the potential to become useful predictors of post-transplant ***,as the studies were typically conducted in one center on relatively low-to-moderate number of patients,verification of the results in other centers is warranted to resolve these ***,of 29 reviewed studies,28 used gene candidate approach and only one implemented a genome wide association *** wide association multicentric studies are needed to facilitate the development of personalized transplant medicine.

Approach to persistent ascites after liver transplantation

Spinal cord injury results in the loss of sensory,motor,and autonomic functions,which almost always produces permanent physical ***,in the search for more effective treatments than those already applied for years,which are not entirely efficient,researches have been able to demonstrate the potential of biological strategies using biomaterials to tissue manufacturing through bioengineering and stem cell therapy as a neuroregenerative approach,seeking to promote neuronal recovery after spinal cord *** of these strategies has been developed and meticulously evaluated in several animal models with the aim of analyzing the potential of interventions for neuronal repair and,consequently,boosting functional *** the majority of experimental research has been conducted in rodents,there is increasing recognition of the importance,and need,of evaluating the safety and efficacy of these interventions in non-human primates before moving to clinical trials involving therapies potentially promising in *** article is a literature review from databases(PubMed,Science Direct,Elsevier,Scielo,Redalyc,Cochrane,and NCBI)from 10 years ago to date,using keywords(spinal cord injury,cell therapy,non-human primates,humans,and bioengineering in spinal cord injury).From 110 retrieved articles,after two selection rounds based on inclusion and exclusion criteria,21 articles were ***,this review arises from the need to recognize the experimental therapeutic advances applied in non-human primates and even humans,aimed at deepening these strategies and identifying the advantages and influence of the results on extrapolation for clinical applicability in humans.

Hydrocarbons in particulate samples from wildfire events in central Portugal in summer 2010

In summer 2010, twenty eight （14 PM=2.5 samples plus 14 samples PM2.5-10） smoke samples were collected during wildfires that occurred in central Portugal. A portable high-volume sampler was used to perform the sampling, on quartz fibre filters of coarse （PM=2.5-10） and fine （PM2.5） smoke samples. The carbonaceous content （elemental and organic carbon） of particulate matter was analysed by a thermal-optical technique. Subsequently, the particulate samples were solvent extracted and fractionated by vacuum flash chromatog- raphy into three different classes of organic compounds （aliphatics, polycyclic aromatic hydrocarbons （PAHs） and carbonyl compounds）. The organic speciation was performed by gas chromatography-mass spectrometry （GC-MS）. Emissions were dominated by the fine particles, which represented around 92% of the PM10. A clear predominance of carbonaceous constituents was observed, with organic to elemental carbon （OC/EC） ratios ranging between 1.69 and 245 in both size fractions. The isoprenoid ketone 6,10,14-trimethyl-2-pentadecanone, a tracer for secondary organic aerosol formation, was one of the dominant constituents in both fine and coarse particles. Retene was the most abundant compound in all samples. Good correlations were obtained between OC and both aliphatic and PAH compounds. Pyrogenic processes, thermal release of biogenic com- pounds and secondary processing accounted for 97% of the apportioned PM2.5 levels.
