Inflammatory bowel disease (IBD) affects a part of the young population and has a strong impact upon quality of life. The underlying etiology is not known, and the existing treatments are not curative. Furthermore, a significant percentage of patients are refractory to therapy. In recent years there have been great advances in our knowledge of stem cells and their therapeutic applications. In this context, autologous hematopoietic stem cell transplantation (HSCT) has been used in application to severe refractory Crohn’s disease (CD), with encouraging results. Allogenic HSCT would correct the genetic defects of the immune system, but is currently not accepted for the treatment of IBD because of its considerable risks. Mesenchymal stem cells (MSCs) have immune regulatory and regenerative properties, and low immunogenicity (both autologous and allogenic MSCs). Based on these properties, MSCs have been used via the systemic route in IBD with promising results, though it is still too soon to draw firm conclusions. Their local administration in perianal CD is the field where most progress has been made in recent years, with encouraging results. The next few years will be decisive for defining the role of such therapy in the management of IBD.

Microbial electrosynthesis(MES) can potentially provide a mean for storing renewable energy surpluses as chemical energy. However, the fluctuating nature of these energy sources may represent a threat to MES, as the microbial communities that develop on the biocathode rely on the continuous existence of a polarized electrode. This work assesses how MES performance, product generation and microbial community evolution are affected by a long-period(6 weeks) power off(open circuit). Acetogenic and H2-producing bacteria activity recovered after reconnection. However, few days later syntrophic acetate oxidation bacteria and H2-consuming methanogens became dominant, producing CH4 as the main product, via electromethanogenesis and the syntrophic interaction between eubacterial and archaeal communities which consume both the acetic acid and the hydrogen present in the cathode environment. Thus,the system proved to be resilient to a long-term power interruption in terms of electroactivity. At the same time, these results demonstrated that the system could be extensively affected in both end product generation and microbial communities.

The adsorption capacities for the removal of hexavalent chromium from aqueous solutions by six carbon nanomaterials have been evaluated. Single-walled and multi-walled carbon nanotubes as received and after oxidation treatment, graphene oxide and reduced graphene oxide are the materials with different dimension and functionalization compared in this research. Carbon nanotubes have been modified using hydrogen peroxide as oxidizing agent under microwave radiation. The oxidation treatment on carbon nanotubes has a positive effect increasing the adsorbent–adsorbate interaction. Rate-controlling mechanisms and equilibrium data are analyzed using non-linear models. non-linear method is proposed as the most suitable method for determining the kinetic and equilibrium parameters. The values of adsorption energy(E) obtained from the Dubinin–Radushkevich isotherm,have been found around 0.371 and 0.870 k J·mol-1, indicating physical adsorption. Therefore, the pseudo-second order model represents better the kinetic experimental data. The results show that the Langmuir isotherm provides a slightly better fit to the experimental data compared with the Freundlich isotherm, indicating homogeneous distribution of active sites on carbon nanomaterials and monolayer adsorption. The separation factors RLare found in the range of 0–1, suggesting that the adsorption process is suitable for all adsorbents. The mechanisms for hexavalent chromium removal have been proposed as electrostatic interactions and hydrogen bonding.

The role of clinical diagnosis criteria on the frequency of accommodative insufficiency

AIM: To estimate and compare the frequency of accommodative insufficiency(AI) within the same clinical population sample depending on the type of clinical criteria used for diagnosis. Comparing the frequency within the same population would help to minimize bias due to sampling or methodological variability. METHODS: Retrospective study of 205 medical records of symptomatic subjects free of any organic cause and symptoms persisting despite optical compensation evaluated. Based on the most commonly clinical diagnostics criteria found in the literature, four diagnostics criteria were established for AI(Ⅰ, Ⅱ, Ⅲ and Ⅳ) based on subjective accommodative tests: monocular accommodative amplitude two or more diopters below Hofstetter's minimum value [15-(0.25×age)](Ⅰ, Ⅱ, Ⅲ, Ⅳ); failing monocular accommodative facility with minus lens, establishing the cut-off in 0 cycles per minute(cpm)(Ⅰ) and in 6 cpm(Ⅱ, Ⅲ); failing binocular accommodative facility with minus lens, establishing the cut-off in 0 cpm(Ⅰ) and in 3 cpm(Ⅱ).RESULTS: The proportion of AI(95%CⅠ) for criteria Ⅰ, Ⅱ, Ⅲ and Ⅳ were 1.95%(0.04%-3.86%), 2.93%(0.31%-4.57%), 6.34%(1.90%-7.85%) and 41.95%(35.14%-48.76%) respectively, with a statistically significant difference shown between these values(χ2=226.7, Pn revealed that the proportion of AI detected for criterion Ⅳ was significantly greater than the proportion for the rest of the criteria(P-adjustedn all cases).COnCLUSIOn: The prevalence of cases of AI within the same clinical population varies with the clinical diagnostic criteria selected. The variation is statistically significant when considering the monocular accommodative amplitude as the only clinical diagnostic sign.

BACKGROUnD: This study aimed to compare the rates of open and laparoscopic cholecystectomies and outcomes in patients with or without type 2 diabetes mellitus (T2DM) in Spain from 2003 to 2013. METHODS: We collected all cases of open and laparoscopic cholecystectomies using national hospital discharge data and evaluated the annual cholecystectomy rates stratiifed by T2DM status. We analyzed tendency for in-hospital mortality (IHM). We also analyzed the impact of T2DM on IHM in patients who underwent cholecystectomies. RESULTS: We identiifed 611 533 cholecystectomies (71.3%laparoscopic) in the patients, in whom 78 227 (12.8%) patients had T2DM. The rates of open cholecystectomies were 3-fold higher (130.0/105 vs 41.1/105) in patients with T2DM than in those without T2DM, and the rate of laparoscopic cholecys-tectomies was almost 2-fold higher (195.2/105 vs 111.8/105) in patients with T2DM. The annual rate of laparoscopic pro-cedures showed an 11-year relative increase of 88.3% (from 117.0/105 to 220.3/105) in T2DM and 49.2% (from 79.2/105 to 118.2/105) in patients without T2DM (Pn procedures showed an 11-year relative decrease of 27.6% in patients with T2DM and 37.9% in those without T2DM (Pncy laparoscopic cho-lecystectomy was increased in the 11 years, whereas the rate of emergency open cholecystectomies was decreased (both Panalysis revealed that older age, higher comorbidity and emergency cholecystectomy were associated with a higher IHM. Compared with patients without T2DM, patients with T2DM demonstrated a lower IHM after open cholecystectomy [OR=0.82 (0.78-0.87)], but a higher IHM after laparoscopic cholecystectomy (OR=1.18 (1.03-1.35))Time-trend analyses showed a signiifcant reduction in IHM in patients with or without T2DM after the two procedures.
