Acute upper gastrointestinal bleeding in octοgenarians:Clinical outcome and factors related to mortality

AIM: To evaluate the aetiology, clinical outcome and factors related to mortality of acute upper gastrointestinal bleeding (AUGIB) in octogenarians. METHODS: We reviewed the records of all patients over 65 years old who were hospitalised with AUGIB in two hospitals from Jan.ary 2006 to December of 2006. Patients were divided into two groups: Group A (65-80 years old) and Group B (> 80 years old). RESULTS: Four hundred and sixteen patients over 65 years of age were hospitalized because of AUGIB. Group A included 269 patients and Group B 147 patients. Co-morbidity was more common in octogenarians (P = 0.04). The main cause of bleeding was peptic ulcer in both groups. Rebleeding and emergency surgery were uncommon in octogenarians and not different from those in younger patients. In-hospital complications were more common in octogenarians (P = 0.05) and more patients died in the group of octogenarians compared to the younger age group (P = 0.02). Inability to perform endoscopic examination (P = 0.002), presence of high risk for rebleeding stigmata (P = 0.004), urea on admission (P = 0.036), rebleeding (P = 0.004) and presenceof severe co-morbidity (P n multivariate analysis, only the presence of severe co-morbidity was independently related to mortality (P = 0.032). COnCLUSIOn: While rebleeding and emergency surgery rates are relatively low in octogenarians with AUGIB, the presence of severe co-morbidity is the main factor of adverse outcome.

Amorphous carbons are promising anodes for high-rate potassium-ion *** low-temperature annealed amorphous carbons display unsatisfactory ***-induced defect engineering of amorphous carbons could enhance their reversible ***,most lignocellulose biomasses lack heteroatoms,making it a challenge to design highly heteroatom-doped carbons(>10 at%).Herein,we report a new preparation strategy for amorphous carbon ***/sulfur co-doped lignin-derived porous carbons(nSLPC)with ultra-high nitrogen doping levels(21.6 at%of n and 0.8 at%of S)from renewable lignin biomacromolecule precursors were prepared through a supramolecule-mediated pyrolysis *** supermolecule/lignin composite decomposes forming a covalently bonded graphitic carbon/amorphous carbon intermediate product,which induces the formation of high heteroatom doping in the obtained *** unique pyrolysis chemistry and high heteroatom doping of nSLPC enable abundant defective active sites for the adsorption of K+and improved *** nSLPC anode delivered a high reversible capacity of 419 mAh g^(-1)and superior cycling stability(capacity retention of 96.6%at 1 A g^(-1)for 1000 cycles).Potassiumion hybrid capacitors assembled by nSLPC anode exhibited excellent cycling stability(91%capacity retention for 2000 cycles)and a high energy density of 71 Wh kg^(-1)at a power density of 92 W kg^(-1).

Soil Organic Carbon Loss and Turnover Resulting from Forest Conversion to Maize Fields in Eastern Thailand

Soil organic carbon(SOC) content and its stable carbon isotopic composition(within the upper 1 m) were measured to determine the e?ect of land-use changes from dry evergreen forest to maize fields in eastern *** land cover maps,derived from aerial photography and satellite images for years 1989,1996,and 2002 were used in association with field surveys and farmer interviews to derive land-use history and to assist in study site *** from forest to maize cultivation for the duration of 12 years reduced SOC stocks at the rate of 6.97 Mg C ha-1 *** was most pronounced in the top 10 cm soil layer,which was 47% after 12 years of *** carbon isotope data revealed that the main fraction lost was forest-derived *** low input rates of maize-derived C were not sufficient to maintain SOC at the level prior to forest *** 12 years of continuous maize cultivation,the maize-derived C fraction made up about 20% of total SOC(5 Mg ha-1 of the total 25.31 Mg ha-1).

In this work, the effect of nitriding current density on hardness, crystalline phase composition, layer thickness and corrosion rate of AlSl 4340 steel has been studied. X-ray diffraction analysis shows that thin layers formed during nitriding process are constituted of 7-Fe4n for samples processed between 1 and 2.5 mA/cm2. Thickness of nitrided layer increases proportionally to current density （0 μm for 0.5 mA/cm2 to 15 pm for 2.5 mA/cm2）. Plasma nitriding increased the surface hardness from 300 HV50g for untreated sample, to around 800HV5og for nitrided samples at 1 mA/cm2. While the untreated samples exhibited a corrosion rate of 0.153 mm per year, the corrosion performance was improved up to 0.03 mm per year at current densities above 1 mA/cm2, which is about one fifth of the corrosion rate of the untreated sample.

According to various worldwide statistics,most car accidents occur solely due to human *** person driving a car needs to be alert,especially when travelling through high traffic volumes that permit high-speed transit since a slight distraction can cause a fatal *** though semiautomated checks,such as speed detecting cameras and speed barriers,are deployed,controlling human errors is an.arduous *** key causes of driver’s distraction include drunken driving,conversing with co-passengers,fatigue,and operating gadgets while *** these distractions are accurately predicted,the drivers can be alerted through an.alarm ***,this research develops a deep convolutional neural network(deep Cnn)models for predicting the reason behind the driver’s *** deep Cnn models are trained using numerous images of distracted *** performance of deep Cnn models,namely the VGG16,Resnet,and Xception network,is assessed based on the evaluation metrics,such as the precision score,the recall/sensitivity score,the F1 score,and the specificity *** Resnet model outperformed all other models as the best detection model for predicting and accurately determining the drivers’activities.

Finite element analysis of overlay incorporating stress absorbing membrane interlayers against reflective cracking

A wheel tracking test was modelled to gain better understanding of the deflection and stress-strain distribution in an overlaid cracked pavements with and without membrane interlayer （SAMI）. For this purpose, commercial finite element software Abaqus 6.7-1 was used. Two different models were considered, one incorporating stress absorbing membrane interlayers （SAMIs） and the other without SAMI. In the study, full bond condition was assumed for the boundaries between the layers, and a linear elastic model was used for the analysis. The results show that introduction of SAMI caused greater deflection of the pavement. It is found that although with SAMIs, low stiffness is required, a very low stiffness may yield undesirable results. The results show that the introduction of SAMIs results in high strain concentration around the crack region, whilst the strain in the overlay is smaller than the values predicted in the models without SAMIs.

Portal vein ligation accelerates tumor growth in ligated,but not contralateral lobes

AIM: To investigate the mechanisms of liver growth and atrophy after portal vein ligation (PVL) and its effects on tumor growth. METHODS: Mice were subjected to PVL, partial hepatectomy, or sham surgery. The morphological alterations, activation of transcription factors, and expression of cytokines and growth factors involved in liver regeneration were evaluated. In a separate set of experiments, murine colorectal carcinoma cells were injected via the portal vein and the effect of each operation on liver tumor growth was studied. RESULTS: Liver regeneration after PVL and partial hepatectomy were very similar. In ligated lobes, various cytokines, transcription factors and regulatory factors were signifi cantly upregulated compared to nonligated lobes after PVL. Atrophy in ligated lobes was a result of early necrosis followed by later apoptosis. Tumor growth was signifi cantly accelerated in ligated compared to non-ligated ***: Tumor growth was accelerated in ligated liver lobes and appeared to be a result of increased growth factor expression.
