Interest of Cardiac Rehabilitation in Patients with Heart Disease in a Cardiology Department in Senegal

Cardiac rehabilitation helps improve the prognosis and quality of life for patients with heart disease. To show its interest in the African context, in the management of heart disease, a prospective study was carried out. Its objective was to evaluate the effects of physical rehabilitation on cardiac function. Methods: Patients were evaluated at the beginning and at the end of cardiovascular rehabilitation by cardiovascular, biological, and echocardiographic parameters including the size of the heart chambers, the kinetics of the walls, and the systolic function of the ventricles. Results: The study involved 12 patients, 67% of whom were men. After cardiac rehabilitation, the mean hemodynamic constants had not been significantly modified. However, a significant decrease in Total-Cholesterol, LDL-Cholesterol and triglyceride levels was noted. Mean fasting blood sugar decreased from 1.25 ± 0.48 g/L to 0.92 ± 0.18 g/L and glycated hemoglobin from 7.72% ± 0.01% to 6.45% ± 0.008%. The echocardiographic parameters studied showed an improvement in the dilation of the heart chambers in 8.33% of the patients, the normalization of the ejection fraction of the left ventricle in 16% patients, the improvement of the kinetic disorders in 16% of patients and recovery of right ventricular systolic function in all patients. Conclusion: Cardiac rehabilitation as a secondary preventive measure for cardiovascular disease has contributed significantly to improving the clinico-biological parameters of the disease.

Assessment of Satisfaction in the Reception and Emergency Department of a Health Center

Introduction The main objective of any healthcare establishment must be to ensure the quality of patient care and customer satisfaction. It is necessary to regularly assess patient satisfaction. The aim of this study was to assess the level of satisfaction of customers aged over 18 years attending the emergency department of the health center. Methodology This was a descriptive and analytical cross-sectional study of patients aged 18 years and over, who attended the Samu Municipal emergency department between 02 and 30 May 2023. The satisfaction index was determined using the adapted 2009 SAPHORA-MCO questionnaire and the Likert satisfaction scale. Results A total of 400 patients were surveyed. The average age was 35 years, with a standard deviation of 14.7. Of those surveyed, 51% were women, 87% were educated, 50% lived in Grand Yoff and 59.5% were unemployed. Satisfaction levels linked to perception of the cost of care (72%), waiting time (64.3%), information given to patients (69.1%) and pain management (74 .5%) are fair. On the other hand, the levels of satisfaction linked to administrative procedures (82.5%), staff attitudes towards patients (84%), staff availability (86.4%), patient privacy (89.2%), general atmosphere (87.2%), staff competence (87.3%), and the effectiveness of care (89.4%) were satisfactory. The average waiting time was 38 minutes. However, 32% of patients waited less than 30 minutes and 92% less than an hour. The satisfaction index linked to administration and reception was 72.9% and 79.85%, respectively. The satisfaction index linked to the administration and technical quality of care is equal to 85.8% and 83.7%, respectively. The overall satisfaction index is equal to 80.6%;the level of satisfaction of users of the health structure is satisfactory. Conclusion Patient satisfaction is an essential part of quality care. Study of Factors Associated with Knowledge, Attitudes and Practices Regarding Rabies in the Commune of Niakhéne in Senegal, in 2022

Introduction: Rabies is a serious disease, as it is always fatal, but it can be prevented by sero-vaccination. It is a neglected tropical disease endemic in Asia and Africa. The aim of this study was to assess knowledge, attitudes and practices regarding rabies and to determine the factors associated with them among people aged 18 and over in the commune of Niakhène. Methods: This was a cross-sectional, descriptive and analytical survey of subjects aged 18 and over living in the commune of Niakhène. A sample of 300 individuals was drawn from a two-stage cluster survey stratified by age and sex. Bivariate analysis was performed using association tests. Results: The mean age of respondents was 35.3 ± 16.9 years. It was noted that 67% (201) of respondents had a good knowledge of rabies. The results showed that 7.3% (22) of respondents owned a dog. Of the 278 people who did not own a dog, 78.4% (218) said they would have vaccinated their dog if they had had one. It should be noted that 83.7% (251) of respondents said they would go to a health facility if an animal bit them. None of the dog owners had vaccinated their dogs against rabies. Of the 41 people exposed to rabies, 39% went to a health facility. The age and education of the respondents had statistically significant associations with knowledge of rabies. Respondents’ age and education were statistically significantly related to whether they had vaccinated a domestic dog. The age, education and economic well-being quintile of respondents’ households had statistically significant associations with the use of a health facility in the event of being bitten or scratched by an animal vector. The education of respondents who had been bitten by an animal vector was statistically significantly associated with the use of a health facility. Background: The welding trade includes many industrial activities in both the formal and informal sectors. Workers in this professional environment do not have access to individual and collective means of protection adapted to the activity, which exposes them to the dust emitted during their activity and therefore contributes to increasing the risk of developing respiratory disorders in this population. Objective: Our study aimed to evaluate the ventilatory function of metal welders in the city of Dakar. Method: This was a comparative study based on the analysis of the spirometric parameters of metal welders working in the city of Dakar (Senegal) with those of a control group of the same kind, selected from the general population. Measurements of forced expiratory volume in the first second (FEV1), forced vital capacity (FVC), peak expiratory flow (PEF) and maximum mid expiratory flow (MMEF) were measured. Results: A total of 75 men between the ages of 19 and 61 were recruited and divided into two groups (43 controls and 32 welders). The welding population had more frequent distal and central obstructions and pulmonary restriction compared to the control group. The metals most commonly used by welders were aluminum for welders who had an obstruction, while for those who had a restriction, iron was the most used metal. Signs of metal fever were frequently found in welders, especially those with ventilatory disorders. Conclusion: The prevalence of respiratory disorders observed is close to that observed in industrial environments, where exposure levels are the highest. The lack of personal protective equipment could also be a significant risk factor in the development of these disorders.

Influence of Pterocarpus erinaceusPoir. on the Growth and Yields of Groundnut (Arachis hypogeaL.) in Agroforestry Parks in the Districts of Tenghory and Kataba 1 (Bignona Department, Lower Casamance)

In Lower Casamance, Pterocarpus erinaceus occupies a very important place in agroforestry parks. It is often grown in association with the most important cash crop of the area, groundnut (Arachis hypogea L.). Thus, the objective of this study is to contribute to a better understanding of the interactions between trees and groundnut cultivation in agroforestry parks in Lower Casamance. To this end, in each of the two districts (Tenghory and Kataba 1) three Pterocarpus erinaceus trees were selected in three separate communes. For each P. erinaceus tree selected and following each of the four cardinal directions, three situations were defined: at R/2, at 1.5 R and at 2 R from the trunk, with R the radius of the crown. Thus, 12 yield squares were installed around each selected tree, for a total of 72 yield squares installed in the two districts. The parameters measured were the height of the peanut plants, the number of pods produced per plant, and the yield in flakes and pods. The results show that geographical orientation significantly influenced plant height (p = 0.004) but not the number of pods/plant (p = 0.6). As for the distance to the trunk of Pterocarpus erinaceus, it induced a significant variation in the number of pods/plant (p = 0.009) but did not induce a significant effect on the other parameters. Yields varied between 1.56 Tons M S /ha and 2.02 Tons M S /ha for the hay, and between 1.03 Tons M S /ha and 1.45 Tons M S/ha for the pods. The results of this study constitute a useful source of information for a better management of the productivity of the agroforestry parks of the department of Bignona.

Management of High Complicated Genital Infections at the Aristide De Le Dantec Hospital: Apropos of Six Cases

Objective: To study the epidemiology, the diagnostic elements and the prognosis after laparotomy surgery, of complicated upper genital infections. Patients and method: This is a retrospective study of six cases of complicated upper genital infections collected over a period of 20 months in the Obstetrics and Gynecology department of the Aristide Hospital Center of LEDANTEC. Results: The average age of the patients was 37.67, in two patients;a hysterosalpingography had been performed less than seven days before the start of the symptoms, a notion of multiple sexual partners was noted in two patients. All patients presented with acute abdominal-pelvic pain associated with vomiting in three patients. An infectious syndrome was found in two patients, adnexal pain caused in four patients and signs of peritoneal irritation (guarding and umbilical tenderness) in three patients. Para-clinically, a biological inflammatory syndrome was found in all patients, including an elevation of C-reactive protein in all patients and hyperleukocytosis in four patients and an elevation of CA 125 in two patients. The cytobacteriological examination of the pus taken intraoperatively was positive in two patients, the isolated germs were Pseudomonas aeruginosa and Enterobacter cloacae. Pelvic ultrasound was performed in all patients and revealed a cystic adnexal mass in five cases, a pyosalpinx and peritoneal effusion in two cases. Abdominal & pelvic computed tomography was performed in four patients and revealed a lateral-uterine adnexal mass in two cases, pyosalpinx in two cases, uretero-hydronephrosis and peritoneal effusion in one case. An exploratory laparotomy was performed in each patient and confirmed the diagnosis of tuboovarian abscess, the presence of pelvic adhesion and purulent effusion in four patients, the presence of false membranes in two patients, pyometra and isolated bilateral pyosaplinx were found in one patient. Practice of Screening for Precancerous Cervical Lesions in Koumpentoum Health District (Senegal)

Introduction: Cervical cancer is preventable through vaccination against papillomaviruses and screening for precancerous lesions. The objective of this study was to determine the frequency of screening practice and to identify associated factors. Methods: This was a cross-sectional descriptive and analytical study. Data were collected from individual interviews during household surveys. Data collection took place from (1st) to (30th) September (2021). The following were explored: socio-demographic characteristics, knowledge, attitudes and practices of women regarding screening. Results: A total of (294) women aged between (18) and (69) years were included. The mean age was (35.79) years (±9.81). The median age was (34) years. The frequency of screening was (16%);that of precancerous lesions was (4.26%). Factors statistically and significantly associated with screening uptake were cervical cancer information (OR = 5.42 (1.27 - 23.16);p = 0.003), knowledge of the availability of screening (ORa = 7.73 (3.01 - 18.51);p - 7.31);p = 0.014). Conclusion: Frequency of pre-cancerous lesions of cervical cancer is high in Koumpentoum health district (4.26%), while the practice of screening is low there (16%). The factors associated with this low performance are related to a lack of adequate knowledge of women about cervical cancer.

True Nodes of the Umbilical Cords of a Mono-Amniotic Pregnancy

The incidence of twin pregnancies, has increased due to assisted reproductive technologies [1]. We note a doubling between 1970 and 2010 from less than 8‰ to nearly 16‰ of pregnancies (Pison et al., 2014;Pison et al., 2015) in developed countries [2] and 17‰ to 20‰ in sub-Saharan Africa. Monozygotic twins are the product of fertilization of a single egg by a single sperm, with the egg splitting into two within the first few days after fertilization. If the division takes place after the 8th day, the pregnancy is mono-chorionic mono-amniotic (less 1% of cases of monozygotic pregnancies) and represents 1 in 30,000 pregnancies. It is the development of two fetuses within the same amniotic sac, representing the rarest type of twin pregnancy (1% of monozygotic twin pregnancies) but being the one with the highest fetal and maternal morbidity. The main complication feared is coiling and knots of the umbilical cords responsible for abnormal heartbeat, fetal suffering and eventual fetal death. We report the case of in utero fetal death of twins from a mono choral mono amniotic pregnancy secondary to knots and coiling of the cord in order to discuss the diagnostic and therapeutic aspects.

Associated Factors with Vaccination among Girls Aged between (11) and (13) against Papillomaviruses in Koumpentoum Health District (Senegal)

Introduction: Cervical cancer can be prevented by early vaccination of young people against papillomaviruses and screening for precancerous lesions. After a successful pilot phase, vaccination coverage in the generalization phase is low. The aim of this study was to determine papillomavirus vaccination coverage and to identify associated factors. Methods: This was a cross-sectional, descriptive and analytical study conducted from (1st) to (31th) September (2020) in Koumpentoum district. After a literature review, we conducted two-stage cluster sampling and direct structured interviews. Socio-demographic characteristics, knowledge, attitudes, and practices of mothers or guardians about papillomaviruses vaccination were collected using a standardized questionnaire. Multiple logistic regression was used to estimate odds ratios. Results: A total of (228) mothers or guardians were interviewed. Coverage for the first dose was (44.74%) CI95% (38.17 - 51.44) compared to (25.88%) CI95% (19.52 - 31.17) for the second dose. Factors statistically and significantly associated with coverage of the first dose of papillomaviruses vaccine were instruction of mothers or guardians (OR = 5.62 (3.16 - 9.99);p 0.001), schooling of the young girls (OR = 4.1 (2.23 - 7.53);p 0.001), information on cervical cancer (OR = 18.97 (5.68 - 63.24);p 0.001), knowledge of risks factors (OR = 8.04 (4.41 - 14.63);p 0.001), information on papillomaviruses vaccine (ORa = 10.26 (1.69 - 62.23);p = 0.011), knowledge on vaccine target (OR = 17.11 (8.51 - 34.41);p 0.001), knowledge of schedule vaccine (ORa = 3.67 (1.2 - 22.51);p = 0.022), knowledge of prevention methods (OR = 26.86 (12.22 - 59.05);p 0.001), and to be favorable in expanded vaccination program in general (ORa = 18.71 (1.5 - 128.41];p = 0.02). Comparative Effects of Natural and Synthetic Fungicides on the Pink Root Disease of Onion (Allium cepa L.), in Nursery

Onion is an important vegetable in Senegal horticulture, where it occupies the first place. However in the greatest areas production like the Gandiolais, in the North of Senegal, Pink root disease caused by a soil born fungus Pyrenochaeta terrestris (Hansen) Gorentz, Walker and Larson affects its productivity especially in nursery. On soil sterilized and inoculated by the fungus, we tested 5 natural fungicides and 2 synthetic fungicides, to measure their effectiveness on the Pink root disease on onion, in nursery. The device used is a split plot with 4 repetitions and 8 treatments. The measured variables are the emergence rates, the survival rates and the infestation rates. Our experimentations have shown that, for the emergence rates, there is no significant difference between thiophanate-methyl (Pelt 44), Carbendazim (Bavistine), Azadirachta indica A. Juss and Carica papaya L. It is the same between C. papaya and Moringa oleifera Lam, between M. oleifera and Cassia alata L. It is also the same between C. alata and Crotolaria retusa L. Regarding the survival rates, there is no significant difference between thiophanate-methyl, Azadirachta indica, Carbendazim, C. papaya and M. oleifera. The same is true between M. oleifera and Cassia alata, and between C. alata and C. retusa. Concerning the infestation rates, apart from the control check, all other treatments show no difference.
