AIM: To assess the cost-effectiveness of two populationbased hepatocellular carcinoma(HCC) screening programs, two-stage biomarker-ultrasound method and mass screening using abdominal ultrasonography(AUS).METHODS: In this study, we applied a Markov decision model with a societal perspective and a lifetime horizon for the general population-based cohorts in an area with high HCC incidence, such as Taiwan. The accuracy of biomarkers and ultrasonography was estimated from published meta-analyses. The costs of surveillance, diagnosis, and treatment were based on a combination of published literature, Medicare payments, and medical expenditure at the National Taiwan University Hospital. The main outcome measure was cost per lifeyear gained with a 3% annual discount rate. RESULTS: The results show that the mass screening using AUS was associated with an incremental costeffectiveness ratio of USD39825 per life-year gained, whereas two-stage screening was associated with an incremental cost-effectiveness ratio of USD49733 per life-year gained, as compared with no screening. Screening programs with an initial screening age of 50 years old and biennial screening interval were the most cost-effective. These findings were sensitive to the costs of screening tools and the specificity of biomarker ***: Mass screening using AUS is more cost effective than two-stage biomarker-ultrasound screening. The most optimal strategy is an initial screening age at 50 years old with a 2-year inter-screening interval.

Lipid concentration and composition in xylem sap of woody angiosperms from a tropical savanna and a seasonal rainforest

Lipids may play an important role in preventing gas embolisms by coating nanobubbles in xylem *** studies on xylem sap lipids have been reported for temperate plants,and it remain unclear whether sap lipids have adaptational significance in tropical *** this study,we quantify the lipid composition of xylem sap for angiosperm species from a tropical savanna(seven species)and a seasonal rainforest(five species)using mass *** found that all twelve species studied contained lipids in their xylem sap,including galactolipids,phospholipids and triacylglycerol,with a total lipid concentration ranging from 0.09 to 0.26 nmol/*** was no difference in lipid concentration or composition between plants from the two sites,and the lipid concentration was negatively related to species’open vessel ***,savanna species showed little variation in lipid composition between the dry and the rainy *** results support the hypothesis that xylem sap lipids are derived from the cytoplasm of individual conduit cells,remain trapped inside individual conduits,and undergo few changes in composition over consecutive seasons.A xylem sap lipidomic data set,which includes 12 tropical tree species from this study and 11 temperate tree species from literature,revealed no phylogenetic signals in lipid composition for these *** study fills a knowledge gap in the lipid content of xylem sap in tropical trees and provides additional support for their common distribution in xylem sap of woody *** appears that xylem sap lipids have no adaptive significance.

Objective: IMpower210(NCT02813785) explored the efficacy and safety of single-agent atezolizumab *** as second-line treatment for advanced non-small cell lung cancer(NSCLC) in East Asian ***: Key eligibility criteria for this phase Ⅲ, open-label, randomized study included age ≥18 years;histologically documented advanced NSCLC per the Union for International Cancer Control/American Joint Committee on Cancer staging system(7th edition);Eastern Cooperative Oncology Group performance status of 0 or 1;and disease progression following platinum-based chemotherapy for advanced or metastatic NSCLC. Patients were randomized 2:1 to receive either atezolizumab(1,200 mg) or docetaxel(75 mg/m^(2)). The primary study endpoint was overall survival(OS) in the intention-to-treat(ITT) population with wild-type epidermal growth factor receptor expression(ITT EGFR-WT) and in the overall ITT ***: Median OS in the ITT EGFR-WT population(n=467) was 12.3 [95% confidence interval(95% CI),10.3-13.8] months in the atezolizumab arm(n=312) and 9.9(95% CI, 7.8-13.9) months in the docetaxel arm[n=155;stratified hazard ratio(HR), 0.82;95% CI, 0.66-1.03]. Median OS in the overall ITT population was 12.5(95% CI, 10.8-13.8) months with atezolizumab treatment and 11.1(95% CI, 8.4-14.2) months(n=377) with docetaxel treatment(n=188;stratified HR, 0.87;95% CI, 0.71-1.08). Grade 3/4 treatment-related adverse events(TRAEs) occurred in 18.4% of patients in the atezolizumab arm and 50.0% of patients in the docetaxel ***: IMpower210 did not meet its primary efficacy endpoint of OS in the ITT EGFR-WT or overall ITT populations. Atezolizumab was comparatively more tolerable than docetaxel, with a lower incidence of grade3/4 TRAEs.

Inorganic relaxor ferroelectric solid solution single crystals are spurring new generations of high performance electromechanical devices,including transducers,sensors,and actuators,due to their ultrahigh electric field induced strain,large piezoelectric constant,high electromechanical coupling factor and low dielectric ***,relaxor ferroelectric single crystals found in organic-inorganic hybrid perovskites are very limited,but achieving these superior properties in them will be of great significance in the design of modern functional ***,here the first two-dimensional(2D)organic-inorganic hybrid relaxor ferroelectric single crystal,[Br(CH_(2))_(3)NH_(3)]_(2)PbBr_(4)(BPA_(2)-PbBr_(4),BPA=3-bromopropylamine),achieves some of superior ***,BPA_(2)-PbBr_(4)reveals a successive relaxor ferroelectric-ferroelectric-paraelectric phase transitions accompanying by a large degree of relaxationΔT_(relax)=61 K and ultralow energy loss(tanδ<0.001).Meanwhile,it exhibits a superior second harmonic generation(SHG)effect with maximum value accounts for 95%of the standard KDP due to great deformation of structure(3.2302×10^(-4)).In addition,temperature dependent luminescence spectra(80-415 K)exhibit fluorescence and phosphorescence overlapping emission originated from inorganic and organic components with the nanosecond-scale short lifetime and the millisecond-scale long lifetime,respectively,and the color of the emitted light is continuously adjustable,which is the first to achieve luminescence and relaxor ferroelectricity compatibility.

Predicting interfacial thermal resistance by machine learning

Various factors affect the interfacial thermal resistance(ITR)between two materials,making ITR prediction a high-dimensional mathematical *** learning is a cost-effective method to address ***,we report ITR predictive models based on experimental *** physical,chemical,and material properties of ITR are categorized into three sets of descriptors,and three algorithms are used for the *** descriptors assist the models in reducing the mismatch between predicted and experimental values and reaching high predictive performance of 96%.Over 80,000 material systems composed of 293 materials were inputs for *** the top-100 high-ITR predictions by the three different algorithms,25 material systems are repeatedly predicted by at least two *** of the 25 material systems,Bi/Si achieved the ultra-low thermal conductivity in our previous *** believe that the predicted high-ITR material systems are potential candidates for thermoelectric *** study proposed a strategy for material exploration for thermal management by means of machine learning.

Symmetry of cardiac function assessment

Both right and left ventricles are developed from two adjacent segments of the primary heart tube. Though they are different with regard to shape and power, they mirror each other in terms of behavior. This is the first level of symmetry in cardiac function assessment. Both cardiac muscle contraction and relaxation are active. This constructs the second level of symmetry in cardiac function assessment. Combination of the two levels will help to find some hidden indexes or approaches to evaluate cardiac function. In this article, four major indexes from echocardiography were analyzed under this principal, another seventeen indexes or measurement approaches came out of the shadow, which is very helpful in the assessment of cardiac function, especially for the right cardiac function and diastolic cardiac function.

Stressful life events,psychosocial health and general health in preschool children before age 4

Background The impact of stressful life events(SLEs)in early childhood is often *** aimed to examine longitudinal associations between SLEs and psychosocial and general health in preschool *** Twelve SLEs occurring before the age of 24 months were assessed and categorized by frequency(no events,1-2 SLEs,and>2 SLEs)and overall tension(no events,low,and high)(n=1431).Psychosocial and general health were meas-ured three times at the age of 24,36 and 45 *** associations were examined by logistic regression models using generalized estimating equations to handle repeated *** Half(48.4%)of the families experienced SLEs,and 23.8%perceived high-tension SLEs before the children were aged 24 *** differences were observed in the association between SLEs and psychosocial *** to girls without SLEs,girls who experienced>2 SLEs[OR=3.31,95%confidence interval(CI)2.05-5.35]or high-tension SLEs(OR=3.01,95%CI 2.07-4.39)had higher odds of psychosocial problems from 24 to 45 *** odds ratios in boys were 2.10(95%CI 1.36-3.24)and 1.47(95%CI 1.06-2.03),***,only girls'risk of psychosocial problems increased after experiencing 1-2 SLEs(OR=2.15,95%CI 1.54-3.00)or low-tension SLEs(OR=1.90,95%CI 1.31-2.74).Regarding general health,children who experienced>2 SLEs(OR=1.96,95%CI 1.21-3.18)and high-tension SLEs(OR=1.60,95%CI 1.12-2.28)had higher odds of poor general health from 24 to 45 *** The findings emphasized that young children's psychosocial and general health can be impacted by experienc-ing SLEs in early *** and adequate support for families experiencing SLEs are needed to minimize the potential negative effect of SLEs on child health,particularly in girls.

Trogocytosis of CD80 and CD86 by induced regulatory 7 cells

Trogocytosis is a process which involves the transfer of membrane fragments and cell surface proteins between cells. Various types of T cells have been shown to be able to acquire membrane-bound proteins from antigen-presenting cells and their functions can be modulated following trogocytosis. However, it is not known whether induced regulatory T cells （iTregs） can undergo trogocytosis, and if so, what the functional consequences of this process might entail. In this study, we show that iTregs can be generated from CD80-/-CD86-/- double knockout （DKO） mice. Using flow cytometry and confocal fluorescence microscopy, we demonstrate that iTregs generated from DKO mice are able to acquire both CD80 and CD86 from mature dendritic cells （mDCs） and that the acquisition of CD86 occurs to a higher extent than that of CD80. Furthermore, we found that after co-incubation with iTregs, dendritic cells （DCs） downregulate their surface expression of CD80 and CD86. The trogocytosis of both CD80 and CD86 occurs in a cytotoxic T lymphocyte-associated antigen-4 （CTLA-4）, CD28 and programmed death ligand-1 （PDL1）-independent manner. Importantly, we showed that iTregs that acquired CD86 from mDCs expressed higher activation markers and their ability to suppress naive CD4＋ T-cell proliferation was enhanced, compared to iTregs that did not acquire CD86. These data demonstrate, for the first time, that iTregs can acquire CD80 and CD86 from mDCs, and the acquisition of CD86 may enhance their suppressive function. These findings provide novel understanding of the interaction between iTregs and DCs, suggesting that trogocytosis may play a significant role in iTreg-mediated immune suppression.

Risk factors and therapeutic response in Chinese patients with peptic ulcer disease

AIM:To assess the risk factors and the eff icacy of medications of patients with gastric and duodenal ulcers among Chinese patients in ***:Patients with peptic ulcers,diagnosed by upper endoscopy,were retrospectively collected between January 2008 and December *** differences were ***:Among all 448 cases,254(56.6%) and 194(43.4%) patients had gastric ulcers and duodenal ulcers *** with gastric ulcers were younger than those with duodenal *** more men existed,there was a female predominance in middle-aged *** with duodenal ulcers had a higher rate of Helicobacter pylori(***) infection(62.4% vs 43.3%,P = 0.001),and those with gastric ulcers owned a significantly higher amount of aspirin and nonsteroidal anti-inflammatory drug(NSAID) use(7.5% vs 1.5%,6.7% vs 2.1%,P = 0.001).Tobacco smoking and alcohol drinking had no different impact between these two ***-pump inhibitors and H2-receptor antagonists(H2RA) were effective,but significantly less so in cases with duodenal ulcers receiving H2RAs,or in those with *** infection and a history of NSAID ***:Patients with gastric ulcers had lower *** infection but more aspirin or NSAID *** therapy was ineffective in gastric ulcers underwent H2RA treatment,and cases combined *** infection and NSAID use.

Motivations for Loan Herding by Chinese Banks and Its Impact on Bank Performance

This study uses a dynamic herding model that considers intertemporal and crosssectional correlation to confirm that loan herding occurs among joint-stock commercial banks (JSCBs) and city commercial banks (CCBs). We clarify the motivations for bank loan herding. We find that loan herding by both JSCBs and CCBs results more from following the behavior of other same-type banks than different-type banks because of characteristic herding or reputational concerns. Loan herding by JSCBs is motivated by investigative herding, whereas loan herding by CCBs results from informational cascades. Moreover, loan herding has a significantly harmful impact on the operating performance of CCBs but not JSCBs, which may be explained by the irrational behavior of CCBs. Our results will help Chinese bank supervisors develop appropriate policies for handling loan herding.
