Distance caregivers(DCGs) are a growing phenomenon in the United States Family members are struggling to provide care to loved ones with chronic illnesses such as cancer,from a *** local caregiving research,distance caregiving research is limited and inconsistent definitions of distance make it difficult to compare *** date,DCGs have not been afforded the opportunities for educational and emotional support that local caregivers have received from the health care *** they are not usually present at medical appointments,DCGs do not receive first-hand information from the health care team about the patient's condition,disease progression,and/or treatment *** caregivers report feeling left out of important family *** experience anxiety related to the uncertainty of the family members' well-being and guilt related to not being available to help local caregivers *** challenges of distance caregiving are especially evident when the distance caregiver has a parent with advanced ***-centered care,attending to the needs of the whole family regardless of their geographic location is critical for quality cancer *** this manuscript,the sparse literature on distance caregiving is *** for future research and for the development of creative technologically advanced interventions for this underserved caregiving population are suggested.

Sour cherry(Prunus cerasus l.)is a valuable fruit crop in the Rosaceae family and a hybrid between progenitors closely related to extant Prunus fruticosa(ground cherry)and Prunus avium(sweet cherry).Here we report a chromosome-scale genome assembly for sour cherry cultivar Montmorency,the predominant cultivar grown in the *** also generated a draft assembly of *** to use alongside a published *** sequence for syntelog-based subgenome assignments for‘Montmorency’and provide compelling evidence *** is also an *** hierarchal k-mer clustering and phylogenomics,we show‘Montmorency’is trigenomic,containing two distinct subgenomes inherited from a ***-like ancestor(A and A’)and two copies of the same subgenome inherited from a ***-like ancestor(BB).The genome composition of‘Montmorency’is AA’BB and little-to-no recombination has occurred between progenitor subgenomes(A/A’and B).In Prunus,two known classes of genes are important to breeding strategies:the self-incompatibility loci(S-alleles),which determine compatible crosses,successful fertilization,and fruit set,and the Dormancy Associated MADS-box genes(DAMs),which strongly affect dormancy transitions and flowering *** S-alleles and DAMs in‘Montmorency’and *** were manually annotated and support subgenome ***,the hybridization event‘Montmorency’is descended from was estimated to have occurred less than 1.61 million years ago,making sour cherry a relatively recent ***‘Montmorency’genome highlights the evolutionary complexity of the genus Prunus and will inform future breeding strategies for sour cherry,comparative genomics in the Rosaceae,and questions regarding neopolyploidy.

Wet sedge meadows are the most productive plant communities in the High ***,the controls on carbon dioxide(CO_(2))exchange processes within wet sedge communities-and the scale at which they operate-are poorly ***,the factors controlling CO_(2)exchange of wet sedge meadows experiencing different moisture regimes are *** data are used to create predictive models of CO_(2)exchange on multiple temporal *** chamber systems recorded CO_(2)fluxes at 30-minute intervals at wet sedge sites in the Canadian High Arctic from June to August in 2014 and *** chambers were also deployed over a larger spatial extent in *** results show that wet sedge communities were strong CO_(2)sinks during the growing season(−7.67 to−44.36 g C·m^(−2)).CO_(2)exchange rates in wetter and drier areas within wet sedge meadows differed significantly(Wilcoxon,pl moisture regimes within vegetation types influence net CO_(2)*** Forest models explained a significant amount of the variability in CO_(2)flux rates over time(R2=0.46 to 0.90).The models showed that the drivers of CO_(2)exchange in these communities vary *** moisture regimes indirectly influenced CO_(2)fluxes given that they exhibit different vegetation and temperature-response *** suggest that the response of a single vegetation type to environmental changes may vary depending on microenvironment variability within that community.

Essential oil has been used as sedatives,anticonvulsants,and local anesthetics in traditional medical remedies;as preservatives for food,fruit,vegetable,and grain storage;and as bio-pesticides for food ***(ll),along with a few other major components such as methyl eugenol(ME),estragole(EG),and citronellal,are the active chemicals in many essential oils such as basil *** oil and the aforementioned monoterpenoids are potent against insect ***,the molecular mechanism of action of these chemical constituents is not well *** is well-known that the c-aminobutyric acid type A receptors(GABAARs)and nicotinic acetylcholine receptor(nAChR)are primary molecular targets of the synthetic insecticides used in the market ***,the GABAAR-targeted therapeutics have been used in clinics for many decades,including barbiturates and benzodiazepines,to name just a *** this research,we studied the electrophysiological effects of ll,ME,EG,and citronellal on GABAAR and nAChR to further understand their versatility as therapeutic agents in traditional remedies and as *** results revealed that ll inhibits both GABAAR and nAChR,which may explain its insecticidal *** is a concentration-dependent,noncompetitive inhibitor on GABAAR,as the half-maximal effective concentration(EC50)values of c-aminobutyric acid(GABA)for the rat a1b3c2l GABAAR were not affected by ll:(36.2±7.9)lmol-1 and(36.1±23.8)μmol·l-1 in the absence and presence of 5 mmol·l-1 ll,*** half-maximal inhibitory concentration(IC50)of ll on GABAAR was approximately 3.2 mmol·*** that multiple monoterpenoids are found within the same essential oil,it is likely that ll has a synergistic effect with ME,which has been previously characterized as both a GABAAR agonist and a positive allosteric modulator,and with other monoterpenoids,which offers a possible explanation for the sedative and anticonvulsant effects and the insecticidal activi

Classically, hepatic artery pseudoaneurysms(HAPs) arise secondary to trauma or iatrogenic causes. With an increasing prevalence of laparoscopic procedures of the hepatobiliary system the risk of inadvertent injury to arterial vessels is increased. Pseudoaneurysm formation post injury can lead to serious consequences of rupture and subsequent hemorrhage, therefore intervention in all identified visceral pseudoaneurysms has been advocated. A variety of interventional methods have been proposed, with surgical management becoming the last step intervention when minimally invasive therapies have failed. The authors present a case of a HAP in a 56-year-old female presenting with jaundice and pruritis suggestive of a Klatskin's tumor. This presentation of HAP in a patient without any significant past medical or surgical intervention is atypical when considering that the majority of HAP cases present secondary to iatrogenic causes or trauma. Multiple minimally invasive approaches were employed in an attempt to alleviate the symptomology which included jaundice and associated inflammatory changes. Ultimately, a right hepatic trisegmentectomy was required to adequately relieve the mass effect on biliary outflow obstruction and definitively address the HAP. The presentation of a HAP masquerading as a malignancy with jaundice and pruritis, rather than the classic symptoms of abdominalpain, anemia, and melena, is unique. This presentation is only further complicated by the absent history of either trauma or instrumentation. It is important to be aware of HAPs as a potential cause of jaundice in addition to the more commonly thought of etiologies. Furthermore, given the morbidity and mortality associated with pseudoaneurysm rupture, intervention in identifiable cases, either by minimally invasive or surgical interventions, is recommended.

Cystic fibrosis (CF) is a genetically inherited disease which is characterized by excessive inflammation and inability to resolve infection with pathogens such as Pseudomonas aeruginosa. Treatment options have improved with correctors and potentiators, but a cure remains elusive. Human mesenchymal stem cells (hMSCs) have the potential to be both anti-inflammatory and anti-microbial, which makes them ideal candidates for exploration as an innovative new therapeutic for CF. Using a sublethal CF mouse model of chronic Pseudomonas aeruginosa infection, we show that hMSCs and wild type bone marrow derived macrophages (BMM) have the capacity to attenuate inflammation while at the same time improving the ability to resolve infection. Animals infected with bacteria and treated with hMSCs and BMM had less weight lost, decreased pro-inflammatory cytokines, decreased severity of gross lung pathology as well as clinical score. Importantly, even though the inflammation was decreased in vivo, both BMM and hMSC treatment resulted in significant decrease in lung bacterial load. The improvement in the CF model was consistent with hMSC induced anti-inflammatory and anti-microbial activity which may involve the cathelicidin ll-37. These studies suggest that both healthy MSCs and BMM may provide important new direction toward cell based therapies in CF.

美国法院要求医生在确定治疗方案时必须得到患者的知情同意.而近来伦理学家提出,尊重患者的自主权不仅要使其知情,还应让病人在医疗决策中发挥作用.然而有调查结果显示,事实上病人在参与医疗决策时起到的作用微乎其微.尽管医生也在为患者罗列出各种医疗方案,但多数医生并不希望由病人参与决策.而这些调查对为什么有时候病人不能充分参与医疗决策并未作出深入探讨,并忽视了对医生就病人参与医疗决策的认知分析.因此,笔者希望通过本次调查,在医生对患者参与医疗决策的态度、医生的态度如何影响病人以及临床医疗决策的构建等方面,做一次试探性、描述性的研究.
