背景:尽管对有系统结构的交接班文档(用于引导有效交接班)的需求在不断增加,针对ICU交接班的交班指导工具却是有限的。目的 :此项研究的目的在于帮助ICU小组长收集患者的主要病情项目,更有效地进行班次之间的交接。方法 :研究设在澳洲的一个有21张床位的危重病房内/外科ICU。参加人员为参与交接班的高资历护士。参与小组采用名义群体法(Nominal Group Technique,NGT)选择并且排序需要收集患者主要病情资料的重要性。接班的护士从前一班护士组长的交班内容中选择哪些项目应该包含在基本病情资料中。选择结果以出现频率和百分率作总结。结果 :参与研究的三个小组由17位高资历护士组成。参与人员一致同意ISBAR(识别-基本情况-背景-检查分柝-建议)是指导临床交接班的一项有效的应用工具。经筛选应该包含在基本资料组成的项目(至少65%同意)包括识别(姓名、年龄、ICU住院天数)、基本情况(诊断、手术名称)、现状背景(主要病症)、主要病症的治疗措施、建议(下一班对患者的治疗和观察计划及跟进措施)。总的来说,检查项目中选出的67项中有30项(45%)被认为应该包含为基本资料的组成部分,具体到对身体特定系统的观察和治疗。其他和ISBAR没有关联的项目,例如ICU收住入院、工作人员和技术能力配置、手术病例等,以及患者病情(感染状态、感染部位、濒死计划)都作为重要考虑因素。其他增加的项目并应包含在交接班的内容仅在与患者病情有关时讨论。结论:研究结果表明,使用专为ICU设计的ISBAR能够帮助ICU小组长有效收集患者病情资料,更有效地进行交接班,并保证患者的安全和治疗计划得以实施。

AIM: To define the cost-effectiveness of strategies, including endoscopy and immunosuppression, to prevent endoscopic recurrence of Crohn's disease following intestinal ***: In the "POCER" study patients undergoing intestinal resection were treated with post-operative drug therapy. Two thirds were randomized to active care(6 mo colonoscopy and drug intensification for endoscopic recurrence) and one third to drug therapy without early endoscopy. Colonoscopy at 18 mo and faecal calprotectin(FC) measurement were used to assess disease recurrence. Administrative data, chart review and patient questionnaires were collected prospectively over 18 ***: Sixty patients(active care n = 43, standardcare n = 17) were included from one health service. Median total health care cost was $6440 per patient. Active care cost $4824 more than standard care over 18 mo. Medication accounted for 78% of total cost, of which 90% was for adalimumab. Median health care cost was higher for those with endoscopic recurrence compared to those inremission [ $ 26347( IQR 25045-27485) vs $2729(IQR 1182-5215), P < 0.001]. FC to select patients for colonoscopy could reduce cost by $1010 per patient over 18 mo. Active care was associated with 18% decreased endoscopic recurrence, costing $861 for each recurrence prevented. CONCLUSION: Post-operative management strategies are associated with high cost, primarily medication related. Calprotectin use reduces costs. The long term cost-benefit of these strategies remains to be evaluated.

Background: Using ultrasound to measure optic nerve sheath diameter(ONSD) is an emerging bedside technique to noninvasively assess intracranial pressure(ICP) in patients with brain injury. This technique is unique among bedside ultrasonography and is often performed by providers who have no formal ultrasound training. We sought to create a low-cost, 3 D, reusable ONSD model to train neurology, neurosurgery, and critical care providers in measuring ***: We identified 253 articles, of which 15 were associated with models and 2 with simulation. One gelatin model was reported, upon which we based our initial design. We could not validate the visual findings of this model;however, after constructing multiple beta models, the design most representative of human eye anatomy was a globe made of ballistics gel and either a 3 mm, 5 mm, or 7 mm×50 mm 3 D-printed optic nerve inserted into a platform composed of ballistics gel, all of which sat inside a 3 D-printed skull. This model was used to teach ONSD measurements with ultrasound at a continuing medical education event prior to training on a live human ***: A simple 3 D ballistic ONSD model allows learners to practice proper hand placement and pressure, basic landmarks, and ONSD measurement prior to operating on a human eye. This model is replicable and sustainable given that the globe and platform are composed of ballistics gel.

For any given genotype,the environment in which an apple is grown can influence the properties of the fruit *** there has been extensive research on the mechanism of the genetic control of fruit quality traits,less effort has been made to investigate the way that these genetic mechanisms interact with the *** address this issue,we employed a large‘Royal Gala’×‘Braeburn’population of 572 seedlings replicated over sites in three climatically diverse apple-growing regions in New *** for traits including fruit maturation timing,firmness and dry matter content was performed at each of these three sites for a single growing season(2011),and at two sites(Motueka and Hawke’s Bay)for two seasons(2009 and 2010).The phenotype data collected over 2 years at two sites enabled the detection of 190 quantitative trait loci(QTL)that controlled these traits regardless of year or growing location,as well as some chromosomal loci that influenced the traits in a single given environment or *** those loci that were environmentally stable over three sites,there was an interdependency of fruit maturation date,dry matter content and storage potential within this population,with two regions on Linkage Groups(LGs)10 and 16 strongly *** these loci were used in a marker-assisted selection programme to select for progeny bearing firmer fruit,this would have the unintentional consequence of selecting,high dry matter content,later maturing *** addition,a further 113 new QTLs with a smaller effect were identified,some of which were exhibited only in a single growing environment,demonstrating the underlying complexity of control of traits determining fruit quality,in addition to the need for being aware of environmental effects when developing new apple varieties.

Given the likelihood of future reductions in the strength of the Atlantic Meridional Overturning Circulation(AMOC),it is important to document how changes in the AMOC have altered climate patterns in the past and to assess the skill of coupled climate models in reproducing these *** past abrupt changes in the AMOC,the 8.2 ka event provides a particularly useful case study because its duration,magnitude of AMOC reduction and background climate state are closest to conditions expected in the *** this research,we present an expanded proxy synthesis of the 8.2 ka event in monsoonal Asia,including new high-resolution lake and bog records,more sites from the East Asia monsoon region and proxies of winter monsoon *** compare proxy evidence with a new simulation of the 8.2 ka event using the Community Climate System Model version 3(CCSM3) and prescribing North Atlantic freshwater forcing according to the latest *** find clear and objectively-determined evidence for 8.2 ka climate anomalies at nearly all of the fourteen proxy sites,emphasizing the strong and widespread impacts of the event in monsoonal Asia during both summer and winter *** model simulation corroborates that these anomalies,described generally as a weakening of the summer monsoon and strengthening of the winter monsoon,were likely caused by a reduction of the *** of regional anomalies in East Asia reveals some spatial heterogeneity,however,that in the model simulation is caused by contraction of the seasonal migration of the subtropical monsoon *** duration of climate anomalies at 8.2 ka in monsoonal Asia,both in proxy records and the model simulation,generally matches the duration of the event in Greenland ice core δ^(18)O,further supporting a tight connection to the North Atlantic.

目的:探讨哺乳动物细胞能否修复DNA链间交联(imerstrand crosslink,ICL)。方法:构建带有丝裂霉素(mito mycin C,MMC)交联的质粒pCMV-Luc／ICL,将该交联质粒转染无修复酶缺陷和有修复酶缺陷的哺乳动物细胞系,观察细胞对 MMC链间交联的修复能力。结果:(1)无酶缺陷的哺乳动物细胞对DNA的修复能力很强,即使缺乏同源序列,也能有效地去除DNA链间交联;(2)修复过程需要许多酶类参与,其中与核苷酸切除修复(nucleotide excision repair,NER)相关的酶类起着关键性作用,提示NER参与MMC链间交联的修复过程;(3)序列分析表明,非同源性重组修复为易错修复(error-prone re pair)。结论:哺乳动物细胞能有效地修复MMC诱导的DNA链间交联,核苷酸切除修复是重要的修复途径。

Objective:Bone metastasis occurs in up to 90%of men with advanced prostate cancer and leads to fractures,severe pain and *** metastases induce a spectrum of types of bone lesions which can respond differently to therapy even within individual prostate cancer ***,the special environment of the bone makes the disease more complicated and incurable.A model in which bone lesions are reproducibly induced that mirrors the complexity seen in patients would be invaluable for pre-clinical testing of novel *** microstructural changes in the femurs of mice implanted with PCSD1,a new patient-derived xenograft from a surgical prostate cancer bone metastasis specimen,were ***:Quantitative micro-computed tomography(micro-CT)and histological analyses were performed to evaluate the effects of direct injection of PCSD1 cells or media alone(Control)into the right femurs of Rag2/gc/male ***:Bone lesions formed only in femurs of mice injected with PCSD1 *** volume(BV)was significantly decreased at the proximal and distal ends of the femurs(p<0.01)whereas BV(p<0.05)and bone shaft diameter(p<0.01)were significantly increased along the femur ***:PCSD1 cells reproducibly induced bone loss leading to osteolytic lesions at the ends of the femur,and,in contrast,induced aberrant bone formation leading to osteoblastic lesions along the femur ***,the interaction of PCSD1 cells with different bone region-specific microenvironments specified the type of bone *** approach can be used to determine if different bone regions support more therapy resistant tumor growth,thus,requiring novel treatments.

Sperm function testing, once commonly performed for the infertile couple before employing assisted reproductive technology （ART）, has fallen out of favour in many reproductive medicine centers throughout the world. Indeed, the most recent addition of the ＇World Health Organisation （WHO） Laboratory Manual for the Examination and Processing of Hu- man Semen＇ now groups many of these procedures into a section termed Research Procedures. In large part, this reflects the current clinical practice of bypassing the in-depth evaluation of the male partner, while assuming that if a spermatozoon can be found for intracytoplasmic sperm injection （ICSI）, it must be a healthy cell capable of achieving fertilization. Never- theless, sperm function testing can provide valuable clinical insights into defects causing male infertility. Admittedly, in some cases, functional sperm deficiencies can be overcome using an ART. In other cases, couples will be empowered by the knowledge of the cause of their infertility, and for some couples, perhaps even the likelihood of ICSI success （relative to the spermatozoa）. The knowledge allows them to make truly informed reproductive decisions, including （perhaps） the de- cision to seek donor insemination, to adopt or to remain childless. Knowledge of the cause of their infertility may provide closure for couples and a sense of confidence regarding their choice of reproductive treatment.
