Since the 1950s,when the Turing Test was introduced,there has been notable progress in machine language *** modeling,crucial for AI development,has evolved from statistical to neural models over the last two ***,transformer-based Pre-trained Language Models(PLM)have excelled in Natural Language Processing(NLP)tasks by leveraging large-scale training *** the scale of these models enhances performance significantly,introducing abilities like context learning that smaller models *** advancement in Large Language Models,exemplified by the development of ChatGPT,has made significant impacts both academically and industrially,capturing widespread societal *** survey provides an overview of the development and prospects from Large Language Models(LLM)to Large Multimodal Models(LMM).It first discusses the contributions and technological advancements of LLMs in the field of natural language processing,especially in text generation and language ***,it turns to the discussion of LMMs,which integrates various data modalities such as text,images,and sound,demonstrating advanced capabilities in understanding and generating cross-modal content,paving new pathways for the adaptability and flexibility of AI ***,the survey highlights the prospects of LMMs in terms of technological development and application potential,while also pointing out challenges in data integration,cross-modal understanding accuracy,providing a comprehensive perspective on the latest developments in this field.

Minimally Invasive Total Arterial Coronary Artery Bypass Grafting in Left Main Stem Disease

Objective: The aim was to show that Minimally Invasive total arterial revascularization for left main stem coronary artery disease, via a left anterior Mini thoracotomy using bilateral internal thoracic arteries is feasible, reproducible and safe. Further, there has been no exclusive data or experience with minimally invasive coronary artery bypass grafting in left main stem disease. Methods: From April 2019 to March 2024, 41 patients with left main stem stenosis, left main equivalent disease and unprotected left main with triple vessel disease underwent off pump minimally invasive multivessel coronary artery bypass grafting using either in situ pedicled Bilateral Internal Thoracic arteries or Left and Right Internal Thoracic artery Y composite conduits at three centers. Bilateral Internal Thoracic arteries were harvested under direct vision. All patients had an Intra-Aortic Balloon Pump inserted via the femoral artery prior to induction of anesthesia, to prevent any hemodynamic instability, arrhythmias, and was removed following completion of the procedure in the operating room without any complications. Efficacy and outcomes were evaluated by i) Primary (MACCE)-Major Adverse Cardiac and Cardiovascular events and ii) Secondary outcome measures including total length of stay, return to full physical activity and quality of life. Mean follow-up was 1.4 years (Maximum was 2.5 years). Results: 41 patients with left main stem coronary artery stenosis, underwent total arterial revascularization using bilateral internal thoracic arteries. Left main stem stenosis was present in 29 patients, Unprotected left main stem stenosis with triple vessel disease in 7 and left main equivalence in 5 patients. In this cohort, 29 patients with only left main stem stenosis had 2 grafts each, 7 patients with left main and triple vessel disease had 3 grafts and 5 patients with left main equivalent disease had 2 grafts respectively. With the development of hardware devices and the upgrading of smartphones,a large number of users save privacy-related information in mobile devices,mainly smartphones,which puts forward higher demands on the protection of mobile users’privacy *** present,mobile user authenticationmethods based on humancomputer interaction have been extensively studied due to their advantages of high precision and non-perception,but there are still shortcomings such as low data collection efficiency,untrustworthy participating nodes,and lack of *** this end,this paper proposes a privacy-enhanced mobile user authentication method with motion sensors,which mainly includes:(1)Construct a smart contract-based private chain and federated learning to improve the data collection efficiency of mobile user authentication,reduce the probability of the model being bypassed by attackers,and reduce the overhead of data centralized processing and the risk of privacy leakage;(2)Use certificateless encryption to realize the authentication of the device to ensure the credibility of the client nodes participating in the calculation;(3)Combine Variational Mode Decomposition(VMD)and Long Short-TermMemory(LSTM)to analyze and model the motion sensor data of mobile devices to improve the accuracy of model *** experimental results on the real environment dataset of 1513 people show that themethod proposed in this paper can effectively resist poisoning attacks while ensuring the accuracy and efficiency of mobile user authentication.

BACKGROUND Hepatitis C virus(HCV),hepatitis B virus(HBV),and human immunodeficiency virus 1(HIV-1)are the most epidemic blood-borne viruses,posing threats to human health and causing economic losses to nations for combating the infection *** diagnostic methodologies that depend on the detection of viral nucleic acids are much more expensive,but they are more accurate than sero-logical *** To develop a rapid,cost-effective,and accurate diagnostic multiplex polymerase chain reaction(PCR)assay for simultaneous detection of HCV,HBV,and *** The design of the proposed PCR assay targets the amplification of a short conserved region featured with a distinguishable melting profile and electro-phoretic molecular weight inside each viral ***,this diagnostic method will be appropriate for application in both conventional(combined with electrophoresis)and real-time PCR *** in silico investigations were conducted to prove the capability of the approached PCR assay to detect variants of each ***,Egyptian isolates of each virus were subjected to the wet lab examination using the given diagnostic *** The in silico investigations confirmed that the PCR primers can match many viral variants in a multiplex PCR *** wet lab experiment proved the efficiency of the assay in distinguishing each viral type through high-resolution melting *** to related published assays,the proposed assay in the current study is more sensitive and competitive with many expensive PCR *** This study provides a simple,cost-effective,and sensitive diagnostic PCR assay facilitating the detection of the most epidemic blood-borne viruses;this makes the proposed assay promising to be substitutive for the mistakable and cheap serological-based assays.

AIM:To develop normative data for meibomian gland dysfunction(MGD)parameters,using non-contact meibography technique of Sirius Costruzione Strumenti Oftalmici(CSO)machine,in an Egyptian population ***:Observational,cross-sectional,analytic study,in which 104 Egyptian volunteers were *** upper lids were examined,using“Sirius CSO”*** eyelid was given a degree of meibomian gland loss(MGL),which was calculated by the software of the ***:Mean percentage MGL in right upper lid was of 30.9%±12.6%,and that of left upper lid was 32.6%±11.8%.Thirty-four volunteers(32.7%)had first-degree MGL in their right upper lid,and 67.3%had second-degree *** volunteer(1%)had zero-degree MGL in left upper lid,28(26.9%)had first-degree loss,and 75(72.1%)had second-degree *** of MGL in right upper eyelid was not related to age,but degree of MGL in left upper eyelid increased with *** was statistically significant difference between both genders for degree of MGL in right eye(P=0.036)and in left eye(P=0.027).CONCLUSION:Noncontact meibography is a useful non-invasive tool for diagnosing *** is diagnosed in 100%of apparently normal individuals;26.9%-32.7%of which have first-degree MGL,and 67.3%-72.1%have second-degree MGL.

Background:Repair of proximal hypospadias remains a challenge with no consensus on the best surgical *** reports have shown recurrence of chordee after plate-preserving *** this report,we present our experience in managing cases who presented with persistent/recurrence of chordee after tubularized incised plate(TIP)repair for proximal ***:Between 2015 and 2023,the study included eight patients who presented with ventral penile curvature after previous hypospadias repair in infancy(TIP urethroplasty).Reoperation to correct persistent ventral curvature was performed several years after the primary TIP repair(3-15 years;mean 7.9 years;median 7.5 years).The reoperation was a two-stage procedure:the first stage comprised transection of the urethra and grafting(lower lip mucosal graft);the second stage(6 months later)was a Thiersch-Duplay ***:Complications included poor taking of the graft after the first stage in one case(12.5%),which was regrafted using buccal(cheek)*** after second stage included urethro-cutaneous fistulae in five(62.5%)that were successfully surgically treated at a later ***-up after second stage ranged between 6 months and 7 years(mean 2.4 years;median 1 year).Satisfaction with reoperation to correct penile curvature was noted by parents as well as older children(adolescents)who appreciated better cosmesis by increasing ventral penile length and improved ***:Preservation of the urethral plate in proximal hypospadias may result in shortened penile length,mostly on the ventral aspect.A two-stage reoperation to increase the length of the penile urethra can successfully reverse this complication.

D-transposition of the great arteries(d-TGA)is surgically repaired with the arterial switch operation(ASO)with excellent results,however short and long-term morbidities still develop including neurocognitive *** significant central sleep apnea is uncommon in non-premature infants,but when present indicates immature autonomic control of respiration likely due to a neurologic *** report the unanticipatedfinding of central sleep apnea in four-term neonates with d-TGA after uncomplicated ASO,with the short-term complication of delayed hospital discharge and long-term concerns regarding this early marker of brain immaturity and its hindrance to normal *** this report,we will review each patient’s clinical course and then examine the literature on pediatric central sleep apnea,neurodevelopmental outcomes after ASO,and the important overlap of these entities in the care of patients going forward.

The increasing trend for integrating renewable energy sources into the grid to achieve a cleaner energy system is one of the main reasons for the development of sustainable microgrid(MG)*** typical power-electronized power systems,MGs make extensive use of power electronics converters,which are highly controllable and flexible but lead to a profound impact on the dynamic performance of the whole *** with traditional large-capacity power systems,MGs are less resistant to perturbations,and various dynamic variables are coupled with each other on multiple timescales,resulting in a more complex system instability *** meet the technical and economic challenges,such as active and reactive power-sharing,voltage,and frequency deviations,and imbalances between power supply and demand,the concept of hierarchical control has been introduced into MGs,allowing systems to control and manage the high capacity of renewable energy sources and ***,as the capacity and scale of the MG system increase,along with a multi-timescale control loop design,the multi-timescale interactions in the system may become more significant,posing a serious threat to its safe and stable *** investigate the multi-timescale behaviors and instability mechanisms under dynamic inter-actions for AC MGs,existing coordinated control strategies are discussed,and the dynamic stability of the system is defined and classified in this ***,the modeling and assessment methods for the stability analysis of multi-timescale systems are also ***,an outlook and discussion of future research directions for AC MGs are also presented.
