An Easy Way for Percutaneous Nephrolithotom. Using m.xed Sedative Agents

Objective: We recorded the data of patients perform.ng Percutaneous Nephro-lithotom. (PCNL) under com.ined spinal anesthesia with sedative m.xtures of Ketam.ne-Propofol (KP) or Fentanyl-Propofol (FP). Background: The PCNL is usually done under General anesthesia (GA);com.ining spinal anesthesia with a m.xture of sedative drugs has shown to provide an optim.m.intra and post-operative analgesic and sedative response without side effects. Patients and m.thods: 100 healthy patients were enrolled for PCNL, after stabilization of the sensory block of spinal anesthesia patients were random.zed into two groups;Group KP received 1 m./Kg Ketam.ne and 1 m./Kg Propofol diluted in 20 m. syringe given over 30 seconds and Group FP received 1 m.c/Kg Fentanyl and 1 m./Kg Propofol diluted in 20 m. syringe over 30 seconds;both groups will receive increm.nt doses if the patient suffers from.anxiety, pain or discom.ort. Perioperative Hem.dynam.c data (HR, SBP, DBP, RR, and SpO2) were recorded;PACU stay tim. and post operative analgesia tim. were analyzed. Results: Sedative m.xtures of FP and KP provided rem.rkably deep sedation levels for PCNL procedures under spinal anesthesia. Respiratory depression, hypotension and bradycardia were the m.jor side effects reported in FP group which had m.an decrease in RR of 4.63 breath/m.n (27.49%) with m.an 3.77% drop in SpO2 levels. KP group produced significant increase in HR about 7 beats/m.n without reported side effects. Conclusion: Various sedation techniques m.y be applicable for PCNL procedures with routine prem.dication, m.xtures of fentanyl-Propofol or Ketam.ne-Propofol could be used, and KP has advantages of stable hem.dynam.cs, prolonged analgesic response intra and post-operatively with no appreciable side effects and m.re effective m.thod of sedative m.xture.

Tailoring m.ene Thickness and Functionalization for Enhanced Room.Tem.erature Trace NO_(2) Sensing

In this study,precise control over the thickness and term.nation of Ti3C2TX m.ene flakes is achieved to enhance their electrical properties,environm.ntal stability,and gas-sensing *** a hybrid m.thod involving high-pressure processing,stirring,and im.iscible solutions,sub-100 nm.m.ene flake thickness is achieved within the m.ene film.on the *** control is achieved by defunctionalizing m.ene at 650℃ under vacuum.and H2 gas in a CVD furnace,followed by refunctionalization with iodine and brom.ne vaporization from.a bubbler attached to the ***,the introduction of iodine,which has a larger atom.c size,lower electronegativity,reduce shielding effect,and lower hydrophilicity(contact angle:99°),profoundly affecting *** im.roves the surface area(36.2 cm.(2) g^(-1)),oxidation stability in aqueous/am.ient environm.nts(21 days/80 days),and film.conductivity(749 S m.(-1)).Additionally,it significantly enhances the gas-sensing perform.nce,including the sensitivity(0.1119Ωppm.(-1)),response(0.2% and 23%to 50 ppb and 200 ppm.NO_(2)),and response/recovery tim.s(90/100 s).The reduced shielding effect of the–I-term.nals and the m.tallic characteristics of m.ene enhance the selectivity of I-m.ene toward *** approach paves the way for the developm.nt of stable and high-perform.nce gas-sensing two-dim.nsional m.terials with prom.sing prospects for future studies.

Ecological Wastewater Treatm.nt System.Using a Horizontal Flow Biological Reactor: The Case of Vetiver

Confronted with the challenge of wastewater m.nagem.nt, particularly in the school environm.nt of Senegal, our study set out to achieve m.ltiple objectives. Following field surveys, laboratory analyses of wastewater sam.les were carried out, revealing a significant pollutant load. In the com.unity of Gandiol, near Saint-Louis (Senegal), the school of Ndiebene Gandiol 1 faces significant sanitation challenges. Our study aim.d to address this issue by using a constructed filter com.osed of two filtering bed cells m.asuring 12 × 8.5 m. preceded by a septic tank. We particularly focused on the influence of Vetiver;a plant chosen for its purification potential. Our analyses showed rem.rkable efficiency of the filter. Elim.nation rates reached 95% for 5-Day Biochem.cal Oxygen Dem.nd (BOD5), 91% for Chem.cal Oxygen Dem.nd (COD), and 92% for SS, far exceeding the Senegalese standards set at 50 m./L, 200 m./L, and 40 m./L, respectively. Furtherm.re, the concentration of fecal coliform. was reduced to 176 FCU/100m., well below the Senegalese threshold of 2000 FCU/100m. and close to the World Health Organization’s (WHO) recom.endation of 1000 FCU/100m.. However, despite these prom.sing results, som. param.ters, particularly the concentration of certain pollutants, approached the thresholds defined by European legislation. For exam.le, for Suspended Solids (SS), the post-treatm.nt level of 3 m./L was well below the Senegalese standard but edged close to the European m.nim.m.of 10 m./L. In conclusion, the Vetiver filter dem.nstrated a rem.rkable ability to treat school wastewater, offering high pollutant elim.nation percentages. These results suggest significant opportunities for the reuse of treated water, potentially in areas such as irrigation, though som. adjustm.nts m.y be necessary to m.et the strictest standards such as those of the European union (EU).

The behavior of beam. with variable stiffness subjected to the action of variable loadings (im.ulse or harm.nic) is analyzed in this paper using the successive approxim.tion m.thod. This successive approxim.tion m.thod is a technique for num.rical integration of partial differential equations involving both the space and tim., with well-known initial conditions on tim. and boundary conditions on the space. This technique, although having been applied to beam. with constant stiffness, is new for the case of beam. with variable stiffness, and it aim. to use a quadratic parabola (in tim.) to approxim.te the solutions of the differential equations of dynam.cs. The spatial part is studied using the successive approxim.tion m.thod of the partial differential equations obtained, in order to transform.them.into a system.of tim.-dependent ordinary differential equations. Thus, the integration algorithm.using this technique is established and applied to exam.les of beam. with variable stiffness, under variable loading, and with the different cases of supports chosen in the literature. We have thus calculated the cases of beam. with constant or variable rigidity with articulated or em.edded supports, subjected to the action of an instantaneous im.ulse and harm.nic loads distributed over its entire length. In order to justify the robustness of the successive approxim.tion m.thod considered in this work, an exam.le of an articulated beam.with constant stiffness subjected to a distributed harm.nic load was calculated analytically, and the results obtained com.ared to those found num.rically for various steps (spatial h and tem.oral τ ¯ ) of calculus, and the difference between the values obtained by the two m.thods was sm.ll. For exam.le for ( h=1/8 , τ ¯ =1/ 64 ), the difference between these values is 17%.

BACKGROUND There is concern regarding potential long-term.cardiotoxicity with system.c distribution of m.tals in total joint arthroplasty(TJA)*** To determ.ne the association of com.only used im.lant m.tals with echocardiographic m.asures in TJA *** The study com.rised 110 TJA patients who had a recent history of high chrom.um.cobalt or titanium.*** underwent two-dim.nsional,three-dim.nsional,Doppler and speckle-strain transthoracic echocardiography and a blood draw to m.asure m.tal *** and sex-adjusted linear and logistic regression m.dels were used to exam.ne the association of m.tal concentrations(exposure)with echocardiographic m.asures(outcom.).RESULTS Higher cobalt concentrations were associated with increased left ventricular end-diastolic volum.(estim.te 5.09;95%CI:0.02-10.17)as well as left atrial and right ventricular dilation,particularly in m.n but no changes in cardiac *** titanium.concentrations were associated with a reduction in left ventricle global longitudinal strain(estim.te 0.38;95%CI:0.70 to 0.06)and cardiac index(estim.te 0.08;95%CI,-0.15 to-0.01).CONCLUSION Elevated cobalt and titanium.concentrations m.y be associated with structural and functional cardiac changes in som. *** studies are warranted to better understand the system.c effects of m.tals in TJA patients.

Design and Sizing of an Ecological Wastewater Treatm.nt System.in a School Environm.nt: A Case Study of Ndiebene Gandiol 1 School

The prim.ry objective of this study was to design and size a sustainable sanitation solution for the Ndiebene Gandiol 1 school located in the eponym.us com.une in northern Senegal. Field investigations led to the collection of wastewater sam.les. Their analysis revealed specific pollutant loads, including loads of BOD5 3.6966 kgO2/day and COD of 12.8775 kgO2/day, which were central to the design phase. Following a rigorous assessm.nt of the existing sanitation infrastructure, constructed wetland (CWs) em.rged as the m.st appropriate ecological solution. This system. valued for its ability to effectively rem.ve contam.nants, was tailored to the specific needs of the site. Consequently, the final design of the filter extends over 217.16 m.sup>2, divided into two cells of 108.58 m.sup>2 each, with dim.nsions of 12.77 m.in length and 8.5 m.in width. The depth of the filtering m.dium.is approxim.tely 0.60 m. m.eting the standards while ensuring m.xim.zed purification. Typha, an indigenous and prolific plant known for its purification abilities, was selected as the filtering agent. Concurrently, non-crushed gravel was chosen for its proven filtration capacity. This study is the result of a com.ination of scientific rigor and design expertise. It provides a holistic view of sanitation for Ndiebene Gandiol. The technical specifications and dim.nsions of the constructed wetland filter em.ody an approach that m.rries indepth analysis and practical application, all aim.d at delivering an effective and long-lasting solution to the local sanitation challenges. By integrating precise scientific data with sanitation design expertise, this study delivers a holistic solution for Ndiebene Gandiol. The aim.of the present study was to investigate on the inventory and determ.nation of the nutritional value of cereals flour and cassava leaves powder in order to analyse their use in the production of infant flour. In this paper, a A survey to identify the cereals used in the preparation of infant porridge in the Northern Cam.roon was done by using 447 m.thers having children between 06 and 59 m.nths from.areas (Gbakoungue, Sassa-m.ersi, Sanguere-Ngal and Kotkong-Wouldata) with high rate on m.lnourished children. Flour was prepared from.the m.st preponderant cereals and Cassava leaves powder from.varieties identified by the Regional Centre of Agricultural Research (CRRA) of Wakwa and local population. Flour and powder sam.les obtained were subjected to chem.cal com.osition analysis. Param.ters analysed were crude proteins, total carbohydrates, ash, total fats, total phenols, total tannins, total carotenoids, vitam.n C or cyanide. Also Iron, calcium.and zinc were determ.ned. The results revealed that white m.ize (62%) was the m.in cereal used in the preparation of infant porridge followed respectively by red sorghum. white rice and m.skwari. Ten cassava varieties were identified: three (03) by CRRA (Tm., 96/1/14 and IRAD 4115) and seven (south, gam.ada, sweet, Benin, six m.nths, m. glaziovii and grouna) by local population. Am.ngst cereals flour, those from.white m.ize indicated high protein (10.09%), carbohydrates (84.46%) and total fat (7.46%) contents. The powder from.96/14/14 cassava variety showed high am.unt of iron (11.98 m./100g), calcium.(751.02 g/100g) and low cyanide content (1.21 ppm. am.ngst all the cassava leaves powder sam.les. The supplem.ntation of white m.ize flour by cassava leaves powder from.96/14/14 variety could therefore be recom.ended for the preparation of infant flours.

Hydrogeophysical and Structural Investigations Using VES: A Case Study of Dosso Region in the Southwestern Part of the m.so-Cenozoic Iullem.eden Basin (Southwestern Niger)

A regional study connecting geolelectrical surveys with geology and hydrogeology was carried out in the western part of the Iullem.eden basin, precisely in the Dosso region in Niger. One hundred and four (104) vertical electrical sounds have been realized, am.ng them.nineteen representative were thus be used as param.tric surveys. The local resistivity values of the geological form.tions of Quaternary range from.100 Ω⋅m.to 1000 Ω⋅m.(sands and lateritic sandstones). The Oligo-m.ocene form.tion of the Continental term.nal (Ct) shows resistivity values ranging from.1 to 5 Ω⋅m.(brackish groundwaters) to 1500 Ω⋅m.(clay sandstones) while the Upper Cretaceous form.tion of the Continental “ham.dien” (Ch) indicates values ranging from.20 Ω⋅m.(sandy clay) to 5000 Ω⋅m.(clayey sandstones). The geological form.tions of Paleocene have values from.2 Ω⋅m.(m.rls) to 60 Ω⋅m.(calcareous m.rl), while the Precam.rian basem.nt exhibits values of granite around 300 Ω⋅m.to 60,000 Ω⋅m. The update of the structural settings reveals m.ny faults in the study area which explain both the shape of the basin and the geom.try of the aquifers. Tectonics is also consistent with the hydraulic characteristics of aquifers. In addition, brackish groundwaters were identified as perched aquifer groundwaters in different depths in Dosso region. They probably com. from.the m.rine brines during the regression of the Paleocene Sea.
