Therapeutic Mammoplasty in Management of Breast Cancer: A Prospective Clinical Study

Background: The introduction of oncoplasticbreast surgery, with a concomitant contralateral symmetrization procedure, allows the surgical oncologist the freedom to perform wider excisions, with more favorable cosmetic outcomes. Objective: To assess the feasibility of bilateral breast reduction for the management of breast cancer, and to determine the most suitable surgical technique of therapeutic mammoplasty for each patient. And to evaluate the cosmetic outcome during the postoperative period. Methods: This study was from June 2014 until June 2016, and included 86 female patients with breast cancer;divided in 3 groups;group A;had bilateral superior pedicle reduction mammoplasty, group B;had bilateral inferior pedicle reduction mammoplasty, group C;had bilateral batwing mastopexy. 1 month postoperative, patients were asked to answer a 5-scale questionnaire evaluating their own cosmetic outcome. Criteria they were asked to evaluate were: symmetry, shape and volume, projection, correction of ptosis, visibility of the scars and overall satisfaction. A similar questionnaire was answered by a panel made up of a surgeon and breast-care nurse after seeing the pre- and post-operative photos of the patients in question. Results: Group A had a mean tumor size of 2.7 ± 0.69 cm, least resected margins 1.2 - 4.3 cm, Group B had a mean tumor size of 3.2 ± 1.19 cm, least resected margins 0.7 - 3.8 cm, Group C had a mean tumor size of 3.4 ± 1.12 cm, least resected margins 1.7 - 5.2 cm. Cosmetic results fared high above average. Group C fared better in overall satisfaction but Group B fared better in ptosis correction, projection and symmetry. The panel answers showed difference regarding shape and volume;the only two criteria to be almost of the same result among the three groups. BACKGROUND Type 2 diabetes is one of the most prevalent chronic diseases worldwide,significantly impacting patients'quality of *** treatment options like metformin(MET)effectively counteract hyperglycemia but fail to alleviate diabetes-associated complications such as retinopathy,neuropathy,nephropathy,hepatopathy,and cardiovascular *** To propose the supplementation of cholecalciferol(CHO)and taurine(TAU)to enhance MET efficacy in controlling diabetes while minimizing the risk of associated *** The study involved sixty rats,including ten non-diabetic control rats and fifty experimental rats with type 2 diabetes induced by *** experimental rats were further subdivided into positive control and treatment *** four treatment groups were randomly allocated to a single MET treatment or MET combined with supplements either CHO,TAU,or *** Diabetic rats exhibited elevated levels of glucose,insulin,Homeostasis Model Assessment of Insulin Resistance(HOMA-IR),glycated hemoglobin percentage,lipid markers,aspartate aminotransferase,and malondialdehyde,along with reduced levels of antioxidant enzymes(catalase and superoxide dismutase).The administration of CHO and TAU supplements alongside MET in diabetic rats led to a noticeable recovery of islet *** antioxidative,anti-inflammatory,and anti-apoptotic properties of the proposed combination therapy significantly ameliorated the aforementioned *** The supplementation of CHO and TAU with MET showed the potential to significantly improve metabolic parameters and protect against diabetic complications through its antioxidative,anti-inflammatory,and antiapoptotic effects.

Formation and Biomimetic Deposition of Bone-like Hydroxyapatite on Wollastonite-gypsum Composites

Bone cements are often used for bone repair and fixation. The main objective of formulating a bone cement is to achieve structure and properties similarity to bone. In addition, bioactivity of a bone cement provides a major advantage that helps achieving better binding and interaction with the surrounding tissues. In the current study, gypsum was used as a matrix for a composite that comprises highly crys- talline prestine and acid-treated wollastonite （CaSiO3） fibers. Composites made by mixing their powder precursors with deionized water at room temperature, were investigated for their composition using X-ray diffraction （XRD） and Thermogravimetric Analysis （TGA）, for their microstructure using Scanning Electron Microscopy （SEM）, for their compressive strengths and modulus of elasticity, and for setting time measurements. In addition, cement composites were evaluated for their preliminary bioactivity in a protein-free Simulated Body Fluid （SBF） for up to 14 days. Results show the improvement of mechanical properties and bioactivity of the composites using acid-treated wollastonite fibers. This was attributed to the formation of hydrated silica layer on the surface of the acid-treated fibers which improved the binding with the gypsum matrix and provided nucleating sites for the deposition of bone-like apatite spherolites from SBF media.

Changes in corneal biomechanics and intraocular pressure following Femto-LASIK using Goldman applanation tonometry and ocular response analyzer

AIM:To compare intraocular pressure(IOP)measurements before and after laser in situ keratomileusis(LASIK)with a femtosecond laser for flap creation using ocular response analyzer(ORA)and Goldmann applanation tonometry,and to identify factors that may influence the preoperative and postoperative ***:A prospective study conducted on myopic patients who underwent LASIK using a femtosecond laser for flap *** patients were evaluated preoperatively,6 wk and 3 mo postoperatively for manifest refraction(MR),keratometric(K)readings and central corneal thickness(CCT)using a scheimpflug-based *** resistance factor(CRF),corneal hysteresis(CH),Goldmann correlated IOP(IOPg)and corneal compensated IOP(IOPcc)were measured using ORA besides IOP assessment by Goldman applanation tonometry(GAT).RESULTS:There was a statistically significant decrease in measures of IOPg by 3.35±0.83 mm Hg,followed by GAT which decreased by 2.2±0.44 mm Hg,and the least affected by operation was IOPcc which decreased only by 0.87±0.1 mm Hg after 6 *** 3 mo follow up there was a statistically significant decrease in IOPcc which decreased only by 0.76±0.4 mm Hg,followed by IOP GAT by 1.6±0.5 mm Hg,and the most affected by operation was IOPg which decreased by 2.3±0.3 mm ***,there was a statistically significant decrease in CH and CRF after 6 wk and 3 *** 3 mo,the preoperative MR and preoperative GAT were prominent significant predictors of the postoperative GAT *** prediction equation was ***:IOP measurements and corneal biomechanical factors reduce significantly after LASIK with a femtosecond laser for flap *** IOPcc values are less influenced by changes in corneal properties than IOPg and GAT,indicating that IOPcc may provide the most reliable measurement of IOP after this procedure.

Four isolates of Bacillus subtilis coded,B4,B7,B8 and B10 were examined as biocontrol agents for their abilities and antagonistic effect on the in vitro growth of certain phytopathogenic fungi of peanut,Rhizoctonia solani and Sclerotium *** subtilis isolate B4(GenBank accession ***150884)was the highly effective one for inhibiting the fungal mycelial *** fermentation of *** isolate B4 was carried out and the maximum biomass achieved was 4.53 g L-1 at 11 *** subtilis isolate B4 was formulated and evaluated as a biofungicide to reduce peanut soil-borne diseases under greenhouse and field conditions at the side of Rizolex-T(fungicide)as *** by formulated plant growth-promoting rhizobacteria(PGPR)*** B4 and Rizolex-T in a soil infested with ***,*** and mixture of them were more effective in decreasing percentage of damping-off,root and pod rot disease incidence(%)in greenhouse and open field environment during the two seasons 2015 and *** by PGPR gave highly dry weight and number of healthy pods compared to control of fungi treatment which was nearby to dry weights of healthy pods achieved by treatments by Rizolex-T in a soil infested with ***,*** and mixture of *** PGPR *** B4 gave higher increasing of yield percentage than treatment by Rizolex-T in the two evaluated seasons 2015 and *** can conclude that the produced bioforumlated agent was more efficient as fungicide when compared with the other chemical synthesized fungicides,safe for human and the environment and economy.

We present a new method for determining the age and relative contribution of different stellar populations in galaxies based on the genetic *** apply this method to the barred spiral galaxy NGC 3384,using CCD images in U,B,V,R and I *** analysis indicates that the galaxy NGC 3384 is mainly inhabited by old stellar population(age〉10^9 yr).Some problems were encountered when numerical simulations are used for determining the contribution of different stellar populations in the integrated color of a *** results show that the proposed genetic algorithm can search efficiently through the very large space of the possible ages.

The differential cross sections of elastic and inelastic scattering of ~3He ions on ^（13）C and ^（14）C have been studied at an energy of 37.9 MeV with a double folding model based on M3Y-Reid effective nucleon-nucleon *** resulted parameters have been used for the standard Distorted Wave Born Approximation calculations of angular distributions corresponding to different excitations levels of ^（13）C and ^（14）C and deformation parameters have been deduced.

Surgical management of primary hyperparathyroidism guided by double-phase Tc-99m-MIBI scintigraphy

Objective:The aim of this work was to study the clinicopathological features of cases with primary hyperparathyroidism (PHPT) referred to National Cancer Institute (NCI), Cairo University in the last six years and to study the role of nuclear medicine techniques in the initial diagnosis and preoperative localization directing surgical management of these cases. Methods: This study included 27 patients with PHPT properly diagnosed and treated in the NCI, Cairo University from January 2005 to December 2010. Preoperative neck U/S and Tc-99m-MIBI scintigraphy were done in all cases. If preoperative localization detected a single lesion, unilateral exploration was done. Bilateral exploration was done if multiple foci of active parathyroid glands or no lesions were detected. Results: This study included 27 patients (8 males and 19 females). The median age was 43 years (range from 19 to 68 years). All cases presented with bone disease in the form of bony pain in 23 patients, bony swellings in 15 patients and pathological fractures in 6 cases. Twenty one patients (77.8%) had single adenoma, 5 cases (18.5%) had parathyroid hyperplasia, and only one case had double adenomas. The sensitivity of neck U/S in detecting single adenoma was 61.9% (13/21), with 81% positive predictive value (PPV) while the sensitivity of Tc-99m-MIBI scintigraphy was 90.5% (19/21) with 100% PPV. Based on preoperative localization tests, unilateral exploration was done in 19 patients with solitary adenomas. All cases in this group were cured with no reported case of persistent or recurrent hypercalcemia. Conclusion: Presentation of PHPT may mimic malignant bone tumors but fortunately these patients were correctly diagnosed with bone scan which confirm the presence of metabolic bone disease rather than bone metastases. Correlation between IL28B/TLR4 genetic variants and HCC development with/without DAAs treatment in chronic HCV patients

In Egypt,Sofosbuvir(SOF)in combination with Dataclasvir(DCV)is the broadly used DAAs with excellent therapeutic *** study is designed to explore the relation between IL28B/TLR4 genetic variants and each of the followings;HCC development post SOF/DCV treatment,progression to HCC in naı¨ve patients and SOF/DCV therapy outcome.A total of 493 blood samples were collected(controls(n = 70);HCV patients treated with SOF/DCV(n = 252)of whom 65 patients developed HCC,187 patients didn’t develop HCC(125 responders,62 relapsers);naı¨ve HCV patients(n = 171)had early(n = 48),late liver fibrosis(n = 21)and HCC(n = 102)).Both SNPs were genotyped using a TaqMan 50 allelic discrimination *** IL28B rs12979860 SNP,the C allele was significantly correlating with the response rate more than T allele(OR 1.9,95%CI 1.29e2.9,p=0.004),while at TLR4 rs4986791 SNP,no association was found(OR 6.5,95%0.57e75.28,p = 0.09).Both SNPs couldn’t detect the probability for HCC emergence after *** naı¨ve patients,the protective alleles were detected in their lowest frequency in HCC patients(p = 0.1,for rs12979860 and,p = 0.001 for rs4986791).SOF/DCV combination improved SVR rates in HCV genotype 4a infected patients regardless of IL28B genotype,with the best rates in those lacking the T allele.
