Non-alcoholic fatty liver disease (NAFLD) is the most common form of chronic liver disease in the Western world. It is more prevalent in male gender, and with increasing age, obesity, and insulin resistance. Besides weight loss, there are limited treatment options. The use of anti-diabetic medications has been studied with mixed results. In this review, we discuss the use of anti-diabetic medications in the management of NAFLD with a specific focus on sodium-glucose cotransporter 2 inhibitors. We shed light on the evidence supporting their use in detail and discuss limitations and future directions.

Background:Liver fibrosis is a hallmark determinant of morbidity in biliary atresia(BA)even in successfully operated *** factors for this rapid progression of fibrosis are not completely *** expression of the transcription factor SOX9 and hepatic progenitor cells(HPCs)proliferation have roles in fibrogenesis in cholestatic ***,they were not investigated sufficiently in *** aimed to delineate the relation of SOX9 and HPCs to fibrosis and its progression in ***:Forty-eight patients with BA who underwent an initial diagnostic liver biopsy(LB)and consequent intraoperative LB were recruited and compared to 28 cases with non-BA cholestasis that had an LB in their diagnostic *** fibrosis,tissue SOX9 and HPC expressions were studied in both BA and non-BA-cholestasis *** fibrosis,SOX9,and HPCs’dynamic changes in BA cases were *** of fibrosis and its progression to SOX9 and HPCs in BA was ***:SOX9 and HPCs in ductular reaction(DR)form were expressed in 100%of BA and their grades increased significantly in the second *** rapidly progressive fibrosis in BA,represented by fibrosis grade of the intraoperative LB,correlated significantly to SOX9-DR and HPC-DR at the diagnostic(r=0.420,P=0.003 and r=0.405,P=0.004,respectively)and the intraoperative(r=0.460,P=0.001 and r=0.467,P=0.001,respectively)*** the other hand,fibrosis,SOX9-DR,and HPC-DR were significantly lower in non-BA cases at a comparable age(P<0.001,P=0.006,and P=0.014,respectively).Conclusions:Fibrosis in BA is rapidly progressive within a short time and is significantly correlated to SOX9 and *** of targeting SOX9 and HPCs on fibrosis progression is warranted.

Nanothermites (metal oxide/metal) can offer tremendously exothermic self sustained reactions. CuO is one of the most effective oxidizers for naonothermite applications. This study reports on two prospectives for the manufacture of CuO nanoparticles. Colloidal CuO particles of 15 nm particle size were developed using hydrothermal synthesis technique. Multiwalled carbon nanotubes (MWCNTs) with surface are 700m2/g was employed as a substrate for synthesis of CuO-coated MWCNTs using electroless plating. On the other hand, aluminium particles with combustion heat of 32000 J/g is of interest as high energy density material. The impact of stoichiometric nanothermite particles (CuO/Al & Cuo-coated MWCNTs/Al) on shock wave strength of Al/TNT nanocomposite was evaluated using ballistic mortar test. While CuO-coated MWCNTs decreased the shock wave strength by 15%;colloidal CuO enhanced the shock wave strength by 30%. The superior performance of colloidal CuO particles was correlated to their steric stabilization with employed organic solvent. This is the first time ever to report on fabrication, isolation, and integration of stablilized colloidal nanothermite particles into energetic matrix where intimate mixing between oxidizer and metal fuel could be achieved.

Objective:To investigate the effects of a single oxytocin injection on plasma steroid concentrations,testicular blood flow measures(resistive and pulsatility indexes),and testicular echogenicity in Baladi ***:Twelve Baladi goat bucks were randomly allocated into 2 groups and received an intravenous injection of either 0.7 IU/kg oxytocin or normal saline 0.9%.Venous blood samples were collected,and testicular blood flow Doppler parameters(i.e.,peak systolic velocity,end-diastolic velocity,time average maximum velocity,resistive index,and pulsatility index)were assessed for supratesticular arteries in the left and right testes immediately before(0),and at 5,30,60,and 120 min after ***:Plasma concentrations of testosterone significantly decreased in the oxytocin group at 60 min post-treatment compared to the control group,whereas plasma concentrations of estradiol-17βwere not affected(P>0.05).Both resistive index and pulsatility index declined in the oxytocin group at 60 min post-treatment compared to the control group(P<0.05).Testicular pixel intensity increased at 30 min post-treatment in the oxytocin group compared to the control group(P<0.05).Conclusions:Oxytocin treatment significantly increases testicular blood flow and decreases plasma testosterone concentrations in male Baladi goats.

Assuring medical images protection and robustness is a compulsory necessity *** this paper,a novel technique is proposed that fuses the wavelet-induced multi-resolution decomposition of the Discrete Wavelet Transform(DWT)with the energy compaction of the Discrete Wavelet Transform(DCT).The multi-level Encryption-based Hybrid Fusion Technique(EbhFT)aims to achieve great advances in terms of imperceptibility and security of medical images.A DWT disintegrated sub-band of a cover image is reformed simultaneously using the DCT ***,a 64-bit hex key is employed to encrypt the host image as well as participate in the second key creation process to encode the ***,a PN-sequence key is formed along with a supplementary key in the third layer of the ***,the watermarked image is generated by enclosing both keys into DWT and DCT *** fusions ability of the proposed EbHFT technique makes the best use of the distinct privileges of using both DWT and DCT *** order to validate the proposed technique,a standard dataset of medical images is *** results show higher performance of the visual quality(i.e.,57.65)for the watermarked forms of all types of medical *** addition,EbHFT robustness outperforms an existing scheme tested for the same dataset in terms of Normalized Correlation(NC).Finally,extra protection for digital images from against illegal replicating and unapproved tampering using the proposed technique.

Dynamic spectrum access(DSA) in cognitive radio(CR) networks became a challenging research area recently. In CR technology, the DSA between primary users(PUs) and secondary users(SUs) simultaneously can be achieved without degrading the performance of the PUs by SUs interference. This can be achieved by donating incentive power to the PUs in order to compensate the interference caused by the SUs. Consequently, PUs allow SUs to share the spectrum. In this paper, orthogonal codes-based dynamic spectrum access(OC-DSA) technique is proposed. OC-DSA technique employs orthogonality between PUs and SUs transmitted data symbols in addition to the incentive power donation. Compared to other techniques, the proposed technique uses a simple encoder at the SU network for the same PU network infrastructure. By applying orthogonal codes, the interference caused by SUs is canceled and hence the donated power to incentivize the PUs is reduced. Also, the SU packet rate is increased significantly. The simulation results show that the proposed technique provides effective improvements over other existing techniques in the signal strength and the bit error rate performance of both the PU network and the SU network at the receiver side. Moreover, the proposed technique requires less donated power to incentivize the PU and has higher packet rate.

With the development of hardware devices and the upgrading of smartphones,a large number of users save privacy-related information in mobile devices,mainly smartphones,which puts forward higher demands on the protection of mobile users’privacy *** present,mobile user authenticationmethods based on humancomputer interaction have been extensively studied due to their advantages of high precision and non-perception,but there are still shortcomings such as low data collection efficiency,untrustworthy participating nodes,and lack of *** this end,this paper proposes a privacy-enhanced mobile user authentication method with motion sensors,which mainly includes:(1)Construct a smart contract-based private chain and federated learning to improve the data collection efficiency of mobile user authentication,reduce the probability of the model being bypassed by attackers,and reduce the overhead of data centralized processing and the risk of privacy leakage;(2)Use certificateless encryption to realize the authentication of the device to ensure the credibility of the client nodes participating in the calculation;(3)Combine Variational Mode Decomposition(VMD)and Long Short-TermMemory(LSTM)to analyze and model the motion sensor data of mobile devices to improve the accuracy of model *** experimental results on the real environment dataset of 1513 people show that themethod proposed in this paper can effectively resist poisoning attacks while ensuring the accuracy and efficiency of mobile user authentication.

Since transactions in blockchain are based on public ledger verification,this raises security concerns about privacy *** it will cause the accumulation of data on the chain and resulting in the low efficiency of block verification,when the whole transaction on the chain is *** order to improve the efficiency and privacy protection of block data verification,this paper proposes an efficient block verification mechanism with privacy protection based on zeroknowledge proof(ZKP),which not only protects the privacy of users but also improves the speed of data block *** is no need to put the whole transaction on the chain when verifying block *** just needs to generate the ZKP and root hash with the transaction information,then save them to the smart contract for ***,the ZKP verification in smart contract is carried out to realize the privacy protection of the transaction and efficient verification of the *** the data is validated,the buffer accepts the complete transaction,updates the transaction status in the cloud database,and packages up the ***,the ZKP strengthens the privacy protection ability of blockchain,and the smart contracts save the time cost of block verification.

Determinants of Blood Pressure Variability in Individuals with Essential Hypertension: A Survey-Based Study

Background: Blood pressure variability (BPV) in hypertensive patients is implicated as a remarkable feature leading to additional cardiovascular complications. The aim of the study was to identify the determinants influencing BPV among patients with essential hypertension seen at the Cardiology department of the faculty of medicine, University of Alexandria, Egypt. Methods: This was a descriptive cross-sectional hospital-based study conducted from August 2019 to November 2019. All the eligible patients were made to fill out a standard questionnaire to obtain family and personal clinical history and undergo routine physical examination, laboratory tests and 24-hour ambulatory blood pressure monitoring. BPV was summarized as the standard deviation (SD) of all-day systolic and diastolic BP in both normal patients (with SD 11) and abnormal patients (with SD ≥ 11). Results: Out of a total of 114 patients, 18 (15.8%) non-hypertensive patients were included in the control group and the remaining 96 (84.3%) were classified based on the degree of hypertension. BPV in all these patients was found to be significantly related to the male gender, a mild or moderate degree of hypertension, high prevalence of non-dipping, diabetes, use of beta-blockers as antihypertensive medication, heart rate variability, BMI, and increased day-time variability. Conclusion: Variability in blood pressure influenced by different intrinsic and extrinsic factors plays an important role in the management of hypertension. In order to reduce the burden of disease and for a better quality of life for hypertensive patients, it is important that physicians start considering lowering BPV in addition to reducing physiological BP levels.
