Combined anterior and posterior ring fixation versus posterior ring fixation alone in the management of unstable Tile B and C pelvic ring injuries:A randomized controlled trial

Purpose: Combined anterior and posterior ring (APR) fixation is classically performed in Tile B2 and C1 injuries to achieve superior biomechanical ***,the posterior ring (PR) is the main weight bearing portion that is responsible for weight transmission from the upper parts of the body to the lower limbs through the sacrum and the linea *** is hypothesized that isolated PR fixation can achieve comparable radiological and clinical outcomes to APR ***,we conducted this study to compare the two fixation principles in managing Tile B2 and C1 ***: Our study included 20 patients with Tile B2 injuries and 20 patients with Tile C1 *** study was a randomized control single-blinded study via computerized random numbers with a 1:1 allocation by using random block *** study was performed at a level one trauma center.A total of 40 patients with Tile B2 and C1 injuries underwent combined APR or isolated PR fixation (Group A and B,respectively).Matta & Tornetta radiological principles and Majeed pelvic scoring system were used for the assessment of primary outcomes and postoperative *** outcomes included operative time,amount of blood loss,intraoperative assessment of reduction,need of another operation,length of hospital stay,ability to weight bear postoperatively and pain control *** used studentt-test to compare the difference in means between two groups,and Chi-square test to compare proportions between two qualitative *** set the confidence interval to 95% and the margin of error accepted to 5%.So,p ≤ 0.05 was considered statistically ***: The mean follow-up duration was 18 *** operative time (mean difference 0.575 h) and the intraoperative blood loss (mean difference 97.5 mL) were lower in Group ***,despite the higher frequency of rami displacement before union in the same group,there were no significant differences in terms of radiological outc

Fair subcarrier-power allocation scheme for multiuser multicarrier systems

The main objective of multiuser orthogonal frequency division multiple access(MU-OFDM) is to maximize the total system capacity in wireless communication systems. Thus, the problem in MU-OFDM system is the adaptive allocation of the resources(subcarriers, bits and power) to different users subject to several restrictions to maximize the total system capacity. In this work, a proposed subcarrier allocation algorithm was presented to assign the subcarriers with highest channel gain to the users. After the subcarrier allocation, subcarrier gain-based power allocation(SGPA) was employed for power and bit loading. The simulation results show that the proposed subcarrier-power allocation scheme can achieve high total system capacity and good fairness in allocating the resources to the users with slightly high computational complexity compared to the existing subcarrier allocation algorithms.

Chemotherapy Toxicity Profile in Adjuvant Treated Colorectal Carcinoma Patients

Background:?Colorectal cancer (CRC) is the third most common cancer and the fourth leading cause of cancer-related deaths in the world. The treatment of stage III CRC consists of surgery followed by adjuvant chemotherapy with either single agent capecitabine or combination therapy consisting of FOLFOX?or XELOX, all of which have been shown to be effective in improving disease-free survival (DFS) and overall survival (OS). However, toxicities acquired from chemotherapy can affect a cancer patient’s quality of life and result in early?treatment discontinuation. Common toxicities include hematological, gastrointestinal (GI), constitutional, dermatological, and neurological.?Aim of the Work: The present work was aimed to assess and evaluate chemotherapy toxicities in adjuvant treatment in CRC patients and analyze certain factors that might increase chemotherapy toxicity. Patients and Methods: This is retrospective study included a total of 72 patients of colorectal carcinoma received adjuvant chemotherapy at Clinical Oncology Department, Ain Shams University. The study was conducted between Jan. 2012 and Jan. 2017. Results: We found that?the most chemotherapy reported was neurological toxicity in (73.6%), gastrointestinal symptoms (diarrhea 52.7%, nausea 30.6%, vomiting 27.8% & oral mucositis 24%), hematological toxicity (neutropenia 40.3%, anemia 34.7%, thrombocytopenia 12.5%), fatigue 20.9%, hepatic toxicity 18.1%, dermatological toxicity 9.7% & renal toxicity 5.6%). older patients have significant incidence of neurological toxicity (p-value?= 0.023) and fatigue (p-value?= 0.038). females have significant incidence of anemia (p-value = 0.017). increase of oxaliplatin?cumulative dose increase incidence of neurological toxicity (p-value?= 0.024), thrombocytopenia (p-value?= 0.007) and renal toxicity (p-value?= 0.030). Factors Predicting Transformation of Non-Severe Pre-Eclampsia into Pre-Eclampsia with Severe Features

Background: Pre-eclampsia (PE), a complex, multisystem, pregnancy-associated hypertensive disorder, typically developing after the 20th week of gestation, that complicates 2% - 8% of pregnancies, is a leading cause of neonatal and maternal mortality and morbidity. Aim of the Work: To identify different factors predicting transformation of non-severe pre-eclampsia in to pre-eclampsia with severe features. Patients and Methods: This prospective cohort study was conducted at tertiary care hospital at Ain Shams University hospitals from June 2021 till January 2022 and performed on total of 100 patients who diagnosed as non-severe pre-eclampsia after exclusion of severity features. Results: The current study revealed that transformation to severe pre-eclampsia occurred in 33% of the studied cases. Body mass index (BMI), past and family histories of preeclampsia statistically were significantly higher in cases transformed into preeclampsia with severe features. Admission blood pressure, albumin dipstick, Oligohydramnios and IUGR statistically were significantly higher in cases with transformation from non-severe pre-eclampsia into pre-eclampsia with severe features. Platelet count statistically was significantly lower in cases with transformation from non-severe pre-eclampsia into pre-eclampsia with severe features Conclusion: Our study results identified the most important clinical risk factors for transformation to severe features of pre-eclampsia from non-severe features and provided new information on the level of risk associated with specific combinations of risk factors (BMI ≥ 35.4, admission systolic blood pressure, admission diastolic blood pressure, albumin dipstick 4+ and platelets count) with low significant diagnostic performance in predicting transformation from non-severe pre-eclampsia into pre-eclampsia with severe features.

The identification of jets originating from quarks and gluons,often referred to as quark/gluon tagging,plays an important role in various analyses performed at the Large Hadron Collider,as Standard Model measurements and searches for new particles decaying to quarks often rely on suppressing a large gluon-induced *** paper describes the measurement of the efficiencies of quark/gluon taggers developed within the ATLAS Collaboration,using √s=13 TeV proton–proton collision data with an integrated luminosity of 140 fb^(-1) collected by the ATLAS *** taggers with high performances in rejecting jets from gluon over jets from quarks are studied:one tagger is based on requirements on the number of inner-detector tracks associated with the jet,and the other combines several jet substructure observables using a boosted decision tree.A method is established to determine the quark/gluon fraction in data,by using quark/gluon-enriched subsamples defined by the jet *** in tagging efficiency between data and simulation are provided for jets with transverse momentum between 500 GeV and 2 TeV and for multiple tagger working points.
