Management of tear trough deformity with and without tear trough-orbicularis retaining ligament complex release in transconjunctival blepharoplasty: a comparative study

AIM: To evaluate the effect of releasing the orbicularis retaining ligament(ORL) complex of the tear trough combined with the transconjunctival blepharoplasty in the surgical management of tear trough deformity compared with the effect of blepharoplasty alone. METHODS: A prospective, randomized(by closed envelope technique), controlled surgical trial which included 50 patients(100 eyes) with bilateral visible tear trough deformity and lower eyelid fat bulging, was divided into two groups where 25 patients(50 eyes) had tear trough-ORL release with blepharoplasty and 25 patients(50 eyes) didn't. Qualitative and quantitative assessments of tear trough deformity were done at 6 mo postoperatively, achieving grade 0 or 1 of Barton's classification was considered a success. Standardized photographic documentation of each patient was done pre and postoperatively, also assessment of the patients' satisfaction postoperatively was done and ranked as excellent, very good, good or fair. RESULTS: There was statistically significant difference between the two groups in the overall aesthetic results postoperatively regarding the qualitative and quantitative analysis of the tear trough deformity, where patients who had tear trough-ORL complex release had more successful outcomes than those of the second group. CONCLUSION: ORL release should be done in patients with tear trough deformity in order to release the tethering effect of this ligament which causes the prominence of the naso jugal groove.

Early diagnostic evaluation of miR-122 and miR-224 as biomarkers for hepatocellular carcinoma

Hepatocellular carcinoma(HCC)is one of the common lethal types of tumor all over the *** lethality of HCC accounts for many *** of them,the lack of reliable diagnostic markers at the early stage,in this context,serum miRNAs became promising diagnostic ***,we aimed to identify the predictive value of two miRNAs(miR-122 and miR-224)in plasma of patients with HCC preceded by chronic HCV *** miRNA assays specific for hsa-miR-122 and hsa-miR-224 were used to assess the expression levels of the chosen miRNAs in plasma samples collected from three groups;40 patients with HCC related to HCV,40 with CHC patients and 20 healthy *** study revealed that the mean plasma values of miRNA-122 were significantly lower among HCC group when compared to CHC and control groups(P1.2(RQ)and(AUC Z 0.93,P<0.001),while the accuracy of AFP to diagnose HCC was(AUC:0.619;P Z 0.06).In conclusion,the expression plasma of miR-122 and miR-224 could be used as noninvasive biomarkers for the early prediction of developing HCC at the early stage.

Expanded Endoscopic Endonasal Approach (EEEA) for Clival Chordomas

Objective: To evaluate the role of expanded endoscopic endonasal approach in removal of clival chordomas. Patients & Methods: Nine patients with clival chordomas were operated upon in Cairo University hospital from September 2015 to September 2018 using the EEEA a recurrent case and seven new cases were involved in these study and ten operations were done. All patients had preoperative neurological and radiological examination. The study was focusing on the approach, efficacy of tumor removal, reconstruction of the base and complications related to this approach. Results: Nine patients were operated in this study in which ten operations were done. It included six males (66.6%) and three females (33.3%) with age ranging from 4 years to 63 years with average age 40.7 years. Headache and diplopia were the most common symptoms found in six patients (66.6%). Brainstem affection was found in two patients (22.2%). Lower cranial nerves affection was found in two patients (22.2%). One case developed CSF leakage postoperatively (11.1%). Two patients underwent tracheostomy. We achieve total removal in four patients (44.4%), near total removal in one patient (11.1%) and subtotal tumor resection in four patients (44.4%). Conclusion: EEEA for clival chordomas is safe and effective approach regarding the results of the incidence of complications, and the percentage of tumor resection.

This paper provides a comprehensive examination of El Sallam Garden in Port Said City,concentrating on its landscape characteristics and potential for design *** study looks at how space syntax can be used to assess the impact of a tree planting design’s spatial configuration on an urban park’s visual *** play an important role in determining the spatial characteristics of an outdoor *** to space syntax theory,an urban area is a collection of connected spaces that can be represented by a matrix of quantitative properties known as syntactic *** simulations can be used to measure the quantitative properties of these *** study uses space syntax techniques to assess how tree configurations and garden area which can affect the social structures of small-scale gardens in Port *** also looks at how these techniques can be used to predict the social structures of four garden zones in El Sallam *** study includes an observational and space syntax study through comparative analysis of four garden zones in El Sallam *** results of the study show that the area and planting configurations of the garden had a significant effect on the syntactic social and visual measures of the urban *** conclusions and recommendations can be a useful tool for landscape architects,urban planners,and legislators who want to enhance public areas and encourage social interaction in urban settings.

Simulation of Radial Variation of Neutral Atoms on Edge Plasma of Small Size Divertor Tokamak

Simulations have been performed to investigate the impact of radial variation of neutral atoms (neutral puff) on the edge plasma of small size divertor tokamak. It was demonstrated that, the variation of neutral atoms (neutral puff) in edge plasma of small size divertor tokamak generates additional large radial electric field and large radial electric field shear near separatrix which can significantly influence global confinement by affecting the transition from low (L) to high (H) confinement. This simulation was performed by using B2SOLPS0.5.2D fluid transport code based on a reduced form of the transport form of transport equations. These transport equations are implemented in B2SOLPS0.5.2D fluid transport code and solved for the parameters of Small Size Divertor tokamak. The results of simulation by fluid transport B2SOLPS0.5.2D code can be summarized as follow: 1) The Plasma Parameters are significantly effect by neutral atoms puffing in the edge plasma of small size divertor tokamak;2) Contrary to previous expectation [1] of the predominant role of neutral viscosity on toroidal flux, anomalous transport was found to be mainly effect on toroidal flux;3) Puffing of gas (neutral) in the edge plasma of small size divertor tokamak produce strong ITB;4) Puffing of gas (neutral) in the edge plasma of small size divertor increase plasma density especially in SOL through charge exchange and ionization processes;5) Puffing of gas (neutral) in the edge plasma of small size divertor has significant effect on the distribution of plasma heat flux;6) The radial electric field is affected by gas (neutral) puffing in the edge plasma of small size divertor;7) Puffing of neutral (atoms) in the edge plasma of small size divertor tokamak produce large radial electric field shear which contribute to L-H transition;8) The centrifugal effect has no influence on distribution of the radial profile of parallel (toroidal) velocity of edge plasma of small size divertor tokamak during gas

Evaluation of GALAD Score in Diagnosis and Follow-up of Hepatocellular Carcinoma after Local Ablative Therapy

Background and Aims:Strategies for detection of early hepatocellular carcinoma(HCC)are still *** GALAD score is a serum biomarker-based model designed to predict the probability of having *** aimed to assess the ability of GALAD score to diagnose early HCC and its validity to follow patients after local ablation ***:This multicenter prospective study included 108 patients in two groups,58 HCC patients(67 focal lesions)with local ablative therapy(study group),and a control group of 50 patients with liver *** GALAD scores of the study and control groups,and of the HCC patients before and after ablative therapy were ***:Most patients were men(74.1%in study group and 76%in controls)with hepatitis C virus infection(98.30%in the study group,and 94%in controls).GALAD scores were significantly higher in HCC patients than in those with benign cirrhosis(2.65 vs.−0.37,p=0.001).Ablative therapy was successful in 94.4%of focal lesions<2 cm,and in 86.10%of 2–5 cm *** GALAD score was also significantly lower at 1 month after ablation in patients with well-ablated tumors(2.19 vs.0.98,p=0.001).The best cutoff values of GALAD score for diagnosis of early HCC,and for prediction of well ablation of HCC were 0.74 and≤3.31(areas under the curve of 0.92 and 0.75,sensitivities of 84.48%and 76.19%,specificities of 89.13%and 83.33%,positive predictive values of 90.74%and 94.1%,and negative predictive values of 82%and 35.7%respectively).Conclusion:The GALAD score was effective for the diagnosis of early HCC and for followup after ablative therapy.

A Note on the Precision of Stratified Systematic Sampling

Conflicting views had greeted the use of systematic sampling for sample selection and estimation in stratified sampling in terms of the precision of the population mean base on the inherent characteristics of the population. These conflicting views were analyzed using Cochran data (1977, p. 211) [1]. When the population units are ordered, variance of systematic sampling for all possible systematic samples provides equal, non-negative and most precise estimates for all the variance functions considered i.e. , unlike when a single systematic sample is used and when variance of simple random sampling is used to estimate selected systematic samples.
