Jejuno-Ileal Ruptures after Blunt Abdominal Trauma at the Teaching Hospital of Bouake

Objective: The aim of this study is to determine the causes, pattern, management, and outcome of jejunal-ileal rupture following blunt abdominal trauma at the teaching Hospital of Bouake. Methods: The study included 27 patients who underwent laparotomy for jejunoileal injuries from blunt abdominal trauma at the Teaching Hospital of Bouake over a period of 14 years from January 2007 to December 2020. A retrospective study was conducted and the patients were analyzed with respect to patient demographics data, cause, injury mechanisms, presentation, anatomical distribution, diagnostic methods, associated injuries, treatment and outcomes. Results: During the 14 year period from 2007 to 2020, 27 patients with blunt small bowel injuries were treated at our Teaching Hospital. That is 2.9% of all blunt abdominal trauma. Male to female ratio was 4.4:1 and the average age was 26.3 years (range: 15 and 50 years). The majority (66.7% 18 cases) were Victims of road traffic accident. Median delay between injury and arrival at hospital for these patients was 10.1 hours (range: 1 - 72 h). A single intestinal injury was present in 22 patients, while 5 patients suffered from 2 injuries. There were 32 perforations of the small intestine in 27 patients with two perforations being transection. All perforations were located on the antimesenteric border of the intestine. Out of the 32 jejuno-ileal ruptures 18 were located on the jejunum while 14 were located on the ileum. Associated intra-abdominal injuries were present in 6 patients and nineteen associated extra-abdominal injuries were present in 10 (70.4%) patients. A one-stage therapeutic strategy was performed in 20 cases (74.1%) and a two-stage strategy was performed in seven cases (25.9%). Intestinal continuity was restored 93.8 days later (range 60 and 140 days). The average length of hospital stay of the operated patients was 11.4 days. The operative morbidity was 25.9%. Economic Value of Crop Productivity Biodiversity in ljebu Ode Agricultural Zone of Ogun State, Nigeria

Biological diversity involves genetic, species, and ecosystem diversity. Genetic diversity denotes the variation within species in the functional units of heredity present in any plant or animal, microbial or other origin of living things. Species diversity encompasses the variety of species--whether wild or domesticated, within a geographical area. This study assesses the economic value of crop productivity biodiversity in Ijebu Ode agricultural zone of Ogun State. The authors proposed a dynamic model of market equilibrium with vertical product differentiation that enabled us to take into account the economic consequences of respondents＇ monthly income and species crop of three different communities. The objectives of the study are to identify the personal and socio-economic characteristics of farmers and determine the importance of biodiversity in the study area using Simpson＇s Index. The result showed that 52.50% were male, majority （32.50%） were within the age range of 31 and 40 years old, 44.17% were married, 62.50% had household size of 6-10, and majority （45.83%） had secondary education while 46.67% had up to 20 years farming experience. In conclusion, the authors find that effective factors play an important role in explaining the support for biodiversity conservation, particularly when people do not have specific knowledge about species; while ecological-scientific considerations appear when people are well-informed.

Inheritance of Angular Leaf Spot [Phaeoisariopsis griseola （Sacc.） Ferr] Resistance in Common Bean （Phaseolus vulgaris L,） Population Developed from Kablanketi x Mexico 54

The genetic resistance to angular leaf spot （ALS） caused by Phaeoisariopsis griseola in the common bean cultivar Mexico 54 was investigated on disease reactions in parental, F1, F2 and backcross generations derived from crosses between a resistant cultivar Mexico 54 and a susceptible cultivar Kablanketi under screen house conditions. The heritability （h2） estimate was as high as 0.719 indicating a successful transfer of ALS resistance among progenies and thus selection can be performed in early generation. High heritability coupled with high expected genetic advance of 39.5% is considered to be more useful in predicting the outcome of selecting the best individuals. Chi-square values were computed to determine whether the observed ratios for disease reactions deviated from expected Mendelian ratios for a single, dominant gene controlling resistance to angular leaf spot in common bean. Based on the resistance of the F2, and the backcross generation to the resistant parent, a 3 resistant： 1 susceptible segregation ratio in the F2 and a 1 resistant： 1 susceptible segregation ratio in the backcross generation to the susceptible parent was obtained implying that resistance to the isolate ofPhaeoisariopsis griseola is governed by a single, dominant gene.

Food Consumption Patterns among Pre-School Children 3 - 5 Years Old in Mateka, Western Kenya

Hunger and malnutrition are burdens that are pronounced in developing countries where they manifest themselves in the forms of protein energy malnutrition. Malnutrition compromises the child’s immune system leading to direct mortality and increased vulnerability to infectious diseases stunting and poor brain development. This study sought to analyze the food consumption patterns of children 3 - 5 years old attending Mateka Primary school, Bungoma County. This study adopted a cross sectional survey. One hundred and twenty five (125) children selected from three ECD classes in the school formed the sample size. Purposive and simple random sampling techniques were used to select the study area and the children respectively. Structured questionnaires were administered to the caregivers of the children. Information on demographics, socio-economic status, and food consumption patterns were gathered. Food consumption patterns were assessed using a HDDS and a quantitative food frequency questionnaire. Socio-demographic data was analyzed using (SPSS) Version 21 (2007) and dietary data was analyzed using Nutri-Survey for Windows (2007). Results revealed that most of the households were of low socio-economic characteristics. The most consumed foods were cereals, roots and tubers. Majority (55.2%) of the children had low dietary diversity, 29.1% had medium dietary diversity and 15.7% had greater dietary diversity. The children were deficient in energy, protein, Vitamin A, Iron and Zinc. It can be concluded that the diets fed to the children are inadequate to meet their nutrient intakes for physiological development and growth. Further research should be done to document the prevalence of micronutrient malnutrition among the children.

Noise Exposure Levels in Basic School Environments in a City in Ghana

Basic schools that are located in heavily populated residential and commercial areas in Ghana are exposed to environmental noise that can have detrimental effects on the academic attainments of children. Limited studies have been conducted on noise level in basic schools to date in Ghana. The objective of the study was to assess noise levels in and around learning spaces in basic schools in Ghana with a focus on Kumasi. A survey employing the use of interviews and an empirical monitoring of noise levels in and around learning spaces in four selected basic schools were adopted. The study revealed that mean outdoor and indoor noise levels exceed the World Health Organisation permissible limits by 30 - 40 percent and 90 - 107 percent respectively for schools in the commercial zones and schools that are located near highways. The school that is sited in an educational environment with a setback from the highway attained mean noise levels ranging from 1.2 percent below to 3.7 percent above the permissible limits for outdoor and between 64 - 105 percent for indoor. The outcome of the study is expected to engender the design of environmental noise resilient buildings to facilitate teaching and learning in basic schools in Ghana.

Hydatid Cyst of the Liver: About a Case at the Digestive and General Surgery Department of the University Hospital of Bouaké(Côte d’Ivoire)

Hydatid cyst of the liver (KHF) is a parasitic condition caused by the development of the larval form of the dog tapenia Echinococcus granulosus. In sub-Saharan Africa, few writings are interested in this pathology, which remains common and constitutes a public health problem in highly endemic countries [1] [2]. We report the observation of a 61-year-old hypertensive patient owner of a dog admitted for abdominal pain without abdominal mass. In whom ultrasound and computed tomography have made it possible to make the diagnosis of hydatid cyst of the liver (KHF). The patient underwent surgery. We performed an associated conservative treatment post-operative albendazole. The post-operative follow-up was simple. In our patient the evolution was good after a setback of more than 2 months. Through this observation and a review of the literature, we insist on the contribution of imaging in diagnosis and treatment, which is essentially surgical.

A CASE OF INSOMNIA IN PREGNANCY

Insomnia in pregnancy :is a very common condition in Shanghai, It is always mild, and often lasts only a few nights. When it reduces a woman’s sleep to one to two hours every night for a period of six to seven months, it becomes a rarity. The following is a case of this kind which is given in detail. A Chinese housewife, aged 27, para ii, consulted me for the first time on January 8th, 1935, with the complaint of insomnia and mild symptoms of morning sickness. Her last menstrual period was October 23rd, 1934, and her expected date for delivery was July 30th, 1935.
