Background Acute bilirubin encephalopathy (ABE) still represents a significant cause of morbidity and mortality throughout the world,especially in developing *** aimed to determine the prevalence of ABE based on the Johnson bilirubininduced neurologic dysfunction (BIND) score and to describe the discharge *** We prospectively studied all newborns ≥ 35 weeks with ABE by evaluating signs of mental sensorium,muscle tone,and cry patterns over 1 *** 12% (81 out of 674) of the newborns admitted for neonatal hyperbilirubinemia had a BIND score > *** admission age was 6 ± 4.1 days;total serum bilirubin (TSB) was 31.2± 10 mg/dL (range 17.5-75.2).Of these newborns,40.7% and 21% had evidence of haemolysis and sepsis,*** mortality was 9.9%;58% of the newborns showed signs of mild-to-moderate BIND at discharge,while 32.1% survived with an apparently normal *** BIND score was significantly correlated with admission TSB (r=0.476,P < 0.001).Similarly,BIND score at discharge was correlated with admission TSB (r=0.442,P < 0.001) and admission BIND score (r =0.888,P < 0.001).The regression model showed that admission TSB (P < 0.001) and maternal illiteracy (P =0.034) were predictors of the BIND score at admission,while admission BIND score was the best indicator of the discharge score (P < 0.001).Conclusions ABE is still a major problem in our *** TSB and maternal illiteracy are good predictors of bilirubin encephalopathy at admission and discharge.

BACKGROUND Elective cholecystectomy(CCY)is recommended for patients with gallstone-related acute cholangitis(AC)following endoscopic decompression to prevent recurrent biliary ***,the optimal timing and implications of CCY remain *** To examine the impact of same-admission CCY compared to interval CCY on patients with gallstone-related AC using the National Readmission Database(NRD).METHODS We queried the NRD to identify all gallstone-related AC hospitalizations in adult patients with and without the same admission CCY between 2016 and *** primary outcome was all-cause 30-d readmission rates,and secondary outcomes included in-hospital mortality,length of stay(LOS),and hospitalization *** Among the 124964 gallstone-related AC hospitalizations,only 14.67%underwent the same admission *** all-cause 30-d readmissions in the same admission CCY group were almost half that of the non-CCY group(5.56%vs 11.50%).Patients in the same admission CCY group had a longer mean LOS and higher hospitalization costs attrib-utable to *** the most common reason for readmission was sepsis in both groups,the second most common reason was AC in the interval CCY *** Our study suggests that patients with gallstone-related AC who do not undergo the same admission CCY have twice the risk of readmission compared to those who undergo CCY during the same *** readmis-sions can potentially be prevented by performing same-admission CCY in appropriate patients,which may reduce subsequent hospitalization costs secondary to readmissions.

BACKGROUND In severe cases of coronary artery disease,percutaneous coronary intervention provide promising *** stent used could be a drug-eluting stent(DES)or a titanium-nitride-oxide coated stent(TiNOS).AIM To compare the 5-year effectiveness and safety of the two stent *** The following systematic review and meta-analysis was conducted in accordance with the preferred reporting items for systematic reviews and meta-analysis guidelines,and PubMed/MEDLINE,Scopus,and Cochrane Central were searched from inception till August *** outcomes were major adverse cardiac events(MACE),cardiac death,myocardial infarction(MI),cardiac death or MI,and ischemia-driven total lesion revascularization(ID-TLR).RESULTS Four randomized controlled trials(RCT),which analyzed a sum total of 3045 patients with acute coronary syndrome(ACS)after a median follow-up time of 5 years were *** statistically insignificant,an increase in the ID-TLR was observed in patients receiving TiNOSs vs *** addition,MI,cardiac death and MI,and definite stent thrombosis(DST)were significantly decreased in the TiNOS *** analysis revealed no significant results with meta-regression presenting non-ST elevated MI(NSTEMI)as a statistically significant covariate in the outcome of *** TiNOS was found to be superior to DES in terms of MI,cardiac death or MI,and DST outcomes,however,the effect of the two stent types on ID-TLR and MACE was not significant.A greater number of studies are required to establish an accurate comparison of patient outcomes in TiNOS and DES.

Aim: To investigate depression, anxiety, PTSD, and insomnia levels among Sudanese citizens after a year has passed since the start of Armed Forces conflict in Sudan. Methods: An online survey was distributed. It is composed of five parts, covering the following areas: 1) sociodemographic data;2) depression assessment;3) generalized anxiety disorder assessment;4) post-traumatic stress disorder assessment;and 5) insomnia assessment. Statistical Package for Social Sciences version 27 was used for data analysis;frequency and percentage were used to describe the qualitative variables. Spearman’s correlation analysis and Chi-square test were used for correlation and association analysis;a P-value equal to or less than 0.05 was considered statistically significant. Results: The study included 283 participants, primarily female (76.0%), with a mean age of 35.26 ± 6.96 years. The majority of participants were married (50.5%) and had a university level of education (50.2%). At the start of the war, 70.3% of participants were inside the war zones, and at the time of data collection, 73.5% were outside Sudan. The length of stay in the war zone ranged from 1 to 300 days, with a mean of 48.59 ± 70.284 days. Additionally, 63.3% of participants stated that they did not receive any form of mental health intervention, and the remaining did not provide an answer. There was a high prevalence of moderate-severe depression (50.5%), moderate anxiety (35.3%), PTSD (56.5%), and sub-threshold insomnia (53.4%) among the participants. Correlation analyses revealed associations between demographic factors such as sex, age, marital status, and education and mental health issues. Notably, female participants showed a higher prevalence of PTSD (53.0%) compared to male participants (67.6%). Rhabdomyolysis, acute kidney injury and diabetic ketoacidosis all are life-threatening conditions. Coincidence of them can be missed because the patient is usually asymptomatic or has mild symptoms related to rhabdomyolysis. The development of rhabdomyolysis as a complication of diabetic ketoacidosis is not well understood and only a few clinical studies address the phenomenon. We are reporting, along with a literature review, a 39-year-old male, who presented with diabetic ketoacidosis complicated with rhabdomyolysis and acute kidney injury. To the best of our knowledge, this is the first case reported in the United Arab Emirates. We highlight the syndrome because it is curable with early detection and proper treatment.

Rift Valley Fever (RVF) is an emerging, mosquito-borne disease with serious economical and negative implications on human and animal health. This study was conducted to verify the factors which influenced the spatial pattern of Rift Valley Fever occurrence and identified the high risk areas for the occurrence of the disease at Sinner State, Sudan. The normalized difference vegetation index (NDVI) derived from Moderate Resolution Imaging Spectroradiometer (MODIS) satellite and rainfall data in addition to the point data of RVF clinical cases in humans were used in this study. In order to identify the RVF high risk areas, remote sensing data and rainfall data were integrated in a GIS with other information including, soil type, water body, DEM (Digital Elevation Model), and animal routes and analyzed using Spatial Analysis tools. The information on clinical cases was used for verification. The Normalized Difference Vegetation Index (NDVI) was used to describe vegetation patterns of the study area by calculating the mean NDVI. The results of the study showed that, RVF risk increased with the increase in vegetation cover (high NDVI values), and increase in rainfall, which both provided suitable conditions for disease vectors breeding and a good indicator for RVF epizootics. The study concluded that, identification of high risk area for RVF disease improved the understanding of the spatial distribution of the disease and helped in locating the areas where disease was likely to be endemic and therefore preparedness measures should be taken. The identification represents the first step of prospective predictions of RVF outbreaks and provides a baseline for improved early warning, control, response planning, and mitigation. Further detailed studies are recommended in this domain.

Invasive fungal infections have grown significantly over the last two decades, owing to an increase in immunocompromised hosts and geriatric patients. When the host’s defenses are compromised, such infections are associated with severe morbidity and mortality. Here, a rare case of fungal infection in a 61-year-old immunocompetent male patient from Saudi Arabia was reported, who suffered from pulmonary hemorrhage and Systemic Lupus Erythematous. Bronchoalveolar Lavage was used as a diagnostic tool to identify the fungus reported in the case. The pathogenic fungal specie identified as Magnusiomyces capitatus, in macroscopic and microscopic morphological characteristics of the colonies. Based on clinical evidence, liposomal amphotericin formulation was recommended for initial therapy against fungal infection. Also, liposomal amphotericin B induced mycological eradication up to 70 percent in patients with proven Magnusiomyces capitatus infection. In addition to addressing suspected Systemic lupus erythematosus, the patient’s health has improved with no evidence of pulmonary bleeding and hemoptysis.

Background and objective The expression of various trace elements and markers in lung cancer is *** aim of this study is to evaluate the presence of calcium(Ca),copper(Cu),iron(Fe) and carcinoembryonic antigen(CEA) in cancerous untreated lung tissues and to determine a possible association between these markers and lung *** Fourty-eight cancerous lung tissue blocks,from Sultan Qaboos University Hospital,Sultanate of Oman,were ***,Ca,Cu,and CEA were demonstrated in the tissue blocks using Perl's Prussian blue,Von Kossa's,modified rhodanine and immunohistochemical staining methods,*** Twenty-three of 48 specimens showed positive Fe staining,2 showed positive Ca staining and Cu was absent in all specimens.93.7% expressed CEA in varying degree of positivity.81.25% of these sections showed high expression of *** Tissue concentrations of trace elements were not elevated in lung cancer and therefore cannot be considered as a potential *** the low sensitivity and specificity of CEA as previously reported,tissue CEA should be considered as a potential marker in the evaluation of lung cancer.
