Mental Health Consequences among Sudanese Due to the Armed Conflicts and Civil Unrest of 2023: One Year Later

Aim: To investigate depression, anxiety, PTSD, and insomnia levels among Sudanese citizens after a year has passed since the start of Armed Forces conflict in Sudan. Methods: An online survey was distributed. It is composed of five parts, covering the following areas: 1) sociodemographic data;2) depression assessment;3) generalized anxiety disorder assessment;4) post-traumatic stress disorder assessment;and 5) insomnia assessment. Statistical Package for Social Sciences version 27 was used for data analysis;frequency and percentage were used to describe the qualitative variables. Spearman’s correlation analysis and Chi-square test were used for correlation and association analysis;a P-value equal to or less than 0.05 was considered statistically significant. Results: The study included 283 participants, primarily female (76.0%), with a mean age of 35.26 ± 6.96 years. The majority of participants were married (50.5%) and had a university level of education (50.2%). At the start of the war, 70.3% of participants were inside the war zones, and at the time of data collection, 73.5% were outside Sudan. The length of stay in the war zone ranged from 1 to 300 days, with a mean of 48.59 ± 70.284 days. Additionally, 63.3% of participants stated that they did not receive any form of mental health intervention, and the remaining did not provide an answer. There was a high prevalence of moderate-severe depression (50.5%), moderate anxiety (35.3%), PTSD (56.5%), and sub-threshold insomnia (53.4%) among the participants. Correlation analyses revealed associations between demographic factors such as sex, age, marital status, and education and mental health issues. Notably, female participants showed a higher prevalence of PTSD (53.0%) compared to male participants (67.6%). Magnusiomyces capitatus in Immune-Competent Patients with Pulmonary Haemorrhage and Systemic Lupus Erythematosus

Invasive fungal infections have grown significantly over the last two decades, owing to an increase in immunocompromised hosts and geriatric patients. When the host’s defenses are compromised, such infections are associated with severe morbidity and mortality. Here, a rare case of fungal infection in a 61-year-old immunocompetent male patient from Saudi Arabia was reported, who suffered from pulmonary hemorrhage and Systemic Lupus Erythematous. Bronchoalveolar Lavage was used as a diagnostic tool to identify the fungus reported in the case. The pathogenic fungal specie identified as Magnusiomyces capitatus, in macroscopic and microscopic morphological characteristics of the colonies. Based on clinical evidence, liposomal amphotericin formulation was recommended for initial therapy against fungal infection. Also, liposomal amphotericin B induced mycological eradication up to 70 percent in patients with proven Magnusiomyces capitatus infection. In addition to addressing suspected Systemic lupus erythematosus, the patient’s health has improved with no evidence of pulmonary bleeding and hemoptysis.

Important operator characteristics (such boundedness or compactness) for particular classes of operators on particular reproducing kernel Hilbert spaces may be impacted by the behaviour of the operators on the reproducing kernels. These results have been shown for Toeplitz operators on the Paley-Wiener space, a reproducing kernel Hilbert space over C. Furthermore, we show how the norm of such an operator has no relation to the supremum of the norms of the pictures of the normalization reproducing kernels of the space. As a result, if this supremum is finite, the operator is implicitly bounded. To further demonstrate that the operator norm is not the same as the supremum of the norms of the pictures of the real normalized reproducing kernels, another example is also provided. We also set out a necessary and sufficient condition for the operators’ compactness in terms of their limiting function on the reproducing kernels.

Prognostic significance of peritoneal metastasis from colorectal cancer treated with first-line triplet chemotherapy

Protective effects of Balanites aegyptiaca extract, melatonin and ursodeoxycholic acid against hepatotoxicity induced by methotrexate in male rats

Objective: To compare the degree of ameliorative effects of Melatonin(MEL), Ursodeoxycholic acid(UDCA) and Balanites aegyptiaca(BA) against hepatotoxicity induced by MTX for one month. Methods: Eighty adult male rats(Sprague Dawely) weighing(190±10g), were randomly divided into eight equal groups: Control, MTX, MEL, BA, UDCA, MTX+MEL, MTX+BA, MTX+UDCA. Liver function biomarker enzymes, liver tissue oxidative stress parameters, together with total antioxidant capacity and tumor necrosis factor(TNF-α) were determined. Histopathological and immunohistochemistry examinations for TNF-α were also done. Results: MTX showed significant increase in alanine transaminase(ALT), aspartate transaminase(AST), alkaline phosphatase(ALP), gamma glutamyl transferase(GGT), total and direct bilirubin, as well as TNF-α levels, oxidized glutathione(GSSG), malodialdehyde(MDA) and nitric oxide(NO). whereas, total protein, albumin, total antioxidant capacity, reduced glutathione(GSH), glutathione peroxidase(GPx), glutathione reductase(GR), glutathione S-transferase(GST), superoxide dismutase(SOD) and catalase(CAT) levels were significantly decreased in MTX treated group. These alterations were improved by MEL and BA treatment, whereas no improvement was noticed in UDCA treatment. Conclusions: BA may be as promising as MEL in the hepatoprotection against MTX toxicity through their antioxidant and radical scavenging activities. In addition, it is not recommended to co-administer UDCA with MTX as it enhanced inflammation and damage to the liver.

Investigation of Spatial Risk Factors for RVF Disease Occurrence Using Remote Sensing &GIS—A Case Study: Sinnar State, Sudan

Rift Valley Fever (RVF) is an emerging, mosquito-borne disease with serious economical and negative implications on human and animal health. This study was conducted to verify the factors which influenced the spatial pattern of Rift Valley Fever occurrence and identified the high risk areas for the occurrence of the disease at Sinner State, Sudan. The normalized difference vegetation index (NDVI) derived from Moderate Resolution Imaging Spectroradiometer (MODIS) satellite and rainfall data in addition to the point data of RVF clinical cases in humans were used in this study. In order to identify the RVF high risk areas, remote sensing data and rainfall data were integrated in a GIS with other information including, soil type, water body, DEM (Digital Elevation Model), and animal routes and analyzed using Spatial Analysis tools. The information on clinical cases was used for verification. The Normalized Difference Vegetation Index (NDVI) was used to describe vegetation patterns of the study area by calculating the mean NDVI. The results of the study showed that, RVF risk increased with the increase in vegetation cover (high NDVI values), and increase in rainfall, which both provided suitable conditions for disease vectors breeding and a good indicator for RVF epizootics. The study concluded that, identification of high risk area for RVF disease improved the understanding of the spatial distribution of the disease and helped in locating the areas where disease was likely to be endemic and therefore preparedness measures should be taken. The identification represents the first step of prospective predictions of RVF outbreaks and provides a baseline for improved early warning, control, response planning, and mitigation. Further detailed studies are recommended in this domain.

The oncofetal protein IMP3 is an indicator of early recurrence and poor outcome in mucoepidermoid carcinoma of salivary glands

Objective: Mucoepidermoid carcinoma(MEC) is the most common primary malignancy of the salivary glands. Insulin-like growth factor-II m RNA-binding protein-3(IMP3) is an important prognostic factor in some cancers and a tool that differentiates between benign and malignant pancreatic lesions. This study aimed to identify a relationship between the expression of IMP3 and the outcome of salivary gland MEC, as well as to differentiate MEC from pleomorphic adenoma(PA).Methods: Tissue specimens from 70 cases of salivary gland MEC, 40 cases of PA, and 10 cases with normal salivary gland were examined immunohistochemically for IMP3. The association among the expression of IMP3, clinicopathological characteristics and patient's survival was ***: IMP3 was present in 51.4% of MEC but absent in PA and normal salivary gland tissues. IMP3 expression was associated with age > 60 years, submandibular gland tumors, tumor size > 4 cm, high-grade tumors, lymph node metastasis, involvement of surgical margins, perineural invasion, distant metastasis, advanced TNM stage, tumor relapse, and death(Postic factors of disease-free survival(DFS). In addition, IMP3 was a strong predictor of overall survival(os) together with distant metastasis and intermediate and high-grade ***: IMP3 expression is highly important in evaluating the outcome of MEC. IMP3 can be used to differentiate MEC from PA of salivary glands.

Aim of the study: To identify all clinico-pathological data, different treatment modalities and the different prognostic factors which affected the locoregional control (LC), disease-free survival (DFS), and overall survival (os) of Type II endometrial cancer patients. Patients and methods: Data of Type II endometrial carcinoma patients who presented to the Radiation Oncology department, National Cancer Institute, Cairo University during the period from (2000-2012) were retrospectively reviewed. Results: Multivariate analysis identified stage as an independent prognostic factor for os & DFS, and age was an independent prognostic factor for DFS and LC. Low pretreatment hemoglobin levels significantly affected os. Conclusion: Large and multicentric clinical trials are required to further study this group of patients and define optimum treatment modalities.

Impact of Pre-Radiotherapy and/or Chemoradiotherapy Hemoglobin Level on Response to Treatment in Laryngeal and Hypophayrngeal Squamous Cell Carcinoma

Background: Anemia is reported by many studies as an important risk factor for poor locoregional disease control and survival in head and neck carcinoma. We aimed to study the frequency and prognostic effect of low hemoglobin (Hb) level in head and neck squamous cell carcinoma (HNSCC) before radiotherapy (RT) and/or chemoradiotherapy (CRT). Material and Methods: We retrospectively studied the charts of 86 patients diagnosed with laryngeal and hypopharyngeal SCC in a university hospital in Cairo, Egypt. Based on the World Health Organization (WHO), anemia was diagnosed in males at Hb levels 13 g/dl in 44/75 (58.7%). Tumor sites were larynx in 77 (89.5%) and hypopharynx in 9 (10.5%) patients. Disease stage 3, 4 was common in 59/86 (68.6%) patients. Thirty-six (41.8%) patients had surgery. 13/86 (15.1%) patients received induction chemotherapy, while 31/86 (36.0%) had concomitant chemoradiotherapy. Radiotherapy was given in 65/86 (75.58%) patients. The mean RT dose was 66 Gy. Non-anemic patients had significant higher inci-dence of complete response (CR) to treatment (p = 0.034). 25.8% of male pa-tients with Hb 13 g/dl (p = 0.403). Female patients whether anemic or non-anemic had no recurrence (p = 0.036 and p = 0.403 respectively). The median duration of DFS and os was 6.52 and 9.33 months respectively. Diagnostic and Prognostic Value of Serum Tissue Polypeptide Antigen in Advanced Lung Cancer

Lung cancer is a lethal malignancy, however, no serum marker is routinely recommended till now. Prospectively two groups of patients included: Group I: Patients with advanced lung cancer. Group II: patients with benign lung disease as control. Serum Tissue Polypeptide Antigen (TPA) levels were measured by ELISA technique before the first line chemotherapy. The TPA cutoff taken was 1800 pg./ml. End points were comparison of high TPA in cases and controls and correlation between high TPA and disease progression (PD), progression free survival (PFS) and overall survival (os). 30 patients with advanced lung cancer (16 non-small and 14 small cell lung cancer) and 15 patients with benign lung disease were included and followed up during the period from October 2008 to October 2011 with median follow-up of 1.5 years. High TPA was found in 50% of lung cancer cases compared to 26% in controls (p = 0.014). High TPA was found in 64% of cases showing PD versus 36% normal TPA (p = 0.08). 1 year PFS in high TPA was 32% versus 39% in normal TPA, (p = 0.2). 1 year os in high TPA was 46% versus 73% in normal TPA (p = 0.6). Serum TPA is a potential marker for advanced lung cancer.
