Self-Expanding Metal Stenting for Malignant Colonic Tumours: A Prospective Study

Background: Self-expanding metal stents (SEMS) have been used in the management of malignant colorectal obstruction for palliation or as a bridging tool to single-stage surgery. We present the clinical results of a series of patients with colonic cancer in whom SEMS were inserted endoscopically under radiological guidance. Methods: Between September 2007 and January 2010, prospectively collected data from 21 patients who underwent SEMS insertion was analysed. This data includes demographics, indication for stenting, stent size, technical success, clinical success, complications, survival and duration of hospitalisation. Results: 14 male and 7 female patients with malignant colonic obstruction underwent SEMS insertion: 19 requiring palliation and 2 bridging to surgery. The rate of technical success was 100% and of initial clinical success was 100%. In 16/19 (84.2%) of the palliation group, clinical success was maintained at mean follow up of 3.4 months (1-6 months), while 3/19 (15.8%) died, two with functioning stents and one with stent occlusion. The two patients with operable tumours were successfully bridged to one-stage elective surgery at 1 month and 4 months following stenting. Post-procedure complications occurred in 5 patients: 1 perforation, 2 pain, 1 migration and 1 stent occlusion. All patients were discharged alive and the median hospital stay was 1 day (range: 1 to 13 days). Conclusion: SEMS provides an effective and safe option in the palliation of malignant colorectal obstruction. In operable patients, it provides a useful option to avoid colostomy, by facilitating safer single-stage surgery. In this prospective study of SEMS insertion, high rates of technical and initial clinical success were achieved. This could be attributed to performing the procedure under combined endoscopic and radiological guidance.

The simultaneous advances in the Internet of Things(IoT),Artificial intelligence(AI)and Robotics is going to revolutionize our world in the near *** recent years,LoRa(Long Range)wireless powered by LoRaWAN(LoRa Wide Area Network)protocol has attracted the attention of researchers for numerous applications in the IoT *** is a low power,unlicensed Industrial,Scientific,and Medical(ISM)bandequipped wireless technology that utilizes a wide area network protocol,i.e.,LoRaWAN,to incorporate itself into the network *** this paper,we have evaluated the LoRaWAN communication protocol for the implementation of the IoT(Internet of Things)nodes’communication in a forest *** outdoor performance of LoRa wireless in LoRaWAN,i.e.,the physical layer,has been evaluated in the forest area of Kashirampur Uttarakhand,***,the present paper aims towards analyzing the performance level of the LoRaWAN technology by observing the changes in Signal to Noise Ratio(SNR),Packet Reception Ratio(PRR)and Received Signal Strength Indicator(RSSI),with respect to the distance between IoT *** article focuses on estimating network lifetime for a specific set of LoRa configuration parameters,hardware selection and power *** the experimental results,it has been observed that transmissions can propagate to a distance of 300 m in the forest environment,while consuming approx.63%less energy for spreading factor 7 at 2 dBm,without incurring significant packet loss with PRR greater than 80%.

Recurrent Parotitis: Extrapulmonary Manifestation of Childhood Tuberculosis

Recurrent parotitis of childhood is a rare condition involving the largest salivary gland. The aetiology is multi-factorial and is usually non-obstructive or non-suppurative type of inflammation. It involves recurrent swelling of the parotid gland unilaterally or bilaterally over a period of months to years. Tuberculosis rarely involves the parotid gland, however in developing countries where tuberculosis prevalence is very high, any child with recurrent parotitis should be investigated for the same. We hereby, describe a 2 and half-year-old female child with recurrent parotid gland swelling below the right ear since 6 months. The swelling was acute in onset, gradually increasing in size with no discharge or pain. There was no history of fever, cough, cold, dryness of mouth or eyes, joint pain or rashes. There was history of koch’s contact in the maternal grandfather. A positive mantoux test and significant cervical lymphandenaopathy on Computed Tomography scan was noted. Common causes of recurrent parotitis, i.e. sjogrens syndrome, immunodeficiency and obstruction were ruled out. On the basis of the above history and investigations, the child was diagnosed as tuberculous parotitis.
