The world of healthcare is constantly evolving,and one area that has seen significant advancements in recent years is clinical pharmacy practice.1 In the past,pharmacists were mainly responsible for dispensing medications and ensuring that patients understood how to use them ***,the role of clinical pharmacists has expanded significantly in recent years,and they now play a critical role in optimizing medication regimens,providing personalized patient counselling,and collaborating with other healthcare professionals to improve patient outcomes.2-3 In the extension of the role of the clinical pharmacist,nowadays health economics is also added,which includes cost-benefit analysis,cost-effectiveness analysis,cost-utilization analysis and costminimization analysis,which will ultimately help the policy and decision makers to make the cost of the drugs affordable,so that all the patient population can avail the benefits.

Objective:Polycystic ovary syndrome(Pcos)is a pathophysiological disorder affecting reproductive and metabolic indices in *** present study was designed to compare the efficacy of metformin and D-chiroinositol in PcoS ***:In a tertiary care hospital in North India,prospective observational research was undertaken on 1o0 patients with PCOS,which was diagnosed based on European Society of Human Reproduction and Embryology Guidelines and ultrasound of lower *** study involves various clinical characteristics into consideration for the determination of statistical significance(P<0.05)in PCOS ***'s t-test along with the association between PCOS and patients taking metformin and D-chiro-inositol,as well as their impact on various biochemical parameters,were investigated finally using Pearson Correlation ***:This study comprises 50 patients taking metformin and 50 patients taking D-chiro-inositol in women suffering from *** mass index(BMI)and waist-to-hip ratio(WHR)were statistically significant(P<0.05)within the groups of both metformin and *** parameters such as luteinizing hormone(LH),follicle stimulating hormone(FSH),anti-mullerian hormone(AMH)and glycated hemoglobin(HbA1c)were found to be statistically significant(P<0.05)in both ***,FSH and AMH(14.40±0.52;14.28±0.53;1.99±0.10)were comparatively lower in patients taking D-chiro-inositol as compared to metformin group(14.17±0.42;19.88±1.01;2.61±0.04).HbA1c(3.71±0.08)with P<0.05 was found to be decreased more in metformin group as compared to patients taking D-chiro-inositol(4.90±0.09).A positive correlation was found between HbAlc and LH in metformin,&HbAlc and FSH in D-chiro-inositol groups,***:The results indicate that D-chiro-inositol shows better results in reducing clinical variables involved in causing PCOS as compared to metformin whereas metformin has better glycemic control in PCOS patients.

Objective:To study chemical constituents of essential oil of the roots of Nardostachys jatamansi found in different regions of Nepal and also to investigate phytochemical as well as antimicrobial activity of the sample with high yield of essential ***:The essential oils of roots of plant from five different regions were extracted by hydro-distillation and analyzed by gas chromatography-mass spectroscopy for their chemical *** root samples were also subjected to hydro-alcoholic extraction and then fractionated with hexane,chloroform,n-butanol and water so as to perform phytochemical screening and antimicrobial ***:The essential oil yield of sample from Jumla was found to be the highest followed by a sample from Nepalgunj,Surkhet and Kathmandu whereas that of sample from Dharan was found to be the *** chromatography-mass spectrometer analysis of essential oil of five samples showed that“2-beta pinene”appeared dominated in three samples,namely,6VJ Nepalgunj,9VJA Jumla and 10VJ ***“alkohol aus neoclovenoxid”in 8VJ Dharan and“methoxy phenyloxime”in 13VJA Kathmandu was found to be present in the highest *** screening of different fractions of sample 9VJA Jumla showed the presence of alkaloids,terpenoids,glycosides,proteins and amino acids,and carbohydrates *** susceptibility test of same fractions showed the n-butanol fraction potent against all pathogens and most affected one was Escherichia ***:Our study suggests that the essential oil of Nardostachys jatamansi found in Nepal contains more than 80 compounds with their quality and quantity differing from place to place.

Objective Robotic-assisted live donor nephrectomy(LDN)is being gradually adopted across transplant *** left donor kidney is preferred over right due to anatomical factors and ease of *** aimed to study donor and recipient outcomes after robotic procurement and subsequent open implantation of right and left *** All fully robotic LDNs and their corresponding open kidney transplants performed at our center between February 2016 and December 2021 were retrospectively *** Out of 196 robotic LDN(49[right]vs.147[left]),10(5.1%)donors had intra-operative events(6.1%[right]vs.4.8%[left],p=0.71).None of the LDN required conversion to open *** operative times were comparable for the two ***(13.3%)was the most common post-operative *** was no mortality in either LDN ***,we report our outcomes on 156 recipients(39 right and 117 left allografts)excluding robotic implants,exports,and pediatric *** were no significant differences between right and left kidney recipients with respect to 1-year post-transplant patient survival(100.0%vs.98.1%,p=0.45)or graft survival(93.9%vs.97.1%,p=0.11),or delayed graft function(7.7%vs.5.1%,p=0.55).Conclusion Non-hand-assisted robotic live donor nephrectomies can be safely performed with excellent *** LDN was not associated with higher incidence of complications compared to left *** implantation of robotically procured right renal allografts was not associated with higher risk of recipient complications.

In the present paper,we report a reproducible,cost effective,fast response method for detection of malathion and its effects on leukocytes in different human blood *** methods（UV-Vis spectrometry）and Fourier transform infrared coupled with solid phase extraction were applied for analyzing malathion content in human blood *** spiking levels of malathion in the range of 0.1-1.7μg/mL were extracted from blood plasma samples using *** present active functional groups（C=0;P-O-C;-OH;P=S）were also *** recovery rate of malathion was 80%±4.5%.The calculated correlation coefficient was 0.9799,indicating the linearity of the *** limit of detection（LOD）and limit of quantification（LOQ）were（0.1-1.7）μg/mL and（0.3-1.5）μg/mL,***〈1.0μg/mL showed no significant change while higher levels of malathion exposure（1.5μg/mL and 3.0｜ig/mL）reduced the number of white blood *** conclusion,the spectroscopic results may be useful to understand the mechanism of other pesticides such as methyl parathion and parathion.

The purpose of this study was to investigate the nuclear magnetic resonance （NMR） assignments of hydrolyzed products extracted from human blood plasma. The correlations between chemical, functional and structural prop- erties of highly toxic pesticides were investigated using the PreADME analysis. We observed that toxic pesticides possessed higher molecular weight and, more hydrogen bond donors and acceptors when compared with less toxic pesticides. The occurrence of functional groups and structural properties was analyzed using ~H-NMR. The ~H- NMR spectra of the phosphomethoxy class of pesticides were characterized by methyl resonances at 3.7-3.9 ppm （8） with the coupling constants of 11-16 Hz （Je-cns）. In phosphoethoxy pesticides, the methyl resonance was about 1.4 ppm （8） with the coupling constant of 10 Hz （Je-cH2） and the methylene resonances was 4.2-4.4 ppm （8） with the coupling constant of 0.8 Hz （Jp-cH3）, respectively. Our study shows that the values of four parameters such as chemical shift, coupling constant, integration and relaxation time correlated with the concentration of toxic pesticides, and can be used to characterise the proton groups in the molecular structures of toxic pesticides.

Objective:To investigate antimicrobial and bronchodialator effect of hydroalcholic extract of polyherbal drug Shirishadi containing Shirisha(Albezzia lebbeck),Nagarmotha(Cyprus ratandus) & Kantakari(Solanum xanthocarpum).Methods:Antimicrobial activity was evaluated by discdiffusion method and MIC,MBC,MFC were calculated by micro dilution *** extract of this preparation was investigated for its phytochemical analysis,phenol and Oavonoid were determined by speclrophotometric method and in vivo bronchodilator effect was analysed by convulsion ***:The phytochemical tests revealed presence of alkaloids, anthraquinones,carbohydrates,flavonoids,saponins and *** antimicrobial result showed the MIC of 6.25 mg/*** Staphylococcus aureus and 12.5 mg/*** Escherichia coli and 12.5 mg/mL against remaining bacteria tested,with strong antifungal *** maximum inhibition zone is found against Pseudomonas aeruginosa with MIC 16 mg/mL Drug showed significant bronchodilator effect with 27.86%& 36.13%increase in preconvulsion time of guinea pigs prebeated with 100 & 200 mg/kg body weight of ***:The study reveals that the extracts possess antibacterial activity and antifungal activity in a dose dependent manner. This antimicrobial property may be due to presence of several saponins,further studies are highly needed for the drug development.
