A new integrated scheme based on resource-reservation and adaptive network flow routing to alleviate contention in optical burst switching networks is proposed. The objective of the proposed scheme is to reduce the overall burst loss in the network and at the same time to avoid the packet out-of-sequence arrival problem. Simulations are carried out to assess the feasibility of the proposed scheme. Its performance is compared with that of contention resolution schemes based on conventional routing. Through extensive simulations, it is shown that the proposed scheme not only provides significantly better burst loss performance than the basic equal proportion and hop-length based traffic routing algorithms, but also is void of any packet re-orderings.

A novel optical burst switching （OBS） high speed network architecture has been proposed. To verify its feasibility and evaluate its performance, just-enough-time （JET） signaling has been considered as a high performance protocol. In the proposed architecture, to avoid burst losses, firstly, a short-prior- confirmation-packet （SPCP） is sent over the control channel that simulates the events that the actual packet will experience. Once SPCP detects a drop at any of the intermediate nodes, the actual packet is not sent but the process repeats. In order to increase network utilization, cost effectiveness and to overcome some limitations of conventional OBS, inherent codes （e.g., orthogonal optical codes （OOC））, which are codified only in intensity, has been used. Through simulations, it shows that a decrease in burst loss probability, cost effectiveness and a gain in processing time are obtained when optical label processing is used as compared with electronic processing.

BACKGROUND Non-alcoholic fatty liver disease(NAFLD)increases the risk of cardiovascular diseases independently of other risk factors.However,data on its effect on cardiovascular outcomes in coronavirus disease 2019(COVID-19)hospitalizations with varied obesity levels is scarce.Clinical management and patient care depend on understanding COVID-19 admission results in NAFLD patients with varying obesity levels.AIM To study the in-hospital outcomes in COVID-19 patients with NAFLD by severity of obesity.METHODS COVID-19 hospitalizations with NAFLD were identified using International Classification of Disease-10 CM codes in the 2020 National Inpatient Sample database.Overweight and Obesity Classes Ⅰ,Ⅱ,and Ⅲ(body mass index 30-40)were compared.Major adverse cardiac and cerebrovascular events(MACCE)(all-cause mortality,acute myocardial infarction,cardiac arrest,and stroke)were compared between groups.Multivariable regression analyses adjusted for sociodemographic,hospitalization features,and comorbidities.RESULTS Our analysis comprised 13260 hospitalizations,7.3% of which were overweight,24.3% Class Ⅰ,24.1% Class Ⅱ,and 44.3% Class Ⅲ.Class Ⅲ obesity includes younger patients,blacks,females,diabetics,and hypertensive patients.On multivariable logistic analysis,Class Ⅲ obese patients had higher risks of MACCE,inpatient mortality,and respiratory failure than Class Ⅰ obese patients.Class Ⅱ obesity showed increased risks of MACCE,inpatient mortality,and respiratory failure than Class I,but not significantly.All obesity classes had non-significant risks of MACCE,inpatient mortality,and respiratory failure compared to the overweight group.CONCLUSION Class Ⅲ obese NAFLD COVID-19 patients had a greater risk of adverse outcomes than class Ⅰ.Using the overweight group as the reference,unfavorable outcomes were not significantly different.Morbid obesity had a greater risk of MACCE regardless of the referent group(overweight or Class Ⅰ obese)compared to overweight NAFLD patients admitted with COVID-19.

In this paper,the performance of a two-stage three-phase grid coupled solar photovoltaic generating system(SPVGS)is analyzed by using a novel reweighted Lo norm variable step size continuous mixed p-norm(RLo-VSSCMPN)of a voltage source inverter(VSI)control scheme.The efficacy of the system is determined by considering unbalanced grid voltage,DC offset,voltage sag and swell,unbalanced load and variations in solar insolation.RLo-VSSCMPN is used for inverter control and it ex-tracts fundamental components of load current for generating the reference grid current with a faster convergence rate and lesser steady state oscillations.With the proposed control,harmonics in the grid current follows the IEEE-519 norm and the performance is also satisfactory under varying environmental/load conditions.The power generated from SPvGS is transferred optimally using a DC-DC boost converter utilizing the incremental conductance(INC)maximum power point technique.The proposed system is simulated using MATLAB/Simulink 2018a and test results are verified experimentally using dSPACE1202 in the laboratory to ensure the validity of a novel proposed robust RLo-VSSCMPN.Index Terms-INC maximum power point tracker,power quality,reweighted LoVSSCMPN algorithm,solar PV generating system,total harmonic distortion,voltage source inverter.

E-Learning during COVID-19 Outbreak: Cloud Computing Adoption in Indian Public Universities

In the COVID-19 pandemic situation,the need to adopt cloud computing(CC)applications by education institutions,in general,and higher education(HE)institutions,in particular,has especially increased to engage students in an online mode and remotely carrying out research.The adoption of CC across various sectors,including HE,has been picking momentum in the developing countries in the last few years.In the Indian context,the CC adaptation in the HE sector(HES)remains a less thoroughly explored sector,and no comprehensive study is reported in the literature.Therefore,the aim of the present study is to overcome this research vacuum and examine the factors that impact the CC adoption(CCA)by HE institutions(HEIs)in India.The scope of the study is limited to public universities(PUs)in India.There are,in total,465 Indian PUs and among these 304 PUs,(i.e.,65%PUs)are surveyed using questionnaire-based research.The study has put forth a novel integrated technology adoption framework consisting of the Technology Acceptance Model(TAM),Technology-Organization-Environment(TOE),and Diffusion of Innovation(DOI)in the context of the HES.This integrated TAM-TOE-DOI framework is utilized in the study to analyze eleven hypotheses concerning factors of CCA that have been tested using structural equation modelling(SEM)and confirmatory factor analysis(CFA).The findings reveal that competitive advantage(CA),technology compatibility(TC),technology readiness(TR),senior leadership support,security concerns,government support,and vendor support are the significant contributing factors of CCA by Indian PUs.The study contends that whereas the rest of the factors positively affect the PUs’intention towards CCA,security concerns are a significant reason for the reluctance of these universities against adopting CC.The findings demonstrated the application of an integrated TAM-TOE-DOI framework to assess determining factors of CCA in Indian PUs.Further,the study has given useful insights into the successful CCA by Indian PUs,which will facilitate

As an innovation,Blockchain has transformed numerous industries and sparked the interest of the research community due to its abundance of benefits,opening up diverse research routes in the healthcare sector in the last decade.With Health 4.0 becoming ubiquitous in the healthcare industry,end-user transactions are being carried out on a decentralized network,making Blockchain profitable to meet the demands of the modern healthcare sector.Therefore,a detailed analysis of Blockchain is very crucial.This study emphasizes the evolution of science and the preliminary research of Blockchain in healthcare through bibliometric analysis.All the data are extracted from the Scopus database,and the VOSviewer tool is used for analysis.A total of 1152 Scopus articles published between 2018 and 2022 are examined.Results reveal that in 2022,the field of Blockchain experienced a notable increment in the number of publications and a significant growth rate.IEEE Access became well known in this field and had a large number of citations.It is observed that China and India are the leading countries in terms of publications on Blockchain.This study offers a number of recommendations that amateur and professional researchers can use as a benchmark before commencing a Blockchain investigation in the future.

Neck, being not protected by skeleton, is vulnerable to external trauma and injury which involves blood vessels, trachea, esophagus and other endocrine and nervous system organs. Vascular injuries can not only cause potentially life-threatening hemorrhage but also need profound surgical expertise in management. Development of collateral circulation in neck is well known; however, there is scarcity of literature on the role of collateral formation in neck trauma. Here, we present a unique case of penetrating gunshot injury to neck with right common carotid and right subclavian artery injury with hemorrhagic shock managed with ligation of these vessels as a life-saving procedure. The patient presented with no neurological or motor deficits in immediate postoperative period owing to the collateral circulation between right vertebral artery and right common carotid and right subclavian artery.

Candida endocarditis is extremely rare in term neonates,and gall bladder involvement due to candidemia has never been reported amongst neonates and infants.A term,appropriate for gestational age neonate developed Candida tropicalis blood stream infection in second week of life.He was started on conventional amphotericin B.However,he failed to show any clinical improvement,and candidemia keep on persisting.Repeat sanctuary sites screening revealed multiple echogenic masses in heart(vegetations) and gall bladder.On changing the treatment to liposomal amphotericin B and fluconazole,he recovered clinically,echogenic masses in gall bladder disappeared,and intracardiac vegetations decreased in size.

Varicocele-associated male infertility has classically been managed using surgery or assisted reproductive techniques. With increasing evidence of oxidative stress as a pathophysiological factor in varicocele-associated infertility, medical therapy especially antioxidants might become a treatment option with lower risks. We reviewed the existing literature on the role of various medical agents in the management of male infertility attributed to varicoceles. Medical therapy is typically evaluated in three different situations such as （a） comparison of two drugs or one drug with placebo, （b） comparison of drugs versus surgery, and （c） comparison of drugs as adjuvant therapy with surgery versus drug therapy alone. Due to heterogeneity of data and lack of well-conducted studies, there is insufficient data to recommend routine use of medical therapy for men with varicocele-associated infertility and surgery remains the treatment of choice. Pregnancy and live birth rates are usually not reported in most studies and mere improvement in sperm parameters or antioxidant capacity is insufficient to support its routine use. Antioxidant therapy is a potential option due to its theoretical benefit, data from preclinical studies, and lack of major side effects. Adjuvant therapy with antioxidants after surgical repair of varicocele may improve the outcome and is a potential area for further research.
