The Impact of Fresh Gas Flow Sevoflurane Anesthesia on Perioperative Hypothermia in Adult Patients Undergoing Elective Open or Laparoscopic Digestive Surgery: A Prospective Randomized Controlled Trial

Background: Because body temperature is such an important indicator and a basic requirement for all kinds of life, even tiny variances might induce undesired changes. This study looked at the influence of FGFs sevoflurane anesthesia on heat preservation in patients undergoing open or laparoscopic digestive surgery. Materials and Methods: Two hundred and forty adult patients (18 - 75 years) with an ASA of I-II were scheduled for open and laparoscopic digestive surgery: open surgery (Group O, n = 120 patients) and laparoscopic surgery (Group L, n = 120 patients). Each group was separated randomly into four subgroups (n = 30 patients) based on FGFs (0.7, 1, 1.5, or 2 L/min). Each patient’s HR, MBP, SpO2, FiO2, fluid infusion amount, urine volume, pre/post-Hb, surgery time, and nasopharyngeal temperatures were investigated and recorded every 15 min from 0 to 120 min. Results: Between groupings, there were no significant changes in demographic features. In 240 patients, the results of various FGFs (0.7, 1, 1.5, and 2 L/min), no statistically significant differences were found in core body temperature over time within each subgroup, with no statistically significant differences between the two (open and laparoscopic) (P > 0.05). Conclusions: The study concluded that FGFs (0.7, 1, 1.5, and 2 L/min) could be utilized safely in adult patients undergoing open or laparoscopic digestive surgery. FGFs (0.7, 1, 1.5, and 2 L/min) provide better body heat preservation during surgical operations.

Evaluation of a newly Developed Laser Pen as a Home Care Device for Pain Reduction of Recurrent Aphthous Stomatitis (Preliminary Study)

Objective: The aim of this survey is to evaluate the effect of a new developed laser pen as a home care device for patients’ pain reduction with recurrent aphthous stomatitis. Background data: Recurrent aphthous stomatitis (RAS) is one of the most common and painful ulcers in the oral cavity. Low Level Laser Therapy (LLLT) has been used successfully for reducing pain and healing time in comparison with corticosteroids and placebo in some studies. But these treatments are done in the clinics and it seems that there is no report of the laser treatment as a home care device up to this study. Methods: This was a prospective, randomized trial pilot study. Thirty patients with RAS were divided into three similar groups. The group one (n = 10) was treated with a diode laser, 660 nm, 40 mw, continuous wave, 1.2 J, 6 J/cm2, for five consecutive days (twice per day, 30 seconds each session). The second group (n = 10) was treated with a topical triamcinolone (adcortyl in orabase) ointment four times per day and the third group (n = 10) was treated with placebo (red light pen) the same way as Group 1. All the patients were evaluated with a Visual Analogue Scale (VAS) before and after treatment on the consecutive days. The data was analysed by one way AnOVA and PostHoc tests. Results: The results show that the laser pen and triamcinolon in orabase ointment have similar results and both have better results than placebo in the control of the recurrent aphthous stomatitis pain (p value: 0.001). Conclusion: In this clinical pilot study the laser pen as a home care device seems to be useful for the treatment of RAS.

液压油粘度的变化会对液压可变气门系统(VVA)性能产生重要影响,为提高内燃机的性能,应根据发动机的运行工况调整最佳的气门正时及升程。该文建立了全可变气门机构的AMESim仿真模型,分析了液压油粘度对气门升程的影响。基于能量守恒定律建立了系统平均模型,利用Matlab/Simulink设计了滑模控制器。仿真结果表明:所设计的滑模控制器通过调节液压泵的转速,调节气门升程,使气门工作精度提高了9.3%左右。因此,该控制器降低了液压油粘度变化带来的扰动影响。

电控液压可变气门控制（VVA）系统能够确保气门正时与升程可变,但是系统却消耗大量能量。为了提升它的节能性,本论文提出了一种带有能量回收系统（ERS）的VVA系统。该系统利用一个蓄能器来储存液压缸卸油浪费的能量,蓄能器积累的油压能确保更低主泵能耗。通过在GT-power模型中优化气门正时与升程,来确保带有VVA系统的发动机的燃油消耗率（BSFC）达到最低。结果表明：增加能量回收系统,使气门正时和升程柔性可调,并减少VVA的能量消耗,发动机动力性提升7%~11%,燃油经济性提升6%~9%。

First Report of an Outbreak of Contagious Ecthyma in Camels （Camelus dromedarius and Camelus bactrianus） in Iran

Camel contagious ecthyma （CCE） has been reported in Mongolia, Kenya, Somalia and Sudan. CCE is caused by a Parapoxvirus affecting young animals by producing lesions around the lips and nostrils. The generalized form of CCE is uncommon. The aim of this study was to submit the first report of contagious ecthyma in Iran and also to help clinician to diagnose this disease with heavy economic losses easier. In the paper, an outbreak of CCE in dromedary and bactrian camels in Qom province of Iran was described and clinical signs were observed in one camel herd in October 2009. nodules and scabs from seven affected animals were collected for virus identification. Total extracted DnA was used for polymerase chain reaction （PCR） to amplify a fragment of Parapoxvirus B2L gene. Results showed that camel calves （n = 27） less than one year old and one male bactrian camel were affected （no adult female camels were found to be infected）. The prevalence of the disease in the herd, adult camels and camel calves was 30.33%, 1.5% and 100%, respectively. Affected animals showed the swelling of head with nodular lesion around the lips. It then developed to pustules and fissured crusts. Previous involvement with this disease, history of contact with sheep or goats, food resources and season all can have a role in epidemiology of the disease.

Comparative Study between General Anesthesia versus General Anesthesia Combined with Thoracic Epidural Analgesia on Cytokine Response in Laparoscopic Cholecystectomy Patients

Background and Objectives: The main benefits of laparoscopic surgery in comparison to open surgery involve the rapid discharge from recovery room, decreased postoperative hospital stays, reduced postoperative discomfort, easier getting back to work and faster return to ordinary daily life as well as cosmetic surgical wounds. The anesthesia type has an essential role in attenuation of the surgical stress and achievement of these advantages. We aimed to determine the outcome of giving general anesthesia in conjunction with thoracic epidural analgesia (TEA) compared to general anesthesia alone on stress response to surgery and anesthesia by investigating cytokine reaction (interleukin 6 and 8 levels), hemodynamic changes (BP, HR, RR, SPO2), and Visual Analogue Scale (VAS) scores postoperatively in patients subjected for laparoscopic cholecystectomy. Methods: This study included 40 patients aged 20 - 60 years old with American Society of Anesthesiologists physical status (ASA) I and II. They were planned for laparoscopic cholecystectomy at Aswan University Hospital from April 2017 to March 2018. They were randomly allocated into two groups. Group A (n. 20) received general anesthesia only and Group B (n. 20) received general anesthesia in conjunction with thoracic epidural analgesia using fentanyl and bupivacaine in the epidural catheter. Thermosensetive poly(n-isopropylacrylamide)-based magnetic nanoparticles were synthesized by free radical polymerization of n-isopropylacrylamide (nIPPAMs), methacrylic acid (MAA), and vinyl pyrrolidone (VP) in the presence of methylene-bis-acrylamide as cross linking agent. The Fe3O4 magnetic nanoparticls were prepared by chemical precipitation of Fe salts in the ratio of 1:2 under alkaline and inert condition. Thermosensitive crosslinked P (nI-PAAM-MAA-VP) copolymers were characterized by FT-IR and H-nMR. The pH and thermosensitive copolymer was used for preparation of drug loaded magnetic nanoparticles, and doxorubicin (DOX) was used as a typical anticancer drug. The amount of the loaded drug and drug release amount were determined by UV measurements. Scanning electron microscopy (SEM) and lower critical solution temperature (LCST) were used to determine the particle surface morphology and the phase transition temperature of the nanoparticles respectively. The release behavior of DOX at pH = 7.4 and 37°C was studied. The result indicated that this thermosensetive magnetic nanoparticle has a high drug loading capacity and favorable linear release property for DOX without initial burst release. Thus this system is promising for the application in targeted smart anticancer drug delivery.
