Randomized controlled trial of remote ischemic preconditioning and atrial fibrillation in patients undergoing cardiac surgery

AIM To study whether remote ischemic preconditioning(RIPC) has an impact on clinical outcomes, such as post-operative atrial fibrillation(POAF).METHODS This was a prospective, single-center, single-blinded,randomized controlled study. One hundred and two patients were randomized to receive RIPC(3 cycles of 5 min ischemia and 5 min reperfusion in the upper arm after induction of anesthesia) or no RIPC(control). Primary outcome was POAF lasting for five minutes or longer during the first seven days after surgery. Secondary outcomes included length of hospital stay, incidence of inpatient mortality, myocardial infarction, and stroke. RESULTS POAF occurred at a rate of 54% in the RIPC group and 41.2% in the control group(P = 0.23). No statistically significant differences were noted in secondary outcomes between the two groups. CONCLUSION This is the first study in the United States to suggest that RIPC does not reduce POAF in patients with elective or urgent cardiac surgery. There were no differences in adverse effects in either group. Further studies are required to assess the relationship between RIPC and POAF.

Water quality and occurrence of water-borne diseases in the Gaza strip are vivid examples for most developing societies. In recent years, the quality and quantity of groundwater, the only source of waters in the Gaza strip, have deteriorated markedly. A general rundown of the infrastructure and water distribution networks, in particular, the spread of cesspools, excessive use of pesticides and fertilizers, and improper treatment and disposal of wastewater remain major contributing factors to the continued deterioration in the water status in the Gaza strip. Without a doubt, the (Israeli)-Palestinian conflict had a clear negative impact on the water sector in the Gaza strip. Apparently, there is a dire need to adopt the WHO’s water safety plan in the management of Gaza’s water supply systems from catchment to consumer’s tap in order to maintain the sustainability and quality of water resources and prevent outbreaks of waterborne diseases. Therefore, this review has been prepared to highlight the overall picture of the water dilemma in the Gaza strip in the last years and in addition, to identify the sources, sorts, levels, and health risks of consequence to microbial contamination of water. The impact of political conflicts on the water sector in the Gaza strip also was reviewed. Furthermore, recommendations were formulated in order to assist and guide future researchers, stakeholders, and policymakers to avoid the more exacerbation of water contamination as well as to protect public health.

Since December 2019,a novel coronavirus that represents a serious threat to human lives has *** is still no definite treatment for severe cases of the disease caused by this virus,named coronavirus disease 2019(COVID-19).One of the most considered treatment strategies targets the exaggerated immune regulator,and interleukin(IL)-6 is a crucial pro-inflammatory *** acute respiratory syndrome coronavirus 2(SARSCoV-2)cases show an elevated level of IL-6 related to disease ***-6 activity can be inhibited by the following:IL-6 itself,IL-6 signaling pathways such as Janus kinase and signal transducer and activator of transcription(JAK-STAT),gp130,IL-6R,and downstream activated ILs,such as IL-17 and IL-6 ***,according to these studies and their results,IL-6 blockade with anti-IL-6 or its receptor antibodies such as tocilizumab in COVID-19 is beneficial in severe cases and may reduce the mortality ***-STAT inhibitors block the cytokine storm by inhibiting several crucial pro-inflammatory mediators such as TNF-αand IL-6 and have shown various results in clinical ***-6 induces IL-17 secretion,and IL-17 is involved in the pathogenesis of inflammatory *** trials of anti-IL-17 drugs are currently recruiting,and anti-gp130 antibody is ***,this agent has shown positive effects in inflammatory bowel disease clinical trials and could be tested for *** study aimed to review the role of IL-6 in the cytokine storm and studies regarding IL-6 and blockade of its inflammatory pathways in COVID-19 to determine if any of these agents are beneficial for COVID-19 patients.

Relationship between religion and school students＇ road behavior in southern Iran

Purpose： Unsafe behaviors are an important cause of accidents in adolescent age groups. This study was designed to examine the behaviors of adolescent pedestrians in southern Iran. Methods： This is a descriptive analytical cross-sectional study. The study population consisted of high school students in Shiraz, capital city of Fars Province, Iran. Five hundred and sixteen students were selected by multi-stage sampling. Data were collected by the use of three questionnaires, which included Persian copies of adolescent road user behavior questionnaire （ARBQ）, Duke University Religious Index （DUREL）, as well as the context and independent variables questionnaire. Results： The results showed that a decrease in dangerous behaviors on the road resulted in an increase in respondents＇ intrinsic religiosity. Also, engagement in unsafe crossing behavior in the road decreased with increasing respondents＇ intrinsic religiosity. Another finding showed that female students were less involved in dangerous play and planned protective behaviors on the road. Conclusion： Findings clearly indicate that intrinsic religiosity has a significant role in reducing the risky road behaviors of students. Hence, religion may improve road safety in school students＇ road behavior in Iran.
