Spatial Disparity in Availability of Tuberculosis Diagnostic Services Based on Sector and Level of Care in Nigeria

Background: Delay in Tuberculosis (TB) diagnosis can contribute to late presentation, severe disease, and continued transmission. KNCV TB Foundation Nigeria through the United States Agency for International Development (USAID) funded the TB Local Organization Network (LON) 1 and 2 projects that explored the availability of Tuberculosis services based on sector and levels of care. Methods: TB Patient Pathway Analysis was carried out in 14 states comprising 92 facilities. It involved primary, secondary, and tertiary levels of health care in both the public and private sectors. This was a cross-sectional study under program implementation. Proforma was used to collect data on the available TB diagnostic services. Results: In public health facilities, GeneXpert was available at 100% in tertiary facilities in 8 (57%) states;up to 82% in 4 (33%) states, 50% available at secondary facilities in 2 states, and There is none at the primary facilities. Smear microscopy was available at 100% in tertiary facilities in 9 (64%) states and 3 (25%) states have 50% to 82%;secondary -10 (71%) states have > 70% at facilities;primary 1 (7%) state has it in 61% of facilities. Loop-mediated isothermal amplification (TB-LAMP) in tertiary 2 (17%) states have 20% and 100% respectively;secondary 4 (<30%) states have in 1 or 2 facilities;none for primary facilities. In private health facilities, 79% of states have Smear microscopy at both primary and secondary facilities, and only 2 states (14%) at tertiary facilities. Only 1 (7%) state has GeneXpert in all tertiary facilities, 2 (14%) states have secondary facilities, and 4 states in about 1% of facilities. TB LAMP was not available in any tertiary facility, one (7%) state at secondary with coverage of 1%, and 2 (14%) states at primary both with 4% overall facility coverage. Conclusions: There is an inequitable distribution of TB diagnostic services in both sectors and levels of care in Nigeria. TB care and control will improve with enhanced equitable distribution of TB diagnostic service

Patient Health Seeking Behavior and Choice of Place of Care among Tuberculosis Clients in Selected States in Nigeria

Background: Tuberculosis (TB) remains a major global public health problem. Early detection and initiation of treatment shortens infectious period and is key to TB control. A considerable proportion of TB patients presenting with advanced symptoms suggests delay in seeking care. As TB control programs rely on passive case finding, TB care-seeking behavior is critical as un-diagnosed cases act as reservoirs for transmission. This study assessed patient health seeking behavior and choice of place of care among TB patients in Nigeria. Methods: This was a pro-gramme implementation, facility-based cross-sectional study in 14 states, 92 facilities across three levels of care. Interviewer-administered semi-structured questionnaires were used. Information on personal characteristics and health seeking behavior was collected from June 2020 to December 2021. The analysis was based on the various healthcare providers where the respondents first sought care. Data analysis was done using IBM SPSS and summarized using frequency and percentages. Chi square test was used for associations of characteristics of patients and choice of place of care at p Results: In all 14 states, distribution of overall first place of seeking care for TB symptoms was: 7208 (75.8%) health facilities (range 57% to 88%), while 2294 (24.2%) visited other places including community pharmacist, patent medicine vendor, traditional/home, un-specified/none. For Health facilities, the majority were at public facilities 6563 (69.1%) and private 641 (6.7%). Moreover, 6 states had >20% of respondents who first sought care at PMVs, while 3 states (Delta, Imo, and Rivers) had 10% of respondents who first sought care at Community Pharmacist. Conclusions: Nearly a quarter of people with TB first sought care outside health facilities, suggesting the need to align availability of services with their needs.

The development of bio-sourced materials is essential to ensuring sustainable construction;it is considered a locomotive of the green economy.Furthermore,it is an abundant material in our country,to which very little attention is being given.This work aims to valorize the waste of the trunks of banana trees to be used in construction.Firstly,the physicochemical properties of the fiber,such as the percentage of crystallization and its morphology,have been determined by X-ray diffraction tests and scanning electron microscopy to confirm the potential and the impact of the mode of drying on the quality of the banana fibers,with the purpose to promote the use of this material in construction.Secondly,the results obtained with the gypsum matrix allowed us to note a preponderant improvement in the composite’s thermal properties thanks to the variation of the banana fiber additive.Thirdly,the impact of the nature of the banana fiber distribution(either fiber mixed in matrix or fiber series model)on the flexural and compressive strengths of the composites was studied.The results obtained indicate that the insulation gain reaches up to 40%.It depends on the volume fraction and type of distribution of the banana fibers.However,the thermal inertia of the composites developed,represented by thermal diffusivity and thermal effusivity,was studied.Results indicate a gain of 40%and 25%,respectively,in terms of thermal diffusivity and thermal effusivity of the developed composites compared to plaster alone.Concerning the mechanical properties,the flexural strength depends on the percentage of the volume fraction of banana fibers used,and it can reach 20%more than the flexural strength of plaster;nevertheless,there is a significant loss in terms of the compressive strength of the studied composites.The results obtained are confirmed by the microstructure of the fiber banana.In fact,the morphology of the banana fibers was improved by the drying process.It reduces the amorphous area and improves the cellulosic crystalline surfaces,w

为探究城市建筑拆除固废再生骨料部分或全部取代天然骨料用于粗粒土路基填料的可行性,开展不同再生骨料取代率、含水率、围压和剪应力比等组合下的室内大型静动三轴试验,定量研究土性参数和应力状态对试样抗剪强度和累积塑性应变特性的影响规律。基于半对数坐标下累积塑性应变发展的多阶段特征,分别考虑不同阶段塑性变形累积速率以及相邻两阶段的塑性变形累积速率的差异,提出适用于建筑固废再生骨料路基填料的新型安定行为判定准则。研究结果表明:再生骨料路基填料试样的累积塑性应变随含水率和剪应力比的增大而增大,当再生骨料路基填料试样在剪应力比为0.3和0.5时,抗累积变形性能与天然骨料路基填料试验所得的抗累积变形性能接近,综合考虑抗剪强度和累积塑性变形特性的再生骨料路基填料最优取代率为85%;新安定行为判定准则具有较高的准确性,可为相似路基填料的长期路用性能评定提供理论依据。

In this paper,we study a pursuit-evasion differential game problem in the Hilbert space L2.Dynamics of countable number of pursuers and evader expressed as nth-order differential equa-tions with geometric constraints on the control functions of the players.The game terminates at a given fixed time which is denoted by 0.The game's payoff is the infimum of the distances between the evader and pursuers at the time 0.According to the rule of the game,pursuers try to minimise the distance to the evader and the evader tries to maximises it.We found value of the game and constructed players'optimal strategies.

Performance of aerogel as a thermal insulation material towards a sustainable design of residential buildings for tropical climates in Nigeria

The building sector accounts for nearly 40%of global energy consumption.In Nigeria,more than two-thirds of the consumption comes from residential buildings.Energy efficiency measures through the adoption of insulation materials are tools that could crash the peak demand of energy in buildings while improving its thermal comfort and aerogel is considered as the most effective material for insulation,owing to its unique thermal properties.In this paper,we present the performance of aerogel as a thermal insulation material towards a sustainable design of residential buildings for tropical climates in Nigeria.First,a typical residential building in the tropical region was modeled with conventional materials utilized in the region and was later modified through the application of aerogel material on various surfaces of the model.A whole building energy simulation was then carried out in each variation and the outcome was compared to effectively conclude on the significance of aerogel in terms of thermal comfort improvement and energy consumption reduction.Results show that aerogel had the highest influence when inserted in the attic and floor slabs of the designed model.A reduction of more than 6%was attained in the recorded indoor mean air and operative temperatures while still maintaining an acceptable humidity range.Concerning energy consumption,a reduction of more than 15%was achieved.However,the high price of aerogel may hinder its application on the studied building but could be a good investment where climate change and sustainability are of high importance and less concern is given to expenditure.Aerogel demonstrated significant potential with respect to both thermal comfort improvement and energy consumption reduction on the designed model.The outcome of the study is hoped to serve as a base reference for the insulation of residential buildings with similar climate and characteristics to the adopted case building.

Thermal evolution of aliphatic and aromatic moieties of asphaltenes from coals of different rank: possible implication to the molecular architecture of asphaltenes

The compositional and structural changes that coal asphaltenes undergo with increasing thermal maturation were investigated using solid-state13 C NMR and Fourier transform infrared spectroscopy. The results show a gradual increase in the relative proportion of carbon content with a concomitant decrease in the hydrogen, sulphur and oxygen content. The amount of aromatic carbon also increases while the aliphatic carbon decreases with increasing maturity. Ester and carboxylic groups are particularly sensitive to maturation and decrease in relative abundance with increasing maturity while the amount of aromatic carbonyl groups increases. In general, the asphaltenes were observed to evolve towards a more thermally stable structure with increasing amounts of aromatic moieties and relatively lower amounts of aliphatic moieties. The results are in agreement with the ultimate dominance of the island molecular architecture(the Yen–Mullins model) in asphaltenes.

Textual data streams have been extensively used in practical applications where consumers of online products have expressed their views regarding online products.Due to changes in data distribution,commonly referred to as concept drift,mining this data stream is a challenging problem for researchers.The majority of the existing drift detection techniques are based on classification errors,which have higher probabilities of false-positive or missed detections.To improve classification accuracy,there is a need to develop more intuitive detection techniques that can identify a great number of drifts in the data streams.This paper presents an adaptive unsupervised learning technique,an ensemble classifier based on drift detection for opinion mining and sentiment classification.To improve classification performance,this approach uses four different dissimilarity measures to determine the degree of concept drifts in the data stream.Whenever a drift is detected,the proposed method builds and adds a new classifier to the ensemble.To add a new classifier,the total number of classifiers in the ensemble is first checked if the limit is exceeded before the classifier with the least weight is removed from the ensemble.To this end,a weighting mechanism is used to calculate the weight of each classifier,which decides the contribution of each classifier in the final classification results.Several experiments were conducted on real-world datasets and the resultswere evaluated on the false positive rate,miss detection rate,and accuracy measures.The proposed method is also compared with the state-of-the-art methods,which include DDM,EDDM,and PageHinkley with support vector machine(SVM)and Naive Bayes classifiers that are frequently used in concept drift detection studies.In all cases,the results show the efficiency of our proposed method.

近年来,随着世界各国尤其是中国以及非洲的广大发展中国家城市化进程的迅速发展,新建基础设施和老旧城区改造产生的建筑垃圾数量急剧攀升,但综合循环再生利用率普遍不高,以中国为例,截止到2022年底,对于建筑垃圾的综合利用率仅约10%,距《“十四五”循环经济发展规划》提出的“到2025年,建筑垃圾综合利用率达到60%”仍存在巨大差距。另外,根据联合国环境规划署的数据,非洲每年产生约9000万吨建筑垃圾,但对建筑垃圾的回收利用率仍处于较低的水平,同时,产出的建筑垃圾通常用于土地复垦、填沙和当地道路升级,没有适当的技术指导或监管框架,导致了诸如空气和水污染和土壤退化等重大的环境问题。上述数据表明,一方面,大量建筑拆除废弃物目前仍采取直接倾倒填埋或焚烧的处理方式,造成一系列环境问题和安全隐患;另一方面,如若将建筑固废加工处理所得再生骨料用于公路和铁路等交通基础设施建设,将会极大地促进交通基础设施建设由传统资源依赖型向绿色、低碳和经济方向转型。因建筑物类型以及产出年代和地区的不同,导致建筑固废再生填料的成分复杂、变异性大,且再生骨料中的砂浆和砖块存在强度低、密度低、吸水率高和孔隙大等特点,使得建筑固废再生骨料相较于传统路基填料存在明显差异,故仍需针对建筑固废再生料作为公路和铁路基层填料等的可行性开展进一步研究。本文旨在探究在长期循环荷载条件下建筑固废(CDW)再生骨料用于铺设路面基层和底层的可行性。针对建筑固废再生骨料与天然骨料二元混合基层填料,开展一系列室内大型静动三轴试验,研究分析了再生骨料掺量、含水率、应力水平以及围压对填料试样回弹模量、抗剪强度和累积塑性变形的耦合影响效应,并建立了适用于建筑固废再生骨料路基填料的回弹模量预估模型和累积塑性变形预估模型,以期为公路建筑固废路基的结构设计和性能评估提供理论依据。本文的第一章首先阐述了建筑固废再生料对于实现绿色、低碳、经济型交通基础建设的重要性;然后分别概述了目前针对建筑固废再生材料和粗粒土路基填料的抗剪强度特性、永久变形特性及其影响因素以及回弹模量特性及其影响因素的研究现状,并总结了现有关于路基填料的永久变形预估模型和回弹模量预估模型,探讨了人工智能神经网络在岩土工程中的应用及其在预测建筑固废再生骨料的力学性能方面的潜在应用;最后交代了本文的研究目标、研究内容和技术路线。本文的第二章介绍了室内大型静动三轴试验的试验材料和试验方案。其中,本章的第一节阐述了建筑固废再生骨料的基本物理性质如:比重、吸水率、最优含水量和最大干密度。结果表明,与原始骨料相比,回收的建筑和拆除废物组合具有低比重和高吸水率的特性。本章的第二节介绍了建筑固废再生料和天然骨料的来源和组分,说明了本试验试样的颗粒级配和再生骨料与天然骨料的配比选择。本章的第三节详细介绍了再生料的基本物理性质。本章的第四节首先介绍了压实试验的试验流程,测得了试样的最优含水率和最大干密度;然后介绍了三轴试验的试样制备方式、试验仪器组成以及静动三轴试验的加载方案,其中,对于静三轴试验,采取固结不排水的方式,设置34.5Kpa、69kPa和103.5kPa三种围压,以恒定1mm/min的速率进行加载;对于动三轴试验,设计了 4种不同剪应力比(SSR,Shear Stress Ratio)加载条件,即 SSR=0.3、0.5、0.7和0.9,并设置三种围压条件(σ3=50 kPa、100 kPa、150 kPa),加载频率为5Hz,动荷载作用次数设置为50,000次;最后介绍了回弹模量试验的试验方案。本章详细概述了再生骨料的物理特性和室内试验测试方案,为后续分析和讨论研究结果奠定了基础。本文的第三章研究了应力状态、建筑固废再生骨料掺量、含水条件和压实度对试样回弹模量特性的影响,并对比分析了现有回弹模量预估模型对本文试验材料的适用性,研究了围压、偏应力、体积应力、八面体剪应力等不同应力参数以及含水率、压实度等物理参数对试样回弹模量的影响,通过进一步引入再生骨料掺量的影响,建立了最优回弹模量预估模型,得到了模型参数值并对模型的准确性加以验证。结果表明:偏应力、围压和再生骨料掺量对试样的回弹模量有显著影响,回弹模量随含水率的增加而减小,随压实度的减小而减小,推荐相关道路工程中路基填料的再生骨料合理掺量为85%,改进的回弹模量预估模型具有较高的准确性,能够反映应力状态和材料物理性质对回弹模量的影响。本文的第四章研究了不同围压和再生骨料掺量下试样的抗剪强度特性,以及不同剪应力比、围压、再生骨料掺量和含水率下试样的累积塑性变形特性。研究结果表明:(1)建筑固废再生骨料的抗剪能力明显劣于天然骨料,掺混适量比例的天然骨料或者提高围压水平均可使其抗剪强度显著提升。85%掺量的混合路基填料在中、高围压水平的峰值强度与天然骨料相近,但在低围压下明显低于天然骨料。(2)建筑固废再生骨料-天然骨料混合路基填料试样的累积塑性变形与偏应力、围压和含水率呈正相关。剪应力比和围压
