Impact of cytochrome P450 2C19*2 polymorphism on the clinical cardiovascular events after stent implantation in patients receiving clopidogrel of a southern Tunisian region

Introduction: The concept of clopidogrel resistance, first described in biology is being strengthened by recent data from clinical epidemiology. The cardiologists have been sensitized to this concept because of its possible involvement in the occurrence of coronary stent thrombosis. Purpose of the study: The purpose of this study was to investigate the genetic variant of the gene CYP 2C19 inour population and to assess the involvement of this genetic profile in the occurrence of major cardiovascular events (MACE) during the follow-up period. Methods: Our prospective study was conducted between May 2009 and September 2010 including 100 patients admitted to the cardiology department for percutaneous coronary stenting. The patients were divided into 2 groups: those with at least one CYP2C19*2 allele (*2 carriers) and non-carriers. Results: The mean age of our patients was 56.7 years ± 10, 5. No remarkable differences in the baseline characteristics were noted between the two groups. The prevalence of CYP2C19*2 allele in our population was 11.5%. Hospital mortality was estimated at 3%. No statistically significant differences were noted between the two groups regarding the occurrence of intra hospital MACE. The mean follow up was 7.5 ± 4.87 months for the entire study population. The rate of MACE during the follow-up of patients receiving clopidogrel was 8.2% throughout the study population: 5.3% in the *2 non-carriers versus 18.2% in the *2 carriers with a statistically significant difference (p = 0.075) at the risk of error of 10%. Concerning the occurrence of stent thrombosis, there was no significant statistical difference between the two study groups. Conclusion: From these results it is suggested that CYP2C19*2 polymorphism is associated with increase in the occurrence of MACE among Tunisian patients receiving clopidogrel. A larger study is needed to assess the role of genotyping in the evaluation of the phenomenon of clopidogrel resistance.

E-fuel system: a conceptual breakthrough for energy storage

The demand for renewables such as solar and wind power has become stronger than ever in the past few decades and their numerous advantages have nearly risen to the status of common knowledge. However, as solar and wind power are intermittent in nature, the electricity generated from these renewable sources only comprise a low fraction of global electricity production. Enabling the widespread use of renewables requires energy storage technology that is site-independent, efficient, scalable, stable, cost-effective, safe, energy dense and continuous in operation. To date, none of the existing technologies have adequately met the attributes necessary for storing intermittent and scattered solar, wind and other sources of renewable energy. Pumped hydro and compressed air are scalable and cost-effective, but are limited by geography and geology [1].

In this paper,we propose the global dynamics of an SIR epidemic model with distributed latent period,immunity,relapse,homestead-isolation of the susceptible and infectious individuals and general incidence *** resulting model has a disease-free equilibrium and if Ro>1,then the SIR epidemic model admits a unique endemic *** using suitable Lyapunov functionals and LaSalle's invariance principle,the global stability of the disease-free equilibrium and the endemic equilibrium is established,under suitable monotonicity conditions on the incidence function.

Hepatitis C virus non-structural 5A protein can enhance full-length core protein-induced nuclear factor-κB activation

AIM： To study the effects of hepatitis C virus （HCV） core and non-structural 5A （NS5A） proteins on nuclear factor- k B （NF- k B） activity for understanding their biological function on chronic hepatitis caused by HCV infection. METHODS： Luciferase assay was used to measure the activity of NF-kB in three different cell lines cotransfected with a series of deletion mutants of core protein alone or together with NS5A protein using pNF- k B-Luc as a reporter plasmid. Western blot and indirect immunofluorescence assays were used to confirm the expression of proteins and to detect their subcellular localization, respectively. Furthermore, Western blot was also used to detect the expression levels of NF- k B/p65, NF- k B/p50, and inhibitor k B-a（k B-a）. RESULTS： The wild-type core protein （C191） and its mutant segments （C173 and C158） could activate NF- k B in Huh7 cells only and activation caused by （C191） could be enhanced by NS5A protein. Moreover, the full-length core protein and its different deletion mutants alone or together with NS5A protein did not enhance the expression level of NF- k B. The NF- k B activity was augmented due to the dissociation of NF-k： B-I k： B complex and the degradation of Ik B-a. CONCLUSION：NF- k B is the key transcription factor that can activate many genes that are involved in the cellular immune response and inflammation. Coexpression of the full-length core protein along with NS5A can enhance the NF- k B activation, and this activation may play a significant role in chronic liver diseases including hepatocellular carcinoma associated with HCV infection.

Mechanical Characterisation of Interface for Steel/Polymer Composite Using Pull-out Test: Shear-Lag and Frictional Analysis

Fibre-matrix interface is known to have contribution to the mechanical performance of fibre-reinforced composite by its potential for load transfer between the fibre and the matrix. Such load transfer is of great importance in dentistry when a post is used for fixing a ceramic crown on the tooth. In this study, a pull-out test was carried out to analyse the interfacial properties of a steel fibre embedded in a polyester and epoxy matrices. It was found that the fibre-matrix interface is debonded on the whole embedded length when the fibre stress reached the debonding stress. Then, the fibre stress fell down to the initial extraction stress required to pulling out the debonded fibre from the matrix. Both debonding stress and initial extraction stress initiated a linear increase with the implantation length after the debonding stress reached horizontal asymptotes. To analyse the fibre-matrix load transfer before debonding, an analytical shear-lag model was adopted to in this test conditions. Fitting the experimental results with the analytical model provided the interfacial shear strength. By considering the Coulomb friction at the fibre-matrix interface during the fibre extraction process, an analytical model which considers Poisson＇s effects on both fibre and matrix, was developed. In this model, knowledge of the initial extraction stress of the fibre provides the residual normal stress at the fibre-matrix interface.

Numerical investigation of the stresses in backfilled stopes overlying a sill mat

Backfill is commonly used in underground mines to help increase the ore recovery rate and reduce the ore dilution. The use of a part of mine waste as underground backfill material also helps reduce the environmental impact of mining operations. After all, backfill is used to provide a working platform or safer working space. Its primary and most important role is to improve the rock mass stability around mine openings. However, most available solutions to stress analyses were developed for an isolated stope, without taking into account the influence of mine depth, or of adjacent stopes. In this paper,results from a numerical study carried out to evaluate the stresses in backfilled stopes overlying a sill mat are presented. Mine depth and excavation of the underlying stope below the sill mat(horizontal pillar)are both taken into consideration. The influence of stope geometry, backfill, sill mat and rock properties on the stresses is also evaluated. Compared with the case of a single isolated backfilled stope, the numerical results show that the stress magnitudes in the overlying backfill are considerably increased due to the excavation of the underlying stope. In general, the stresses also increase with mine depth and backfill stiffness, while these tend to decrease with an increase in the surrounding rock mass *** results suggest that existing solutions for backfill design may need to be revised.
