Objective::To identify Candida species in asymptomatic subjects in Bobo-Dioulasso(Burkina Faso) by the matrix—assisted laser desortption ionization—time of flight mass ***:A cross-sectional study was conducted from January to February 2013 in BohoDioulasso to coiled fecal and urine specimens from voluntary *** strains were isolated on Sabouraud dextrose agar and analyzed using matrix—assisted laser desorption ionisation—time-of—flight mass ***:A total of 135 samples including stools(78.5%,106/135) and urine(21.5%,29/135) were *** results revealed that fecal specimens contained mainly Candida krusei(***)(42.5%) followed by Candida albicans(29.3%),Candida glabrata(18.0%) and Candida tropicalis(***)(4.7%).***(34.6%) was also found to be the most frequently identified in urine samples followed by Candida albicans(27.0%).***(15.4%) and Candida parapsilosis,However,uncommon species such as Candida *** *** norvegensis,Candida *** lusitaniae and Candida robusta were also identified from fecal and urines ***:This study noted the emergence of species such as ***,Candida glabrata,Candida parapsiolosis,***,Candida nivariensis,Candida norvegensis,and *** is an imperative to take into account the existence of these species in the therapeutic management of patients in Bobo—Dioulasso.

Etiological and Radiological Profile of Acute Lower Respiratory Infections during the Pre-COVID Period in the Paediatric Ward of the Teaching Hospital of Mali and in the Community Health Centre of Yirimadio in Bamako

Introduction: Every year, nearly 4 million people pass away from acute respiratory infections. 98% of such deaths are due to lower respiratory tract infections. Even though studies have been carried on lower respiratory infections x-ray aspects in Mali, very few studies have been done to reveal bacteriological and virological evidence of this disease. Materials and methods: It is about a descriptive prospective study carried out from January to December 2018 having involved patients of all ages, coming for medical consultation at the Yirimadio Community-based health center as well as children from 6 months to 15 years old coming at the pediatric department of UHC Hôpital du Mali for a lower respiratory infection. They had all undergone chest X-ray and a PCR. The purpose: of this work is to study etiological and x-ray aspects of acute lower respiratory infections at the Yirimadio Community-based Health Center and at the UHC pediatric department of Hôpital du Mali. Findings: From January to December 2018, we recorded a frequency of 1.19%. The age group 0 - 5 years was the most represented (64.5%) with a sex ratio of 0.97 for women. Cough was the most common clinical sign (98.7%) followed by fever (58.9%). Standard frontal chest X-ray was pathological in 70% of our patients. It was bronchitis in 75.4% of cases, pneumonia (13.5%), and bronchopneumonia (12.3%). PCR positive was in 83.9% of patients. It revealed a co-infection in more than half of the patients (52.5%), bacterial infection (16.1%) and viral infection (15.2%). Pathogens isolated ranked by frequency were Streptococcus pneumoniaa (87.6%) followed by Staphylococcus aureus (24.9%) and human rhinovirus (17%). The most common viral causes were human rhinovirus (17%), followed by influenza A and B virus (7%) and human parainfluenza virus (7%). Assessment of Wastewater Management and Health Impacts in African Secondary Cities: Case of Dimbokro (Côte D’Ivoire)

The majority of population growth in developing countries will occur in secondary cities where there is limited access to clean water and sanitation. Wastewater management priorities in these cities will be different from those in larger cities and developed countries. For this reason, it is important to assess wastewater management in these cities of sub-Saharan Africa. This study assessed clean water supply, wastewater and faecal sludge management and associated health risks in Dimbokro, C?te D’Ivoire. The study was performed in March 2015 in 12 neighborhoods with a total of 703 households. In these neighborhoods, the questions were addressed and discussed with household head. The study revealed that the households of the city were structured on three topology habitats: residential (5%), economic (42%) and evolutive or yards housing (53%). For water supply, 99.4% of the households used water from SODECI, the local public water supply network, while the remaining 0.6% of surveyed household obtained water from wells. Domestic wastewater, used for doing laundry, dishwashing and showering, was disposed of in the following ways: directly into the street (33%), septic tank (23%), backyards (18%), storm drain (16%), sewer network (6%), pit latrine (3%) or into soakaway (2%). In addition, untreated wastewater was sometimes discharged into the N’Ziriver by certain households. 52% of households surveyed emptied latrines manually and 48 % used mechanic drain services. The most frequent diseases occurring in the households were Malaria (63%), Diarrhoea (13%) and Typhoid fever (9%) in the households of the city. In the light of this study, the municipal authorities of Dimbokro should provide the city with wastewater systems that drain to a central outlet system where the waste-water can be easily treated before final disposal for reuse in agriculture.

Cavernous lymphangioma of the chest wall is a very rare disease entity, and only a few cases have ever been documented in the literature. Cases of recurrent cavernous lymphangioma after surgical excision of a cystic lymphangioma on the same side of the chest wall are quite uncommon. We report a case of a 10-year-old girl, with a giant cavernous lymphangioma of the left lateral chest wall extending into the axilla, who had undergone surgical excision of a cystic lymphangioma 9 years earlier.

Congenital Lobar Emphysema in an Infant: A Case Report from the University Hospital of Mali

Congenital lobar emphysema is characterized by distress due to hyperinflation of the affected lung lobe(s). We report the case of a 2-month-old infant with congenital left lower lobe emphysema. A 2-month-old female infant, born at term without incident, was hospitalized for respiratory distress. On a postnatal day 15, respiratory distress occurred. On admission, he weighed 3400 g and was apyretic. He had a polypnea of 58 cycles/min and a oxygen saturation of hemoglobin of 90% on room air. A chest radiograph revealed hyperclarity of the left lung. Chest CT revealed left lower lobe emphysema. He underwent a left lower lobectomy. The postoperative course was uneventful. He was discharged from the hospital 7 days after surgery without sequelae. Physicians should be aware that congenital lobar emphysema can present with respiratory distress in infants. A chest CT scan confirms the diagnosis. Surgical treatment is effective.

Pneumothorax in Children: A Study of 47 Pediatric Cases at Mali Hospital 2017-2020

Objectives: Pediatric pneumothorax, whether primary or secondary (infection, trauma, or iatrogenic), is sometimes life-threatening. We tried to determine the clinical characteristics of pediatric pneumothorax in Mali. Materials and Methods: We collected all pediatric patients (0 - 15 years) admitted for pneumothorax in the pediatric department of the Mali Hospital from January 2017 to December 2020. Results: During this period, a total of 5569 children aged 0 - 15 years were hospitalized, of whom 47 had pneumothorax or 0.84%. Children under 5 years of age were the most affected (80%). Dyspnea was the most common reason for hospitalization (95.7%). The causes of pneumothorax were (in order): infectious causes dominated by bullous dystrophies (70.2%), traumatic (25.5%), and iatrogenic (4.2%). Pneumothorax was compressive in 42.6%. The frequency of anemia was 79.3%. Surgical drainage was performed in 87.2% and almost all (97.6%) were treated with amoxicillin/clavulanic acid. The mean duration of hospitalization was 10.06 days (range: 0 - 35 days): 74.5% were cured while 25.5% died. Conclusions: We reconfirmed that pneumothorax is a pediatric emergency. Children under 5 years of age are the most affected. The main etiologies are infectious bronchopneumonia. The prevention and treatment of respiratory infections, the implementation of good bacteriological diagnostic tools, and the use of surgical video-thoracoscopy will reduce its mortality rate.

Evolutive Trend of Water Level in the Ebrie Lagoon by Reconstitution of the Tide Gauge Time Series in Front of the Abidjan Coastline (Côte d’Ivoire)

The latest Intergovernmental Panel on Climate Change (IPCC) report shows that sea-level rise, which has been accelerated since the 19th century resulting to the global warming, threatens coastal areas with high population growth. A Global Sea Level Observing System (GLOSS) assessment highlighted the lack of data in Africa, and in Côte d’Ivoire in particular. In order to estimate the evolutionary trend of sea level along the Ivorian coast, and to draw up preventive plans to protect properties and populations, we digitized 65 years of historical tidegrams recorded in the Ebrie Lagoon, using the “Surfer” and “Nunieau” software, then processed them using “T-Tide” and “U-Tide” software. The average levels were calculated using the Demerliac filter from complete daily (day and night) recordings for providing a usable database of 31 years of hourly lagoon data from 1979 to 2015. Our results show that a mean water level in lagoon is 1.04 m. The evolutionary trend in sea level, estimated in the lagoon via the Vridi canal, during the rainy season is the most significant at 2.93 mm/year. This is followed by the dry season, with a trend of 2.89 mm/year. The flood season trend is 2.78 mm/year. This suggests that marine water inflows dominate continental inflows. Our results highlight the vulnerability of Côte d’Ivoire’s coasts to the risk of marine submersion.

Role of Laparoscopic Surgery in the Management of Female Infertility at the Department of Gynecology of Hôpital du Mali

Infertility is a socio-cultural drama in Africa, especially in Mali and remains difficult for couples to overcome. Laparoscopy, also called minimally invasive surgery or keyhole surgery, is an operative technique that permits to explore the pelvis and perform an appropriate therapeutic procedure. The objective of our work was to assess the role of laparoscopic surgery in the treatment of female infertility at Hopital du Mali. It was a retrospective descriptive study that was conducted over a period of 5 years (January 2013 to December 2018). Any patients followed for infertility and who underwent laparoscopic surgery in the gynecology department over a period of two (2) years were included. The outcome of laparoscopic surgery was evaluated in terms of conception of pregnancy. We had collected 103 infertile patients out of 2984, with a frequency of 3.45%. The mean age of our patients was 30.2 years. Housewives and out-of-school women accounted for 68.93% and 54.37% respectively. Nulligravida represented 41.75%. Regarding infertility, it was primary in 41.75% and secondary in 58.25%, with an average duration of 6.2 years. Systematic chlamydial serology was positive in 62.14% of our patients. Hysterosalpingography (HSG) revealed bilateral tubal obstruction in 53.33% of cases. Laparoscopy found tubal adhesions in 97.80% of cases. Adhesiolysis was the main procedure performed with 84.47% bilateral tubal patency achieved in the methylene blue test. Among our patients who had contracted a pregnancy (22.99%), 85% had carried their pregnancies to term. Tubal damage was the main cause of infertility. Adhesiolysis remains the main laparoscopic procedure for treatment.

Introduction: Myasthenia is a rare and disabling autoimmune disease. Few studies were devoted to this pathology. We report a clinical case of myasthenia in the Department of Internal Medicine at the Point G University Teaching Hospital. Observation: This is a 41-year-old patient of Malian origin, a trader with a history of hemorrhoidectomy in 2011 and familial hypertension, was admitted on August 19, 2014, for muscle weakness, dysphonia, and dysphagia. The disease started 1 year before admission at the department with progressive muscle weakness of the upper limbs aggravated by repetitive movements and spreading to the head “drooping head” and inferior limbs, associated with general fatigue, dysphonia and selective dysphagia for solid foods. The diagnosis of myasthenia was maintained based on the clinical signs such as ptosis, bilateral diplopia, and weakness with lower limbs muscle strength rated at 3/5, and confirmed with confirmatory exams;a positive anti-acetylcholine receptor antibody (RIA), the post-synaptic neuromuscular conduction block at ENMG and the positive neostigmine pharmacological test. The patient received Neostigmine (prostagmine) 0.5 mg one ampoule in IM/day and Prednisone at a dosage of 1 mg/kg/day. The evolution was marked by a moderate improvement of the symptomatology afterward the patient was evacuated to Tunisia on family request where he received a course of immunoglobulin 2 g/kg in 2 days. The evolution in Tunisia was favorable. The patient returned in Mali and death occurred after 2 months in a context of respiratory distress. Conclusion: Myasthenia is a rare but serious disease requiring careful management and monitoring to reduce respiratory complications.
