Assessing donor-recipient arterial pressure dynami.s in STA-MCA bypass for moyamoya disease

Background In bypass surgery for moyamoya disease (MMD), the superficial temporal artery’s (STA) pressure needs to surpass that of the cortical M4 recipient of the middle cerebral artery (MCA), boosting cerebral blood flow into the MCA and enhancing cerebral circulation. This study investigates the STA-MCA arterial pressure parameters and gradients during bypass surgery, aiming to deepen our understanding of hemodynami. shifts pre- and *** DSA imaging data were prospectively collected from patients diagnosed with bilateral MMD who underwent STA-MCA bypass surgery between 2022 and 2023 and stratified according to the Suzuki stage. The mean arterial pressure (MAP) of the donor and recipient arteries was directly measured during the STA-MCA bypass procedure, and these data were statistically analyzed and *** Among 48 MMD patients, Suzuki grading revealed that 43.8% were in early stages (II and III), while 56.2% were in advanced stages (IV, V, and VI). Predominantly, 77.1% presented with ischemic-type MMD and 22.9% with hemorrhagic type. Pre-bypass assessments showed that 62.5% exhibited antegrade blood flow direction, and 37.5% had retrograde. The mean recipient artery pressure was 35.0 ± 2.3 mmHg, with a mean donor-recipient pressure gradient (δP) of 46.4 ± 2.5 mmHg between donor and recipient arteries. Post-bypass, mean recipient artery pressure increased to 73.3 ± 1.6 mmHg. No significant correlation (r= 0.18,P= 0.21) was noted between δP and Suzuki *** Our study elucidated that cerebral blood pressure significantly decreases beyond the moyamoya network at the distal M4 segment. Furthermore, we observed bidirectional flow in MCA territories and a significant positive pressure gradient between the STA and M4 segments. Isolated antibody to hepatitis B core antigen in patients with chronic hepatitis C virus infection

AIM： To evaluate the prevalence of isolated anti-HBc in patients with chronic hepatitis C virus （HCV） infection, and its relation to disease severity. METHODS： We screened all patients with chronic HCV infection referred to King Faisal Specialist Hospital and Research Center for hepatitis B surface antigen （HBsAg）, antibody to hepatitis B surface antigen （anti- HBs）, and anti-HBc. One hundred and sixty nine patients who tested negative for both HBsAg and anti-HBs were included in this study. RESULTS： Pathologically, 59 had biopsy-proven cirrhosis and 110 had chronic active hepatitis （CAH）. Of these 169 patients, 85 （50.3%） tested positive for anti-HBc. Patients with CAH had significantly higher prevalence of isolated anti-HBc than patients with cirrhosis, 71 （6d.5%） and 14 （23.7%） respectively （P 〈 0.001）. Twenty-five patients were tested for HBV DNA by qualitative PCR. The test was positive in 3 of them （12%; occult HBV infection）. CONCLUSION： Isolated anti-HBc alone is common in Saudi patients with chronic HCV infection, and is significantly more common in those with CAH than those with cirrhosis. Therefore, a screening strategy that only tests for HBsAg and anti-HBs in these patients will miss a large number of individuals with isolated anti-HBc, who may be potentially infectious.

Harmony search optimization applied to reservoir engineering assisted history matching

Based on the analysis of characteristics and advantages of HSO(harmony search optimization) algorithm, HSO was used in reservoir engineering assisted history matching of Kareem reservoir in Amal field in the Gulf of Suez, Egypt. HSO algorithm has the following advantages:(1) The good balance between exploration and exploitation techniques during searching for optimal solutions makes the HSO algorithm robust and efficient.(2) The diversity of generated solutions is more effectively controlled by two components, making it suitable for highly non-linear problems in reservoir engineering history matching.(3) The integration between the three components(harmony memory values, pitch adjusting and randomization) of the HSO helps in finding unbiased solutions.(4) The implementation process of the HSO algorithm is much easier. The HSO algorithm and two other commonly used algorithms(genetic and particle swarm optimization algorithms) were used in three reservoir engineering history match questions of different complex degrees, which are two material balance history matches of different scales and one reservoir history matching. The results were compared, which proves the superiority and validity of HSO. The results of Kareem reservoir history matching show that using the HSO algorithm as the optimization method in the assisted history matching workflow improves the simulation quality and saves solution time significantly.

Cubic equation-of-state solid models are one of the most widely used models to predict asphaltene precipitation *** parameters are needed to model precipitation under different pressures and temperatures and are usually obtained through tuning with multi asphaltene onset *** the purpose of enhancing the cubic Peng–Robinson solid model and reducing its dependency on asphaltene experiments,this paper tests the use of aromatics and waxes correlations to obtain these thermodynami. *** addition,weighted averages between both correlations are *** averaging is based on reported saturates,aromatics,resins,asphaltene(SARA)fractions,and wax *** the methods are tested on four oil samples,with previously published data,covering precipitation and onset *** proposed wax-asphaltene average showed the best match with experimental data,followed by a SARA-weighted *** new addition enhances the model predictability and agrees with the general molecular structure of asphaltene molecules.

Trabeculectomy with Ologen implant versus mitomycin C in congenital glaucoma secondary to Sturge Weber Syndrome

AIM： To compare the efficacy and safety of collagen matrix implant [Ologen（OLO） implant] versus mitomycin C（MMC） with subscleral trabeculectomy（SST） for the surgical treatment of congenital glaucoma（CG） in SturgeWeber Syndrome（SWS）.METHODS： A prospective comparative randomized study of 20 eyes of 16 patients with CG associated with SWS was divided into two groups. The first group（MMC Group） included 10 eyes that were subjected to SST with MMC. The second group（OLO Group） included 10 eyes that were subjected to trabeculectomy with a collagen matrix implant（OLO implant）. Postoperative evaluation included intraocular pressure（IOP） level, bleb evaluation, complications, and the need for further medication or surgical intervention. RESULTS： The mean preoperative IOP was 29±3.16 mm Hg in MMC and 29.8±3.08 mm Hg in OLO eyes. Mean 12-month percentage reduction in IOP was significant in both groups（57.9% and 56.3%）. At the end of the 12 postoperative follow-up month, in the MMC Group, 80% of eyes achieved the complete success, 20% of eyes had qualified success with no failed surgery in comparison to OLO Group which 70% of eyes achieved the complete success, 20% of eyes had qualified success with 10% failed surgery. In terms of complications, the MMC Group had a higher rate of complications than the OLO Group in the form of thin polycystic bleb in 6 eyes（60%）, blebitis in only one eye（10%） treated with topical antibiotics, shallow anterior chamber in two eyes（20%）.CONCLUSION： This study proves that the use of a collagen matrix implant yields equally effective results as MMC when combined with trabeculectomy for the treatment of CG in SWS. Furthermore, OLO implantation is safe and has low incidences of complications.

The Safaga Region(SR)is part of the Red Sea mountain range in *** flash flooding is now an inescapable event,wreaking havoc and causing massive loss of life and *** majority of the floodwater,however,has been wasted as runoff to the Red Sea,which,if used wisely,could meet a fraction of the water demands for a variety of applications in this *** current work aims to use GIS techniques to integrate remote sensing data for evaluating,mitigating,and managing flash floods in *** data set comprised Tropical Rainfall Measuring Mission(TRMM)thematic rainfall data,1:50,000 scale topographical map sheets,geological maps,the ASTER Digital Elevation Model(ASTER GDEM),Landsat 7 Enhanced Thematic Mapper"(ETM7+),and Landsat 8 Operational Land *** flash flood risk model of SR is developed using ArcGIS-10.3 geoprocessing tools integrating all the causal factors thematic *** final flood risk model for the SR suggests that 57%of the total basins in the SR are at high risk of *** 38%of all basins are at moderate flood *** remaining 5%of basins are less prone to ***-prone zones were identified,suitable dam-building sites were located,and extremely probable areas for water recharge were *** the basis of reliable scientific data,structural and non-structural mitigation strategies that might reduce the damage susceptibility,alleviate the sensitivity of the flash flood,and best utilize its water supply were recommended.

Impact of the injected water salinity on oil recovery from sandstone formations:Application in an Egyptian oil reservoir

In the last decade,there has been an increasing interest in understanding the effects of changing injected water salinity on the performance of oil *** paper aims to investigate the effects of injected water salinity on oil recovery of an Egyptian oil reservoir(Bahariya formation).An experimental work program has been performed using 25 core plugs and 5 different water salinities to study the effects of changing water salinity during both secondary and tertiary stages of *** objectives of the experimental work were to(1)investigate the effects of the low water salinity on oil recovery and(2)identify the optimum water salinity and the main reservoir parameters for application of low salinity waterflooding project(LSWF)in Bahariya *** results revealed that there is an optimum salinity for waterflooding in the secondary flooding ***,for the tertiary flooding stage,the results showed that the controlling factor is not decreasing the salinity,but rather changing the salinity(*** increasing or decreasing).It was also clear that applying the optimum salinity in the secondary recovery stage is more effective than applying it in the tertiary recovery ***,the results showed that the positive impact of LSWF may be expected in reservoirs with high amount of kaolinite,high values of CEC,and low amounts of plagioclase *** study is an original contribution to develop guidelines for designing optimum salinity waterflooding projects in sandstone reservoirs.

INTRODUCTION Preservation of the freshwater resources is a big challenge for a healthy civilized *** water consumption increased more than twice the population increase rate,drinking water pollution and freshwater shortage increased dramatically during the last decades(Kar et al.,2012;Mueller and Nowack,2010).Only 0.01%of freshwater resources are suitable for human needs(Rashid et al.,2019),even in the areas of abundant freshwater(Yeston et al.,2006).By 2050,over 5 billion of the world population will be under severe fresh water supply conditions due to climate change and urbanization exacerbate(Sealy,2019;Oves et al.,2015;Kar et al.,2012).Currently,one billion people drink unhygienic water,especially in developing countries which leads to severe waterborne diseases which might cause death(ami.iyan et al.,2018).Twelve million people die every year due to this problem(Zhao et al.,2019;Oves et al.,2015).
