Millard-Gubler Syndrome in a Patient with Pontine Infarction: A Case Report

Millard-Gubler Syndrome is a rare neurological condition caused by damage to the sixth and seventh cranial nerves, as well as the corticospinal tract in the brainstem. It is characterized by the presence of ipsilateral facial paralysis and contralateral hemiplegia. We report a 55-year-old male patient who presented with sudden onset of left-sided weakness. Imaging revealed a pontine infarct. The patient therefore, was diagnosed with Millard-Gubler Syndrome also known as Ventral Pontine Syndrome based on his symptoms and imaging findings. He was treated with Aspirin and Atorvastatin and was referred to neurology for further consultation and to physiotherapy for his weakness. This case report highlights the importance of prompt recognition and diagnosis of Millard-Gubler Syndrome in patients with pontine infarction. Early identification especially with the use of high-resolution MRI can facilitate appropriate management and treatment, ultimately improving patient outcomes.

In this study, we will construct numerical techniques for tackling the logarithmic Schrödinger’s nonlinear equation utilizing the explicit scheme and the Crank-Nicolson scheme of the finite difference method. These schemes will be subjected to accuracy and stability tests before being used. Efficacy and robustness of the techniques under consideration will be demonstrated using an exact solution, one-Gausson, as well as conserved quantities. Interaction of two-soliton will be conducted. The numerical findings revealed, the interplay behavior is flexible.

Background: Simulation is perceived as an important part to the nursing clinical educational experience since it can offer a safe learning environment for the students’ clinical practice. Objective: The purpose of this study was to explore the Lebanese student’s experience of benefits of high fidelity simulation in nursing education. Design: A qualitative study using open-ended questionnaire. Setting: This study was implemented at private university in Lebanon between January and April 2015. Participants: A purposive sample was sought from those who have a simulated experience. Methods: Qualitative data were collected via interviews and analysed using a thematic approach. Findings: Four major themes were identified: “Bridging Theory to Clinical Practice”, “Developing Critical Thinking and Decision-making”, “Practicing Safely Leads to Enhancing Confidence”, “Teamwork Spirit and students’ motivation to earn and practice more”. Conclusion: Simulation was found to be an acceptable learning strategy for novice nursing students. The use of simulations is a useful and effective learning strategy. Students with a higher positive attitude towards the simulation experience displayed better learning outcomes. Thus, the integration of simulation into the learning experience of nursing students will bridge the gap between theory and practice;enhance their critical thinking and motivation aptitudes along with traditional clinical practice by sparing them a safe milieu.

IEEE 802.11 Wi-Fi networks are prone to many denial of service(DoS)attacks due to vulnerabilities at the media access control(MAC)layer of the 802.11 *** to the data transmission nature of the wireless local area network(WLAN)through radio waves,its communication is exposed to the possibility of being attacked by illegitimate ***,the security design of the wireless structure is vulnerable to versatile *** example,the attacker can imitate genuine features,rendering classificationbased methods inaccurate in differentiating between real and false *** security standards have been proposed over the last decades to overcome many wireless network attacks,effectively detecting such attacks is crucial in today’s real-world *** paper presents a novel resource exhaustion attack detection scheme(READS)to detect resource exhaustion attacks *** proposed scheme can differentiate between the genuine and fake management frames in the early stages of the attack such that access points can effectively mitigate the consequences of the *** scheme is built through learning from clustered samples using artificial neural networks to identify the genuine and rogue resource exhaustion management frames effectively and efficiently in *** proposed scheme consists of four modules whichmake it capable to alleviates the attack impact more effectively than the related *** experimental results show the effectiveness of the proposed technique by gaining an 89.11%improvement compared to the existing works in terms of detection.

Groundwater Vulnerability Assessment for the Corridor Wellfield Using DRASTIC and Modified DRASTIC Models: A Case Study of Eastern Jordan

Groundwater vulnerability maps were created for the Corridor wellfield (~300 km2) in the eastern Jordan using the DRASTIC and modified DRASTIC groundwater vulnerability assessment models. The study area is considered as one of the most important well fields therein providing partially three governorates with the needed drinking water. Detailed geological and hydrogeological parameters as well as the land-use map of the area were obtained from various sources to utilize both models. ArcGIS software was used for calculations and maps preparation. As a result, the generic DRASTIC vulnerability index ranges between 109 and 168. Thus, two vulnerability classes were observed, moderate (9.9%) and high (90.1%) vulnerability classes. On the other hand, the modified DRASTIC model (risk map) is taking into account the land-use map classes in the study area. The output risk map reveals two main classes, the moderate and high-risk areas. The moderate-risk areas occupy 9.3% of the total volume of the study area while the high-risk areas are 90.7%. Due to the high depth to groundwater within the area (between 90 m and 390 m), the depth to groundwater intervals was modified in the model to become more comfortable with the situation in Jordan. The high percentage of the high vulnerable areas against pollutants reflects the need to do more investigation for the studied area.

AIM: To study the significance of cap-fitted colonoscopy in improving cecal intubation time and polyp detection rate. METHODS: This study was a prospective randomized controlled trial conducted from March 2008 to February 2009 in a tertiary referral hospital at Sydney. The primary end point was cecal intubation time and the secondary endpoint was polyp detection rate. Consecutive cases of total colonoscopy over a 1-year period were recruited. Randomization into either standard colonoscopy (SC) or cap-assisted colonoscopy (CAC) was performed after consent was obtained. For cases randomized to CAC, one of the three sizes of cap was used: D-201-15004 (with a diameter of 15.3 mm), D-201-14304 (14.6 mm) and D-201-12704 (13.0 mm). All of these caps were produced by Olympus Medical Systems, Japan. Independent predictors for faster cecal time and better polyp detection rate were also determined from this study. RESULTS: There were 200 cases in each group. There was no signif icant difference in terms of demographic characteristics between the two groups. CAC, when compared to the SC group, had no signif icant difference in terms of cecal intubation rate (96.0% vs 97.0%, P = 0.40) and time (9.94 ± 7.05 min vs 10.34 ± 6.82 min, P = 0.21), or polyp detection rate (32.8% vs 31.3%, P = 0.75). On the subgroup analysis, there was no significant difference in terms of cecal intubation time by trainees (88.1% vs 84.8%, P = 0.40), ileal intubation rate (82.5% vs 79.0%, P = 0.38) or total colonoscopy time (23.24 ± 13.95 min vs 22.56 ± 9.94 min, P = 0.88). On multivariate analysis, the independent determinants of faster cecal time were consultant-performed procedures (P < 0.001), male patients (P < 0.001), non-usage of hyoscine (P < 0.001) and better bowel preparation (P = 0.01). The determinants of better polyp detection rate were older age (P < 0.001), no history of previous abdominal surgery (P = 0.04), patients not having esophagogastroduodenoscopy in the same setting (P = 0.003),

Breast cancer in women is a complicated and multifaceted disease. Studies have demonstrated that hyperglycemia is one of the most significant risk factors for breast cancer. Hyperglycemia is when the sugar level in human blood is too high, which means excess glucose. Glucose excess can encourage the growth, invasion, and migration of breast cancer cells at the cellular level. Though, the effects of glucose on the dynamics of breast cancer cells have been examined mathematically by a system of ordinary differential equations. However, the non-instantaneous biological occurrences leading to the secretion of immuno-suppressive cytokines by tumors to evade immune surveillance and the immune cells’ derivation of cytokines to attack the tumor cells are not yet discussed. Therefore, investigating the biological process involved in the dynamics of tumors, immune and normal cells with excessive glucose concentration is inviolable to determining the best procedure for controlling tumors’ uncontrollable growth. Time delay, denoted by τ, is used to describe the time tumor cells take to secrete immunosuppressive cytokines to evade immune surveillance and the time immune cells take to recognize and attack the tumor cells. We have studied the local stability analysis of the biological steady states in both delayed and non-delayed system. The Routh-Hurwitz stability criterion is used to analyze the dynamical equilibrium of the cells’ population. Hopf bifurcation was analyzed by using time delay s as a bifurcation parameter. The analytical results suggest an unstable scenario for a tumor-free equilibrium point as normal cells are bound to grow to their carrying capacity. The result predicts a stable system for coexisting equilibrium when the interaction is instantaneous (τ = 0). However, when τ > 0, the coexisting equilibrium point switches from stable to unstable.
