Seroprevalence of brucellosis among exposed agro-pastoral communities in southern Saudi Arabia

Objective: To investigate the prevalence and risk factors of brucellosis in human and animal’s communities in southern Saudi ***: A cross-sectional sero-epidemiological study was conducted in Aseer and Jazan, Saudi Arabia(October 2017-October 2018). Human serum samples(n=339) were initially screened for Brucella antibodies and positive samples were further titrated for Brucella antibodies by immunocapture assay(titer of ≥1:320 as positive). Animal samples(n=828) were screened using the Rose Bengal test. Relationship status was dichotomized to measure and predict independent contributions to variations in human using univariate and multivariate stepwise binary logistic regression model. Results: The rate of brucellosis among the 339 human samples in the two regions was 33.9%, and the rate of acute brucellosis was 12.4%. The rate of brucellosis in animals was 4.7%. Human brucellosis among the target groups was higher in northwestern Aseer(53.3%) compared to Southeastern Aseer(25.9%) and Jazan region(20.6%). The disease was more prevalent among non-Saudi nationals(35.2%) compared to Saudis(30.5%). The rate of brucellosis among butchers and shepherds was 37.5% and 37.2%, respectively. The rate of brucellosis was 37.8% in people over 30 years of age. Our univariate analysis showed that residing in Aseer region(OR: 2.60, 95% CI: 1.50-4.40), especially residing in northwestern Aseer region(OR: 4.40, 95% CI: 2.40-7.90), frequent consumption of raw meat(OR: 2.90, 95% CI: 1.50-5.50), shepherds(OR: 2.10, 95% CI: 0.80-5.30), owning sheep(OR: 2.20, 95% CI: 1.10-4.40), daily contact with animals(OR: 2.10, 95% CI: 0.75-5.80), and those > 30-year-old(OR: 1.50, 95% CI: 1.00-2.40) were significantly associated with increased risks of brucellosis. Our multivariate analysis further showed that residing in northwestern Aseer(OR: 9.16, 95% CI: 3.39-24.76) and having sheep(OR: 1.16, 95% CI: 1.00-1.35) were significant and independent risks of brucellosis while residing in agric

In this paper we consider the most general form of non-static cylindrically symmetric space-times in order to study proper teleparallel homothetic vector fields using the direct integration technique and diagonal tetrads. This study also covers static cylindrically symmetric, Bianchi type I, non-static and static plane symmetric space-times as well. Here, we will only discuss the cases which do not fall in the category of static cylindrically symmetric, Bianchi type I, non-static and static plane symmetric space-times. from the above study we show that very special classes of the above space-times yield 6, 7 and 8 teleparallel homothetic vector fields with non-zero torsion.

In reliability analysis,the stress-strength model is often used to describe the life of a component which has a random strength(X)and is subjected to a random stress(Y).In this paper,we considered the problem of estimating the reliability𝑅𝑅=P[Yf both stress and strength are independent and follow exponentiated Pareto *** maximum likelihood estimator of the stress strength reliability is calculated under simple random sample,ranked set sampling and median ranked set sampling *** different reliability estimators under median ranked set sampling are *** estimators are obtained when both strength and stress have an odd or an even set *** two other estimators are obtained when the strength has an odd size and the stress has an even set size and vice *** performances of the suggested estimators are compared with their competitors under simple random sample via a simulation *** simulation study revealed that the stress strength reliability estimates based on ranked set sampling and median ranked set sampling are more efficient than their competitors via simple random *** general,the stress strength reliability estimates based on median ranked set sampling are smaller than the corresponding estimates under ranked set sampling and simple random sample ***:Stress-Strength model,ranked set sampling,median ranked set sampling,maximum likelihood estimation,mean square *** estimates under ranked set sampling and simple random sample methods.

This paper deals with the Bayesian estimation of Shannon entropy for the generalized inverse exponential *** that the observed samples are taken from the upper record ranked set sampling(URRSS)and upper record values(URV)*** of Bayesian estimators are derived depending on a gamma prior distribution considering the squared error,linear exponential and precautionary loss functions,in addition,we obtain Bayesian credible *** random-walk Metropolis-Hastings algorithm is handled to generate Markov chain Monte Carlo samples from the posterior ***,the behavior of the estimates is examined at various record *** output of the study shows that the entropy Bayesian estimates under URRSS are more convenient than the other estimates under URV in the majority of the ***,the entropy Bayesian estimates perform well as the number of records *** obtained results validate the usefulness and efficiency of the URV *** data is analyzed for more clarifying purposes which validate the theoretical results.

Treatment of hepatitis C virus genotype 4 with peginterferon alfa-2a: Impact of bilharziasis and fibrosis stage

AIM： To evaluate pegylated interferon alpha2a （PegIfN-α2a） in Egyptian patients with HCV genotype 4, and the impact of pretreatment viral load, co-existent bilharziasis and histological liver changes on response rate. METHODS： A total of 73 nafve patients （61 with history of bilharziasis） with compensated chronic HCV genotype 4 were enrolled into： group A （38 patients） who received 180 mg PegIfN-alpha2a subcutaneously once weekly for a year and group B （35 patients） received IfN alpha-2a 3 MU 3 times weekly. Ribavirin was added to each regimen at a dose of 1200 mg. Patients were followed for 72 wk and sustained response was assessed. RESULTS： Significant improvement in both end of treatment response （ETR）（P 〈 0.002） and sustained response （SR）（P 〈 0.05） was noted with pegylated interferon, where ETR was achieved in 29 （76.3%） and 14 patients （40%） in both groups respectively, and 25 patients in group A （65.8%） and 9 （25.7%） in group B could retain negative viraemia by the end of follow up period. Sustained virological response （SVR） showed a significant negative correlation with age and positive correlation with pretreatment inflammation in patients receiving PegIfN. Viral clearance after 3 mo of therapy was associated with high incidence of ETR and SR （P 〈 0.001）, but without significant difference between both forms of interferon. Significant improvement in response was achieved in patients with high grade fibrosis （grade 3 and 4） with PegIfN-α2a, where SR was seen in 5 out of 13 patients in group A, but none in group B. There was no significant difference in response between bilharzial and non-bilharzial patients in both groups. In terms of safety and tolerability, neutropenia was the predominant side effect, both drugs were comparable. CONCLUSION： PegIfN-~2a combined with ribavirin results in improvement in sustained response in HCV genotype 4, irrespective of history of bilharzial infestation.

Microalbuminuria in hepatitis C-genotype 4:Effect of pegylated interferon and ribavirin

AIM:To study the relation between hepatitis C virus (HCV) genotype 4 and microalbuminuria and renal impairment in relation to hepatic histology, and viremia in the absence of cryoglobulinemia, and to examine the effect of treatment on ***: Three hundred subjects, including 233 HCV genotype-4 infected patients, were tested for cryoglobulinemia, microalbuminuria, albumin creatinine ratio (ACR), urea, creatinine, and estimated glomerular filtration rate (eGfR). The parameters were measured again in the HCV patients after 48 wk of treatment with pegylated interferon and ***: Significantly higher levels of microalbumin- uria were detected in HCV-positive patients compared to HCV-negative controls (median 9.5 vs 5.9, respectively, Kruskal-Wallis P=0.017). Log microalbuminuria was significantly correlated with hepatic inflammation (r=0.13, P=0.036) and f ibrosis (r=0.12, P=0.061), but not with viral load (r=-0.03, P=0.610), or alanine transaminase (r=-0.03, P=0.617). Diabetes mellitus neither significantly moderated (χ2=0.13, P=0.720), nor mediated (Sobel test P=0.49) the HCV effect. HCV status was signifi cantly associated with log microalbu-minuria (χ2=4.97, P=0.026), adjusting for age, gender, diabetes, cryoglobulinemia, urea and creatinine. A positive HCV status was not significantly associated with low eGfR (fidence interval (CI):0.2-1.4], nor with high ACR (OR: 1.7, 95% CI:0.7-4.1). End-of-treatment response (ETR) was achieved in 51.9% of patients. Individuals with ETR had significantly lower microalbuminuria post-treatment (χ2=8.19, P=0.004).CONCLUSION: HCV affected the development of microalbuminuria independent of diabetes or cryoglobulinemia. Combination therapy of pegylated interferon-ribavirin had a positive effect in reducing microalbuminuria.

Small-animal PET demonstrates brain metabolic change after using bevacizumab in a rat model of cerebral ischemic injury

ABSTRACT To evaluate the effect of bevacizumab on cerebral ischemia, we used 2-deoxy-2-18f-fluoro-D-glucose （18f-fDG） small-animal positron emission tomography （PET） in the middle cerebral artery occlusion （MCAO） rat model. After baseline neurologic function tests and PET studies, MCAO Sprague-Dawley rats received bevacizumab or normal saline （controls）. Weekly PET imaging and neurologic function tests showed that the 18f-fDG accumulation in the bevacizumab group was similar to that in the controls during the first 2 weeks, but lower than in controls at weeks 3 and 4. However, no difference was found in neurological scores between the groups. The number of von Willebrand factor-positive cells in the bevacizumab group was lower than that in controls. The expression of vascular endothelial growth factor was higher than in controls at week 4. These results suggested that bevacizumab does not influence functional recovery in this model of cerebral ischemia during a 4-week period, but inhibits vascular formation and metabolic recovery, which may be considered in cancer patients with a recent ischemic stroke.

Bound states in the continuum （BICs） are perfectly localized resonances despite embedding in the continuum ***,an isolated BIC is very sensitive to the structure ***,we report merging acoustic BICs in a single open resonator,robust against the structure *** find that both symmetry-protected BIC and friedrich-Wintgen BIC are sustained in a single coupled waveguide-resonator *** varying the height and length of the resonator,these two BICs move toward each other and merge into a single one at a critical *** to an individual BIC,the merged BIC is robust against fabrication error because its Q-factor is proportional to ΔL-4,whereΔL embodies the structure *** essence of this extraordinary phenomenon is perfectly explained by the two-and three-level approximations of the effective nonHermitian ***,we present direct experimental demonstrations of the moving and merging of BICs in a coupled 3D waveguide-resonator,which are evidenced by the vanishing of the linewidth of fano resonance in the transmission *** results may find exciting applications in designing high-quality acoustic sources,sensors and filters.

Stealth Considerations for Aerodynamic Configurations Design of Missiles

The aerodynamic design of a strategic weapon is of interest, especially when the radar signatures are included in the conceptual design phase. The basics of stealth configurations and stealth mechanisms for missiles are reviewed. The Radar Cross Sections （RCS） of some generic missiles are predicted and compared to analyze the trade-offs involved between low RCS and aerodynamic performance. The consideration of RCS prediction in the conceptual design phase gives a quick insight into the stealth performance prior to detailed design.

Usually,rural areas can be electrified via three-phase distribution transformers with relatively large *** such areas,low voltage lines are used for long distances,which cause power losses and voltage drop for different types of *** losses and improving voltage profiles in rural distribution networks are significant challenges for electricity distribution *** different solutions were proposed in the literature to overcome these challenges,most of them face difficulties when applied in the conventional distribution *** address the above issues,an applicable solution is proposed in this paper by installing a number of small-capacity distribution transformers instead of every single large-capacity transformer in rural *** proposed approach is implemented in the branch network of Al-Hoqool village,which belongs to the Nineveh distribution *** network has been inspected on-site,drawn,and analyzed using the electrical systems analysis program(ETAP).The analysis showed that using the single-phase pole-mounted transformers can improve the voltage in the network’s end by 29%and enhance the voltage profile for all *** analysis has also demonstrated that the modification can reduce the total power losses by 78%compared to the existing *** the economic aspect,the payback period for the proposed network is assigned to be 20 months.
