Current concepts on osteonecrosis of the femoral head

It is estimated that 20000 to 30000 new patients are diagnosed with osteonecrosis annually accounting for approximately 10% of the 250000 total hip arthroplasties done annually in the United States. Thelack of level 1 evidence in the literature makes it difficult to identify optimal treatment protocols to manage patients with pre-collapse avascular necrosis of the femoral head, and early intervention prior to collapse is critical to successful outcomes in joint preserving procedures. There have been a variety of traumatic and atraumatic factors that have been identified as risk factors for osteonecrosis, but the etiology and pathogenesis still remains unclear. Current osteonecrosis diagnosis is dependent upon plain anteroposterior and frog-leg lateral radiographs of the hip, followed by magnetic resonance imaging(MRI). Generally, the first radiographic changes seen by radiograph will be cystic and sclerotic changes in the femoral head. Although the diagnosis may be made by radiograph, plain radiographs are generally insufficient for early diagnosis, therefore MRI is considered the most accurate benchmark. Treatment options include pharmacologic agents such as bisphosphonates and statins, biophysical treatments, as well as joint-preserving and joint-replacing surgeries. the surgical treatment of osteonecrosis of the femoral head can be divided into two major branches: femoral head sparing procedures(FHSP) and femoral head replacement procedures(FHRP). In general, FHSP are indicated at pre-collapse stages with minimal symptoms whereas FHRP are preferred at post-collapse symptomatic stages. It is difficult to know whether any treatment modality changes the natural history of core decompression since the true natural history of core decompression has not been delineated.

Towards designing telepresence robot navigation for people with disabilities

Aneurysmal fibrous histiocytoma is often clinically *** this study,we put forward an insight on how to help diagnose this disease clinically.A retrospective chart review was performed on all patients diagnosed with aneuiysmal fibrous histiocytoma from 2007 to 2017 in the Department of Dermatology,Union Hospital,China,and all clinical data were collected from the hospital *** a total of 418 patients diagnosed with cutaneous fibrous histiocytoma,only 30 patients were confirmed to have aneurysmal fibrous histiocytoma out of which only 2 patients were clinically diagnosed with aneurysmal fibrous *** remaining 28 patients were diagnosed with various types of vascular tumors although pathology classified them as having aneurysmal fibrous *** the 30 patients,9 were male and 21 were *** were following age groups:13-19(mean 16,n=4),20-29(mean 26.25,n=8),30-39(mean 33,n=7),40-49(mean 44,n=4),50-59(mean 56.75,n=4)5 60 and above(mean 61,n=3).Tumors were present on the head,neck,back,waist,hips and upper and lower *** complete excision,there was no recurrence and no ***,lesions showed the typical pseudoangiomatoid spaces without endothelial lining and infiltration of fibrohistiocytes in hemosiderotic *** was suggested that although the prognosis of aneurysmal fibrous histiocytoma is good,accurate diagnosis is paramount to avoid clinical misdiagnosis and subsequent complications.

Background： Bacillus spp. seem to be an alternative to antimicrobial growth promoters for improving animals＇ health and performance. However, there is little information on the effect of Bacillus spp. in combination with different dietary crude protein（CP） levels on the ileal digestibility and microbiota composition. Therefore, the objective of this study was to determine the effect of Bacillus spp. supplementation to low-（LP） and high-protein diets（HP） on ileal CP and amino acid（AA） digestibility and intestinal microbiota ***： Eight ileally cannulated pigs with an initial body weight of 28.5 kg were randomly allocated to a rowcolumn design with 8 pigs and 3 periods of 16 d each. The assay diets were based on wheat-barley-soybean meal with two protein levels： LP（14% CP, as-fed） and HP diet（18% CP, as-fed）. The LP and HP diets were supplemented with or without Bacil us spp. at a level of 0.04%（as-fed）. The apparent ileal digestibility（AID） and standardized ileal digestibility（SID） of CP and AA was determined. Bacterial community composition from ileal digesta was analyzed by Illumina amplicon sequencing and quantitative real-time PCR. Data were analyzed as a 2 × 2 factorial design using the GLIMMIX procedures of ***： The supplementation with Bacillus spp. did not affect both AID and SID of CP and AA in growing ***, there was no difference in AID of CP and AA between HP and LP diets, but SID of cystine, glutamic acid,glycine, and proline was lower（P 〈 0.05） in pigs fed the HP diets. Kinetics of Norepinephrine- and Serotonin-Induced BDNF Release in Cultured Embryonic Hippocampal Neurons

The primary neurotransmitters targeted by currently used antidepressants, such as duloxetine, venlafaxine and fluoxetine, are serotonin and norepinephrine, which also are released in significant amounts in the central nervous system in response to sympathetic nervous system activation. In cultured hippocampal neurons, we have previously shown that norepinephrine induces increased expression of brain-derived neurotrophic factor (BDNF), the PI-3 K/Akt, MAPK pro-survival pathways, the BDNF receptor, TrkB, a transcription factor, and cyclic AMP response element binding protein (CREB). In the present study, we extend these findings of increased BDNF expression to its kinetics of release into the surrounding media. We also evaluate these two cell survival pathways, TrkB and CREB, in response to application of serotonin and/or norepinephrine. Serotonin elicits an earlier, but brief expression and release of BDNF, whereas norepinephrine elicits a more delayed and sustained release of BDNF. In response to both norepinephrine and 5-HT, the presence of BDNF in lysates and subsequent release into the media is significantly increased out to 4 h, as is PI-3 K/Akt activation. Together, these two neurotransmitters increase BDNF expression and release covering the entire 8 h continuum evaluated. The results of this study provide further evidence for a G protein-coupled receptor and a crosstalk-to-TrkB receptor with transactivation signaling across pathways.

Norepinephrine Induces BDNF and Activates CREB and Discriminates among Protein Kinase C Isoforms in Cultured Embryonic Hippocampal Neurons

Medications acting as mood stabilizers work by enhancing and maintaining the concentration of circulating synaptic neurotransmitters, which then activate a plethora of various intracellular signal transduction and second messenger cascades. Previously, we showed that two of these cascades, the PI-3K and MAPK pathways, are activated by cross-talk with the protein kinase A cAMP cascade and by brain-derived neurotrophic factor (BDNF), an immediate-early gene whose expression is the result of phosphorylation of the transcription factor, cAMP response element binding protein (CREB). In the current study, we extend these findings to include the protein kinase C (PKC) pathway. Western blotting studies show that application of norepinephrine to cultured hippocampal neurons leads to increased expression of BDNF, phosphorylation of CREB, activation of growth-associated protein-43 (GAP-43) and activation of PKCμ and PKCt538. Because GAP-43 is a putative substrate for PKC, the results of this study lend further support of a G-protein coupled receptor cross-talking to an entirely distinct intracellular pathway that is known to be involved in neuritogenesis.

Role of Plasma Surface Treatments on Wetting and Adhesion

There are many current and emerging wetting and adhesion issues which require an additional surface processing to enhance interfacial surface properties. Materials which are non-polar, such as polymers, have low surface energy and therefore typically require surface treatment to promote wetting of inks and coating. One way of increasing surface energy and reactivity is to bombard a polymer surface with atmospheric plasma. When the ionized gas is discharged on the polymer, effects of ablation, crosslinking and activation are produced on its surface. In this paper we will analyse the role of plasma and its use in increasing the surface energy to achieve wettability and improve adhesion of polymeric surfaces.
