Relevance and Compliance of Requests for Dental Panoramic Radiography Examination at Yaounde (Cameroon)

Background: Research studies made in Cameroon on compliance and relevance in the various areas of imaging, show the failure to comply with administrative and clinical criteria. These research studies led to recommendations meant for result amelioration. However, the orthopantomogram (OPT) field remains less studied in that regard in Cameroon. Aim: Evaluate the relevance and compliance of examination requests for panoramic dental radiography. Setting and Design: It is a descriptive cross-sectional study with a consecutive sample of dental panoramic requests identified during the study period in 4 radiology centres of Yaoundé. Material and Methods: Variables studied here were the validity criteria of imaging examination. Request: Five of which are of administrative order (date of the request, requesting department, patient’s identity, patient’s age, applicant’s identity) and three of clinical order (anatomic region, reason for the examination and the purpose of the examination). Statistical Analysis: Chi-squared test was used with confidence interval of 95%. Results: Patient’s identity (name) was the criteria carrying the highest information (98.9%), followed by the name of the applicant physician (91.6%). Out of 179 requests analysed, 8.6% had complete information. 46.4% of requests had no indications. Dental surgeon was the top prescriber with a total of 112 (62.6%). Conclusion: The quality of dental panoramic requests was not optimal in Yaounde, with lack of precision mostly noted at the level of clinical criteria.

Utility, Appropriateness of Request and Report of Computed Tomography Scan for the Diagnosis of Paranasal Sinuses Pathologies in a Sub-Saharan Africa Urban Setting

Background: Computed tomography (CT) Scan is the imaging of choice for the diagnosis of sino nasal (SN) pathologies in sub-Saharan African setting. However, it is often requested by non experienced practitioners. We aimed to describe the utility and the appropriateness of CT Scan request forms and results in the diagnosis of SN pathologies. Method: We conducted a retrospective cross-sectional study in four health facilities in Yaoundé, Cameroun, from October 2018 to September 2019. All patients to whom SN CT Scan was performed were called for an appointment. After informed consent, data on the appropriateness and conformity of the request form were collected and compared to the guidelines of the French society of radiology. CT Scan results were also collected and the quality of the report was evaluated. Results: A total of 206 SN CT Scan request forms and reports were included, with a mean patient age of 37.7 ± 16.1 years. The request form was conformed to guidelines in 8.3% of cases and was found appropriate in 62.6% of cases. No CT Scan report had all the items required, and 159 (77.2%) reports were useful. The most common diagnosis in patients with abnormal results was chronic sinusitis (49.5%) and SN polyp or polyposis (19.4%). Conclusion: Most of SN CT Scan requested were appropriate and useful. However, the conformity of request forms and reports to the guidelines is low. Suggesting the need for capacity building for the diagnosis of SN pathologies especially for non specialists like general practitioners and nurses.

Histopathological and Ultrasound Correlation in Women Presenting with Breast Lumps in Yaoundé, Cameroon

Context: Breast cancer is very deadly among women with higher rates in the developing world. Imaging tools such as ultrasound, can be used to differentiate between the types of breast lumps. This study aimed to determine the value of ultrasound as a first-line examination in the diagnosis of breast masses. Methods: This was a retrospective cross-sectional study of women presenting with breast lumps from November 2022 to June 2023 at the Yaounde General Hospital, which lasted seven months, from November 2022 to June 2023. The sampling was exhaustive and consecutive. Association between variables was studied using the x2 test and concordance between ultrasound and histopathological findings was assessed using the Kappa correlation coefficient. Results: 234 women were included in the study. Their mean age was 46.3 ± 11.4 years. Overall, 15 (6.4%) lumps were benign while 219 (93.6%) were malignant. Triple negative (5.6%) was the most recurrent genomic classification. The correlation between the ultrasound and histopathological findings was significant, with an observed concordance rate at 85.1%, kappa = 0.322 and a p-value Conclusion: The performance of ultrasound in differentiating benign and malignant lesion was high. However, the discordant cases highlight the need for a diagnosis system which blends histopathological and radiological findings for an improved management of patients with breast lumps.

Ultrasound Value in the Management of Parapneumonic Pleural Effusions in a Limited-Resource Setting: A Case Report

Pleural effusion is a common complication of acute lung infection, with rising morbidity and mortality. If poorly treated, parapneumonic effusion evolves to the fibrino-purulent stage wherein antibiotic therapy alone becomes inadequate. Chest CT is the gold standard diagnostic imaging tool, however, in a resource-limited context, it may not be performed. Chest ultrasound can therefore be an alternative for drainage and intermittent follow-up of complicated parapneumonic pleural effusions. We report the case of a 4-year-old child who presented with cough, breathing difficulties and fever for over two weeks and in whom an initial chest X-ray revealed a left hemithorax white-out with an air-fluid level. Chest ultrasound revealed a left pleuropulmonary massive fluid collection with an encysted empyema. It also allowed ultrasound-guided pleural effusion drainage of a fibrino-purulent liquid which tested positive for Kocuria kristinae, a bacterium sensitive to gentamycin, vancomycin, norfloxacin and clindamycin. The next follow-up ultrasound checks showed improvement and the control chest X-ray performed one month later demonstrated pulmonary functional recovery. This case highlights the importance of ultrasound in the management and follow-up of this chest pathology in resource-limited settings.

Chest investigation is common in hospital practice. Chest X-ray is readily available and usually the first chest investigation. Thoracic CT scan constitutes an alternative and complimentary chest investigation. It is currently the most efficient investigation for the chest and its contents. Objectives: To evaluate the relevance of chest CT Scan requests in two university teaching hospitals in Cameroon. Material and Methods: We conducted a cross-sectional, retrospective and descriptive study at the Radiology and imaging units of the Yaounde Central Hospital and the Yaounde University Teaching Hospital Centre. Included in our study were files of patients who had a chest CT scan investigation during three years. Results: We had a study population of 323 subjects. The age interval was 23 months to 91 years old. Pulmonologist were the most prescribers with 27.2%. We had 80% conformity of indications with French Society of Radiology (FSR) standards. 50 over of 323 indications were not recommended by the FSR. Conclusion: There is a conformity rate of 80% between indications and the FSR recommendations.

Cancer Occurrence and Associated Factors to Malignancy in BIRADS-3 Lesions in Yaoundé: The Need to Be More Proactive for Patients above 40, Non-Compliant with Imaging Follow-Up and Presenting with Nipple Discharge

Background and objectives: Breast Imaging Reporting and Data System in Category 3 (BIRADS-3) includes probably benign lesions which need a short-term imaging follow-up. However, in our context, the lesions graded BIRADS-3 remain insufficiently evaluated. We therefore conducted this study to assess the cancer occurrence and associated factors in BIRADS-3 lesions during the follow-up in order to propose an adaptation of the management for lesions in this category in our setting. Patients and methods: A retrospective longitudinal study of patients with lesions initially classified as BIRADS-3 and who realised each at least one additional imaging check-up between January 2014 and December 2022 in five Yaoundé hospitals. All clinical and imaging data were analysed using SPSS® 21.0 software with a significant p-value Results: Patients were aged 13 to 73 (33.0 ± 13.4) years, with a history of breast mass (315 cases;79.7%), breast pain (25 patients;6.3%), nipple discharge (20 patients;5.1%) or past family history of breast cancer (25 cases;6.3%). The most common baseline abnormalities were mammogram opacities (64.8%) and microcalcifications (48.6%), whereas initial breast ultrasound showed solid masses (77.0%) and cystic lesions (11.1%). Compliance with imaging appointment periods was low with only 23.9% of all patients performing an imaging control at the scheduled moment. During the follow-up, 115 patients (29.1%) were upgraded to BIRADS-4 and histology performed revealed 43 cancers (10.9% of overall initial BIRADS-3 sample). The presence of malignancies was associated to age above 40 years (p = 0.0001) and to the presence of nipple discharge (p = 0.0375). Conclusion: The frequency of malignancies among initial BIRADS-3 lesions in our series is higher than that described in the guidelines. Introduction: The diagnosis of pneumonia is usually made based on clinical manifestations and chest X-ray. The use of ultrasound in detecting pulmonary diseases in general, and especially consolidation syndrome has been demonstrated. The objective of this study was to determine the accuracy of thoracic ultrasound compared to chest X-ray in the diagnosis of infectious pneumonia in children. Methods: Children between 0 to 15 years were included in our study. The lung ultrasound results obtained were compared with those of the chest X-ray used as the reference. Our data were introduced into the EpiInfo 3.5.4 software and analyzed with the EpiInfo 3.5.4 and IBMSPSS Statistics version 20.0 softwares. Microsoft Office Excel 2016 was used to produce Charts. Continuous quantitative variables were presented. Cohen’s Kappa concordance test was applied with confidence interval of 95%. Results: 52 children were enrolled in the study. In imaging, the dominant sign was consolidation syndrome (75.0%) of cases by chest radiography, and in 78.8% of cases by lung ultrasound (p Conclusion: Our study demonstrated that lung echography is a non-ionizing and reliable tool in the diagnosis of childhood’s pneumonia.

Application of a k-ε Model to Heat Transfer in Impinging Flows

Local heat transfer is predicted in turbulent axisymmetric jets, impinging onto a flat plate. A non-linear k-e model is used, in which both the constitutive law for the turbulent stresses and the transport equation for the turbulent dissipation rate e have an important contribution in the improved heat transfer predictions. The shape of the Nusselt number profiles, expressing dimensionless heat transfer, as well as the stagnation point value, are well predicted for different distances between the nozzle exit and the plate. Accurate flow field predictions are the basis for good heat transfer predictions. For a fixed Reynolds number, the influence of the nozzle-plate distance is well captured. For a fixed distance, the influence of the Reynolds number is correctly reproduced. Comparisons are made to a low-Reynolds standard k-e model and the v2-f model. A thorough discussion is found in [4]. Only a summary of those results is discussed here, while some new results are also presented.

In gas turbine engines, laminar-turbulent transition occurs. However, generally, the turbulence models to describe such transition results in too early and too short transition. Combining a turbulence model with a description of intermittency, i.e. the fraction of time the flow is turbulent during the transition phase, can improve it. By letting grow the intermittency from zero to unity, start and evolution of transition can be imposed. In this paper, a method where a dynamic equation of intermittency combining with a two-equation k-ωturbulence model is described. This intermittency factor is a premultiplicator of the turbulent viscosity computed by the turbulence model. Following a suggestion by Menter et al.[1], the start of transition is computed based on local variables.
