Seismic refraction investigations have been carried out in Bakel, Eastern Senegal. The purpose was to map geometrical relationship between the existing rock types and the Panafrican quarzitic basement, which is valuable information for the project of the Bakel fluviatile port construction. Four seismic refraction profiles were acquired. The obtained data have been processed by inversion. The obtained four seismic P-wave velocity profiles have been integrated to obtain a 3D model. By comparing the outcropping geological formations with the observed seismic data at the surface, it was possible to identify the lithology corresponding to each measured range of seismic velocity for the alluvium, the weathered bed rock, and the fresh rock. The results showed that the depth of the fresh rock of the basement varies from 0 to 18 meters above the sea level, with a deepening toward the Senegal River and toward the Northern part of the studied area. The presence of alluviums and their thickness are linked to the existence of bays and gulfs. The results of this study give valuable information for the river bed dredging cost assessment prior to the port construction phase.

Introduction: The principal postoperative complication of mastectomies with axillary dissection is the lymphocele that can last many months after surgery. The purpose of our study was to prevent its formation using the padding. Methods: Sixty-one patients have been included in our study. The follow-up was 6 months. The patients were divided in two groups through a random draw (simple drainage and drainage associated with padding). All patients had a mastectomy with axillary dissection following the Madden technique. All quantities of lymphoceles during postoperative hospitalization and ambulatory care have been noted. Results: Twenty-five patients had benefited from the padding and 36 of a simple axillary drainage. Six months after the surgery, the patients benefitting from the padding had a quantity of lymphocele equal to half that of the control group (761.83 mL against 1373.60 mL;p = 0.01). During the postoperative hospitalization, the quantities were of 362.80 mL for the padding group versus 630.83 mL;p Conclusion: The production of postoperative lymphocele is heterogenous, varying from one patient to another. Improving the Stability of the Inflatable Soil of Diamniadio by Using Typha Australis in Order to Value It in Partitioning Wall

The construction industry in Senegal is experiencing an upsurge in the development of residential housing units and administrative buildings within the new Diamniadio municipality, an area located just about 35 km from the capital city of Dakar. Laboratory oedometer or expansive-index tests, however, show poor density and compromised shear strength in the soil samples within the area -posing serious construction problems due to significant volume changes (swellings) that occur when subjected to moisture, thus, bringing into question the structural performance integrity of the soil, and the financial implications of substitute soil types used to compensate for its poor tensile strength. By way, the companies collect the soil of this location (Diamniadio) and throw it to make pile or micro-pile (pious) for their structures. This article demonstrates how we can value the soil of Diamniadio and the Typha Australis in residential building with a reasonable cost. Typha Australis is a plant generally luxuriant within the northern and central belts of Senegal but also known for its negative ecological impacts on the agricultural production of rice. The valorisation of the soil of Diamniadio will pass by the ability to increase its stability or to decrease its shrinkage/swelling rate. When we achieve it, the soil associate with Typha Australis will be used for small bricks which can be useful for partition wall and so the soil will not be ever thrown as a non-useful material. Tests results of soil specimens extracted from the site show a shrinkage rate of 29.19% with estimated cracking depths of 3.5 cm. But by adding gradually Typha Australis, the shrinkage rate will pass from 29.19% to 5.13% with the material treats with 10% by mass of Typha Australis. Moreover, the crackings in the cylindrical specimen disappear. That shows the increase of the stability;thus the composition will be used for building bricks. In the literature, the management of enterocutaneous fistula of tuberculosis origin is rare. The aim of this work was to report a case of external digestive fistula of tuberculosis origin in a 10-year-old girl in northern Mali in a situation of armed conflict. She was a 10-year-old patient who was admitted to our general surgery ward for external digestive fistula evolving for more than (4) four years, at admission the general. The karnofsky index was at 40%, she could not sit or hold a cup to drink water. A 3 cm enterocutaneous fistula deafened foul-smelling liquid stools, on clinical examination, the conjunctivas were pale, the lips were dry, the eyes were sunk in the eye sockets, the ribs visible from a distance. The paraclinical aspects (Itra-Dermo-Reaction (IDR) to tubercria, anatomopathology) were positive, specifying the place of medical treatment and evolution under medical treatment. Digestive fistula of tuberculosis origin is a rare condition in surgical settings.

The World Health Organization declared that corona virus diseases-19 (COVID-19) is a public health emergency. The COVID-19 pandemic is more likely to cause disaster in developing area including West Africa due to limited medical resources. COVID-19 reportedly causes severer conditions in adults with advanced age and in patients with underlying comorbidities including sickle cell anemia. We recently experienced two pediatric patients with sickle cell disease (SS), who had COVID-19. We here highlight the difficulties of management and the severity of COVID-19 infection in children with homozygous sickle cell SS.

The Use of Atypical Neuroleptics in Drugs Management in Fann’s Psychiatry Department

Background and Objectives: Neuroleptics have revolutionized pharmacological management in psychiatry. The advent of atypical neuroleptics constitutes a major turning point in this therapeutic challenge. The objective of our study was to determine the prescription of these atypical neuroleptics in Fann’s psychiatry department. Methodology: It was a descriptive retrospective cross-sectional study that involved 223 patients hospitalized in the psychiatry department of Fann Hospital during the period from January 2015 to December 2017. Results: Our study shows a majority prescription of classic neuroleptics in combination Haloperidol-Chlorpromazine 53% and 14% in monotherapy and a prescription of atypical neuroleptics at 4%. They are prescribed in combination with the classics at 4%. Conclusion: These results show a still low prescription of atypical neuroleptics which raises questions about the availability and affordability of such drugs, but also about prescription habits in the context of developing country.

Coronary Angioplasty in a Low Income Sub-Saharan Country: Situational Analysis

Introduction: Coronary angioplasty is not widely conducted in sub-Saharan Africa in general and Senegal in particular. The objective of this work was to describe the indications, techniques, results and difficulties of coronary angioplasty in Senegal. Patients and Methods: We prospectively included all patients who underwent coronary angioplasty between July 2012 and November 2014. We evaluated the clinical characteristics, techniques used, and the peri-procedural complications. Results: 110 coronary angioplasties were performed at the cardiac catheterisation unit of the Clinique du Golf in Dakar. The average age of patients was 60.41 ± 10.93 years. A male predominance was noted with a sex ratio of 6.3. Hypertension was found in 57% of patients. The principal indications for coronary angioplasty were for the management of acute coronary syndromes with or without ST elevation, in 29 and 39 patients respectively. The femoral artery was the puncture site in 104 patients. The lesions treated were type A/B1 or B2/C in 53 and 57 cases respectively. The success rates of coronary angioplasties and 30-day mortality were 91% and 0.9% respectively. Conclusion: In sub-Saharan Africa, Senegal in particular, the practice of coronary angioplasty is low. Indeed, many factors linked to socioeconomic status limit the development of coronary angioplasty in Senegal.

Assessment of Radiofrequency in Otorhinolaryngology at the Military Hospital of Ouakam (MHO)

Introduction: Coblation® is a low temperature-controlled tissue ablation technique widely used in human surgery. Its development in otorhinolaryngology has allowed a spectacular advance in the management of diseases of the nasal and oral cavities. The objective of our work was to study the indications for radiofrequency in ENT, to describe the technical aspects and to evaluate the results of his practice at the HMO. Patients and Methods: This is a retrospective study carried out in the ENT and Head and Neck Surgery department of the Ouakam Military Hospital (MHO) over a period of 3 years (from January 2015 to the end of December 2017), involving 37 patients who received radiofrequency treatment in the ENT sphere. Results: The study was carried out on 37 patients. The sex ratio of 0.19. The mean age of the patients was 33.3 years. The indications were dominated by chronic caseous cryptic tonsillitis with or without halitosis with 24 cases (64.9%) followed by recurrent tonsillitis in 5 cases (13.5%) then we have obstructive hypertrophy of the palatal tonsils with syndrome sleep apnea (OSAS) in 4 cases (10.8%) followed by nasal obstruction (hypertrophy of the inferior turbinates) and hypertrophy of the soft palate and uvula in 2 cases (5.4%). The complication rate was 8.1% and dominated by transient dysgeusia in 1 case or 2.7%, uvula edema in 1 case or 2.7% and bleeding in 1 case or 2.7%. The postoperative follow-up was done one week, 1 month, 3 months, 6 months and 12 months after radiofrequency. Volumetric reduction begins to set in from 1 month (62.5%) and becomes maximum in 3 months (74.3%). Conclusion: Coblation® is a controlled tissue ablation technique at low temperature, patented by ArthroCare® Corporation in 1997. Very widespread in human surgery.
